During the twentieth century, drugs were administered with much less care that in the current medical settings. Your host, Pat Iyer, recalls sometimes-fatal insulin shock therapy being administered to psychiatric patients. However, the rising number of deaths from medical and medication errors in the U. S. has made this crisis a significant issue, both in medical and legal arenas. Christine Dorman, a critical care nurse practitioner, has a doctorate and is a legal nurse consultant who specializes in critical issues. She brings her expertise to our discussion about how medication errors occur and how they can be prevented. She describes the many safeguards in place to ensure that the right medication is administered to the right patient, by the right route, at the right time, and at the right frequency. The participation of onsite critical care pharmacists further reduces the margin of error. Nonetheless, errors continue to occur. Christine and Pat discuss a Vanderbilt Hospital case wherein a nurse inadvertently administered a paralytic agent, Vercuronium, with tragic consequences. She also describes some other medicines that should be administered with great care and oversight. Knowing which drugs require the most attention and supervision helps an LNC in analyzing a medication errors case. When it comes to nursing and doctors' care, procedures, documentation, and much more, a nursing home environment differs greatly from that of an acute care facility. Irene Nobles, an LNC who specializes in long-term care cases, shares some of the most crucial differences. One major difference is regulatory. Very different governmental standards apply to overall care in a nursing home. One reason for this is the heightened vulnerability of the patient population. One complexity is that regulations may vary from state to state, but federal regulations take priority. Those working a nursing home case are far more likely to be faced with piles of paper records. Because many patients may be long-term residents, many of these records may be in storage. Paper records also make creative rewriting of incidents like falls possible, a situation which should put the LNC on alert. Irene emphasizes the importance of asking for or researching supplementary information. These include daily staffing reports, patient complaints, call light records, and other forms of information that can provide a wealth of information. Beth Zorn is one of the pioneers in the LNC field, having worked as a legal nurse consultant for 35 years. She has extensive experience both as an in-house LNC, both for defense and plaintiff firms. One primary distinction between plaintiff and defense work is that in the defense situation, cases may have been prescreened by the insurance company. When I first thought of sharing tips with you in the form of a podcast, the idea of getting the technology straightened out was a bit daunting. A business coach I hired helped me work through the steps and helped me get everything ready. At the beginning of Legal Nurse Podcast, I released 2 shows a week. The first was an interview and two days later, the second show was a monologue I created based on the same topic our guest covered. My cohost Kelly Campbell joined me for about a year and contributed ideas and guests. In July 2020, Barbara Levin and I began planning virtual conferences and I decided to drop the second episode of the week. Here's what people tell me: you listen when taking a walk, driving, cleaning the house and working out. The podcast has over one hundred thirty-six thousand downloads. Our listeners are in 92 countries. The top numbers of listeners are in the United States, Japan, Canada, Australia, UK and Germany. Eighty three percent of you listen to the podcast on mobile devices. The rest are going to our website, podcast.legalnursebusiness.com Here's what I've found. LNCs tell me that they dip into the vault of past shows and listen to current and older episodes. They appreciate the tips and hearing from a variety of LNCs and experts.. In the last 6 years we've seen some major changes in our world. During 2020 when the courthouses closed down in the U.S., attorneys worked harder to settle their cases. They learned how to take depositions by Zoom and even try some cases that way. Now that courts are a bit more open, attorneys continue to focus on settling cases, knowing that there is a backlog of cases waiting for trial. LNCs are essential to helping attorneys sort through cases, rejecting non meritorious ones, and conserving their resources. You help attorneys make sense of murky medical records, gain insights using your analytical skills, and provide decisive guidance. Once attorneys have rewarding relationships with LNCs, they often cannot think of managing cases without them. You may have heard me discuss how I built a large and successful multimillion dollar LNC business – from 1989 to January 2015 when I sold it. In February 2021 I returned to the practice of legal nurse consulting, after fulfilling a 5 year non-compete agreement with my business' purchaser. I work with a few attorneys now and love the experience. Here's what I know is true: you will have a rewarding business when you couple your knowledge with grateful clients. There are many attorneys who don't yet know how useful you can be. There are a few who think they know it all and don't need medical help. There are a few who don't appreciate your experience or undervalue your services. Don't waste your time trying to convert the hard-headed. If you do so, you'll just keep spinning in the wind, unable to satisfy these individuals. Look for the attorneys who want and need help and are willing to pay for it. If you have a bad experience with an attorney, learn from it and move on. When you have a great experience, which will be true for the majority of cases, learn from it and bask in the rewards. Your skills are an excellent way to serve attorneys and their clients. You have the ability to share what you know and serve the needs of your clients. Embrace this, grow with it, and enjoy it. The pandemic has caused widespread awareness of burnout experienced by medical workers. Most people, though, don't realize that burnout was already a critical issue among healthcare personnel. Increased pressure to see ever-greater numbers of patients, 12-hour shifts, and the constant burden of never having enough time ensured that the medical system, especially in hospitals, was already broken. Dr. Hamid Elmyar, who currently works in the area of public health, digs deeply into this problem. As a doctor committed to unearthing the root causes of illness, he has developed a comprehensive and global approach to the question of whether burnout is reversible. He points out that according to WHO, health is not just merely the absence of disease, it's the wellbeing of the social, physical, and mental being. In 2019, over 400,000 adult women reported being sexually assaulted. Although this figure is believed to represent only one third of actual rapes, and though it doesn't include children under the age of 12, it is a huge number. LNCs can play vital roles in those cases that are litigated. As a professional, you want to constantly evaluate new developments in your field. As a patient, family member, or nurse, you may have visited an ICU. Now you have the opportunity to experience the drama of this environment as described by a nurse who works there. Amy McCutcheon, who is both an LNC and a critical care nurse, recounts some of the conditions and circumstances that make the ICU unique among hospital settings. In chaotic and intense circumstances, all medical personnel must learn to work cooperatively. The ready availability of doctors and pharmacists makes the work easier. However, all those who work in ICUs suffer what is called alarm fatigue. In addition to the high noise level, a nurse must be able to discern which one needs to be handled first. If you are an LNC or malpractice attorney, you will surely come across cases in the very high-risk area of labor and delivery obstetrics. Jill Gunther, a highly experienced clinical nurse in this area, breaks down the major risks and how to prevent them. In many areas, obstetrics has evolved technologically, but Jill questions whether postpartum mothers get sent home earlier than they should. Working on a cancer malpractice suit provides special challenges for legal nurse consultants. As in all potential legal cases, attorneys come to LNCs for their expertise, and that requires special attention to discerning in which of several possible areas the error occurred. The rehabilitation nurse has unique challenges in the clinical world. She or he, in addition to general medical considerations, must assess patients for mobility, ability to eat, and potential for skin deterioration. An increasingly fragmented medical system has placed people with psychiatric issues in particular jeopardy. Both the clinical or psychiatric nurse and LNCs need to be keenly aware of cases that may involve misdiagnosis, delayed diagnosis, and nondiagnosis. Lynda Waggoner, a nurse practitioner with both medical and psychiatric experience, describes some of the key medical and psychiatric conditions that can lead to diagnostic errors. Diabetic and thyroid crises can masquerade as psychiatric conditions. Many psychiatric patients may have serious heart conditions that may lead to a diagnosis of acute anxiety. Although EMRs software was designed for greater efficiency in documentation, the large number of different systems, many of which perform different functions, programs that don't “talk” to each other, and the frequent mixing of hand-written and electronic records can make an LNC's journey through the world medical records feel bewildering. Difficulty in deciphering electronic medical records is especially frustrating because it can hamper an LNC's efforts to discover crucial information about the merits of a case. I meet LNCs who are struggling in their businesses. They attribute their struggles to attorneys not being interested in what they have to offer and also an over-saturated market, or not having the right connections. One LNC told me that the only ones who make it in this field are either married to an attorney or had a boyfriend or girlfriend who was an attorney. Beispiele sind die graphische Darstellung von Beziehungen in einem sozialen Netz oder Straßennetze, für deren Verarbeitung das Forschungsgebiet von Christian Schulz immer bedeutender wird, wie wir in seinem Gespräch mit Gudrun Thäter erfahren.Eine wichtige Aufgabe im Umgang mit Graphen ist die Aufteilung (oder fachsprachlich Partitionierung), von Graphen in kleinere Teile. Dies kommt z.B. der Parallelisierung der Arbeit auf den Graphen zugute, und ist unumgänglich, wenn Graphen eine Größe haben, die nicht mehr von einem Prozessor bearbeitet werden kann. Ein wichtiges Merkmal ist hier, die Aufteilung möglichst gleichmäßig vorzunehmen, damit die Aufteilung von z.B. In der Informatik ist der Dijkstra-Algorithmus der passende Basis-Algorithmus für diese Aufgabe, doch er ist für große Graphen in seiner ursprünglichen Form sehr ineffizient. In Kombination mit einer passenden Graphpartitionierung und Algorithmen kann man das Verfahren deutlich effizienter ausführen und beschleunigen.Ein klassisches Verfahren zur Aufteilung ist das Mehrschichtverfahren über die Laplace-Matrix, wo ausgenutzt wird, dass zwischen den Eigenwerten der Matrix und der Schnittstruktur des Graphen enge Zusammenhänge bestehen. Dieses Verfahren wurde zum Mehrschichtverfahren für Graphen weiterentwickelt, bei dem in einer sogenannten Kontraktion benachbarte Knoten und parallele Kanten jeweils zusammengeführt werden, und der Graph zu einem kleinen und kantengewichteten Graph vereinfacht wird. Schließlich wird das Problem auf einem vereinfachten, groben Gitter gelöst, und dann jeweils mit lokalen Suchen auf feinere Graphen erweitert. Für die Kontraktion werden Heuristiken und Kantenbewertungsfunktionen verwendet.Ein weiterer Ansatz sind auch evolutionäre Algorithmen. Dabei wurde eine allgemeinere Umgebung geschaffen, die auf eine weite Klasse von Optimierungsproblemen angewendet werden kann.Die Graphentheorie ist natürlich auch Teil der diskreten Mathematik, und besonders berühmt ist auch das Traveling Salesperson Problem. Gleichzeitig ist das Thema aber auch in der Theoretischen Informatik, im Algorithm Engineering und in der Software-Entwicklung beheimatet.Literatur und Zusatzinformationen C.
