In this episode I am joined by Ryan Sanson, a childhood friend and someone who I call a brother. Ryan Served in the British Army with The Royal Dragoon Guards. He shares with us his childhood, growing up in Bradford, United Kingdom. The north of England. Born to a white mother and black father he talks about the challenges he faced from a young age, being surrounded and dabbling in some criminal activity before reaching a turning point in his life and choosing to join the British Army. Ryan has deployed on 3 operational tours, Irag and Afghanistan twice. We discuss parts of these tours, what happened and how he faced some challenges on his return to the UK. Ryan accomplished a lot of things whilst serving from becoming an Army Physical Training Instructor and also qualifying as a Sniper. Since leaving the Army Ryan has embedded back into his community in Bradford where he works with troubled youth, helping lead and guide them through the challenges they face. Ryan also talks about doing security and how this has led him to travelling to Africa, Democratic Republic of the Congo to provide security for I Heart Africa, a charity that raises money for poverty-stricken people and they help provide drinkable water, electricity and housing for as many people as they can. Ryan is also an active bare-knuckle boxer; he uses this not only as a personal drive to keep him focused but also to raise money for various charities. One of these being the Royal British Legion. Ryan is a humble lionheart who uses his experiences, voice and body to help anyone who seeks it by being an inspiration and example in everything he does. Kenny Pasten and I met at the Wisdom Tree hike in Hollywood. He was hammering in a light into the ground so it would shine on a gigantic American flag. His story was immediately interesting to me. He served and fought in Irag and because of that suffers from PTSD. Upon returning to the states he found the Office of Veteran Affairs ill-equipped to help him has fast as he needed assistance - so he took it upon himself to search for a way to help fend off the nightmares and flashbacks. He obtained a Belgian Malinois and named him Rexitron (after Voltron from the 80s cartoon.) Unfortunately since this interview took place Rexitron has passed away making this story even that much more touching. This man's story is so moving because he is one of thousands who deal with PTSD everyday because of the wars in the Middle East and he is working toward his own recovery daily. Episode 92 with Marine and motivational speaker, Josh Bleill: Losing my legs in Afghanistan didn’t mean I had to give up my life. Summary: Josh Bleill was a Marine stationed in Afghanistan when a bomb explosion took both of his legs, and also the lives of many of his friends. He now works as a motivational speaker for the Indianapolis Colts, and during this inspiring interview he shares how he recovered – both physically and mentally – from the accident. Josh talks about the strategies and techniques he uses to put himself in the right mindset, and how he gets through tough times without giving up. History and background 2:10 Born and raisedin Indiana, Josh had a great family growing up. He played a lot of sports most of his life. And after 9-11, he felt a calling to join the marines. His grandfather and father had served before. So in 2003 he joined the Marine Corps, and in 2006 Josh got called to activated duty. He was in the middle of a tour in Irag and was stationed in Afghanistan when Josh was injured from a roadside bomb attack. He lost two of his good friends in an instant and both of his legs, but he was still thankful to be alive. After a long recovery, Josh has a family now and has a new job with the Indianapolis Colts as a motivational speaker. His faith was tested and he experienced a lot of guilt over the loss of his friends. Getting better 10:30 There were a lot of people around Josh that helped him get better. He mentioned the staff and his physical therapist who kept encouraging him. But it was really the guys and girls who were hurt before him that showed him that life does go on. Realizing that the world was just not going stop just because he felt sorry for himself, Josh realized that he had to get up each day to get his life back. Life after the Marine Corp 13:00 After the Marine Corps, Josh didn’t know what to do. He was not sure what his next career could be, but he believed in his faith that there is a plan for him. Then the Colts owner offered him a job as a spokesperson for them and now Josh goes to different schools and organizations talking as a motivational speaker. After 7 years 21:00 Things have changed for Josh after his injury, and all for the good. Physically his walking is good, he has a great job with the Colts, a wonderful wife, and kids. Josh also enjoys travel and talking to other people. He doesn’t take a lot for granted anymore and notices the small things in life. Josh also mentioned that his family is his greatest accomplishment and his work with the Indianapolis Colts keeps him going. What keeps Him going 29:40 Humor is a big part of Josh’s life. He enjoys a joke and hearing his kids laugh brings him happiness. Even the everyday humor of life or even his own occasional stupidity really brings joy into his life. The Total Tutor will interview Saul Silas Fathi. We will discuss the education system in Irag and his family life. He was born into a well-to-do Jewish family in Baghdad in 1938. Two former prime ministers lived on the same street and it was an “idyllic” place, he recalls. That ended for the country's Jewish population in the late 1940s when the creation of Israel set off a wave of anti-Jewish sentiment. As a young boy, Saul witnessed public executions and his family was falsely accused of treason against the Iraqi government. Like all wealthy Jews, his family was labeled Zionist and Communist. When he was 10, he and his younger brother were smuggled out of Baghdad through Iran and eventually landed in Israel. In 1958, Saul worked his way to Brazil, where he nearly starved. Two years later, he came to the U.S. and joined the Army for three years. In Israel, Saul had studied aeronautics and in the Army was assigned to the 15th Aviation Company, the famed helicopter division in the Vietnam War. Back in the U.S. he earned a degree in electrical engineering from the University of Virginia and went on to found three high-tech companies over a 20-year period. Saul retired in 2003 to write his memoir, Full Circle: Escape from Baghdad and the return (Xlibris 2006). A review by the president of Military Writers Society of America, stated that Saul's autobiography “takes the reader to places and events that one may only get to know about from small glimpses on The History Channel or in some old National Geographic Magazines. This guy has lived a life that would make a great movie.” Saul's second book, published in March, 2006 is History of the Jews and Israel – 4000 years of history condensed into 76 pages. He is fluent in English, Hebrew, Arabic and Portuguese.

Das gasförmige Stickstoffmonoxid (NO) steuert die Relaxation glatter Muskulatur. Dabei aktiviert NO die lösliche Guanylatzyklase im Zytosol der glatten Muskelzelle. Die lösliche Guanylatzyklase bildet den Botenstoff cGMP, der die cGMP-abhängige Proteinkinase (cGK) aktiviert. Die cGK gehört zur Familie der Serin/ Threonin-Proteinkinasen und führt nach Phosphorylierung von unterschiedlichen Signalproteinen zur Relaxation der glatten Muskulatur. Der Beitrag des NO/ cGMP/ cGK-Signalwegs zur glattmuskulären Relaxation kann mit Hilfe von genetisch veränderten Mauslinien, denen Komponenten dieses Signalwegs fehlen, untersucht werden. In der vorliegenden Arbeit werden neben Wildtyp-Mäusen mit intaktem NO/ cGMP/ cGK-Signalweg die cGKI-/--Mäuse, denen das Protein cGKI deletiert ist, und die IRAG∆12/∆12-Mausmutanten, die nach Zerstörung des Exon 12 des IRAG-Gens zwei hypomorphe IRAG-Allele besitzen, verwendet. Im Rahmen dieser Arbeit werden die cGMP-abhängigen Mechanismen der Relaxation nach Hormon- und Kalium-induzierter Stimulation glatter Muskelzellen untersucht. Die Experimente werden einerseits am intakten und permeabilisierten Gefäß und andererseits an kultivierten, vaskulären glatten Muskelzellen durchgeführt. Die Ergebnisse dieser Arbeit zeigen die Existenz unterschiedlicher Mechanismen zur cGMP-vermittelten Relaxation an der Mausaorta. Nach Hormon-induzierter Stimulation wird der Gefäßtonus durch den cGMP/ cGKI-Signalweg relaxiert. Dabei wird der Effekt hauptsächlich über IRAG durch Hemmung der Ca2+-Freisetzung aus den intrazellulären Speichern vermittelt. Die Kalium-vermittelte Kontraktion wird ebenso cGMP/ cGKI-abhängig relaxiert, jedoch unabhängig von IRAG. Überraschenderweise demonstrieren die Ergebnisse an der permeabilisierten Aorta, dass die cGMP/ cGKI-vermittelte Ca2+-Desensitisierung eine untergeordnete Rolle spielt und in Gegenwart hoher cGMP-Konzentrationen über die Protein Kinase A vermittelt wird. Die Kontraktionsexperimente mit Thapsigargin weisen darauf hin, dass vermutlich die SERCA an der Relaxation des Kalium-induzierten Tonus beteiligt ist.

Die cGMP-abhängige Proteinkinase (cGKI) vermittelt den relaxierenden Effekt der NO/cGMP Signalkaskade im glatten Muskel. Die Phosphorylierung des IP3-Rezeptor assoziierten cGMP-Kinase Substrats (IRAG) ist ein Prozess, der in diesem Mechanismus involviert ist. Um dieses Modell genauer zu verifizieren, wurde die cGMP-abhängige Relaxation in Mäusen, die ein modifiziertes IRAG expremieren, untersucht. Bei der IRAGD12/D12 Maus handelt es sich um eine Deletionsmutante, bei der die Interaktionsstelle von IRAG mit dem IP3-Rezeptor Typ I zerstört wurde, was dazu führt, dass IRAG nicht mit dem IP3-Rezeptor Typ I interagieren kann. Diese Mäuse zeigen eine normale Futteraufnahme, der Kotabsatz ist aber signifikant geringer als bei Wildtypmäusen. Eine Röntgenkontrastuntersuchung mit Hilfe von Bariumsulfat offenbarte eine deutliche Verlängerung der Darmpassagezeit, einen Megaoesophagus sowie ein Megacolon. In situ-Erhebungen an der eröffneten Bauchhöhle bestätigen diese Befunde. Gründe für diese Veränderungen könnten Funktionsstörungen in der glatten Muskulatur sein. Zur Stützung dieser Vermutung wurden die cGMP-abhängigen Effekte an der glatten Muskulatur des Darmtraktes untersucht. Die Untersuchung des Hormon induzierten Tonus im Jejunum ergab keinen signifikanten Unterschied in der cGMP-Wirkung zwischen den Wildtyp- und den IRAGD12/D12 Mäusen. Der CCh induzierte Tonus im Colon der Wildtypmäuse wird im Gegensatz zu den IRAGD12/D12 Mäusen durch 8 Br- cGMP um ca. 90% reduziert. Bei den IRAGD12/D12 Mäusen bewirkt 8 Br cGMP nur eine sehr geringe Relaxation (16%) des Hormon induzierten Tonus am Längsmuskel des Colons. Eine Vorinkubation mit dem Phosphatase-Hemmstoff Calyculin A in Präparaten von Wildtypmäusen hebt den relaxierenden Effekt von 8-Br-cGMP im glatten Muskel des Dünndarms auf, im glatten Muskel des Colons findet dagegen keine Aufhebung statt. Die Ergebnisse zeigen, dass IRAG eine entscheidende Bedeutung für die cGMP/cGKI-vermittelte Relaxation im Colon, aber nicht im Jejunum hat. Eine cGMP/cGKI-vermittelte Aktivierung einer Phosphatase kann als möglicher Mechanismus der cGMP abhängigen Relaxation im Jejunum in Frage kommen. Eine mögliche Phosphatase könnte hierbei die Myosin leichte Ketten Phosphatase (MLCP) sein. Es ist aber nach wie vor unklar, ob diese Ergebnisse Grund für die geringe Lebenserwartung der IRAGD12/D12 Mäuse sind. Um die Ursachen dafür zweifelsfrei feststellen zu können, bedarf es weiterführender Untersuchungen.

Der Tonus der glatten Muskulatur wird einerseits direkt durch Aktivierung des kontraktilen Apparates und andererseits über Veränderungen der intrazellulären Calciumkonzentration gesteuert. NO freisetzende Pharmaka aktivieren die lösliche Guanylylzyklase und führen über den cGMP/cGMP-Kinase abhängigen Signalweg zur Erschlaffung der glatten Muskulatur. Wie die Kinase dieses Signal vermittelt ist noch weitgehend unklar, wobei schon einige Substrate der cGMP-Kinase I bekannt sind. In der Mikrosomenfraktion glatter Muskeln wurde ein stabiler Komplex der cGMP-Kinase I und ihrer Substrate IRAG und IP3-Rezeptor Typ I beschrieben, der für die Steuerung des Calciumausstroms aus Speichern des sarkoplasmatischen Retikulums verantwortlich ist. In der vorliegenden Arbeit sollte dieser mikrosomale Komplex genauer untersucht und die beteiligten Proteine gereinigt und charakterisiert werden. Dies geschah vor allem durch Co- Immunpräzipitationen mit spezifischen Antikörpern und Affinitätschromatographie mit cGMP-Agarose. Die Identifizierung der gereinigten Proteine erfolgte durch MALDI-TOF Analyse und Immunoblot, wobei Substrate der Kinase nach cGMP-abhängiger Phosphorylierung mit radioaktiv markiertem ATP detektiert wurden. Der Komplex wurde aus dem glatten Muskel der Trachea von Rindern aufgereinigt und seine Bestandteile isoliert. Dabei ergab sich die Assoziation von Phospholamban, einem Substrat der cGMP-Kinase, das den Calcium Rücktransport durch die Ca2+-ATPase in Speichervesikel des sarkoplasmatischen Retikulums moduliert. Die Interaktion von Phospholamban mit der cGMP-Kinase I β und dem IP3-Rezeptor Typ I konnte nach heterologer Expression der Komponenten in COS 7 Zellen bestätigt werden. Dazu wurde Phospholamban aus cDNA des Herzens der Maus kloniert und mit je einer der beiden Isoformen der cGMP-Kinase I α und β, dem IP3-Rezeptor Typ I und IRAG in COS 7 Zellen exprimiert. Weiterhin konnte die Assoziation des Komplexes an die zytoskelettalen Proteine α-Aktin und Calponin H1 gezeigt werden. Das Zytoskelett kann einerseits zur Stabilisierung des Komplexes im glatten Muskel beitragen. Andererseits kann der Komplex Membranproteine mitdem Zytoskelett verbinden und einen direkten Einfluss auf die Calcium unabhängige Kontraktion der Zelle haben. Als einzige Funktion von Phospholamban ist bisher die Regulation der Ca2+-ATPase beschrieben worden, daher wurde deren Assoziation an den funktionellen Komplex untersucht. Dazu wurde der Komplex an cGMP-Agarose gereinigt und nach Elution eine Co-Immunpräzipitation mit den spezifischen Antikörpern der Komplexbestandteile durchgeführt. Die differenzierte Auftrennung über zwei Säulen deckte die Existenz zweier Proteinkomplexe auf, die über das Zytoskelett miteinander verbunden sind. Einer besteht aus Phospholamban und der Ca2+-ATPase und steuert die Aufnahme von Calcium in intrazelluläre Speicher, der andere enthält einen geringeren Teil Phospholamban, die cGMP-Kinase I β und ihre Substrate IRAG und den IP3-Rezeptor Typ I. Dieser zweite Komplex reguliert wahrscheinlich den Calciumausstrom aus dem sarkoplasmatischen Retikulum.

I was taking the pills and drinking every night before I went to bed. I literally had to lock every single door in my house, including closets and other bedrooms. It was obsessive. I was having issues. So I took the shiny brass off my shoulders, called my fellow Marines and said, ‘Hey, I'm struggling. I'm drinking. I can't figure it out. Who else is having these problems?'
