FREE! CRNA School Interview Prep Guide: https://www.cspaedu.com/uc9a5ih4Ketamine: A Unique Anesthetic with Diverse ApplicationsDive deep into the world of ketamine, exploring its history, pharmacology, mechanism of action, clinical uses, and side effects. Dr. Kelly Elmore, Assistant Program Director at Mary Baldwin University's Nurse Anesthesiology Program, guides listeners through this fascinating drug, highlighting its potential for anesthesia, pain management, and even mental health treatment.Targeting NMDA Receptors for Dissociative AnesthesiaKetamine is classified as a dissociative anesthetic, distinct from depressant anesthetics like propofol. It primarily works by blocking NMDA receptors in the central nervous system, thereby reducing excitatory signals and producing profound analgesia and amnesia. This mechanism also contributes to the characteristic dissociative state, where patients may appear detached from their environment and experience hallucinations.Beyond Anesthesia: Exploring Ketamine's Diverse UsesBeyond its role in anesthesia, ketamine offers a range of benefits in various clinical settings:Pain Management: Ketamine's analgesic properties make it valuable for treating acute and chronic pain conditions, including neuropathic pain, complex regional pain syndrome, and post-surgical pain.Mental Health: Emerging research suggests ketamine's potential in treating treatment-resistant depression, PTSD, and suicidal ideation. Low-dose ketamine infusions combined with psychotherapy have shown promise in these areas in the form of ketamine-assisted therapy.Emergency Medicine: Due to its bronchodilatory effects and ability to maintain airway patency, ketamine is a valuable tool in emergency departments for managing patients with respiratory distress.Important Considerations for Ketamine UseWhile ketamine offers numerous advantages, it's crucial to be aware of its potential drawbacks:Side Effects: Nausea, vomiting, and hallucinations are common side effects, particularly at higher doses.Abuse Potential: Ketamine's popularity as a street drug necessitates caution, especially in outpatient mental health settings.Cardiac Effects: In critically ill patients with depleted catecholamines, ketamine can worsen cardiovascular depression.Mental Health Risks: Ketamine may exacerbate symptoms in some mental health disorders.S-Ketamine: A Promising Option for Treatment-Resistant DepressionThe FDA-approved S-ketamine nasal spray offers a new avenue for treating treatment-resistant depression. This medication is typically used alongside other oral antidepressants. Conclusion: A Multifaceted Drug with Ongoing ResearchKetamine's unique properties make it a valuable tool for CRNAs across various healthcare settings. Ongoing research continues to explore its potential benefits and refine its use for optimal patient care.Join the Free CSPA Community! Explore how memory operates in magical ways in Harry Potter that might be quite unlike the Muggle understanding of memory. Regular contributor Dr. Louise M. Freeman is professor emerita of psychology at Mary Baldwin University and a licensed behavior analyst and service provider for special needs individuals. She considers the implications of being able to store and share memories in a Pensieve. While the basic premise seems somewhat similar to the psychological process of encoding, storing, and retrieving memories, there is almost an element of time travel, in which the user can access details that the initial viewer surely would not have been able to see or remember. But can such details be trusted, or are they part of the viewer's subjective reconstruction and reinterpretation of the memory? While it is obvious that Slughorn has tampered with his memory of Tom Riddle, it is less clear how accurate Snape's memories of the Marauders are. Upon his death, Snape manages to show Harry a clear montage of memories explaining everything - had he been preparing all of the necessary information in order to hand it off so neatly? How difficult is it to show memories smoothly? Could Harry's memory of the Dementor attack in Little Whinging have been used as evidence in his hearing, or would Fudge not have allowed or trusted it, particularly if presented by Dumbledore? What happens when memories are lost to a Memory Charm? Are they fully removed, or just hidden? Victims of such charms seem to have varying degrees of success in recovering their memories. While taking memories can be portrayed as a terrible crime, like what Lockhart intended to do to Harry and Ron, modifying them is often depicted as humorous or necessary, such as when the Ministry of Magic performs Memory Charms on Muggles who have witnessed magic. Hermione alters her parents' memories without their consent, albeit for a noble purpose, but the full consequences are unknown. If they were to be captured and interrogated by Death Eaters, could their memories be forcibly extracted? There are ethical issues surrounding memory in both the wizarding and Muggle worlds. Stay tuned for Part 2 to hear more from Louise on the complicated nature of memory.

It might surprise you to learn that just about every production of a Shakespeare play that you've ever seen onstage has been cut, from student shows to Broadway revivals. Cutting Plays for Performance: A Practical and Accessible Guide, a new book by Aili Huber and Dr. Toby Malone, lays out some of the reasons that theater-makers cut Shakespeare's plays, and suggests some handy questions directors and dramaturgs should ask themselves as they take a pen to the plays. Barbara Bogaev interviews Huber about the argument that brought Huber and her co-author together, strategies for cutting plays, and how a good cut can reveal a new and exciting story. Aili Huber has been a theater director for over 20 years. She holds an MFA in directing from Mary Baldwin University and the American Shakespeare Center. Her new book, co-written with Dr. Toby Malone of SUNY-Oswego, is called Cutting Plays for Performance: A Practical and Accessible Guide. It was published by Routledge in December 2021. Stacy Pethia is the county’s housing policy manager.“The proposed project would serve a total of 470 individuals and households through three distinct programs,” Pethia said. These are $110,000 for a grocery card gift program to serve up to 220 households, and $144,000 for the Local Food Hub’s Fresh Farmacy program to provide fresh produce for 18 months to 100 households. The funding would come specifically from a COVID relief program. Supervisors also agreed to amend a special use permit that allows the Monticello United Soccer Club to operate on land off of Polo Ground Road. Scott Clark is a planner with Albemarle County. “The proposal would increase the number of total number of fields to seven although only four would be used for play at any one time,” Clark said. “This is to enable them to move feels around, rest fields, prepare fields.”The land is within Albemarle’s rural area, and there are no permanent facilities on the property. There is no increase in the number of parking spaces. “This property could easily return to agricultural use in the future with a very low impact on the site,” Clark said. The Mon-U soccer field is on Polo Grounds Road, which is just to the north of where the furthest Charlottesville Area Transit route currently stops. That won’t change when the city-owned and operated agency alters its routes later this year, but CAT is conducting a review of how to expand service to the north. So is Albemarle County and the Thomas Jefferson Planning District Commission. You’re reading Charlottesville Community Engagement and time for another subscriber-supported public service announcement. The Charlottesville Jazz Society at cvillejazz.org is dedicated to the promotion, preservation, and preservation of jazz, and there’s no time like now to find a time to get out and watch people love to play. The Charlottesville Jazz Society keeps a running list of what’s coming up at cvillejazz.org. This week, find out that the Michael Elswick Gathering plays at the Pub at Lake Monticello on Friday and the Eric Franzen Trio plays at Early Mountain Vineyards on Saturday. Take a look at cvillejazz.org. At the same time, Albemarle County and the Thomas Jefferson Planning District Commission are doing the exact same work as part of a study partially funded by the Virginia Department of Rail and Public Transportation. Boris Palchik is a transit planning project manager with Foursquare Integrated Transportation Planning, a firm hired to help conduct the work. The other consultant is Michael Baker International. Palchik ran a meeting on July 26 that sought to get initial feedback for the study.“It’s really a feasibility study and implementation plan for expanding transit service in both population and employment centers in Albemarle County,” Palchik said. The July 26 meeting was for northern Albemarle County along U.S. 29, and one on July 28 was held for Pantops. We’ll focus on July 26 first. (watch the video)Palchik said the study may not result in several new fixed routes, but may include a combination of on-demand routes and other new transit options. The work consists of a market analysis, a service analysis, and stakeholder outreach. “The market analysis is looking at the underlying environment in which transit operates or needs to operate in the study area,” Palchik said. “The service analysis is looking at what’s happening today on the ground in terms of ridership and productivity.”Stakeholder outreach includes the July meetings and other ways to get a sense of what people might want and need in expanded transit. In addition, to Charlottesville Area Transit, Jaunt provides service in the area through on-demand, one fixed-route service, and through its partnership with Greene County Transit. “There’s really many different ways to provide transit service and each of those ways has its own ideal operating environment,” Palchick said. “When we’re looking at the market analysis, we’re trying to understand the environment that exists so we can make recommendations that are appropriate.”That means taking a look at population density, the built environment, employment opportunities, and other factors to measure the potential for public transit to work. “Transit service is most effective and most efficient in areas that have higher density,” Palchick said. “The kind of tipping point for where fixed route transit service really begins to make sense is once you have more than five people or jobs per acre.”Research conducted so far indicates moderate-to-high transit potential south of the South Fork of the Rivanna River. The highest population density in the area is along Commonwealth Drive, which is currently served by CAT’s Route 5. Service gaps are north of Rio Road and in the Hollymead / Forest Lakes area. A slide from the July 26 presentation (download)This work also comes at a time when Albemarle continues to become more dense, with more properties coming online such as North Pointe, Brookhill, and numerous other developments that will be more dense than single family homes. Palchick said the stakeholder analysis specifically sought out information that may not have come through their initial review. During the service analysis, stakeholders were shown older information on CAT routes, several of which are changing in the coming months. There will be alterations to Route 5, Route 7, Route 8, and Route 11, all of which serve Albemarle’s northern urban area.  Learn more about the CAT changes here. Scott Elliff is a member of the Forest Lakes Community Association’s Board of Directors. The FLCA has used a portion of its homeowner association fees to fight development of a mixed-use development on Ashwood Boulevard known as RST Residences. Elliff took the opportunity to speak at a discussion on expanding transit to point out that the existing character of his neighborhood is suburban. “The challenge that’s happening up here is that we’re starting to get developments that are going to be by necessity pretty dense,” Elliff said. “There’s one that’s being planned which we’re opposing and hasn’t come before the Supervisors yet. It would be a huge high story development on the corner of Ashwood and 29.”Currently there is fixed-route transit service in the Forest Lakes area provide by Jaunt through their Route 29 Express.According to Valerie Long of the law firm Williams Mullen, 75 percent of the apartments in the RST development will be rented to people who can demonstrate household incomes between 30 percent and 80 percent of the Area Median Income. Elliff is concerned that if all of those people drive, it will exacerbate traffic congestion out of a neighborhood that only has two direct connections onto U.S. 29. “The only solution from a transportation standpoint that I can think of is to have a dedicated service that picks people up at those affordable housing apartment buildings and takes them non-stop down to Barracks Road, downtown mall, and UVA where the jobs are,” Elliff said. Elliff claimed there were no jobs in his area. In fact, let’s hear more of what he had to say. “We’re up here in a beautiful area,” Elliff said. “There are no jobs. There are retail jobs… in the shopping centers north and south. If it’s going to be heavily affordable housing, these are people who are going to be working retail and they’re going to be working as administrative assistants or something in small companies but not around here. This is completely residential.”Elliff’s claim made me look up the latest information from the Virginia Employment Commission on the top employers in Albemarle County.  Several of them are within close proximity to the Forest Lakes neighborhood and all rough measurements below are taken from the pool at Forest Lakes South using main roads and Google Earth. (VEC profile)#4 is the Department of Defense and the various military installations at Rivanna Station (4.77 miles away) #6 is the Crutchfield Corporation which operates by the Charlottesville Regional Airport (3.5 miles away)#7 is the Northrup Grumman Corporation located in between both sides of Stonefield on U.S. 29 (4.4 miles away)#9 is Wal-Mart located just south of the South Fork of the Rivanna River on US. 29 (2.2 miles away)#18 is Emerson (listed as G.E. Fanuc) on U.S. 29 north of North Pointe (5.2 miles) #29 is Costco in Stonefield on U.S. 29 (4.5 miles away)#32 is Target in Hollymead Town Center (2 miles away)#36 is MicroAire Surgical Instruments in the former U.S. Postal Service building off of Airport Road (2.75 miles away)#38 is Rosewood Village Associates with facilities in Hollymead Town Center (2.4 miles away) The RST rezoning goes to the Board of Supervisors on September 15. From the Albemarle County profile from the Virginia Employment Commission (download)Now, on to the July 28 meeting, which covered the Pantops area. Fewer people attended that virtual call. Pantops is currently served by Charlottesville Area Transit Route 10, which will also be changing as a result of the upcoming route changes. Here’s Boris Palchik with Foursquare once again reviewing a market analysis. “In the Pantops area north of U.S. 250, there are a number of key kind of activity generators like multifamily housing, the Social Security administration building, but it’s still showing fairly low density,” Palchik said. Dick Hiss, the chair of the Pantops Community Advisory Committee, asked if the various analyses conducted take a look at future land use changes. “Have you considered the changes that we see coming in the Pantops area such as the motor vehicle department going somewhere?” Hiss said. “That building has had a sign on it for years saying it is moving.”Hiss said he is also wondering if State Farm employees will return to that building. State Farm is the fifth largest employer in Albemarle according to the VEC. Sentara Martha Jefferson is the third. Gina Morss-Fischer, a public affairs specialist with State Farm, confirmed in an email to me today that employees assigned to the Charlottesville-Albemarle office will continue to work from home. Palchick said the stakeholder meetings are intended to take note of comments such as this. For a time, Albemarle County had been updating development dashboards which depicted what projects were coming up in the near future. These have not been updated since February 2020 in part because of the pandemic and in part because a staff member moved on. Charles Rapp is the planning director in Albemarle County. “The staff member that was previously managing the dashboards is no longer with the county so we have used this as an opportunity to collaborate with our GDS department and create an updated version of the development dashboard,” Rapp said in an email to me this morning. “This new approach will have automated updates regularly and should provide a more streamlined approach toward conveying information.  We are working through the final details now and hope to have it ready for the public soon.”(review the current Development Dashboards on the Albemarle website)But back to transit. Palchick said on-demand microtransit could be an option for parts of Albemarle in the future.“The main difference between microtransit and Uber and Lyft is that Uber and Lyft operate with a fleet of vehicles that are not infinite, but you never quite know what kind of vehicle you are going to get when you request a service,” Palchick said. “Whereas with microtransit you have a set fleet of vehicles and a set group of drivers that are operating the service so it is more predictable and can be more closely branded with the local public transportation service and be more closely affiliated with it.”Currently, Pantops is also served by Jaunt’s Buckingham Connect East service. “So this service operates between Buckingham County and destinations in Charlottesville and Albemarle County,” Palchick said. “Those destinations include downtown Charlottesville, the University hospital, Martha Jefferson Hospital and the Westminster Canterbury retirement community.” Westminster Canterbury is the 14th largest employer in Albemarle County. A slide from the July 28 presentation on Pantops (download)In addition to the meetings on July 26 and July 28, the consultants are holding individual meetings. The goal is to complete the study by next January in order to apply for funding from the Virginia Department of Rail and Public Transportation to pay for a pilot project. Will any of this result in a better transit system? That means to be seen. Another thing I encourage people to see is the staff report of a February 11, 2008 joint meeting of the Albemarle County Board of Supervisors and Charlottesville City Council to discuss a study for a Regional Transit Authority that would be one unified system. That never happened, but eight years later, a Regional Transit Partnership was formed to encourage collaboration between area systems. That body next meets on August 26. I've known that the 5th anniversary of the podcast has been coming up for quite some time now, and I've been wracking my brain on how best to commemorate this milestone. After considering a few different options, I eventually decided that there was no better place to start than by looking back at the first episode of Behavioral Observations, my interview with Dr. Greg Hanley. In this episode, we discussed the origins of what is now called the Practical Functional Assessment approach, back when it was referred to as the IISCA. I wanted to replay this show for a few reasons. First, even though the PFA process has gone through many refinements over the last few years, in this episode, Greg describes the factors that led him to deviate from the Standard Functional Analysis procedure. I think that part of the show on its own is worth revisiting. The second reason for sharing this conversation again is that every day new listeners are coming in contact with the show. While I get emails from some of them who tell me that they binge the back catalog, with nearly 150 shows, I don't expect that everyone is going to subject themselves to that. Lastly, this particular episode is the most downloaded episode when compared to all the other shows I've published. As of this writing, the show has been downloaded over 56,000 times. About 15,000 times more than the second-most downloaded show (which by the way, is Session 7, again with Greg... want to guess who's in the third-most downloaded show?). So for all those reasons and more, I hope you enjoy this episode, whether it is for the first time, or if you're dusting it off for a re-listen. On a broader note, I'd like to talk for a minute about my thoughts on the show turning five. It's not an understatement when I tell you that creating this podcast has been a life-changing experience for me. Most certainly life-changing in a professional sense, and very likely from a personal one as well. With regard to the former, starting the show really re-energized my passion for the field at a time when I was teetering on the brink of burnout (before burnout became a fashionable term). With regard to the latter, I've met several hundred people and made some amazing friends throughout this journey. And when milestones like these come up, it is incredibly overwhelming to think through all of the people who've helped me make this show happen. First, there are so many people who have been listeners and supporters since Session 1. People who've listened to every single episode... all of my filler words, awkward silences, vocal fry, the whole nine yards. I once had someone actually take data on my filler words and would send it to me from time to time. That's dedication! All kidding aside, this show would've quietly faded out over time if it wasn't for the support and encouragement from you, the listener. I'm also grateful to everyone who shares episodes with friends and co-workers. And of course supervisors and professors who who force their mentees and students to consume this content. It is both amazing and gratifying that this fun side project is helping people learn more about the science we all know and love. I've had countless people and organizations support the show financially, whether by purchasing CEUs*, subscribing to my Patreon membership, sponsoring episodes, or inviting me to speak at events. To be perfectly candid, the ability to generate some revenue from Behavioral Observations allows me to put more time into the show, and I'm grateful for everyone who has helped me do just that. It is always a danger to list specific people to thank in situations like these, as it is all too easy to inadvertently leave someone out. As such, I've chosen to limit my shoutouts to one person in particular: my friend John Corley. John is not a Behavior Analyst, but he is a programmer and all around tech-savvy guy. I shared the idea of a podcast with him over a few beers, and he relentlessly encouraged me to follow through with it. In fact, he helped me set up my website, the podcast's RSS feed, showed me the basics of GarageBand, and lots more. He remains on standby to me whenever I have a bug or glitch that needs sorting out. John Corley and me ~ 2017 So huge thanks to both John, and everyone else who has played a part in getting this show to nearly 2.5 million downloads in these last five years. I could say thank you a million times and it wouldn't be enough. I look forward to sharing these conversations with you for the next five years and beyond! So the title of this show may be a bit of a misnomer. Let me explain... In Session 148, Shira Karpel and Shayna Gaunt, founders of HowToABA.com, join me to talk about one of my favorite topics: supporting the newly-minted BCBA. As you might know by now, that's very often my closing question of the podcast. In this episode however, we pretty much spent the entire time talking about strategies for helping early-career professionals succeed. So what is the misnomer all about? As it turns out, the advice that Shira and Shayna provided - on topics ranging from obtaining mentorship to setting professional boundaries - is helpful for BCBA's of all experience levels. Moreover, Shira and Shayna created HowToABA.com to provide materials and continuing education opportunities for BCBA's, as well as a supportive community of like-minded practitioners. And in this episode, they share their experiences and insight and address the following: Common challenges faced by newly-minted BCBA's (and how to address them). Common mistakes early-stage practitioners make. Strategies to establish professional boundaries. What practitioners are looking for in their Continuing Education. Why they started HowToABA.com. The ABA Forum that they're hosting. It's a virtual 3-night event where they bring together recognized speakers in the field who will share on different topics in Behavior Analysis - from supervision to ethics and social skills and everything in between. Again, despite the headline, there's something in this episode for everyone. Also, I want to note that we had some intermittent Zoom connectivity challenges, and Shira's audio flaked in and out in a few spots. I chose not to edit those segments out because I think she was still able to make her points known, and thankfully, we were able to resolve it as the show went on. Here are links to some of the things we discussed: The HowToABA.com homepage (and if you sign up for the BxResource, use code BOP to save 10%). The ABA Forum registration page. Surveys of the use (or lack thereof) Functional Analysis by practitioners (Oliver, Pratt, and Normand, 2015; Roscoe, et al., 2015). Great article about setting boundaries and avoiding burnout. And here's another perspective. Today's episode is brought to you by: The 2021 Virginia Association for Behavior Analysis Conference! This year's VABA conference is April 15th and 16th. While this will be a safe and socially-distanced in-person event, it will also be available online, so don't worry if you're not local to the Virginia area. In this episode I’m talking with Cass Morris, author of GIVE WAY TO NIGHT, the sequel to the historically-inspired fantasy FROM UNSEEN FIRE.  This series, called the AVEN CYCLE, explores a Roman-esque empire filled with divine and elemental magics, dangerous political machinations, and rising war between cultures and territories. Several years of harsh dictatorship have unbalanced the powerful city of Aven. But now, the Dictator is dead, and the resulting power vacuum draws in the late-ruler's allies and enemies to struggle for control of the Republic. Latona of the Vitelliae is a mage of Spirit and Fire, married off to a minor, indifferent noble to keep her life and talents free of notice. But with the dictator finally gone, Latona and her canny, gifted siblings can throw off the objections of society to seize a greater destiny for their people. Sempronius Tarren is an exiled but ambitious senator seeking influence in Aven. But in a political system that refuses office to those who possess magical talent, he hides a dangerous secret—a blessing from the gods that shows him shadows of possible futures.  Conjoined by politics, complicated by romance, Latona and Sempronius must work together to protect the Republic. But can cunning wit and divine magic stand strong against foes that wield brutal violence and wicked sorcery?  GIVE WAY TO NIGHT is the 2nd novel in the AVEN CYCLE series, and is available now from DAW Books: Amazon: https://amzn.to/2WZtO3G Bookshop: https://bookshop.org/a/16632/9780756412319 Barnes & Noble: https://www.barnesandnoble.com/w/give-way-to-night-cass-morris/1136806407 Books-a-Million: https://www.booksamillion.com/p/Give-Way-Night/Cass-Morris/9780756412319 IndieBound: https://www.indiebound.org/book/9780756412319   FROM UNSEEN FIRE is available now:   Amazon: https://amzn.to/2MecfuA Bookshop: https://bookshop.org/a/16632/9780756415174   » Some of these links are affiliate links. Science in Harry Potter? You’re kidding, right? Harry’s adventures are set in a magical world that is the antithesis of the Muggle realm’s dependence on science and engineering; magic by definition is the suspension of the laws of nature, and, as science is the exploration of those laws, the worlds should not meet, even in the potions laboratory of Professor Snape. Except in one field, namely, genetics. Witches and wizards, pure-blood, half-bloods, mudbloods, and even squibs are all subject to and products of the vagaries of chromosomes and genetic coding like all the rest of us and as this is no small part of the larger meaning of the Hogwarts Saga – the prejudice of purebloods to muggleborns – getting the genetics right is an important thing. Joining us today to talk about this is Dr. Eric Spana, an Assistant Professor in the Practice of Biology at Duke, who is an expert on the subject and Priyanka Nadar, founder of the Harry Potter Alliance chapter at Mary Baldwin University who has her Masters in Engineering Sciences from Dartmouth.
