Chuck Todd surveys a political landscape where multiple crises are converging on the Trump administration simultaneously — and none of them are going well. The Iran war, which Chuck reiterates is a war of choice, appears to be devouring Trump's presidency: the administration burned through nearly $6 billion in munitions in just two days, is sending contradictory messages of reassurance and escalation that appear designed to manipulate markets, and seems to be operating entirely by the seat of its pants. He warns that asymmetric warfare has never gone well for the United States, that energy markets are in turmoil as Iran deliberately tries to inflict economic pain, that the threat of stagflation and energy shortages is very real, and that Trump's threat to use the Fed to shape oil markets has alarmed economists. He argues that if Trump could undo the war he would — but this won't be Venezuela 2.0, because there's no opposition on the ground to coordinate with, you can't change a regime without boots on the ground that Trump won't commit, and if the regime simply survives, that counts as victory for Iran. Meanwhile, Trump naively buys Putin's claim that Russia isn't helping Iran with targeting, and that new polling shows a majority of Americans oppose the war — with MAGA influencers notably against it even as older rank-and-file supporters stick with Trump. Beyond Iran, Chuck hits the Ticketmaster settlement as proof that Trump talks a big populist game but the lobbyists always win, warns that a partial DHS shutdown risks snarling air travel and punishing the flying public while ICE has already been funded, and cautions Democrats not to overplay their hand on the shutdown. Then, John Adams, editor of the Montana Free Press, joins the Chuck Toddcast to make the case that local journalism has been in crisis — and that saving it might be one of the most important things Americans can do for their democracy. Adams is on the show to promote Local News Day on April 9th, a nationwide effort involving 700 local newsrooms aimed not at fundraising but at spreading awareness. He traces the origin story of the Montana Free Press back to his appearance in the documentary "Dark Money", a story about outside money quietly trying to buy Montana politics at the local level with zero disclosure — and the journalists best positioned to expose it were losing their jobs. Adams argues the economics of local news changed drastically as advertising revenue collapsed and audiences became hypnotized by social media and smartphones, but that the need and appetite for local reporting never went away. The conversation turns to the deeper consequences of local news deserts: the loss of trusted community voices created a vacuum that bred distrust in the national press, because people no longer had local "character references" — journalists they knew and saw at the grocery store — to anchor their understanding of how media works. Adams warns that the rise of AI-generated misinformation makes reliable local sources more important than ever, noting that while younger people tend to be savvier at spotting junk online, older generations are particularly vulnerable. They close by noting that strong local news doesn't just serve democracy — it helps local businesses target customers, creating an economic ecosystem that benefits everyone — and that journalism ultimately has to reach enough people to really matter, which is exactly what Local News Day at localnewsday.org is designed to help make possible. In this episode of Framework Focus, Dr. Dae Lee breaks down the regulatory and reimbursement shifts reshaping long-term care (LTC) pharmacy. With the Inflation Reduction Act (IRA) moving from policy theory to operational reality, Medicare's Maximum Fair Price (MFP) program, Part D redesign, and inflation rebate enforcement are now active forces that LTC operators must navigate. This conversation moves beyond headlines and into real-world implications: cash flow risk, administrative burden, contracting exposure, and strategic readiness for closed-door and combo LTC pharmacies. Key Discussion Points Medicare Drug Price Negotiation Is Now Operational The IRA authorizes Medicare to negotiate prices for certain high-spend Part D drugs, with the first 10 Maximum Fair Prices (MFPs) effective January 1, 2026. This marks a structural shift in federal drug pricing and directly impacts LTC dispensing economics. Maximum Fair Price and LTC Reimbursement Compression MFP caps reimbursement on selected drugs, creating potential margin compression if acquisition costs and payment timing are misaligned. For LTC pharmacies operating on tight spreads and high-volume chronic utilization, even small deltas can materially affect profitability. Medicare Transaction Facilitator (MTF) and Payment Timing Risk The MTF system introduces new payment mechanics between manufacturers, plans, and pharmacies. LTC pharmacies must understand how effectuation and reconciliation work, particularly if payment timing differs from traditional Part D adjudication flows. Cash flow modeling becomes essential. Administrative Complexity and Claims Reconciliation Identifying which NDCs are subject to MFP, managing claim reversals, handling price disputes, and monitoring plan-level compliance will increase administrative workload. LTC pharmacies will need structured internal workflows to prevent reimbursement leakage. Dispensing Fees and the LTC Service Intensity Problem Unlike retail, LTC pharmacy includes compliance packaging, emergency kits, cycle fills, consultant pharmacist oversight, and regulatory documentation. Current IRA implementation does not automatically adjust dispensing or supply fees to reflect this complexity, raising sustainability concerns. Congressional Efforts to Stabilize LTC Pharmacy Legislative proposals such as the Preserving Patient Access to LTC Pharmacies Act aim to create supply fee protections tied to MFP drugs. The episode explores whether policy corrections are likely — and how quickly DC can realistically respond. Expansion of Negotiated Drug Lists The initial 10 negotiated drugs are only the beginning. Additional rounds of negotiation are underway, expanding exposure across more therapeutic classes. LTC pharmacies must treat MFP as a growing structural feature — not a limited pilot. Part D Redesign and the $2,100 Out-of-Pocket Cap Beginning in 2026, the redesigned Part D benefit changes liability distribution among plans, manufacturers, and CMS. The new out-of-pocket cap alters plan incentives and may lead to tighter utilization management, formulary shifts, and network recalibration — all of which LTC pharmacies must monitor closely. Inflation Rebates and Market Distortion The IRA's inflation rebate provisions penalize manufacturers for price increases above inflation benchmarks. While not directly adjudicated at the pharmacy counter, these provisions influence manufacturer pricing strategy, launch pricing behavior, and downstream PBM negotiations — indirectly affecting LTC acquisition costs. Strategic Readiness as a Competitive Advantage The episode concludes with practical recommendations: • Conduct an MFP exposure audit across top-dispensed NDCs • Reassess PBM contracts and network participation clauses • Model cash flow under delayed reimbursement scenarios • Educate facility partners on regulatory changes • Build internal compliance tracking specific to negotiated drugs Why This Matters Now For LTC pharmacies, this is not simply a policy conversation — it is a structural shift in reimbursement architecture. As federal oversight expands and pricing authority evolves, operational precision and legal literacy will define which organizations adapt successfully. As 2026 gets underway, we answer your questions about the CMS drug acquisition cost survey, what states are doing on 340B this year, and more. Some of the topics we cover:CMS Drug Acquisition Cost Survey Not MandatoryEarlier this year, the Center for Medicare & Medicaid Services (CMS) launched a new survey focusing on hospitals' outpatient drug acquisition costs, which could lead to Medicare Part B payment cuts for 340B drugs. Some hospitals recently saw materials suggesting they are required to complete the survey. Amanda Nagrotsky, vice president of legal and policy for 340B Health, notes that a CMS rule states there are no penalties under the Medicare statute for hospitals that choose not to respond. 340B Health and other groups sent a joint letter asking for the language to be corrected, citing the confusion it has caused.State Legislatures Are Becoming Major Battlegrounds for 2026Just over one month into 2026, statehouses are already shaping up to be one of the biggest venues to debate various aspects of the 340B. Two broad categories of bills are emerging: legislation protecting access to 340B pricing — including protections for contract pharmacy arrangements — and state-level reporting mandates. 340B Health Senior Vice President of Government Relations Tom O'Donnell says the proposed reporting mandates mirror other states' recently enacted requirements, and he argues they can be misleading, burdensome, or modeled on frameworks promoted by large drug companies.Medicare Announces More Drug Price Caps for 2028Medicare is phasing in maximum fair pricing – or MFP – for high-spending drugs over several years. CMS recently announced the next group of 15 drugs that will be subject to these types of price caps in 2028, adding to the 2026 and 2027 drug lists. Starting in 2028, these price caps will apply to both Medicare Part D and Part B drugs, including those purchased through Medicare Advantage. 340B Health Senior Manager of Pharmacy Services Gilda Yeboah says this means hospitals will see reduced 340B savings on certain drugs as Medicare prices move closer to existing 340B ceiling prices. Yeboah says the issue is complex and evolving, and 340B Health is working to share concerns about MFP implementation with federal agencies.Resources340B Health and Allies Urge CMS Contractor To Correct Statement Suggesting Hospitals Must Respond to OPPS Drug Cost SurveyStates Introduce New 340B Legislation in 2026 SessionsMaine Federal Court Rejects Drug Company Challenge to State 340B Contract Pharmacy LawMedicare Expands List of Drugs Subject to Price Caps, Decreased 340B Savings Starting in 2028Manufacturer Notices to Covered EntitiesHRSA Releases 340B Purchase Data for 2024FY 2025 Manufacturer Audit Results

To participate, please go to 340bpodcast.org/survey.January was set to be the first month of a new 340B rebate pilot program, marking a seismic shift in how the drug discount program functions. But a flurry of court activity just before and after the new year put a sudden pause on the rebate program and left the future of the rebate pilot in question. Amanda Nagrotsky, vice president of legal and policy with 340B Health, joins the show to fill us in on the latest.HRSA Pauses Rebate Implementation After Court RulingIn late December, a federal court in Maine issued a ruling that temporarily blocks the Health Resources & Services Administration (HRSA) from moving forward with the 340B rebate pilot. HRSA then announced it was pausing the pilot and instructed drug companies to continue providing 340B pricing through the traditional upfront discount model. Nagrotsky says the judge found that hospitals had demonstrated a high likelihood of irreversible harm if rebates went forward.Government Appeals, Then ReconsidersImmediately after the ruling, HRSA unsuccessfully argued to the Maine court and an appeals court that the agency still should be able to implement the 340B rebate pilot program while it appealed the initial ruling. After the courts denied that request, the government agreed to voluntarily dismiss its appeal and said it would reconsider its rebate program approvals. Nagrotsky stresses that it is unclear what this means. It could mean the government might overhaul or scrap the rebate program, or it might issue revised approval notices to better explain the rebates' purpose and address concerns of potential harm to 340B hospitals.Medicare Maximum Fair Price Provisions Still Take EffectDespite the legal questions surrounding the 340B rebate pilot program, new Medicare maximum fair price (MFP) provisions did take effect at the beginning of January for the drugs that would have been subject to the rebates. Nagrotsky says that while 340B still can be used for these drugs, covered entities are not entitled to MFP refunds from drug companies in those scenarios. She recommends hospitals closely track expected MFP rebates to identify any missing refunds for claims that did not use 340B drugs.ResourcesDismissal of Appeal Sets Stage for HRSA Reconsideration of 340B Rebates
In this episode, Greg and Rob share some initial reactions around the recent judicial activity affecting HRSA's 340B rebate model. They'll discuss thoughts on the federal court ruling that halted the rebate model from going into effect, and also debate what the fallout could be for manufacturers and covered entities given the lack of a regulated deduplication process for MFP and 340B prices. They'll also discuss some of the preliminary steps hospitals might need to take in response to the CMS OPPS Drug Acquisition Cost Survey. It’s a new year, and that means new goals! Cody and Justin share their top tips and tricks to help you set yourself up for success in 2026. The guys are already hitting the ground running with their 2026 goals: Cody is already testing out snowbirding down in Florida Justin is actively recruiting for his first international group trip Don’t forget to share your goals with us in the Facebook Group. To kick off the episode, the guys break down how the methods they’ve used to successfully meet their goals, which include: Tracking is Key “What gets measured, gets managed.” You need to know how you're doing currently to improve For finance, use apps like YNAB, Monarch Money, or a spreadsheet For fitness, use apps like MFP, TrainHeroic, etc. For any goal you want to actually reach, there needs to be tracking Inputs vs. Outputs “I'm going to invest $1,000 every month” vs “I'm going to increase my net worth by $25K” “I'm going to work out 5x per week” vs “I'm going to lose 20 pounds.” Accountability Join online groups or local groups Groups that have people ahead of where you currently are are a big bonus Share milestones/goals with friends Keep talking about it to the point where you'd be embarrassed if you didn't finish Set punishments and rewards for your actions Goal Sizing Goals shouldn't be so big that they paralyze you They also shouldn't be so easy that you hit them without much effort A “good” goal should require quite a bit of effort, but still be possible If you hit your goal too early in the year, you may lose steam (you can always level up your goal, too; it’s just important to know your personality) A / B Goals Many runners/athletes in competition have an A and a B goal, maybe even a C goal. A -> The stars align perfectly, you can see a path to it, you're putting in the work, but it's a reach B -> Fairly confident, little room for error, still can be very proud of the outcome C-> as within your control as possible, room for error, a backstop to avoid just giving up if your more aggressive goal fails Becoming a person who ____ “I'm the type of person who saves 50% of my income.” “I'm the type of person who runs marathons.” “I'm the type of person who volunteers.” Let go of the fear of failure by playing the tape forward If you can look yourself in the mirror and be proud of your effort, what is really the biggest drawback to you trying and failing? Celebrate the wins along the way Maybe your goal is a 10k this year Celebrate your first 5-mile run Celebrate your new 5k PR Cody and Justin’s 2026 Goals! Here’s a sneak peek into the goals we shared publicly in this episode: Cody's Goals: Fewer decisions driven by money Make decisions bc I want to do them (regardless of money) More regular fitness challenges Ironman, Hyrox, 100k steps, etc Spontaneous adventures Always discovering and exploring new places (even at home) Book launch Goal is April/May release, podcast tour, book tour, etc Gold City Ventures Nail down an ad strategy, continue to grow business Real estate Keep running existing properties, only “buying” new properties via syndication. Scaling back a bit feels good Justin's Goals: Run a Marathon in December (CIM) A goal -> sub 3:15 B goal -> sub 3:30 C goal -> sub 4hr New PRs 5k / 10k / half marathon / Deadlift Hyrox Double with Leslie Lead a group fitness-focused trip A goal -> 12+ person international B goal -> any number domestic C goal -> any number in Austin Build Fitness Community Host 6+ in-person fitness meetups Design 12 12-week beginner Hyrox guide Post weekly fostering community This goal is in support of the fitness trip goal Hang out with at least one friend every week we're in Austin Run the greatest number of annual miles of my life A goal -> Avg 30 miles per week post Achilles rehab B goal -> Avg 25 miles per week post Achilles rehab C goal -> Avg 20 miles per week post Achilles rehab Finish House Remodel in Mississippi A Goal – Full remodel, landscaped, short-term rental B Goal – Full remodel, long-term tenant C Goal – Liveable for personal use Complete another amazing multi-month International trip with Leslie Build out Recipes A Goal – 50+ fully documented recipes posted on Substack with social posts B Goal – 50 Recipes Designed, 25 documented and posted on Substack C Goal – 50 Recipes Designed Finish documenting the 2025 trip on Substack We hope you enjoy getting these tried and true methods for hitting your goals. Substance use disorder is one of the most pressing health challenges in rural America. According to the Health Resources & Services Administration (HRSA), nearly 65% of all federally designated primary care shortage areas are in rural communities, and more than 60% of mental health professional shortage areas are rural as well (HRSA, March 31, 2025). These provider shortages are persistent and growing, leaving millions of Americans without timely or reliable access to behavioral health services—even as rural communities continue to experience high rates of substance use and related harms. In this episode of Mental Health Trailblazers: Psychiatric Nurses Speak Up!, Dr. Serge Wandji, MFP at the American Nurses Association alumnus and nurse scientist, shares with host Indrias Kassaye how his early research uncovered the struggles of rural Black men living with addiction. Today, his work expands to underserved communities across the rural South, showing how untreated substance use destabilizes families, harms children, and weakens entire communities.Dr. Wandji discusses why behavioral health must be integrated into primary care, how nurses and behavioral health specialists can apply research to real-world rural settings, and the importance of community-driven, culturally grounded approaches to recovery. According to Dr. Wandji, breaking the silence around substance use in rural America is the first step toward building healthier, more resilient communities.Learn more about Dr. Serge Wandji at https://emfp.org/mfp-fellows/serge-wandjiFor more information on access to care and mental health professional shortages, visit:https://bhw.hrsa.gov/sites/default/files/bureau-health-workforce/state-of-the-primary-care-workforce-report-2024.pdfhttps://data.hrsa.gov/Default/GenerateHPSAQuarterlyReporthttps://data.hrsa.gov/topics/health-workforce/nchwa/ahrfNurses can earn free Continuing Nursing Education (CNE) credits for listening to this episode. About three years after being signed into law, one provision of the Inflation Reduction Act (IRA) that is of particular importance to 340B hospitals is about to take effect: Medicare price caps. Jan. 1, 2026, marks the date that the first 10 Medicare drugs will be subject to a maximum fair price (MFP). Meetali Desai, director of pharmacy business services at UMass Memorial Medical Center, joins us to explain how this will affect 340B hospitals and health centers such as hers. The Good and Bad News for Covered EntitiesThe good news, Desai says, is that there is potential for the 340B ceiling prices to go down for certain medications. This is because the MFP will become the new “best” price in the formula for calculating 340B prices. However, because the law will cap reimbursement rates to MFP when Medicare patients receive those drugs, 340B hospitals will see their 340B savings amounts go down for those dispenses.Updated Calculator Can Help Hospitals Gauge Potential Impact340B Health recently updated its calculator for hospitals to use to estimate the effects of MFP pricing based on the newest data. This new calculator allows users to project what the potential impact from the IRA could be on a hospital, including if drugmakers decide to lower their list prices significantly to avoid IRA inflation penalties. This drop in prices of Medicare drugs could result in 340B hospitals seeing higher ceiling prices and reduced savings.Reduced Savings Could Impact Patient CareDesai says Medicare price caps, combined with other financial pressures on hospitals, could mean some rough times ahead for hospitals that care for a large proportion of low-income patients. She encourages 340B teams to share their results from the IRA calculator with their senior leadership and with 340B Health as the hospital community prepares for the impact of these caps.ResourcesUse Our IRA Calculator and Share How Your 340B Savings Will ChangeRead Our Analysis: Second Federal Court Affirms HRSA's Authority Over 340B Rebates

Other Resources Welcome His Heart: https://welcomehisheart.com/ MFP 165: Forgiveness Brings Freedom and Transformation Videos - Evangelizing Your Kids and Forgiveness in the Family https://messyfamilyproject.org/course/pizza-and-parenting/watch/ Key Takeaways Parents must be very intentional about evangelizing their children and bringing them into a relationship with Christ. Marriage is a sacrament of service to the world. Our marriages are not just for us, they are an office of service. Every mother and father is to act as “priests of the home” and take up the spiritual authority God has given them for the sake of their children. Forgiveness is key to restoring relationships that have been broken and can teach children powerful lessons on relationships. In order to transform your family you will need to start with these three things - create habits to support your goals, make Jesus the king of your home, and build community to support your family. Couple Discussion Questions How are we leading together in the home? Have we taken up that call to spiritual leadership? What is my understanding of forgiveness? How can we live this out better within our home and our relationships? How is our marriage serving others? How are we giving from the well of our sacrament? What are 2 resolutions that we can make today to start living a transformed family life?
