The rise of Martin Vitík. Martin Vitík is a Czech professional footballer who plays as a defender for Sparta Prague and the Czech Republic national team.
Hemorrhagic disease of the newborn (HDNB) was first identified over a century ago, and presents as unexpected bleeding, often with gastrointestinal hemorrhage, ecchymosis and, in many cases, intracranial hemorrhage. In newborns, HDNB is typically caused by vitamin K deficiency as neonates are innately deficient in vitamin K secondary to very little vitamin K transferred through the placenta to fetuses in utero, limited liver storage of vitamin K, and low amounts of vitamin K in breast milk. IM administration of vitamin K for prevention of vitamin K deficiency bleeding (VKDB) has been a standard of care since the American Academy of Pediatrics recommended it in 1961. Despite the success of prevention of VKDB with vitamin K administration, the incidence of VKDB appears to be on the rise. This increase in incidence of VKDB is attributable to parental refusal as well as lowered efficacy of alternate methods of administration. Can parents decline this injection for their babies? In this episode, we will review IM Vit K neonatal administration and discuss the controversial data regarding Vit K oral supplementation.

Emergencies happen in hematology and oncology. This is a fact. But how do we manage these emergencies? Look no further. In this episode, we talk all about our second hematologic emergency: disseminated intravascular coagulation (DIC) with an added bonus of an intro to thrombotic microangiopathic anemias (TMAs).Be sure to check out episode 009 on thrombocytopenia for a general approach and differential!Disseminated intravascular coagulation (DIC):Workup: CBCCMPPT, PTT, INRFibrinogenPeripheral smear - concern for schistocytes. Example of these cells from ASH image bank: https://imagebank.hematology.org/image/60306/schistocytes?type=upload#:~:text=A%20schistocyte%20is%20present%20in,angles%20and%2For%20straight%20borders.Basic mechanism of DIC is consumption of clotting factors leading to coagulopathy Need to be weary of thrombotic microangiopathy: Small blood clots forming in the small vessels leading to endothelial damage, which cause shear stress on the RBCs, which then break down into a schistocyte (AKA triangulocyte or helmet cell) Examples: thrombotic thrombocytopenic purpura (TTP) and hemolytic uremic syndrome (HUS)Management (our opinion!): - Repeat coags q4-6 hours initially (but base interval based on patient) NOTE: INR Is NOT a good assessment of “clotting status” in these situations- Repeat fibrinogen q4-6 hours initially (but base interval based on patient); keep fibrinogen >100 with cryoprecipitate in more stable patients; consider higher thresholds for more acutely ill patients (such as >150) - Repeat CBC q6-8 hours initially; can provide platelets if low, especially if they are bleeding - Workup and treatment for trigger of DIC (infection, trauma, medications, etc.)How does cirrhosis affect data interpretation?- Use clinical context to determine if labs are acutely abnormal or if they have signs/symptoms to suggest underlying liver dysfunction- In the acute setting, always just replace what is missing! How can you tell the difference between nutritional deficiencies vs. consumption (as in with DIC?)- Factor activity levels! My Vit D levels with PCP won't go much above 33. (I had gone from 5 drops to 7 with no change, so settled in on roughly 35units/lbs.) I have heard others say oil based capsules are better than drops for accurate dosing. Do you recommend drops because there is some level of sublingual capillary absorption? If so, do I need to take the DNS a couple hours before Vit D so the cofactors are already in my system, ready to work? Bu kontrol BBT ise travmadan 6-24 saat sonrasında çekilmeli. - Yazarların yorumlarının yorumu: Yazarlar önerilerini kesin bir dille ifade etmiş olsalar da bu sorunun cevabını literatürde bulmanın mümkün olmadığını peşinen belirtmişler. Sonrasında ise özellikle kontrol BBT’nin zamanlamasına karar verme açısından hastanın kliniğini takip etmenin akılcı olduğunu belirtmişler. - Klinik Soru:Peki ilk BBT’si normal hastayı nörolojik takip açısından yatırmalı mıyız? Yatırılacaksa ne kadar süre için takip yapılacak ve hasta monitörizasyonu nasıl yapılacak? - Cevap: Tüm hastalar ilk BBT’si normal olsalar da en az 24 saat nörolojik gözlem için yatırılmalı. Bu hastalarda kontrol BBT ancak nörolojik bozulma (GKS değişimi, pupil yanıt değişimi vb) olan vakalar için endikedir. Nörolojik izlem ilk 4 saatte saat başı, sonraki 8 saatte iki saatte bir ve sonraki 12 saat için 6 saatte bir olacak şekilde yapılmalıdır. - Yazarların yorumlarının yorumu: Literatürde ilk BBT normal olmasına rağmen gecikmiş kanama oranı %0.2 ile %6 arasında raporlanmıştır. Bu nedenle bu hastaların ilk BBT’si normal olsa bile güvenle taburcu edebilmek mümkün değildir. Ancak bu kanamaların bir kısmı günler ve haftalar sonra da görülebilmektedir. Bu durumda ilk 24 saat takip edilseler bile tamamen güvenle taburculuktan söz etmek zor gibi durmakta. Yine de yazarlar önemli bir kısmın ilk 24 saatte görüldüğünü belirtip ilk 24 saat sonrasında taburcu edilebileceğinden bahsetmişler. Ülkemiz yoğun acilleri düşünüldüğünde bu 24 saati de fazla olduğunu düşünenler olabilir tabi :) .

Hey Weight Loss Nation! www.TheWeightLossNation.com ARE BRUSSELS SPROUTS FROM BRUSSELS? Brussels Sprout are part of the “wild cabbage” & “mustard seed” family. According to the “New World Encyclopedia,” Brussels Sprout were likely cultivated in Ancient Rome and possibly as early as 1200 B.C. in Belgium’s Capital City - Brussels. So.....YES....Brussels Sprout comes from Brussels, Belgium! There is written documentation of Brussels Sprout as early as 1587, being used as a vegetable crop and it is believed Brussels Sprout originated from a mutation from the “Savoy Cabbage.” The vegetable grows as a “Stem,” which the “sprouts” grow into a “cabbage-like” head. Many rows of sprouts are produced on a single…..long…..stem. The leafy, green buds look like miniature “cabbages.” There are two main varieties that are grown. One is the “tall” variety, which stands 2-4 feet tall…..and the second is the “short” variety, which grows to a maximum of 2 feet tall. Brussels Sprout like “cool” temperatures and produces best at between 60-65 Degrees Fahrenheit. Fields are ready for “harvest,” between 90 days to 6 months. Depending on the variety, a Brussels Sprout “stalk,” can product 1 ½ - 3 pounds of Brussels Sprout per Stalk! That’s a lot of Brussels Sprout! Brussels Sprout were brought to the U.S, around 1800, when French Settlers brought them to Louisiana. Commercial production began in the Louisiana Delta around 1925, and production was moved to mid-California in 1939. Some of the production was also moved to New York in the same year. Much of the U.S. production of Brussels Sprout is in California, with a smaller percentage grown in Skagit Valley, Washington, where cool springs, mild summers, and rich soil is perfect for the Brussels Sprout. I research fruits, vegetables and grains online…..just like anyone would do. Therefore…..the information I am providing you is INFORMATIONAL and for your entertainment only. With that said…..let’s learn some things about Blueberries! BLUEBERRIES THE KING OF ANTI-OXIDANTS! HISTORY Blueberries are “Native” to North America. We finally have a GREAT Food Representing US! In early America, The Native Americans recognized the health benefits of blueberries using them for medicinal purposes and as flavoring in their foods. It wasn’t until 1910, that Frederick Coville, an American Botanist, discovered blueberries could only thrive in “acidic soil.” In 1911, Elizabeth White, daughter of a New Jersey cranberry farmer read Coville’s Experiments in Blueberries….and invited Coville to use the family farm to partner with her in his experiments in blueberries. In 1916 White & Coville harvested & sold the FIRST COMMERCIAL CROP of “highbush” blueberries in Whitesbog, NJ. “High Bush” means it grows tall!......Blueberries grow on bushes that reach a height…..of about 6 to 8 feet. They also produce a larger amount of berries on the bush. By contrast….”Low Bush” Blueberries…a.k.a. “wild blueberries” grow close to the ground….being no taller than 2 feet high. The berries are smaller and “sweeter” than “High Bush” Low Bush berries are very “hardy” plants, and do well in different types of soil and climate. Between the years 1942 – 1962, “Blueberry Fever” swept the nation and more than 200,000 seedlings were spread across 13 states! By 2014….Blueberry Production across 5 Continents rose to over 1 Billion Pounds! That’s a LOT of Blueberries! Within the United States….the top Four States that produce “highbush” variety of blueberries are - Michigan, Washington, Georgia and Oregon. 60% of the “High Bush” blueberries grown in those States are sold as “Fresh Blueberries” just like you buy a “pint of blueberries” in the store ….and the other 40% are converted into juices, jams or dried fruit. The State which produces the largest amount of “low bush” blueberries is….the State of Maine. More than a dozen U.S, States grow blueberries commercially, and all together, they produce over 550 million pounds of blueberries! Even though the U.S. grows an enormous amount of blueberries, the majority of blueberries consumed in the U.S. is imported! Chile and Canada supply the U.S. with the majority of imported blueberries. BENEFITS OF BLUEBERRIES I’ve stated several times on this show…….dark colored fruits and vegetables get their color due to the Flavonoid called “Anthocyanin” Flavonoids provide the powerful anti-oxidants and anti-inflammatory properties and ……they produce the vibrant, dark color in foods. That means that Blueberries…..are part of our “Pac Man” Family! No surprise there Nation! Flavonoids like….”Flavonois” and “Flavan-3-ols” are highly effective in “searching, Fighting and help in Destroying Free Radical Cells” in your body, that cause cancers. Blueberries also contain Vitamin C, Vitamin A and with the powerful Flavonoids – they help decrease inflammation in the body and help inhibit the growth of cancer cells related to esophageal, lung, mouth, endometrial, pancreatic, prostate and Colon Cancers. Blueberries also contain Folate. Folate plays a role in DNA Synthesis and repair. This can help prevent the forming of cancer cells due to mutations in DNA. Blueberries also contain iron, phosphorous, calcum, magnesium, manganese, zinc and Vit K. What a friggin nutrient-rich Fruit! You Go Blueberry! All of those vitamins and minerals are components of bones. Iron & Zinc play Crucial roles in maintaining bone strength and elasticity of joints. When you have a good intake of Vitamin K you will improve calcium absorption which may help reduce calcium loss or breaking of bones. Blueberries have a HUGE amount of Vit C. One Cup of Blueberries provides 24% of the recommended daily allowance of Vitamin C. Vit C also is an essential nutrient and works to prevent skin damage caused by the sun. Flavonoids have a unique relationship with Vit C, by helping “transport” Vit C throughout your body. Flavonoids also help regulate the function of an enzyme called “Ascorbate Oxidase” which converts Vit C into a NON Vitamin form. I have no idea why that is important to the cells in your body…..but it is….so eating foods rich in Vit C and Flavonoids is important! Blueberries also have a HIGH amount of FIBER. One Cup of Blueberries has 3.6 grams of Fiber! Studies have shown that Type 1 Diabetics who eat high amounts of Fiber, have lower low blood glucose levels and Type 2 Diabetics who eat high amounts of Fiber may also improve their fat and insulin levels. HUGE benefit for Diabetics! Fiber also will help to prevent CONSTIPATION. Fiber helps maintain regularity in the digestive tract. Fiber functions as a “bulking agent” in the Digestive System. High Fiber also makes you feel full! When you feel full ……you don’t EAT as much! That automatically reduces your Calorie intake. Blueberries also have Vit B6 and Folate which helps prevent the buildup of the compound known as “Homocysteine.” Excessive buildup of Homocysteine in the body can damage the blood vessels and lead to heart problems. Blueberries also contain the phytonutrient called “Resveratrol” Sound Familiar……of course it does! There have been lots of publicized research on RED wine, which contains Resveratrol. The Resveratrol is found in the skin of red grapes…..but….Resveratrol is also found in BLUEBERRIES! The Resveratrol is found in smaller amounts in Blueberries, but HEY……Resveratrol is an anti-oxidant and anti-inflammatory phytonutrient! Blueberries also help nervous system responses, by protecting nerve cells from “oxidative” damage with those FREE Radical Cells. There is about 14 gms of sugar. 24% of Vit C 5 % of Vit B6 36 % of Vit K 9mg of Calcium .5 mg of iron 114 mg. of Magnesium 18 mg of Phosphorus 0.25 mg of Zinc and 9 mg of Folate. Blueberries also contain copper, Beta-Carotene, choline, Vit A, Vit E And…..Manganese. Blueberries are definitely the King of Anti-Oxidants !! Hey Weight Loss Nation! www.TheWeightLossNation.com I hope you are enjoying your summer! One of my favorite fruits that is peaking right now, are "raspberries." This high fiber, anti-oxidant rich fruit, kicks ass! Raspberries are in the Rosaceae family, which means they are related to roses. I had no idea the raspberry was related to roses! The Raspberry's classified name is "Rubus idaeus" In Greek mythology, Zeus enjoyed raspberries, which were "white" in color. Zeus's nursemaid was named "Ida." According to legend, "Ida" was picking some raspberries for Zeus and she pricked her finger on a thorn. Her blood stained the white raspberries, turning them red. In honor of "Ida's" passion for the raspberries, Zeus kept the raspberries red and they have remained that way ever since. Americans most commonly recognize Raspberries in two different colors - Red and Black. Raspberries do come in other colors, such as purple and yellow, and come in almost 200 variations. The State of Washington leads the U.S. in Raspberry production, producing over 70 million tons annually. Humans have been using Raspberries for medicinal purposes over thousands of years. The leaves of the raspberry bush contain a property called "fragirine," which tone the female reproductive system. Drinking one to two cups a day of "raspberry tea," will help Menstrual flow and cramps by regulating the uterine muscles. "Raspberry tea" also helps with bladder control in adults and children. A cup of raspberry tea before or with dinner is a perfect time to help with bladder control. Having any liquids later in the evening will only trigger you bladder to empty. If you want to help a younger child control their bladder overnight, try to eliminate any fluids after 6:00pm. "Fragirine" also helps regulate the walls of the intestine. By regulating the intestinal wall, people with digestive issues will benefit from drinking "raspberry tea." Raspberries contain "anthocyanins," which is the pigment that gives plants their purple, blue and red color. Anthocyanins are part of my "Pac Man" family, which means it is search, fight and destroy anti-oxidant. Killing free radical cells and offering anti-inflammatory agents, Anthocyanins are a must in helping your immune system and YOU healthy! Raspberry's also contain "Polyphenols" which are "Flavinoids," even more anti-oxidants and anti-inflammatory agents for YOUR body! The potent Polyphenols will help your white blood cells "eat up" many types of cancer cells, which can slow down or even eliminate some tumors. Polyphenols have shown to have an effect on endometrial cancer cells as well as lung, esophageal, prostrate and colon cancer cells too! Raspberries are also high in FIBER! Fiber helps keep blood sugar stable. If you are a type 2 diabetic, you can reduce blood glucose levels, by eating a cup of raspberries 2-3 times a week. Raspberries also improve insulin levels in your blood. One of the best reasons of eating fiber, is that fiber keeps you "regular." Fiber helps relieve constipation. "Fiber" has also been associated with lowering blood pressure and cholesterol levels. So.....increase your fiber intake and reap the benefits of raspberries! Let's look at the Nutritional Value of Raspberries: Serving Size - 1 Cup of Raspberries 64c 1.5gms Protein 0.8gms Fat 15gms Carbs 8gms Fiber - That's the right figure! 5gms Sugar 1 Cup of Raspberries will yield 55% of your daily intake of Vit C, 41% your daily intake of Maganese and 12% of your daily intake of Vit K. Eating 3 Cups a week of fresh Raspberries will help you attain the most benefits. The best way to eat raspberries? Wash them and eat them right out of your hand! If you aren't fond of that way, you can sweeten up Greek Yogurt. 1 Cup of Plain Greek Yogurt 1 Cup of fresh raspberries 1/4 cup of sliced almonds 1 tsp of raw, unfiltered local honey 1 tsp of real vanilla extract Mix all together and enjoy! You can also add raspberries to your salad or... even put them in your blender with 1/4 cup of olive oil and 1/4 cup of apple cider vinegar for an awesome salad dressing! I use it for several reasons. 1) It seems to be relatively safe with no side effects 2) Ill take anything that helps recovery 3) I currently do lower body work and core work in the gym, and i believe creatine helps me build those muscles However, creatine also causes me to carry extra water weight. I have heard that creatine does this, and I have noticed it myself. I carry a substantial amount of water weight around my midsection. After a long run where I sweat alot, I can see my abs, but if I take creatine and go a day or so without sweating, there is alot of bloating in my abs. I am worried that during taper this will have a negative effect. When I rest for my marathon, should I stop taking creatine? Or do I need to stop a few weeks in advance to get it out of my system. I want to get the benefits of it without carrying extra weight during the race. Also, will there be any problems with stopping taking it, like withdrawal symptoms? I currently take about 5g after workotus and 3g on non workout days in the morning." A month before IMC I was diagnosed with a stress fracture on both of my tibias, I cut back running for the last month and pushed myself through the run on race day.
