On Tuesday the Senate Finance Committee voted to move forward with Robert F Kennedy Jr.’s nomination for Secretary of Health and Human Services. His nomination now heads to the Senate floor, where he’s expected to be confirmed. As the secretary of the HHS, Kennedy would have oversight over 11 agencies including the FDA, the CDC and the National Institutes of Health. Kennedy has come under fire from both Democrats and Republicans for some of his controversial beliefs – the most contentious being his extensive history of anti-vaccine work and rhetoric. Kennedy has in particular focused on Gardasil, the HPV vaccine. A lawyer by trade, he has helped facilitate hundreds of potential lawsuits aimed at pharmaceutical company Merck, which manufactures the drug. The vaccine is nearly 100% effective against cancers caused by the human papillomavirus – most notably cervical cancer. Kennedy has criticized the vaccine, calling it ‘dangerous and defective’ and claiming it can ‘increase the risk of cervical cancer.’ Guest: Dr. Linda Eckert, a professor in the University of Washington School of Medicine, Women's Health Division of the Department of Obstetrics and Gynecology. Columbia Gorge News recently reported on the meeting amid the yearslong effort by the Oregon Occupational Safety and Health Division to update agricultural labor housing rules, some of which haven’t changed in nearly four decades.  Also in April, a coalition of labor unions, racial justice and farmworker advocacy groups sent a letter to Gov. Lyse Brillouet has been the Chief Intellectual Property Officer and Senior Vice President of Licensing for the Orange Group since 2018. She came to IP with a deep and cross-functional capability background in strategy, innovation, and business enablement. Her specific skills include managing critical business disruption projects and staging complex projects to implement high-value digital transformations. She held various positions in business and innovation within the Orange Group, such as Head of the Digital Society Research Program and Head of Strategy & Marketing for the e-Health Division. In the podcast, Lyse describes why IP matters within the Orange Corporation, which has over 130,000 employees worldwide. IP for Orange is an important outcome of various research and development projects. Lyse explains that Orange is a technology company that aims to bring technology and related services to its customers. With that objective, Lyse also leads her IP team, where, in some cases, she must balance the monetization of IP with promoting a new standardized technology. A good example she provides here is Orange's strategy towards patent pools. Orange is very much in favor of patent pools, and Lyse believes it's the most efficient way to license SEPs. In fact, as to the LexisNexis® IPlytics data, no other SEP owner joins as many patent pools as Orange. Lyse provides even more detail here, explaining that Orange is not only a pool member but often even the founder of various patent pools. Lyse elaborates that patent pools are, however, joined by companies with different objectives and that there is always a trade-off about how much royalty should be paid by a patent pool compared to promoting and ensuring the broad adoption of a technology standard. In conclusion, pools not only have the function of creating license revenues but are also tools to provide one-stop market access. As to the LexisNexis® IPlytics data on submitted 5G standards contributions, Orange ranks in the top 20. However, the Orange 5G patent portfolio is ranked much lower. Lyse explains that the contribution of the standards working groups for Orange standards development is about something other than IP or filing SEPs. It's much more about creating widely used and accepted standards. SEPs are an organic outcome of that process. Also, Lyse explains that at Orange, they declared only those patents in the public ETSI IPR databases that are verified essential, which may make the Orange SEP portfolio look much smaller than the over-declared SEP portfolio of some other companies in a 5G ranking.Lyse believes that standards will be adopted by an increasing range of industries, going beyond automotive to the IoT. Orange has been here since last year and cooperates and provides technology to those industries, and she is optimistic that the SEP licensing challenges will also be resolved there. Finally, Lyse comments on the latest EU Commission proposal for a SEP regulation. Lyse feels that setting up a new SEP register or database for existing standards needs to be clarified for her. If at all necessary, a new register of SEPs is essential for future standards. She feels that anything else would not be feasible and too costly for her team, and she would expect a negligible effect on her SEP licensing success.

Today, panelists, Ashley Frost and James Maloney, Deputy Directors, Office of HIV/AIDS USAID, discussed USAID's localization initiative, locally-led development, and the importance of diversified local partners. They discuss USAID's Global Health work, how USAID supported PEPFAR and the successes it has enjoyed, including statistics of lives saved and changed.   James Maloney and Ashley Frost share the new PEPFAR five-year strategy, the USAID Approach to HIV and Optimized Programming (AHOP). AHOP is a framework for USAID to employ assets through efficient, measured, and simplified programmatic approaches to ensure resilient and country-led approaches to the HIV response. The AHOP defines five critical pathways through which USAID will assist countries in sustaining epidemic control and accelerating progress in those that have not yet achieved the 95-95-95 goals. Each pathway outlines planned results and time-bound milestones. If this resonates with you, share successes with Congress to fund the five-year reauthorization of PEPFAR.   IN THIS EPISODE: [00:32] Mike Shanley outlines key talking points regarding the accomplishments that PEPFAR has achieved over the last 20 years, delivering unprecedented impact in the global fight against HIV AIDS.  [02:57] Ashley Frost and James Maloney describe their roles at USAID as Deputy Directors.  [04:40] Discussion where funding originates, the investments they make at the primary healthcare level, and successes they have accomplished through PEPFAR, stating that USAID is the largest donor and they are advancing work in the HIV vaccine space.  [11:00] James shares statistics of the impact made in the world attributed to PEPFAR and reflects on the history of growth and prevention.  [14:50] Discussion of localization, local entities and partners and relationships within country-led staff and senior diplomats in their countries.         [20:06] Looking forward to goals and objectives, James discusses the AHOP framework in response to HIV.  [25:24] Ashley comments on the importance of keeping an open dialogue between all partners within the HIV AIDS space, and James outlines the importance of seeing the 5-year reauthorization of PEPFAR.    KEY TAKEAWAYS:  Oftentimes, hospitals and building new infrastructure is what gets a “ribbon cutting”, but in terms of buying the best health outcome with USAID's investments from U.S. taxpayers, the first area of importance is investments made at a primary care level, which will most impact healthcare outcomes, saving lives and supporting healthy lives. The second thing that drives a lot of USAID's investments across individual disease or health areas is health security. Much of PEPFAR's success lies in engaging with local partners. With regard to localization, the Office of HIV AIDS and the Global Health Bureau have been the front runners in the agency and have been able to guide our agency in these spaces. Right now, about 58 percent of our resources are implemented through local entities, and those sets of partners have been diversified. Small business engagement in the U.S. is essential and has much to offer to the work we can accomplish together. More significant international partners have specific skill sets that are necessary for USAID. As we think about localization, everyone needs to think outside the box and remember that one size does not fit all.  By the end of 2025, the vision is to help countries reach what the USAID calls the global commitment of 95-95-95 goals. That means that 95 percent of those who have HIV know their status. 95 percent of those individuals are on treatment, and 95 percent of those individuals would be virally suppressed. The AHOP defines five critical pathways through which USAID will assist countries in attaining these goals.  RESOURCES: Aid Market Podcast Aid Market Podcast YouTube Mike Shanley - LinkedIn James Maloney - LinkedIn USAID Approach to HIV and Optimized Programming   BIOGRAPHIES:  James Maloney has served as the Deputy Director for the Office of HIV/AIDS (OHA) within the Global Health Bureau at the United States Agency for International Development (USAID) since March 2020. Sexually transmitted infections (STIs) on college campuses pose a significant public health concern, affecting the well-being of students. The unique social dynamics of college life, characterized by increased sexual activity and a transient environment, contribute to the spread of STIs. Despite efforts to promote safe sex practices, many students engage in risky behaviors, such as unprotected intercourse and multiple sexual partners. Limited access to sexual health resources, stigma, and a lack of comprehensive sex education further exacerbate the issue. Colleges must prioritize initiatives that promote awareness, education, and easy access to testing and protection methods to mitigate the spread of STIs and safeguard the overall health of their student populations. Just in time for National Sleep Awareness Week, World Sleep Day, and the change to Daylight Saving Time (March 12), Carlos M. Nunez, M.D., visits with Mark Allyn on Late Night Health. Dr. Nunez talks about the importance of quality sleep on mental and physical health, including why the American Heart Association recently added sleep as a metric for cardiovascular health. Learn what's keeping people up at night, what symptoms of sleep deprivation 4 in 5 people say they have – despite most people saying they're happy with their sleep, and what a “sleep-sumer” is. Feeling bad the next day after a full night's sleep is also an indicator of a sleep disorder like insomnia – or sleep apnea, which an estimated 54 million Americans have. The lack of deep sleep can cause significant fatigue and elevate the risk for serious health problems and alarmingly, most people with sleep apnea are unaware they have it. Dr. Nunez can discuss how to easily identify whether a person might have a sleep disorder along with remedies to help get a better night's sleep, as well as common misconceptions around sleep health and sleep disorders. Carlos M. Nunez, M.D., is ResMed's chief medical officer and the chair of the Consumer Technology Association (CTA)'s Health Division. Before working in the digital health and medtech industry, Carlos was a practicing anesthesiologist and director of critical care research, informatics, and analytics at the Duke University educational affiliation at Carolinas Medical Center Northeast. Dr. Helen Morgan is the Director of Residency Preparation Courses at the University of Michigan. Together, Drs. Hammoud and Morgan are leading the $1.7 million American Medical Association (AMA) Reimagining Residency Grant for Association of Professors of Gynecology and Obstetrics (APGO) Right Resident, Right Program, Ready Day One. The episode begins with Drs. Hammoud and Morgan sharing a call to action regarding the need to transition the emphasis of applying to residency from the lens of individual achievements to that of an individual's competency as a future physician. The two physicians share what inspired them to contribute to making changes in the realm of OB/GYN, including personal experiences from advising medical students at the University of Michigan to their own family members. During this time, they address pitfalls of the current system, including the financial expenses for applications. The group then transitions to describe the 5-year grant that led to the development of Right Resident, Right Program, Ready Day One. The program addresses 2 components: (1) the application process itself, and (2) students' readiness for residency. The physicians cover the topic of “application fever” (i.e., the large increase in the number of applications that each student submits), and how the field of OB/GYN has adopted “program signaling” with the hope to mitigate this ongoing trend. Drs. Hammoud and Morgan also address topics such as growth mindset, holistic review of applications, importance of coaching for preparation for residency, and the increasing popularity of OB/GYN as a specialty. Lastly, the group highlights other aspects of the residency application process, including methods to ensure an equitable review process, the need to increase diversity in the field, the standardized letter of evaluation (SLOE), and best practices to facilitate transition to residency. During this conversation, Drs. Hammoud and Morgan share about the residency preparation courses that are available for medical students at the University of Michigan.

While the CDC acknowledges the duration of the flu season varies, flu activity commonly begins in October and peaks in the winter between December and February. According to the CDC, since the onset of the COVID-19 pandemic, the duration and timing of flu activity has become less predictable. The World Health Organization (WHO) mentions treatments for the flu exist and range from those focused on relieving symptoms to antiviral drugs for those in a high-risk group. While colleges and universities are comprised of several shared spaces, prevention of the flu is possible through social distancing, washing your hands, and staying home when you are sick, among other steps to protect yourself and others from influenza.Learn more about CITI Program: https://about.citiprogram.org/

The virus often shares symptoms like those in smallpox patients, however, less clinically severe according to the World Health Organization (WHO). The WHO reports the first human case of monkeypox was recorded in 1970 and is a disease of global public health importance. The first case of monkeypox was reported in the United States in 2003. The CDC is currently tracking an outbreak of monkeypox in several countries, including the U.S., which does not normally report the virus. Colleges and universities in the U.S. have started to report cases across the country according to an August 2022 report from National Public Radio (NPR). Back in the host's chair this week, Bob Hughes welcomes Dr. Bill Evans, one of the world's foremost experts on muscle aging, to the podcast. Bill is adjunct professor of Human Nutrition at University of California Berkeley and an adjunct professor of medicine in the Geriatrics Program at Duke. Previously, he was vice president and head of Muscle Metabolism Discovery Performance Unit at GlaxoSmithKline and he was also president of the Muscle and Health Division at KineMed. He was also president of the Muscle and Health Division at KineMed. Earlier this year, he was recognized with a Lifetime Achievement Award at the 2022 International Conference on Frailty, and Sarcopenia Research. Today, Bill brings his vast amount of experience and expertise to the podcast to discuss how muscle aging affects longevity in older people and the relationship between muscle aging and age-related diseases. He begins by sharing his experience in the longevity industry, particularly with muscle aging, and goes on to discuss the term ‘sarcopenia', including what it means and how it relates to muscle degeneration. He then explains the differences between sarcopenia and cachexia, and referring to several studies, shares the meaning of frailty, the relation of walking speed with age, and the importance of the brain–muscle connection. At the conclusion of the episode, Bill discusses the future of muscle aging and how the longevity industry hopes to find solutions that will improve the lives of people on a global scale. Episode Highlights: Bill's journey in the muscle aging industry Why muscle degeneration? The origin of the term ‘sarcopenia' The differences between sarcopenia and cachexia Is sarcopenia due to dysregulation of neuronal inputs? The contribution of fast-twitch and slow-twitch muscle fibers in aging Sarcopenia and age-related diseases What is frailty? Why our walking speed slows down as we age The relation between exercise and psychological state The brain-muscle connection The future of the longevity industry from Bill's viewpoint Quotes: “What are some of the causes of late life disability? How does muscle change as we grow older? Why do we lose muscle?” “A large percentage of women in particular over the age of 60 reported that they couldn't even lift 10 pounds. And the muscle weakness progressed as they grew older.” “The primary deficit and functional deficit as we grow older is loss of strength. And that is directly related to how much muscle we have.” “Cachexia is associated with a rapid increase in the breakdown of muscle, while sarcopenia is associated with a more gradual decrease in the rate of synthesis of muscle.” “People generally over the age of 75, have circulating markers of inflammation.” “People with type two diabetes and insulin resistance lose muscle at almost double the rate of people with normal glucose tolerance.” “If we improve strength in an older person, their spontaneous activity goes up. An international perspective with the OECD on utilising RWD for healthcare system benchmarking and information systems

In this second episode of season 2 we discuss an international perspective of using RWD to evaluate country healthcare systems and their information systems with Jillian Oderkirk, Senior Economist, Health Division at the Organisation for Economic Cooperation and Development (OECD), an intergovernmental economic organisation with 38 member countries founded in 1961. Jillian provides a unique perspective from her global position on the health agenda of the OECD, and in particular the need to utilise data at scale to better understand healthcare systems, their impact at a national and international level with regards to quality evaluation, while assisting governments to improve their healthcare systems. Jillian has very relevant, prior experience in Canada, and with the OECD since 2010 in working with governments on assessing how they are transitioning from 20th century models of care provision and systems implementation to a 21st century, data-driven, learning healthcare system goal. In the conversation, we explore some of the more advanced countries, such as France, Finland, Denmark and South Korea with regards to realising this goal, as well as some deeper insights into countries like the Netherlands in OECD's most recent audit report (link). The COVID-19 pandemic has acted as a lens on the problems everyone has with ensuring there is health data and information that is accessible and interoperable, with analytical interoperability, but also a catalyst for governments to prioritise improvements and policy responses to the persistent information deficit. Within the discussion there is less emphasis on technical and methodological aspects of working with data, but more about governance, control and protection of health data, challenged by more nihilistic approaches to not utilising it in research to avoid perceived risks. Political and technocratic hurdles issues, such as parochial planning at local level, and the emphasis on reinventing versus copying best practice illustrate how the OECD is responding to this, such as with their 2016 recommendations on data governance (link). Fitness for purpose of information systems remains a key challenge, albeit with some pockets of improvement. In the latter aspect of the episode we discuss future trends, and indeed hope for the 21st century goal Jillian has, demonstrated in programmes like EHDEN, and global open science collaborations such as OHDSI, based on transparency, reproducibility, collaboration and verifiable data interoperability. The views expressed by the participants are personal and not necessarily reflective of their organisations.

For episode nine, we speak to Saskatoon-based physiotherapist, Juliet Sarjeant. Juliet has been specializing in pelvic health treatment through her own physiotherapy practice, as well as at Saskatoon City Hospital, for almost two decades. She has undertaken a great deal of continuing education in pelvic health and pelvic pain, and she has previously been the chair of the Women's Health Division of the Canadian Physiotherapy Association.During the episode, Juliet starts us off with a “Pain 101” – she breaks down the differences between acute versus chronic pain to provide a strong educational overview. With this, Juliet also introduces listeners to “pelvic pain” and the various systems involved that may be affected by the pelvic floor muscles (such as the bladder, bowels, and sexual organs). Juliet explains the role physiotherapy has in pelvic health to combat chronic or actuate pelvic pain. She reinforces that YOU are the expert on your body, and the role a patient has in educating themselves and putting in the work to help overcome the pain they are dealing with. She also touches on the fear cycle, and the power of visualization and graded exposure therapy in overcoming chronic pain.  We learn throughout the episode that the pelvis, or pelvic pain in general, is not given a great deal of attention due to the sensitive nature of the area, and how there is often a misunderstanding by care providers not specialized in pelvic health about the variety of health improvements patients can have if they attend to their pelvic floor health. Throughout the course of the COVID-19 pandemic, multiple variants of concern have become the dominant strain of the virus, such as Delta and Omicron. According to the Centers for Disease Control and Prevention, the Omicron variant is likely to spread more easily than the original strain of SARS-CoV-2. More data is needed on the Omicron variant to form a better understanding of how it compared to the Delta variant, vaccine protection, spread of the variant, and the severity of illness caused by the variant.Learn more about CITI Program: https://about.citiprogram.org/To learn more about our guest speaker visit: Philip Chan, MD, MS | CITI Program

This week on the Minnesota Native News Health Report, emergency medical teams from the federal government are being sent to the state to support hospitals during the current COVID-19 surge. Also, Dr. Antony Stately with the Native American Community Clinic talks about vaccines for younger children. Plus, an award has been given to two tribal health clinics in northern Minnesota. Here's reporter Cole Premo.   Gov. Tim Walz announced that the federal government accepted his “urgent request” for emergency medical staffing assistance, and now two Department of Defense medical teams are on the way.   The emergency staffing teams, with 22 medical personnel each, will be supporting staff at Hennepin Healthcare in Minneapolis, also known as HCMC, and St. Cloud Hospital. They're expected to arrive the week of Nov. 22.   In Minnesota hospitals as of Nov. 16, there are 1,382 patients being hospitalized for COVID-19, which is the highest figure in 2021. Only 47 intensive care unit beds remain available for Minnesota patients.  Health officials say Minnesotans can support hospitals by getting vaccinated, wearing a mask in indoor public settings, testing as appropriate and social distancing.   Meanwhile, the Food and Drug administration has officially expanded its emergency authorization of Pfizer's and Moderna's COVID-19 vaccine booster shots to all adults. The Centers for Disease Control and Prevention will need to issue recommendations before its officially greenlit, but many states, including Minnesota, are preparing to give out the shots soon.   ----  In other news, COVID-19 vaccine doses are now available for children ages 5 to 11 years old.   In COVID-19 Community Conversations, Antony Stately, PhD, talked about the importance of vaccination and how the virus can spread through families.   STATELY: 5:18-5:46: I just think folks are just beginning to see the value of having your child vaccinated, so we can all enjoy being close to each other, and not have to repeat what happened last year.   STATELY: 6:36-7:50 I think it's really important that your children are going into schools, they're playing in extra curricular activities. They're petri dishes of infection anyways, even before covid. I think it's important to recognize the danger. Maybe they will be fine, but they could bring it home to a family member. Immunocompromised people are very vulnerable”   Check your local clinic or tribal health clinic for vaccines for more information on pediatric vaccinations. You can also go to mn.gov/vaccine and use the vaccine locator map.   ----   Lastly, two northern Minnesota tribal health systems have received awards for their vaccination efforts.   Recently, the Minnesota Department of Health announced that the 2021 MN Rural Health Team Award has been given to Cass Lake Indian Health Services and Leech Lake Band of Ojibwe, Health Division.   The health department says from January to April of this year, both health systems not only developed and implemented a mass immunization model for eligible Native Americans, but the rural community at large.   In 15 mass vaccine events, over 12,000 vaccine doses were provided, 70% of elders were vaccinated before it was a national goal, and 70% of the Cass Lake Indian Health Services user population was vaccinated before the president's July 4th goal.   The pair of health systems are considered two of the best and fastest vaccine administration teams in the state, and their vaccination model has gotten attention from federal, state and private sector partners across the United States, the health department said.   I'm Cole Premo.

Matt is a Research Analyst in the Health Division at The Center on Budget and Policy Priorities, a non-partisan research and policy institute in Washington DC. His policy, research, and analytical work is conducted in the areas of Medicaid and the State Child Health Insurance Programs. We start with an overview of the Medicaid program, the largest insurance program in the US. We touch on the possible benefits of block granting (19:00), and how it may affect other industry stakeholders such as health systems, hospitals, community health centers, and insurers that offer Medicaid managed care products (23:20). Lastly we discuss another block granted program, CHIP and why many see it as a successful block grant program (29:00), and one other suggestion that has been made for Medicaid reform, per-capita funding (32:45). How the Public is Pushing Cities for Better Air Quality with CIMCON Lighting & the Ramboll Group

More and more, societal culture is emphasizing the need to be environmentally conscience. Everyday citizens are becoming more aware and socially active in their concerns for the air quality in their neighborhoods. Traditionally, legal standards have enforced monitoring ambient air for a standard criteria of pollutants, but this wide net is no longer sufficient. Air quality is now a factor that can attract new residents, businesses, and, in some areas, tourism. This social push is setting a demand for more specific, and complex forms of monitoring. Companies such as CIMCON Lighting, and Ramboll's Environment and Health Division are working to deploy sensor solutions that are flexible and can satisfy the unique goals for every region. "We are really starting to see monitoring in new ways. Ways that are social and ways that are fueled by the public and public awareness," said Julia Luongo, Managing Consultant for Ramboll Group's Air Quality division. Luongo came on the podcast, joined by Chris Davis, Vice President of Smart Cities for CIMCON Lighting, to discuss what are the major reasons – regulatory, social, environmental, economic – that compel cities and utilities to measure environmental air quality, and what is driving the need for more distributed and granular air quality monitoring? While the technology is growing in efficiency, the challenge deepens for city planners, as the amount of data can be overwhelming. The question now is how do cities process instrument data and provide meaningful solutions to its citizens.

Listen NowTelehealth and remote monitoring services generally enable physicians to treat patients and monitor their health status remotely.  Because of advances in wireless communication and biosensor technology these services are increasingly being used in the commercial health care market and as well in the Medicaid program and the VA because research shows these services can reduce acute care visits and lengths of stay, iatrogenic harm and improve patient adherence to care.  Nevertheless, the Medicare program restricts reimbursement for these services largely because CMS (the Congress and the CBO) see them moreover as duplicative (v. substitutive) services.   For example, in 2014 Medicare spent just $14 million on telehealth service reimbursement.  (Total Medicare spending in 2014 was well north of $500 billion).    During this 18 minute conversation Ms. Drobac discusses in part how and why reimbursement for telehealth and remote monitoring services are limited under Medicare, how other payers and providers are using telehealth and remote monitoring, what the research literature suggests regarding clinical effectiveness and cost efficiency, proposed Congressional legislation and related regulatory action to broaden Medicare coverage and chances for legislative and regulatory success. Krista Drobac leads the Alliance for Connected Care, a 501(c)(6) coalition formed to create a statutory and regulatory environment in which providers are able to deliver and be adequately compensated for providing telehealth and remote monitoring services regardless of delivery location or technological modality.   Ms. Drobac was previously Director of the Health Division at the National Governors Association's Center for Best Practices.  Prior to that she was senior adviser at CMS, Deputy Director of the Illinois Department of Healthcare and Family Services and spent five years on Capital Hill where she was a Health Adviser to the Senate Majority Whip Senator Richard Durbin and served as a John Heinz Senate Fellow for Senator Debbie Stabenow.  Ms. Drobac earned her BA from the University of Michigan and her MPP from the Kennedy School of Government at Harvard. For information on the Alliance for Connected Care go to: http://www.connectwithcare.org/  This is a public episode.
