I'd love to get your thoughts on a red patch under my right eye. I've suffered through it flaring up and going away over half of this year. My dermatologist has tried 2 creams and we recently did a skin allergy test. While this was helpful, nothing has made it actually get better. I have a sneaking suspicion that it is more gut related than contact allergen related. Do you have any thoughts on ways to go about finding this root cause? Thanks! Melissa: Hi! I just moved out of state and am in my mid 40s with menstrual irregularities. I'm feeling overwhelmed trying to find a new primary care and OBGYN that practices holistically with functional medicine therapies. How does one go about finding one. Or should I opt to doing a hormone therapy specialist? Anonymous: Hi Dr. C, For several mornings now Ive been having a weird, somewhat sour chemical taste in my mouth. I believe this is due to my chronic constipation as my morning bowel movements are often incomplete, and any subsequent movements I have after my first one will smell more strongly of this sour chemical taste I wake up with in my mouth. My urine will have this smell as well. It does not smell of feces, just sort of chemically. I also have lots of pain in the area right below the sternum and seems to be chronic, and only reduces with the more bowel movements I have. Im already taking many magnesium supplements, motility activators, tudca, and miralax but I still find myself in these situations quite often. I'm trying to understand why I tend to get benign growths such as lipomas. I also have a lump on my breast that has been there since I was 20. Finally, I have a lymph node that feels like a tiny hard nodule on my neck (at the top near the ear). Within the last few years, the levels have begun to climb and some of my inflammatory markers are worrisome. I am 53, post menopausal, and live an active lifestyle. I would like to avoid statins; however, I have been taking Icosapent Ethyl and Ezetimibe for the past 6 months and have seen a lowering of both my triglycerides and cholesterol to normal ranges again. My APOB is 139, LpA is 102 but my CT cardiac score is a zero. Where do I go from here? Is there any other testing I should consider to understand if need to continue on these medications, such as a CT angiogram? In this episode we hear the second half of the conversation with Franklin County Public Health Director, Scott LaVigne, in which he shares his views on the role of properly addressing behavioral health, providing a positive patient experience, and the importance of partnerships, and how these elements, and others, work together in order for his team to provide holistic care for patients.I want to go back a little bit to something you mentioned earlier. So we talked, you talked about the needs-based care, what I call the contextualized care and Medicaid is very focused on serving the whole patient, right, which includes some of those social determinants of health. And, and we've talked about access and access to behavioral health is really important. How's Franklin County Health Partner Department partnering or attempting to partner with other agencies to address these needs?Well, one of the things that when I first came down here that that I just said we really needed to do was get our medical staff. And by that I mean everyone from the person that greets somebody when they walk in the door and checks them in to the person that works through everything with their, their claims and submitting and all the financial pieces of all that interaction from start to finish and everything in between that we had a trauma informed and, and with a focus on integrating behavioral health and, and behavioral health is a broad term. I should probably break that down because it's used a lot in different contexts. I don't look at it as a way of, of sanitizing mental health. So I look at it as a collection of mental health and substance use disorder and, and really what we wanted to focus on here and it and it goes to the social or social determinants of health. We wanted to focus on the whole patient, not just one aspect of that patient. I know I don't think I've ever heard of patients say that they felt their life was better because they met all their HEDIS metrics.Me either, by the way.But what, what we did and what I, I did do almost immediately was we purchased an outcome measurement tool because I knew that one of the things that we want to do is we didn't want somebody to have all their screenings done, you know, meet all those metrics like that on the healthcare side, but have housing insecurity and be living in domestic violence and to have substance abuse and mental health problems. Because I know as a mental health provider and a substance abuse provider in my background history that most of the people that show up in emergency rooms with preventable emergency room presentations are people that have mental health and substance use disorders and other things on board or have experience childhood trauma. So we knew that if we didn't look at that whole picture and integrate that in, we were going to have a hard time doing that. So we pulled an outcome measurement tool from behavioral health. It's called the DLA 20 and it, it focuses on 20 areas of a human's existence. And we wanted to make sure that if somebody experienced a good positive health outcomes, that translated into all these other areas as well. And that became our outcome measurement tool. So that was a big piece of what we focused on. Let's see. The other thing I mentioned already was we wanted to do more screening. We, you know, we do screenings routinely as a health department. We have to spend more time with patients because of our funding than providers in the community do. This post outlines the strategies and lessons I've learned over the years in my mission to prevent chronic Lyme disease.Understanding the Challenges of Lyme DiseaseLyme disease is a complex and often misunderstood condition. It can manifest in a variety of symptoms, which differ widely from patient to patient. These symptoms can range from mild to severe and can affect multiple body systems, making diagnosis and treatment particularly challenging. Even when Lyme disease is caught early and treated with antibiotics, some patients continue to suffer from persistent symptoms, leading to what is often referred to as chronic Lyme disease.Early and Accurate Diagnosis: The First Line of DefenseOne of the most critical steps in preventing chronic Lyme disease is ensuring an early and accurate diagnosis. Lyme disease is primarily a clinical diagnosis, meaning that it should be based on a patient's symptoms and history rather than relying solely on test results. This is because the standard two-tiered testing approach can miss early or late-stage infections.In my experience, comprehensive evaluations are essential, especially for patients with persistent symptoms. This includes not only testing for Lyme disease but also for co-infections like Babesia, Bartonella, and Ehrlichia, which are often transmitted by the same ticks that carry Lyme disease. These co-infections can complicate the clinical picture and require different treatment approaches.Tailored Treatment Plans: No One-Size-Fits-All ApproachNot all patients respond to standard treatment protocols. While a typical course of antibiotics might be effective for some, others may require extended treatment or a combination of therapies to address the varying stages of the infection. Monitoring each patient's progress is crucial, and being willing to adjust treatment plans based on their response is key to preventing chronic symptoms from developing.Patient Education and Support: Empowering Patients for Better OutcomesEducating patients about the potential for chronic symptoms, even after early treatment, is vital. Patients need to be aware that symptoms can recur and that it's important to seek medical attention promptly if they do.
