Host, Dr. Catherine Cerulli, a professor of Psychiatry at the University of Rochester, is joined by her guest, Nila Bragg, MSN, CPNP, a certified pediatric nurse practitioner and a Medical Consultant for the New York City Administration for Children's Services (ACS). ACS's mission is to protect and promote the safety and well-being of New York City's children. As part of that work, child protective specialists from ACS conduct investigations to determine what is needed to best keep the children safe. For instance, to help keep families together, child protective specialists often connect families to services that can help, including counseling, substance abuse treatment, and access to concrete resources, like food, diapers clothing and even child care vouchers. Throughout this work, it is important that ACS child protective staff understand a variety of medical issues that the family may be coping with, as well as treatments, in order to better identify whether there are any potential safety risks. As a Medical Consultant for ACS, Nila provides consultation to child protective specialists who have clients with complex medical histories. Nila helps CPS better connect children and their families with services that best support caregivers as they manage their child's medical needs. Nila's key message is that educating families and connecting them to resources is paramount to not only improve children's health, but connections also support the overall well-being of children and their families.

The month of September is Pain Awareness Month. In years past, I've done lots of different things for Pain Awareness Month. I've held online summits, which hosted 30 plus experts. I've done courses for professionals and people with pain. In 2021, I was trying to figure out, "What are we not talking about in the chronic pain space that we need to bring more awareness around? What should we be talking about? What can we bring some more disruption around to disrupt our pain care world out there that isn't always as effective for people with chronic pain?" As I started to reflect on my practice and the patients I've seen over many years I've been treating patients looking at research, I said, "We need to talk more about the link between trauma and chronic pain." This September, I've done a couple of different things to raise awareness around the trauma-pain connection. The first is I've done lots of different posts on my Instagram handle. Recently I moderated a panel with Cleveland Area Healthcare leaders and Ohio Congresswoman Marcy Kaptur, representing Ohio's 9th Congressional District, to discuss constricted access to social services. The virtual event was hosted by CareAdvisors, a healthcare technology firm. They had recently done a survey of Greater Cleveland social workers to determine what needs had changed because of the pandemic, and more importantly, what are their clients saying are their biggest concerns. The responses were eye opening. Panelists included; Terrance “Terry” Byrne, MBA, MJ, FACHE, vice president of Health Center Operations, at Neighborhood Family Practice Community Health Centers; Dr. Patricia Lyons, PhD, LISW-S, owner of Lyons Counseling and Consultation Services, president of the Columbus Association of Black Social Workers, and director of the Molina Healthcare Social Determinants of Health Innovation Center; Dr. Michelle Medina, MD, FAAP, associate chief of population health at Cleveland Clinic; Margaret Mitchell, president and cEO, YWCA Greater Cleveland; Dr. Adam T. Bradford L. Frank, MD, MPH, MBA, conducts a Masterclass on how to provide nursing home consultations for psychiatrists. The documents Dr. Frank refers to during this Masterclass are available at (https://bit.ly/3rWqfcK) Dr. Frank is a board-certified geriatric psychiatrist who provides consultations for more than 30 nursing homes in North Dakota. He has no disclosures. Take-home points Dr. Frank reviews practical information about documentation, prescribing, and diagnoses for psychiatric clinicians who treat individuals living in nursing homes. The Centers for Medicare & Medicaid Services has many rules and regulations governing the psychiatric treatment of individuals in nursing homes, including special mental status testing, a policy of gradual dose reduction, and restrictions on how long certain medications can be used. Documentation Even for geriatric patients who live in nursing homes, a full past psychiatric history, including substance abuse and social history, is essential to diagnosis and treatment. To obtain these histories, Dr. Frank sends documents to the nursing home to be completed ahead of time, and then, while he starts to make his differential diagnoses, he talks with the nursing staff about why they want the consultation. The Brief Interview for Mental Status (BIMS) is a 15-item mental status exam mandated by the CMS during nursing home evaluations. A score of 13-15 indicates that a patient is cognitively intact, 8-12 indicates moderately impaired, and
Behavior is always a hot topic in special education. If a school struggles to support a child’s behavior, the child often struggles comprehensively. One solution is a comprehensive functional behavior assessment, which is an evaluation of the child’s trigger behaviors. Once an FBA is done, the team can develop a BIP.Dr. Solandy Forte of Milestones Behavioral Services walks us through the components to a well-written, comprehensive FBA in today’s episode. Solandy is my friend. We met at the COPAA Convention through her husband, Jeff, who is a special education attorney in Connecticut. I’d met Jeff at Wrightslaw’s ISEA a few years prior. I met Solandy at a happy hour and knew I wanted to sit by her at dinner, as her passion and wisdom are contagious! I could listen to her talk behavior all day! Dr. Forte’s explanation of the FBA is practical and thorough. I have read so many FBAs that tell me very little about the child. She explains the components of the FBA (which are listed in the FREE resource on my website here) and also the reasons that you need them. This is amazing information that you can tote along with you to your next IEP meeting! Then, she explains how to take the recommendations from an FBA to draft a Behavior Intervention Plan, which both supports the child with proactive and preventative strategies but also reactive interventions. I have a lot of cases that involve behavior, and Solandy taught me a thing or two in today’s episode! It’s just that good! Being an Ally is more important than ever in this time of challenge, frustration, and confusion. It is time to remove the confusion around what it means to truly be an ally, specifically for people of color, if you are a white person in America. People of color should not be expected to educate white people about their experience or mere existence; it is the responsibility of white people to listen, to guide each other through the process of recognizing the impacts of privilege, and to move forward to learn and grow as become allies. Their new edited book, entitled Teaching LGBTQ Psychology: Queering Innovative Pedagogy and Practice (American Psychological Association, 2017), covers pedagogical concepts as well as practical suggestions for bringing the material to life and helping students feel at home with it.
