Broadcast from KSQD, Santa Cruz on 8-07-2025: Dr. Dawn opens with groundbreaking news about the first successful islet cell transplant for type 1 diabetes without immunosuppression. A 42-year-old man received gene-edited donor cells that were modified to avoid immune rejection by removing tissue compatibility markers and adding protective proteins. The transplanted cells in his forearm began producing insulin autonomously, reducing his A1C by 42% over 12 weeks. This breakthrough could lead to off-the-shelf diabetes treatments without the cancer and infection risks of immunosuppressive drugs. She critiques a case where a patient was prescribed an expensive new osteoporosis drug without trying conservative treatments first. Dr. Dawn explains how this monoclonal antibody works by blocking sclerostin, essentially cutting the brake lines on bone formation. While effective for severe osteoporosis, the drug increases heart attack and stroke risk by 15%. She advocates for trying lifestyle changes, exercise, and established treatments before resorting to newer, more dangerous medications with questionable risk-benefit ratios. A caller asks about a pustular rash on his upper body that his acupuncturist suggests might be related to COVID vaccination. Dr. Dawn addresses the distribution pattern of the rash, suggesting possible causes like hot tub folliculitis or topical allergies based on the location. She discusses how to evaluate rashes by their characteristics and distribution rather than jumping to vaccine-related conclusions. The caller inquires about vaccine development timelines and whether rushed approval during COVID might have missed serious side effects. Dr. Dawn explains that mRNA vaccine technology existed for 10 years before COVID and discusses her theory about vaccine side effects. She describes how spike proteins may temporarily deplete anti-inflammatory mechanisms, leading to flare-ups of existing conditions like eczema or arthritis, but emphasizes these effects are generally temporary and rebuilding occurs over time. Caller asks about RFK Jr.'s stance on vaccine research and the long-term implications of reducing vaccine development. Dr. Dawn provides a nuanced response about problems with the current vaccine system, including government-guaranteed markets that incentivize overproduction of vaccines. She discusses the evolution from essential vaccines for devastating diseases to newer vaccines for less critical conditions, explaining the economic forces that drive development and the resulting over-vaccination in healthy populations versus the need to protect vulnerable individuals. Dr. Dawn concludes with an innovative medical technology - a robot mattress designed to prevent pressure ulcers in immobilized patients. The mattress contains 1,260 independently controlled pistons that create undulating checkerboard patterns, alternating pressure points to stimulate blood flow. Research showed that brief high pressure followed by relief works better than constant low pressure, reducing pressure ulcer areas by 34 times compared to standard foam padding.

Episode 93 – Side Effects from Psychotropic Medications Side effects are something we ALL think about when prescribing any medication for our patients, not just psychopharmaceuticals. But for some reason, whether it's because these meds are still so unknown to many in the industry, or because of experiences we have heard from people – friends, family – who have been prescribed these medications, conversations around side effects from psychotropics seem more difficult to navigate. In this episode, I unpack the 2 most common caregiver concerns I encounter when talking about psychotropic medications as well as what side effects we commonly expect to see with long-acting medications and how I approach these situations. Here's the study I mention in the episode: Dreschel, N.A., 2010. The effects of fear and anxiety on health and lifespan in pet dogs. Applied Animal Behaviour Science, 125(3-4), pp.157-162. On this episode of Longevity by Design, Dr. Gil Blander sits down with Dr. Karl Pfleger, aging biotechnology investor and creator of AgingBiotech.info, to dissect the rapidly evolving field of aging therapeutics. Karl breaks down the sector into two main categories: interventions that slow aging rates versus rejuvenation strategies that repair accumulated damage.Karl highlights the most promising areas in his investment portfolio, including senolytics for clearing senescent cells and epigenetic reprogramming technologies. He explains why current aging clocks aren't ready for clinical use and discusses the limitations of popular interventions like GLP-1 drugs and fasting protocols. The conversation reveals that 14 Phase 3 clinical trials are currently testing core aging therapeutics. The discussion covers recent FDA approvals for ATTR treatments and stem cell therapies, signaling the field's maturation. Karl emphasizes that meaningful life extension requires a "divide and conquer" strategy, targeting multiple aging mechanisms simultaneously rather than relying on single interventions. Despite exciting therapeutic developments, he maintains that optimizing diet, exercise, and sleep remains the most impactful strategy for extending healthspan today.Guest-at-a-Glance

Trigger Warning:This episode includes discussion of trauma, suicide, abuse, and emotional processing in the context of psychedelic therapy. Please take care while listening. Viewer discretion is advised. You've been told psychedelics are poison. That healing trauma requires years of talk therapy. What if three sessions could change everything? This week on Live Well Be Well, I sit down with Dr. Rick Doblin, who has spent 39 years fighting for something most people thought impossible: bringing psychedelics back into medicine. As the founder of MAPS (Multidisciplinary Association for Psychedelic Studies), Rick has dedicated his life to proving that MDMA isn't just a party drug. It's a revolutionary treatment for trauma that could change how we understand healing itself.Rick doesn't just speak about psychedelics as science or policy. He's lived them, studied them, and helped thousands heal with them. His work isn't about escapism. It's about going deeper. Into the body. Into memory. Into parts of ourselves we've long avoided. And then, through compassion and skilled integration, coming out the other side, changed. We explore:Rick's first psychedelic experience and what it taught him about interconnectednessHow MDMA works in the brain to reduce fear and increase compassionWhy MDMA may be the most effective tool we have for PTSDThe three-phase therapy protocol and why context matters more than the drug itselfWhat went wrong with FDA approval and why Rick believes approval is inevitableThe difference between a "bad trip" and a meaningful oneWhy people with the most difficult childhoods often show the greatest improvement with MDMA therapyHow patients develop empathy (not forgiveness, but understanding) for their perpetratorsYour trauma doesn't define you. But learning to hold it with compassion might just set you free.Love,Sarah Ann
