"The universality of guilt - I think that my life as a child therapist has a lot of advantages; we carry the idea that ‘we are all the ages we've ever been' and those magic years never disappear. There are some great things about that - there are some problems with it too. That kind of irrational self-centered construction to explain what is intolerable and difficult to grasp comes from that. Early in this whole process, I remember talking to the pediatrician taking care of us: ‘Was it something in the environment? Was it the street we lived on? Was it that we lived in the basement? Was that an explanation? How close we were to the boiler room?  What could it be?' And he just looked through all of that to say: ‘You did nothing wrong'. It stays with me as I remember it - how potent that was. There's a part of me that wants to keep manufacturing the explanation that I can get my head around, and there's something penetrating about realizing that those explanations don't serve."    Episode Description: We begin with Phil sharing with us the family tragedy that is described in his book A Short Good Life. It describes the illness and death of his younger daughter Liza, who he says is his second child. When I ask him about his use of the present tense, he says that he "is parenting her memory." He describes the process of writing this book and the help he sought from other parents who have lost children. He reads sections from the book that allow us to listen in on Liza's struggles with her older sister, her facing the guilt of imagining having caused her leukemia, and then her efforts to come to terms with her dying - at the age of six. We conclude with Phil's evolution as a therapist, which now includes his use of psychedelics to assist those who are struggling with trauma. I share how grateful I am to have learned of his work and how he has brought forth something special for others to learn from what I consider to be a tutorial on love. Why are religious and mystical experiences important to our sense of meaning and purpose in life? What is a spiritual emergency and how can it actually help us in the long run? Are transpersonal experiences illusions of the mind or can they tell us anything about the nature of reality? In this episode, we have the extraordinary topic of Transpersonal Psychology to learn about. With the steady rise in popularity of western secular spirituality, meditation, psychedelic research, altered states of consciousness and embodied practices in general, during the 60's some psychologists felt there was a part of psychology missing from the old humanist and behaviourist models. It was as if the overwhelmingly materialist scientific view of humans, that sees our bodies and brains as fundamentally separate from other beings, the natural world and any hypothetically transcendent reality, was missing out a huge source of data about the way our minds work. So a bunch of them coined a new term, Transpersonal psychology, and with it came a new field of study and practice.  It's a really wide field and at its cusps starts to get into areas that science can't actually study using the method; some of which we're going to touch on towards the end of this episode. But above all it makes a place for the importance of the transpersonal that crosses those boundaries between our bodies and brains and everything else out there, both known and unknown. So fortunately today's guest has over 25 years of experience both as a psychologist and a workshops leader, David Lukoff. Dr. Lukoff has published over 80 articles on spirituality and mental health, and is an active workshop presenter internationally on spiritual competency, grief, death, recovery, and spiritual crises. He is a Professor Emeritus of Psychology at the Sofie University, previously known as the Institute of Transpersonal Psychology in Palo Alto, in CA and previously served on the faculties of Harvard University and UCLA. Gizma is a Psy Trance artist and a MAPS (Multidisciplinary Association for Psychedelic Studies) research physician. She is a native Mexican who grew up both there as a young child but is now based in SF. Gizma recieved her M.D. from the University of Michoacan in 2009, and comes from a family of doctors. She talks about coming to San Francisco to attend Pyramind Music & Audio Production Institute because of her passion for music and love for the city. "The Opening" is the first track played, and she credits the instructors at Pyramind that helped her learn the sound design techniques and Apple's Logic Pro that gave her the skills and creative workflow to produce at this level. Greg and Gizma discuss the influences that went into this track, and how her work as a research Physician for MAPS influences the music she makes. Being an indigenous Mexican drew her to her research with MAPS because psychedelic medicine is such a big part of her culture and has inspired the music she makes. They dive into the sound design process of her music, and she talks about many of the plugins and techniques that are staples of producing Psy-Trance music as well as how she likes to sit down and start a track, usually by choosing a vocal or spoken word sample. They then get into talking about the record labels that Gizma has worked with over the years, including Spun Records, and Antu Records where she releases music now. The conversation turns to collaboration with fellow label artist, including the next track they listen to, "Cognitive Liberty" produced in collaboration with Psy Trance artist Ital. Gizma talks about the inspiration for the song, as well as the collaboration process that went into making the track. The Psy-Trance scene in Mexico is discussed, and how it's no longer the underground thing it was back in 2003 when she started going to events there, and why it's now a great thing that it's much more visible and accessible. The conversation then turns to how the psychedelic medicine of her culture and family influences her work and music, and how she applies that knowledge in her medical practice. They close out the episode by listening to the track "Ainu" by Gizma and Amplify, which was inspired by the Aboriginal Ainu people of Japan. In this episode, Philip shares how all his medical degrees might have helped his family and daughter through her life's transition to death, it never prepared him for his grief as a bereaved father.Dr. Philip Lister is an adult and child psychiatrist in private practice in New York City. He received his medical degree from Duke University School of Medicine and moved to New York to enter psychiatric training. Trained in adult and child psychiatry at the Payne Whitney Clinic of Weill Cornell Medical Center, Dr. Lister joined the voluntary faculty there supervising residents and teaching medical students. Overlapping with residency and fellowship, he trained in adult and child psychoanalysis at the Columbia University Center for Psychoanalytic Training and Research. He then joined the faculty at the Psychoanalytic Center, participating in the basic course surveying the work of Freud.Most recently, he has trained with MAPS (Multidisciplinary Association for Psychedelic Studies). That training prepared him to be a team of therapists offering research participants MDMA-assisted psychotherapy in a Phase 3 study to determine whether MDMA will provide a new, more effective treatment for entrenched, treatment-resistant PTSD [Post Traumatic Stress Disorder]. I believe these substances to be medicine even though they're stigmatized as drugs. I will always refer to them as medicine because that is what they have been for me. Types of medicines now being considered for therapy:  MDMA/MDA, Medical Marijuana, LSD, Ibogaine, Ayahuasca, DMT, Ketamine, Mescaline, Peyote, Psilocybin  Treatable conditions & behaviors: Anxiety, Depression, PTSD, Addiction, Chronic Illness, Trauma-induced Mental Illness Disorders, Trauma-related issues MDMA has shown to be incredibly effective in treatment of trauma, specifically PTSD. Psilocybin has shown to be incredibly effective in treatment of anxiety, depression and addiction. “Psychedelics saved my life,” says Payton Nyquvest. “Now [I'm] on a mission to pay that forward.” Nyquvest is the CEO of Numinus Wellness, a publicly traded Vancouver start-up that manufactures such illegal psychedelics as MDMA and psilocybin for clinical drug trials. The intent is to determine how effective they are as a therapy for mental illness. And if their potential pans out, Numinus Wellness wants to administer the drugs to patients in guided sessions across the continent. Plus here's a link that provides information on how TPL is supporting innovative therapies to treat PTSD.    INSIGHTS Nyquvest turned to psychedelics as a last-ditch therapy for chronic pain. He had been trying treatments for years with no results. “When you're in the emergency room,” he says, “and physicians are looking at you sort of compassionately, saying, ‘Sorry… we don't have answers here for you,' it's a very lonely and isolating place.” Eventually, Nyquvest became desperate for relief. “I was getting hospitalised two to three times a week, and had really run out of options, and turned to psychedelics, really as a last ditch effort to try and save my life.” Nyquvest insists that psychedelics aren't a panacea or a magical solution, but they were the first thing that worked for him to treat his chronic pain. [02:55] Rather than making psychedelics available on the open market, Nyquvest believes psychedelic therapies should be administered by experienced professionals in guided sessions. “It's a service-based model, not a product-based model,” says Nyquvest. By way of contrast, cannabis is a product-based model, which sees the substance available for sale for recreational use by civilians. Nyquvest doesn't want to see the same thing happen to psychedelics. “The integration, the working with therapists, the support system is as much, if not more, important than just the psychedelic experience itself,” he says. [12:58] “When you have depression, you become labelled as a depressed person. You get put on some kind of an SSRI, and it's this symptom management thing that pharmaceutical companies have been profiting from significantly over the last number of years,” says Nyquvest. Psychedelics, in contrast, are already showing that they have potential not just to treat, but to cure mental illnesses, Nyquvest says. He mentions a MAPS study where “over 66 percent of the people who did MDMA for post traumatic stress disorder no longer met the PTSD criteria after three treatments.” In other words, two-thirds of participants were cured. Today, psychedelics may be administered as a last-ditch effort to cure a certain mental illness. In future, Nyquvest wants psychedelics to be administered on a “preventive mental health model, where people are able to get ahead of some of these things before they become a significant problem… so that they can get back to continuing to do their jobs, and living more happy and full lives.” [23:20] With the help of psychedelic-assisted therapy, Nyquvest has been able to separate his physical and mental health from the conditions that once plagued him. “I was labelled as someone who was very, very sick… and the recognition that there's actually inherently nothing wrong with me, [but that] there were events that happened in my life that I was struggling with… that was a big thing for me,” he says. Nyquvest uses the metaphor of dentistry to describe the way he sees psychedelics being used preventively. “Most people have great teeth… and still go to the dentist twice a year. I think it's a similar sort of thing in the way we should be treating the brain. I don't know a person who gets out of life unscathed in terms of some kind of event that happens in their life that they could probably use some looking at, and some support. And I think that's a huge opportunity for psychedelics.” [26:03] Nyquvest says that the old stereotype, of psychedelics users simply looking for an escape from reality, is just plain false. “A lot of the people that I know that have worked with psychedelics are very high-performing, entrepreneurial or otherwise type of people,” he says. “They've been able to clear a lot of the noise and really look at what is motivating [them]. The more deeply you understand some of those motivations, the better you can work with them, and the better you are at some kind of control or understanding of what's driving you.” [27:35] When Payton Nyquvest co-founded Numinus Wellness, his focus was on healing, curing, and developing new treatments. “The plan was always to just build a company that could make a significant impact, there was no plans of taking the company public, it wasn't even really something we thought was going to be on our radar screen for quite some time,” he says. But the world's excitement around psychedelics has led to more interest, more research and more funding. “It made sense for us to go public,” he says, because that's provided the funding to acquire mental health clinics, where Numinus may one day provide guided psychedelic therapies to patients at large. Going public came with challenges, though. “People talk about the overnight success and huge amount of enthusiasm in the psychedelic space, but when we went public, it was actually met with a significant amount of challenge and scrutiny.” Numinus has come a long way since then, holding onto a cash balance of more than $63 million in their latest quarter. [29:38] Asked to predict the future of Numinus Wellness, and the psychedelic space in general, Nyquvest says that he believes we're only just starting to learn about what psychedelics can do for mental healthcare. He hopes Numinus will remain at the forefront of the field. “Our goal has always been to be the number one most trusted brand in the psychedelic space… Our licencing out of the lab is an international licence, so we can work with any organisation all over the world… when you think about doing psychedelic therapy, [we hope] you go to a Numinus location in order to do that.” In the short term, Numinus Wellness will continue to collaborate with mental health clinics and MAPS, the Multidisciplinary Association for Psychedelic Studies. [31:20]

I really appreciate you having me. Stu 01:32 I really appreciate you sharing some of your time. I know that we got lots to talk about this morning and there's a particular device that we're really intrigued in finding out a lot more about. Before we dial into that, and for all of our listeners that may not be familiar with you or your work, I'd love it if you could just tell us a little bit about yourself please. Dave 01:53 Sure. I'm a psychiatrist and a neuroscientist. I see patients currently focused on treatment resistant, posttraumatic stress disorder, depression and anxiety, substance use disorders and chronic pain, insomnia, pretty much most mental health conditions that are worsened by stress and some of them that crossover even into physical health. That's a lot of conditions and we see a lot of overlap between those conditions in people. I was working with those patients for a while and I realized that there were a lot of commonalities between these folks that prevent them from achieving their therapeutic goals from treatment. If you have anxiety or depression, your goal of treatment is to feel less anxious and feel happier during the day and have more energy and sleep better at night. 02:41 But less than 50% in a lot of cases are getting that kind of result from their depression treatment in Western medical situations. I started to look at other techniques for how we could help people access those skills and access better sleep, better focus, better energy levels during the day, better mood, that we're starting to look into sort of where Eastern and Western medicine meet, really looking at the breathwork biofeedback and starting to look at meditation and mindfulness practices and the neuroscience of that, right, and how we've evaluated those over the years. 03:19 We know that biofeedback practices boost heart rate variability, something that we'll probably talk about a little later, and heart rate variability turns out to be really interesting, particularly in this day and age, because it is the rate of change of the heartbeat over time, which doesn't sound that interesting. But when you actually look at it more deeply, what you see is that heart rate variability is the single biggest predictor of the effect of stress on our bodies over time. It causes these imbalances in our autonomic nervous system, our stress response system, our recovery response system that prevent us from recovering effectively and then that impairs our performance down the road. Heart rate variability allows us to evaluate that. 03:58 We saw the patients you have PTSD, depression and anxiety all have low heart rate variability. People who have chronic pain, insomnia, they all have low heart rate variability almost across the board. That's really interesting because their bodies and their reports from their mouth when they talk to me would say or my colleagues would say, "I feel really stressed out. My body's always under duress, always under threat. Never feeling like it can rest, take a break, truly feel safe." All of these kinds of language was used. We developed Apollo as a strategy to help give people something that's accessible, that's a wearable technology. 04:37 It vibrates in a very gentle frequency of sound that stimulates the nervous system very much like biofeedback through our sense of touch to help balance the nervous system. We wanted to make something that was accessible and easy for people to use that could actually have realtime benefit in terms of improving our autonomic nervous system balance and helping us access high performance states, high recovery states, and meditative states more effectively.

Charles L. Raison, MD, returns to the Psychcast to conduct a Masterclass on psychedelics for patients with major depressive disorder. Dr. Raison, professor of psychiatry at the University of Wisconsin–Madison, previously conducted a Masterclass on the risks and benefits of antidepressants. He disclosed that he is director of translational research at the Usona Institute, also in Madison. Later, Renee Kohanski, MD, raises questions about the felony child abuse case of pediatric emergency department doctor John Cox. Takeaway points Psychedelics are a range of compounds that share a common mechanism as agonists at the postsynaptic 5-HT2A serotonin receptor. Psychedelic agents have a novel therapeutic quality. Studies suggest that a few or even one exposure to a psychedelic compound, which has a short-term biological effect, leads to long-lasting therapeutic effect, such as remission of mood disorder or change in personality characteristics. The clinical outcomes are mediated by the intensity of the psychedelic experience. A psychedelic experience is characterized by profound, rapid alterations in what is seen, sensed, felt, and thought. It often leads to personal growth with experiences of transcendence. Subjects in trials often report a “mystical experience” they describe as a sense of unity with the universe and understanding of one’s deeper purpose. Psychedelic experiences also are characterized by a difficulty in describing them with words. Because psychedelics are illegal substances, the traditional route of pharmaceutical companies’ funding the research for clinical trials is not available. Organizations such as Usona Institute and MAPS (Multidisciplinary Association for Psychedelic Studies) are leading the way. The Food and Drug Administration has granted psilocybin a “breakthrough therapy designation” for the treatment of major depressive disorder. Summary Psilocybin, lysergic acid diethylamide (LSD), mescaline, ayahuasca (active ingredient: N,N-dimethyltryptamine [DMT]), and 3,4-methylendioxy-methamphetamine (MDMA) are all classified as psychedelics. Psychedelics have been used for thousands of years for spiritual ceremonies. Psychedelics came to the attention of medicine and science after 1943 when Albert Hofmann, PhD, a chemist at a Sandoz Lab in Basel, Switzerland, synthesized LSD and accidentally ingested it, serendipitously identifying its mind-altering properties.  Until 1970, psychedelics were widely used in clinical research, and more than 1,000 academic papers about their use were published. For example, psychedelics were used as a model for schizophrenia and helped identify the role of serotonin in psychosis. They also were studied to treat addiction and as a treatment for existential anxiety in cancer. In 1971, psychedelics were declared illegal under the U.N. Convention on Psychotropic Substances. Researchers returned to psychedelics in the 2000s, examining a variety of uses, including the capability to reliably induce psychedelic experience in healthy normal volunteers (no previous psychiatric diagnosis) and promote emotional well-being in healthy normal volunteers. The role of psychedelics as medicine are once again being studied in a variety of contexts, such as mood disorders, PTSD, addiction, and phase-of-life problems. Most notable from the research is the capability of psychedelic compounds to induce long-lasting effects on personality, mood disorders, and PTSD after one or a few ingestions. What is remarkable is how the therapeutic effect remains long after the biological presence of the compound is gone from the body. The clinical outcomes are mediated by the intensity of the psychedelic experience. The Usona Institute, a medical research organization, started as a nonprofit to advance the research into psychedelics needed for the FDA to approve psychedelics as a treatment. Because psychedelics are still illegal, the traditional route of pharmaceutical companies funding this type of research is not available. The FDA has granted psilocybin a “breakthrough therapy designation” for the treatment of major depressive disorder. The breakthrough therapy designation “indicates that the drug may demonstrate substantial improvement on a clinically significant endpoint(s) over available therapies.” The breakthrough therapy designation is for major depressive disorder, not for treatment-resistant depression, suggesting that the FDA recognizes the shortcomings of current treatments for depression. Dr. Rabin helped to organize the in collaboration with colleagues at Yale, the University of Southern California, Mt. Sinai, Modern Spirit, and MAPS (Multidisciplinary Association of Psychedelic Studies) to determine the mechanisms of the dramatic therapeutic benefits observed following psychedelic-assisted psychotherapy in treatment-resistant mental illness. The study has begun and is collecting DNA samples from patients with treatment-resistant PTSD who are participating in the MAPS-sponsored Phase 3 FDA trial of MDMA-assisted psychotherapy to evaluate the gene expression changes that result from the therapy. He is also the founder and executive director of the Board of Medicine, a nonprofit organization of physicians and scientists spearheading the development of the world’s first evidence-based peer-reviewed clinical guidelines for the safe use of complementary and alternative medicines including medical cannabis. Dr. Rabin works with patients with treatment-resistant conditions like post-traumatic stress disorder (PTSD), depression, and substance use disorders. In his work, he found that when patients were in a calm, safe environment their physiological markers of stress improved, their mood got better, and they were better able to focus and manage their symptoms. He also found that mindfulness techniques and deep breathing worked quite well to reduce stress in patients, but that his patients had a difficult time practicing these techniques on their own in their day-to-day lives. In 2014, Dr. Rabin began his research on what would become the device I'm referring to, "Apollo" in a neuroscience lab at the University of Pittsburgh. After exploring academic research on mindfulness, biofeedback, deep breathing, and a host of sensory techniques including music therapy, Dr. Rabin found that touch is an often overlooked, but an incredibly vital pathway for quickly establishing relaxation in the mind and in the body. For New Year’s Eve, I decided to go on a vision quest with the help of psychedelic mushrooms and I had one of the most beautiful experiences of my life and knew I needed to tell my parents about it. But, I was so nervous about what they were going to think that I presented the idea of recording the experience with them for the podcast as I told them the story. Most likely you will never hear that recording, but it allowed me to feel safe as I was telling them the story. Which then inspired this coffee talk about communication and speaking and living our truth. David Jay Brown is the author of The New Science of Psychedelics: At the Nexus of Culture, Consciousness, and Spirituality. He is also the coauthor of five bestselling volumes of interviews with leading-edge thinkers, Mavericks of the Mind, Voices from the Edge, Conversations on the Edge of the Apocalypse, Mavericks of Medicine, and Frontiers of Psychedelic Consciousness. De ene keer een interview met een expert en de andere keer deelt iemand diens persoonlijke seksuele levensverhaal, onder de noemer Dicht op de Huid.Deze keer komt Laura aan het woord. Na het beluisteren van de aflevering over chemsex mailde ze me om haar eigen ervaringen met drugs en seksualiteit te delen. Verhalen die weer heel anders waren dan die in de chemsex aflevering te horen waren. In deze aflevering vertelt ze over wat ze is tegenkomen toen ze MDMA gebruikte. Verder hebben we het ook over kwetsbaarheid, de behoefte aan controle en het verlangen naar overgave. We praten over het beeld wat we van onszelf hebben, over hoe we denken dat we zouden moeten zijn en hoe bevrijdend het is om dat los te laten. In het gesprek hebben we kort over de onderzoeken die momenteel worden gedaan naar de mogelijkheden van MDMA in de behandeling van PTSD. Dit gebeurt met name in de Verenigde Staten, maar ook in Nederland zijn er onderzoeken aan de gang. It kept developing into “my child” into “my country”, “MY”. Then people started using less psychedelics and started consuming more alcohol and now everything is an over exaggerated male dominance “Females tend to be more nurturing and more cooperative, and it's exactly the factors that are missing in our current world and are causing problems of greed. It could be balanced and harmonized with more feminine energy.” - David There is a crucial imbalance from male and female in history alone But more than an imbalance between just males and females, it's about an imbalance of masculine and feminine energies Each of us, male and females have both a masculine and feminine energy We can see the masculine and feminine imbalance in the world and our planet right now. Dr. Ben Sessa is a consultant child and adolescent psychiatrist, and is currently leading the first clinical trials for the therapeutic use of MDMA in a psychiatric setting in the United Kingdom. As Ben goes over in this episode, the way the psychedelic compound MDMA works on the brain of an individual very dramatically allows them to process and discuss extremely traumatic experiences and effectively work through them. MDMA, in a therapeutic setting, shows enormous potential, and as Ben stated in his TED talk and elsewhere, MDMA may be as revolutionary to psychiatry as antibiotics were to modern medical practice. The use of MDMA, as well as other psychedelics, is heavily prohibited under law, both within the United Kingdom as well as in the United States, and is very difficult and expensive to research to research for scientific and medical purposes. We discuss how the funding and research by organizations like MAPS (Multidisciplinary Association of Psychedelic Studies, an organization Ben works with) is currently opening the door for the therapeutic use of psychedelics, as well as potentially loosening the prohibitive laws surrounding the recreational use of the substances.
