You can find all the shows, including this one, at asco.org/podcasts.  My name is Brittany Harvey, and today, I am interviewing Dr. Lisa Law from Kaiser Permanente and Dr. Randy Taplitz from City of Hope Comprehensive Cancer Center, authors on “Vaccination of Adults with Cancer: ASCO Guideline.” Thank you for being here, Dr. Law and Dr. Taplitz. Dr. Lisa Law: Thank you. Dr. Taplitz: Thank you, Brittany. Brittany Harvey: Before we discuss this guideline, I'd like to take note that ASCO takes great care in the development of its guidelines and ensuring that the ASCO conflict of interest policy is followed for each guideline. The disclosures of potential conflicts of interest for the guideline panel, including Dr. Taplitz and Dr. Law, who have joined us here today, are available online with the publication of the guideline in the Journal of Clinical Oncology, which is linked in the show notes.  So then, to dive into the content, here first, Dr. Taplitz, can you provide a general overview of both the scope and purpose of this guideline on vaccination of adults with cancer? Dr. Randy Taplitz: Yes, so people with cancer often experience a compromised immune system due to a variety of factors. This includes chronic inflammation, impaired or decreased function of the hematopoietic system, and treatments that compromise their immune function. Because of this, people with cancer are at a higher risk for infection, including with vaccine-preventable diseases. Also, response to vaccines in patients with cancer may well be affected by this underlying immune status, and their anticancer therapy, as well as the severity of the underlying malignancy. The purpose of vaccination in this group of patients is to prevent infection or to attenuate the severity of the disease when infection cannot be fully prevented.   This ASCO review builds on a 2013 guideline by the Infectious Diseases Society of America, or IDSA, and uses what's called a systematic literature review of 102 publications between 2013 and 2023. This includes 24 systematic reviews, 14 randomized clinical trials, and 64 non-randomized studies. The largest body of evidence in these studies, not surprisingly, addresses COVID vaccines on the efficacy and safety of vaccines used by adults with cancer or their household contacts. ASCO convened an expert panel to review this evidence and formulate recommendations for vaccinations in this population. Brittany Harvey: Understood. I appreciate that context, Dr. Taplitz. So then, next, Dr. Law, I'd like to review the key recommendations of this guideline. The guideline addresses four overarching clinical questions. So starting with the first question, what are the recommended routine preventative vaccinations for adults with cancer? Dr. Lisa Law: Thank you, Brittany. Before I start, I just want to wholeheartedly thank the first author of this paper, Dr. Mini Kamboj, Dr. Elise Kohn from the NCI, as well as the ASCO staff in putting this publication and guideline together. It is a very, very important guideline, and I echo everything Dr. Taplitz just said.  So going back to your question, what are the recommended routine preventative vaccines for adults with cancer? As per this guideline, there are about 7 to 8 based on patient age and risk. Namely, they are: seasonal flu, RSV for those aged 60 or above, COVID-19, Tdap, Hepatitis B, Shingrix, Pneumococcal vaccine, and the HPV vaccine. These vaccines should ideally be given two to four weeks before therapy. However, non-live vaccines can be given anytime during or after chemo, immunotherapy, hormonal treatment, radiation, or surgery. Brittany Harvey: Excellent. Thank you for reviewing those vaccinations and the timing of them as well. So then, following those recommendations, Dr. Taplitz, what additional vaccinations and revaccinations are recommended for adults undergoing hematopoietic stem cell transplantation, CD19 CAR-T treatment, or B-cell depleting therapy?  Dr. Randy Taplitz: Many studies have shown that stem cell transplant recipients essentially lose immunity from childhood immunizations, and we know that these individuals are very vulnerable to infection, particularly in the first year after transplant. Revaccination is critical to help restore their immunity. The optimal timing of vaccination is based on our understanding of adequate immune reconstitution with B and T-cell recovery so that the individual can mount a response to the vaccine. We know that a lot of factors influence this immune reconstitution, including the age of the stem cell transplant recipient, the source of the donor, the time from transplant, graft-versus-host disease prophylaxis, the treatment and severity of graft-versus-host disease, and the vaccine type and antigens used.   There are a number of bodies throughout the world, IDSA as I mentioned, CDC, American Society for Transplant and Cellular Therapy, European Society for Blood and Marrow Transplant, and European Conference for Infections and Leukemia. All of these bodies have guidelines that approach vaccination in stem cell transplants. However, variation does exist in the use of a variety of things including whether to use immune predictors to help guide vaccination, and there is really not consensus on whether this immune predictor guided vaccination is more likely to produce a protective immune response versus a standardized schedule. In addition, the duration of protection is incompletely understood.  The bottom line in these guidelines is that they recommend complete revaccination starting for most vaccines at 6 to 12 months after stem cell transplant, in order to restore vaccine-induced immunity. And I just want to go through a few of the particulars. For COVID-19, which is a three-dose series in the primary series, influenza - generally high-dose influenza - and pneumococcal vaccine, PCV20 in general, ultimately four doses, can be administered, starting as early as three months after transplant. Although there is really not much data to guide the use of the recombinant zoster vaccine in allogeneic stem cell transplant, the vaccine can be administered after the end of antiviral prophylaxis, which in general is 12 to 18 months after allogeneic and 3 to 12 months after autologous stem cell transplant. Some of the other vaccines, such as hepatitis B, Tdap, meningococcal vaccines, and HPV revaccination in those less than 45 are also recommended.   I want to also spend the moment talking about the two recently licensed RSV vaccines, which were essentially studied in less compromised hosts and really without any immunogenicity data in stem cell transplant, and thus, there is no recommendation in this guideline for the use of these vaccines after transplant. Live vaccines, such as MMR and varicella – varicella would be in varicella-seronegative patients without a prior history of varicella – should be delayed for at least two years and only given in the absence of active graft-versus-host disease or immunosuppression.  Moving briefly to CAR T, which is an immunotherapy that involves adoptive cell therapy, given the available data and after a review by the group, it was recommended that adults with hematopoietic malignancies receiving CAR T therapy directed against B-cell antigens should receive influenza and COVID-19 vaccines either two weeks before lymphodepletion or no sooner than three months after the completion of therapy. Administration of non-live vaccines preferably should occur before CAR T treatment or at least 6 to 12 months after, following the same timing as what we recommend for stem cell transplant. There is really little data to guide the safety and timing of administration of live vaccines after CAR T therapy.   In terms of adults receiving B-cell depleting therapy, they are generally unable for time to mount an effective humoral response but may have at least partially intact cellular immune responses. They are encouraged to be revaccinated for COVID-19 no sooner than six months after completion of B-cell depleting therapy, and they should receive influenza vaccine approximately four weeks from the most recent treatment dose for patients on chronic therapy. For other non-seasonal immunizations, vaccines ideally should be given two to four weeks before commencing anti-CD20 therapy or delayed until 6 to 12 months after completion, except for the recombinant zoster vaccine, which can be given one month after the most recent dose of B-cell depleting therapy. Brittany Harvey: I appreciate you reviewing each of those vaccinations and when they should be given, and reviewing the available data – albeit, limited data – in these situations.  So beyond these routine preventative vaccinations and revaccinations that you've both just described, Dr. Law, what additional vaccinations does the expert panel recommend for adults with cancer traveling outside the United States? Dr. Lisa Law: Good question. As per these ASCO guidelines, adults with solid or blood cancer traveling outside of the United States should follow the CDC standard recommendations for their destination. For the 2024 CDC Yellow Book, travel vaccines, in general, should be delayed until three months from the last chemotherapy or, and for those with solid tumors, ideally when the disease is in remission. Of note, hepatitis A, typhoid, inactivated polio, Hep B, rabies, meningococcal vaccine, and Japanese encephalitis vaccines are considered to be safe. In all cases of travel, patients should be counseled by their healthcare provider about the travel timing, with the additional attention to the regional seasonality of infections, for instance, influenza is more common in late summer in Australia, and also with attention to any outbreaks that may be occurring globally at the time of travel. Brittany Harvey: Absolutely. Those are key points for clinicians to discuss with their patients as they consider upcoming travel.  So then, the final clinical question that the panel addressed, Dr. Taplitz, what vaccinations does the panel recommend for household and close contacts of adults with cancer?  Dr. Randy Taplitz: Thank you. Yes, it is recommended that all household members and close contacts, when possible, be up to date on their vaccinations. And the only further thing I would say is that there are some special considerations for the use of live vaccines in household contacts, particularly in stem cell transplant recipients. Contacts of people who receive stem cell transplants should preferably receive inactivated influenza vaccines. As was mentioned, MMR and varicella vaccines are both safe to administer to close contacts. Vaccine strain transmission to immunocompromised hosts has not been associated with MMR use in family members.   Eleven cases of the varicella vaccine strain transmission are described in the published literature, but none occurred in compromised hosts. Because the vaccine strain can cause severe and fatal varicella in profoundly immunocompromised people, precautions are advised to avoid close contact with a person with a vaccine-induced rash. For household contact travelers, MMR and yellow fever vaccines are considered safe. Oral cholera should be avoided. For smallpox vaccines, the second-generation ACAM2000 has rarely been associated with vaccinia transmission and should be avoided because of this. But the live replication-deficient MVA-based JYNNEOS vaccine is felt to be safe for household contacts of immunocompromised individuals. Brittany Harvey: I appreciate you reviewing the importance of vaccination for household and close contacts, and some of those precautions that individuals should take. I appreciate you both for reviewing all of these recommendations.  So then in your view, Dr. Law, what is the importance of this guideline, and how will it impact both clinicians and adults with cancer? Dr. Lisa Law: In my opinion, this is a very important guideline that is long overdue in the oncology community and will have a huge impact on both clinicians and adults with cancer. Over the years, I have often been asked by my colleagues and patients, “Can I have the flu vaccine, and if so, when?” So this guideline really is going to be helpful. More importantly, our cancer patients are living much longer. They may have years of quality of life even with third or fourth line of treatment, especially, for instance, like CAR T for myeloma and lymphoma. However, we know that with additional treatment, that carries a substantial risk of infection complication among these immunocompromised patients. So it is of paramount importance to inform our patients and colleagues to be proactive in advocating preventive therapy ahead of time, meaning trying to get the patients appropriately vaccinated as early as possible to generate immunity.  Another case in point is the Shingrix vaccine. I used to see lots of shingles, but ever since we have the recombinant Shingrix, I have fewer encounters. And this is huge because post-herpetic neuralgia robs a patient's quality of life. So, again, it is very important to recommend appropriate vaccines for our cancer patients.  Brittany Harvey: Absolutely. It is key to ensure patients receive these preventative vaccines, and we hope that this guideline puts an emphasis on that for clinicians and patients.  So finally, to wrap us up, Dr. Taplitz, what are the current gaps in knowledge regarding the vaccination of people with cancer? Dr. Randy Taplitz: There are a number of really important gaps in knowledge and really critical unmet needs that require research and other dedicated efforts. Among these are, and I think paramount, are really the participation of people with cancer with varied types of immunocompromise in vaccine trials. Where vaccine trials are only for cancer patients, obviously is ideal, testing vaccines in the appropriate population. But when that's not feasible, pre-existing cancer should not preclude eligibility, and inclusion of cohorts of people receiving anticancer treatment should be incorporated prospectively. So that's really critical because the quality of our guidelines is based upon the data. We use the data for developing guidelines and gathering more data in the particular patient population is really, really critical.  Secondly, work for creating more immunogenic vaccines and research to understand the immune response to vaccines after immuno-depleting therapies, particularly with newer therapies such as CAR T and newer B cell therapies, bispecific antibodies, etc. is really critical. We need to really understand the immune response and have the most potent vaccines available to these people who may have impaired immune responses.  Switching gears a little bit, we really need mechanisms to promote institutional commitment to integrate and sustain immunization best practices for people with cancer. This will largely be through multidisciplinary, team-based approaches, protocol-based vaccination standing orders, and leveraging data sharing so that we can all be on the same page with giving vaccines to these individuals. We also need education and evidence-based decision-making tools, emphasizing preventive care through immunization, the availability of educational resources to clinicians and patients to address commonly asked questions and also misconceptions about vaccination, that's absolutely critical.  And finally, I think we need to develop strategies for addressing unique challenges and factors contributing to vaccine hesitancy during cancer therapy. We need to focus on patient and clinician communication, and very importantly, we need to consider health equity considerations in the development and approach to vaccines in these compromised patients. Brittany Harvey: Definitely, we'll look forward to research and advances in these areas that you've just described to support these guidelines and increase vaccine uptake.  So I want to thank you both so much for your work on this important guideline, and thank you for your time today, Dr. Law and Dr. Taplitz. How did India waste over 100 million Covid vaccines?

Somewhere in May last year, when the second wave of pandemic was in the full swing, India was struggling to vaccinate its people. The government and vaccine makers were trading charges while Covid-19 was taking a heavy toll on human lives. India, which was home to the world's largest producer of Covid-19 vaccine, failed to inoculate the entire population during the course of that lethal wave. But a few months on, by the month of July, India became the only country after China to administer over two billion Covid vaccination doses. The vaccination drive, on scale, was hailed as one of the best in the world. So far, over 98% of adults have received at least one dose of the Covid vaccine, while 90% have been fully vaccinated. Nearly 22.7 million doses are available with states as of October 1. And just about 15 months later -- by the end of September -- over 100 million doses of vaccines ended up being wasted as their shelf life expired. Were there no takers of these booster doses or they simply didn't reach the needy? The loss, pegged at Rs 2,250 crore, fades in front of the loss it may have cost to people's health as the world is still facing acute vaccine inequality. The shelf life of Covishield is currently nine months, while that of Covaxin is 12 months. The shelf lives of both the vaccines were extended by three months last year. Non-Covid vaccines typically have a shelf life of three years. Why were 100 million doses wasted? As the pandemic waned, people became complacent. Masks disappeared from public places and there were few takers for booster doses. Over 84% of India's population is eligible for booster dose, but only 27% of them have received the shot. The booster coverage improved, thanks to a 75-day free booster dose campaign by the Centre. In mid-July, the booster dose coverage was at a meagre 8%. Despite the gains, India still has a long way to go in improving booster coverage. Had it not been the free booster dose campaign, the wastage would have been higher. In an interaction with Business Standard earlier, Adar Poonawalla indicated that he was trying to save around 50-100 million doses of vaccines in a free booster dose drive. Some amount of vaccine wastage is inevitable with such a large vaccination drive, however, governments try to minimise the damage. For all vaccines, GAVI recommends countries to keep the maximum wastage of 25% for the first year, with gradual reduction to 15% by the third year. For Covid vaccines, GAVI assumed the wastage rate could be 10%. If we look at the global scenario, the number of Covid-19 vaccines which are likely to have been wasted since the global roll out began is estimated to be 1.1 billion as of mid-July, according to data analytics firm Airfinity. “While wastage at any level is undesirable, it is the result of unprecedented vaccine production which has saved millions of lives. If we want a fast reacting global vaccine response system, we will have to accept some level of wasted doses,” says Airfinity CEO, Rasmus Bech. The other major reason why so many doses ended up being wasted in India was because of mis-anticipation of demand. Experts say companies were unclear about the purchase orders from the Centre and ramped up capacity in a big way. Serum had accumulated 250 million doses of formulated vaccines, and another 250 million in the bulk form by December last year and stopped Covishield production. Bharat Biotech too had a stock of 50 million doses lying at its plants around April and had halted production of Covaxin. The restrictions on exports did not help either. As India grappled with a deadly second wave in April last year, the government halted vaccine exports and resumed only towards the later end of the year. On this episode of This Thing Called Life, host Andi Johnson is talking with E. Denise Peoples. She is a comedian, actress, singer, writer, producer, radio host, and motivational speaker. She is a double lung transplant recipient, and most importantly, she is a Christian. African Americans are at a higher risk of heart disease. Research suggests that African Americans may carry a gene that makes them more salt sensitive, which increases the risk of high blood pressure and heart disease. Tune in for her story and great knowledge.   Episode Highlights:  In December 2000, while coming from the second floor of the house, Denise was short of breath, which lasted until February. She went to her primary doctor, and she was treated for pneumonia but nothing got better. Denise got diagnosed with idiopathic pulmonary fibrosis (IPF), which at that time was an older white male smoker's disease. The only alternative is a lung transplant or double lung transplant as this disease progresses. After being selected for a play audition and receiving health-related confirmation from a pulmonologist, Denise learned a lot about her body and herself while in Paris. When they came home at the end of 2003, things just started going down, and the disease was progressing so fast. Denise ended up on oxygen 24 hours a day.  One night around 10 o'clock Denise told her manager that she  was so tired of living like this. Soon she had a call from the hospital and they said, “Miss Peoples, we've got lungs, we need you to get to the hospital.”  The next day after surgery, Denise woke up and felt her face, but the nurse asked, “Are you looking for the cannula? She said it's there, but you are breathing on your own.” These are the absolute best words she had heard in years. Andi asks Denise, “You are with New Jersey Sharing Network, which procures organs and tissue for transplant, and they have 57 OPO's throughout the country. How did you end up there? What is emergency-use authorisation of Covid-19 vaccines?

It's been almost a year and half since India registered its first coronavirus case. And since then, we have been hearing new terms that define the pandemic's rise and fall. We have also seen a race across the world to produce a vaccine against this virus. A term which has been and is being used very frequently in the field of vaccines is'Emergency Use Authorisation' or EUA. But what is EUA? What is EUA and when is it given? In an emergency to diagnose, treat or prevent serious or life-threatening diseases or conditions, when marketing authorisation is given to an unapproved medical product, or the regulator authorises unapproved use of an approved medical product, it is called EUA. This is unlike full authorisation, for which the regulator assesses more elaborate data collected over a longer duration of time.  Now here is what it means for patients. The Indian regulator gave "restricted emergency use” authorisation to drugs like remdesivir. This means the doctor who prescribes the drug has to explain the risks and benefits to the patient and also clearly state that this drug does not have a full authorisation. The patient has to sign a consent form indicating that she or he has understood the implications. Now, let's look at the process by which a EUA is given. EUA is never granted based on only phase-1 and -2 clinical trial data (which are necessary to ascertain that the product is safe). EUA is considered only after sufficient efficacy data is generated in a Phase-3 trial. For Covid-19 vaccines, the USFDA has specified that it would consider an application for EUA only if the Phase-3 data show it is at least 50 per cent effective in preventing the disease. This data needs to be generated from over 3,000 trial participants who have been followed up for any serious adverse events for at least one month after all doses have been given. So why are vaccines being given EUA, not a full authorisation? The regulator would need long-term immunogenicity data. It means whether the vaccine induces the desired immunity against a disease pathogen from the trials or not. One may produce antibodies against the pathogen now, but it is important to see if the person also develops what we call memory-cell immunity. This means that if exposed to the pathogen after a long period of time he still produces antibodies. At the moment, no vaccine maker would be able to submit this long-term data. Regulators would thus review this interim data from the trials and decide if the vaccine merits an approval for use during the pandemic. Long-term data would be subsequently submitted to the regulator from time to time to get a full authorisation. So, after a vaccine gets EUA, it is first made available to the vulnerable population and eventually in the private market for all. Since we are in the middle of a pandemic, the government and policy makers would have a say in the way the vaccine is distributed so as to ensure effective mitigation of the risks to larger populations.

Chuck Anderson is an amazing musician. Not only is his technique on guitar remarkable, he is also a solid businessman who has made his living from music since he was a teenager. Over his long career, Chuck has worked with the top singers in the music business.But his steady passion is teaching. Since he was 16, he has taught thousands of students. He instructs them not only on the technique of the guitar, but also how to pursue their musical dreams, whether it is becoming a rock star, a studio musician, or just playing for a lifetime of enjoyment.In addition, Chuck has written 26 books, the first when he was 21 years old.I first met Chuck in 1974 when we recorded his first jazz trio album. That was my introduction to the serious jazz idiom. Since then, we recorded a couple of other albums, and recently we did a session where Chuck recorded an entire album of wonderfully improvised compositions in two hours. I didn't, but after talking for a bit, we came up with the idea of recording a new album of solo guitar, entirely improvised in the studio.Chuck came in late one Saturday afternoon in August, after a full day of teaching, and we produced the entire album in about two hours. Setup took minutes, and we were ready to record almost immediately. Chuck played the Gibson L-5 guitar he has used all his life. I used an AEA R88 stereo ribbon microphone on the studio Vox AC-30 amp, with a pair of Coles 4038 ribbon mics out farther in the room. It sounded great right from the first note, so I never changed anything.There were no retakes, no punches, and no overdubs. The recording was done 24-bit, 96kHz sample rate, using D.W. Fearn VT-2 and VT-24 mic preamps into a pair of VT-4 equalizers on the main pickup, with a bit of bass roll-off to compensate for the close mic'ing of the guitar amp. No other eq or other processing was used.The session was recorded with a Merging Technologies Hapi converter, using Pyramix software. During the mix, a bit of Bricasti M7 digital reverb was added.A week later, we recorded this interview, using a pair of AEA R44 mics. Today, we are joined by Jaz, a Ugandan American bride, and her groom Rodney, a Haitian American, who are getting married in October. She reached out after episode 200 to open up a larger conversation about wedding planning while Black. Big Takeaways Suggestions from Jaz's email: 1) Highlighting a current black bride and her planning - not removed from the state of events but not only about that. Black women are disproportionately affected by covid, we are trying to stay positive and constructive during this enraging national uprising, and we still have to sift through wedding industry racism. I'd be willing to share my story with my fiancé on the show if you'd be open to that. 2) Please consider racial inclusion form this point forward. It still doesn't sit well that only two episodes touch upon race. And may seem small but some of your language is marginalizing. Talking to a stylist in one of your episodes about “skin color fabric” who's skin color? Is my skin not “normal”? Or giving advice on makeup and hair and ignoring the nuance and extra labor that black brides have to face to get the service we deserve. It's important and it ripples to brides and other industry professionals. Life after the uprisings, advice from Jaz: Approaching uninviting certain guests who aren't true allies. 1) Completely cut friends out of my life and reevaluating others; when I look out on my wedding day certain people would be a painful reminder that they don't fully see my humanity. 2) Considering breaking ties with vendors who aren't true allies (this is the hot topic in bridal communities right now for brides across cultures/race as they uncover guests who don't align with their values in light of everything going on.) 3) Reevaluating where I get my inspo (no longer supporting publications that ignore black brides) because it perpetuates the idea that our love doesn't matter. Advice from Jaz, in retrospect, and for current brides at the beginning of wedding planning: 1) Lean on your fiancé/fiancee! Rodney handled typical groom stuff like finding our band and handling his attire. But he also completely handled our website, guest list management, engagement shoot (in Morocco!), multi leg honeymoon planning, room-blocks/venue, budget management (Aisle Planner pro) and both our wedding bands! Pretty much anything that isn't planner related he's running it so I can focus on overall vision, design, and vendor management. If he hadn't been so hands on with all the things going on I don't even know where we'd be. 2) Marie Kondo it - we made a pact if something annoyed us, we ditched it. Guestbook, fancy invite designers (Minted baby), registry etc. My guests keep saying “I wish we ditched XYZ too it was such a headache”. New couples you can do that now! 3) For COVID brides (because this may drag on for the foreseeable future!), pay attention and support vendors that give couples some grace. My venue has been incredible and flexible and open to any solutions we put out there. On the other side, I had a potential makeup artist who was recommended left and right but wouldn't budge on a ridiculous client minimum of 10 different people getting services. For the foreseeable future, that's not going to be a thing. I don't think I have 10 guests total who would feel comfortable with getting a makeup service done this fall from a stranger. I personally think they're killing their own prospect conversion and it sends a terrible signal to couples that they aren't adjusting to the current state of affairs. 4) Shorter timeline brides (less than 9 months): you have to do your research and nail down a dream team much faster. I liked that because I loved checking things off but it can be a disaster if you don't know what you want yet or don't have clear budget or a planner helping. 5) Biggest learning from the short time window - try on and order a dress like yesterday if you want made to order (I'm sample size but so short that sample dresses weren't even an option)! 6) Have real, tough conversations - ask questions. If a vendor is not receptive or responsive - they're not the right vendor for you. Don't feel like you can't ‘ruffle feathers.' 7) You described to us how you felt almost forced to hire certain people and only after hiring them could really see where they stood. 8) Make vendors SHOW you not just give lip service. “If you're really representative, tell me about 5-6 black couples and vendors you've worked in the past few years”. 9) Tap into the resources: Black Brides, Munaluchi, B Collective, Junebug black vendor round ups and reach out to vendors you like and ask who they like working with. It took me way too long to realize that black/non-black ally vendors will warn you about venues and professionals who won't align with you. 10) Don't be afraid to travel your professionals if you can. I spent more mindshare trying to find perfect LA vendors than what travel fees would have cost me. Other cities may have way more options and traveling them in from more diverse industry pools could help you build a dream team (particularly the planner)
