Gyalia Rutledge, a highly experienced clinical nurse and legal nurse consultant, got the idea for it when in response to an attorney's calling her about the viability of a case, asked him questions he couldn't answer. Our learners often complain that they do not get good feedback from us as faculty. Providing actionable, formative feedback in real-time while providing patient care in a busy inpatient setting is hard. This week we talked with three neonatologists who asked us to help them to give feedback better. Listen to them describe the two formative feedback models they are now using in a busy NICU setting. Leeann Blaskowsky is a board-certified neonatal nurse practitioner.  In her words:  since 2009 I have been living and working in Colorado, and I am faced, almost daily, with counseling families about marijuana use during pregnancy and breastfeeding. My nursing education began with an Associates degree waaay back in 1984, continuing to a Bachelors and Masters in Nursing more than twenty-five years later, finally finishing with a Doctorate of Nursing Practice in 2020. With more than 40 years in healthcare beginning as a bedside nurse in the ICU/CCU, Emergency Department and Neonatal ICU settings, I have had the opportunity to interact with patients and families with varying opinions and practices regarding the use of marijuana. These interactions have expanded in scope and increased in number as medical marijuana and legalized recreational use arrived in Colorado. I have also cared for many patients struggling with addiction to various substances including mothers and newborns. Optimizing outcomes for our patients has, and always will, remain my driving force in practice when a clear need to continue educating providers and families in our communities arises. Our guest today is Rebecca Wood. Rebecca lived all over Virginia before her relocation to Massachusetts. Rebecca worked with many organizations and offices on Capitol Hill. Her healthcare work includes rallies, protests, press conferences, and conventions. Most notable, she told her and daughter Charlie's story at the introduction of Senator Sanders' Medicare For All Act of 2017 and testified before the House Ways and Means Committee hearing on Pathways to Universal Coverage in June 2019 (Rebecca's statement begins about at about 18:10). Rebecca also regularly volunteers for Remote Area Medical, which provides access to healthcare for underserved communities and is active in the push for universal school meals. https://www.youtube.com/watch?v=WK1XiF5Rco4 Show Notes CW: Medical and dental trauma We don't usually start our podcasts with data or statistics, but last week Gallup released a shocking poll that clearly reflects a new reality for the country: 38 percent of Americans now say they put off medical treatment because of the cost sometime in the past year (2021). This is a HUGE leap over just a year ago, when only 26 percent of Americans were putting off care. Gallup has been conducting this poll for over 20 years, and this is the worst we have ever seen. Even more troubling, 26 percent say they put off treatment for “very serious” conditions or “somewhat serious” conditions, and THAT is another huge leap over just a year ago, when “only” 18 percent of Americans were putting off care for very serious or somewhat serious issues. We're now three years into the pandemic, and employment rates have improved. So what the fuck is going on here that our access to healthcare is now worse than during the height of the pandemic when the economy was shut down and there were no vaccines?!? Rebecca tells a story that illustrates how delayed care hurts American families. Ten years ago her daughter Charlie was born prematurely due to Rebecca's severe early onset pre-ecclampsia, a potentially deadly condition for both mom and baby. Charlie was in the Neonatal ICU for three months. Their family had "good insurance" through Rebecca's then-husband's employer. Over the next several years, like many micro-premies Charlie needed enormous amounts of care. They faced automatic denials, outrageous deductibles and huge copays from their insurance company. Due to the high out of pocket costs for Charlie's care, they went from being financially comfortable, to going through their savings, putting off their plan to buy a home, and living paycheck to paycheck. Even worse, Rebecca had to make choices between paying for her daughter's healthcare and her own. Like so many American parents, Rebecca put Charlie's needs first, sometimes rationing her own medications and eventually putting off an important dental procedure. That delayed dental procedure led to an infection that spread to her entire mouth and jaw, forcing her to the emergency room. Doctors feared the swelling could cut off her airway. After days in the hospital, Rebecca followed up with her dentist, where she had to have all of her teeth pulled and parts of her jaw scraped away. In 2017 when the Affordable Care Act came under attack, Rebecca feared repeal would make Charlie uninsurable due to preexisting conditions. So Rebecca became a vocal activist. She used her family's experience to call out the profound policy failure that has caused so many American families to suffer. The new Gallup Poll found: “In 2022, Americans with an annual household income under $40,000 were nearly twice as likely as those with an income of $100,000 or more to say someone in their family delayed medical care for a serious condition (34% vs. 18%, respectively). Those with an income between $40,000 and less than $100,000 were similar to those in the lowest income group when it comes to postponing care, with 29% doing so. She started her family at a very young age, and after her second child was born emergently three months premature, she knew she wanted to be a L&D or nursery nurse, so she returned to Georgia to attend Columbus State University for her undergraduate nursing degree. While in nursing school she gave birth to her third child under the care of a certified nurse midwife (CNM). Immediately, she knew that was exactly she wanted to become: a certified nurse midwife. She graduated as a Registered Nurse in 1999 and immediately began working in her favorite area of nursing, Labor & Delivery. Over the years as an L&D nurse, she felt honored to provide care to thousands of families during one of the most intimate and memorable events in their lives. Before starting graduate school to become a CNM, she also pursued an internship and worked a few years as a Neonatal ICU nurse. It was important to her to learn more about what happened to the preemie and sick babies when they left the delivery room and how could she best educate and support her future patients as a CNM. During her pregnancy, Sonal was the Hollywood prototype of a pregnant woman - a perfectly round belly, glowing skin, and luscious flowing hair. But life after birth was much different for Sonal. Postpartum tore her apart physically, emotionally, and mentally - leaky, engorged breasts accompanied by waves of intense pelvic pain, diapers on both baby and her, a sense of isolation, and tears never portrayed in any Hollywood blockbuster. Sonal is a physician, specifically a pediatrician further trained in neonatology. Her story concerning postpartum, or the 4th trimester, is not an isolated tale. Many women undergo it far worse. Serena Williams, the greatest of women's tennis, almost died after her c-section. She developed a pulmonary embolism, a lung clot, requiring two surgical operations, and spent her first six weeks of motherhood in bed, half her 4th trimester. In doing more research on the 4th trimester, the statistics are staggering. America's maternal mortality rate of 23.8 maternal deaths per 100,000 live births ranks last in first world nations though spending the most in healthcare dollars.The lastest data reported in September 2022 showed that 22% mothers die in pregnancy, while 78% die after birth, with 25% in the first week of delivery. 4 in 5 deaths are preventable.Interstate disparities find that a mother in Georgia has ten times higher chance of dying versus a California mother.Among black women, maternal mortality is three times higher than that of her white counterparts. Extrapolating the data further, a black woman's higher education level and socioeconomic status does not protect her from these alarming disparities.Women are income-earners through outside avenues of employment as well as through traditional household duties. A new mother's unexpected death not only leaves behind a newborn and maybe other children, but jolts a household's economic foundation.  In 2022, the Council of Foreign Relations estimated that if women were part of the US labor force at the same rate as men, America's economy would grow by $4.3 trillion in five years.   As Dr. Patel says, "There must be a better way." ...and a better way for the 4th trimester will take everyone. Sonal is doing her part. After her own experience, Dr. Sonal Patel transitioned from her Neonatal ICU job and began a new career as a traveling 4th-trimester doctor. Carly Kelly was born with autosomal recessive polycystic kidney disease, a disease that not only affects the kidneys, but also can lead to liver problems. She spent the first month of her life in the Neonatal ICU at Mayo Clinic, where physicians told Carly's family she would eventually need a kidney transplant. "I was the first one to register as a donor," says Tim Kelly, Carly's father. "And I was so blessed to be a match. Carly and I have the exact same blood type: AB negative. There are so many people out there that are waiting for kidney and other organs. It is such a long waitlist, and to be able to give my daughter a kidney right off the bat was so fortunate for both of us."At age 8, Carly had her first organ transplant, receiving a kidney from her father. In 2019, Carly went into liver failure she received a liver transplant on May 2, 2019. 10 months later, Carly received her second kidney transplant. All three transplants have taken place at Mayo Clinic in Rochester, Minnesota.Now 27 years old, Carly is doing well and hasn't let her transplants slow her down. She and her father will compete in the 2022 Transplant Games of America, which will take place July 29-Aug. 3 in San Diego. The games are open to transplant recipients, living donors and donor families in different categories.Carly and Tim will be part of Team MN-DAK, which is sponsored by LifeSource. This will be their third time competing in the games, taking part in cycling, pickleball and cornhole. They've been preparing together for the event."Every day, we'll bike, or we'll go for a walk, or we'll play pickleball or we'll throw some bags," says Carly. "Being together and preparing, it's something just my dad and I do, which is really cool. It's not really about competing. It's more about honoring your donor." There are nearly 106,000 people in the U.S. waiting for a lifesaving organ transplant. Tim and Carly both say that competing in the games is not only about honoring the donors, but also raising awareness. "Please registered to be a donor," implores Tim. "One person can save up to eight lives. And it's easy thing to do. She received her Bachelor of Science in Nursing from the University of Texas and has over 35 years of experience practicing as a Registered Nurse specializing in Neonatal ICU.  In addition, she has continued her education to include Trauma Informed Equine-assisted learning from The Natural Lifemanship Institute having completed the Fundamental and Intensive course sequences.  Leigh utilizes equine-assisted learning with a mindful approach to guide her clients to their inner knowing and promote self-awareness of life's recurring patterns. Her coaching is rooted in offering a meaningful experience centered on building a relationship with the horse as a partner, then allowing and supporting the thoughts or emotions that arise in the space of that relationship and exchange. Holding the belief that all relationships are sacred and it is in the support of relationships that we can explore our true essence.  Leigh is also a trained astrologer and regularly integrates this education with her equine coaching.  I met Leigh at the Miraval Wellness and Spa Resort in Austin, Texas in February of this year. Happy Friday Friends!My next guest has shared her story in the medical field juggling being a mother. I love her openness and her vulnerability about witnessing some very hard cases being in the Neonatal ICU. Please welcome PREMIERE guest Dr. Susan Landers. Susan has 34 years of experience in the neonatal intensive care unit (NICU). She practiced in academic medicine (on faculty of two medical schools) and in private practice. She found her work in the demanding environment of the NICU rewarding & managed to postpone burnout until the end of her career. She and her physician husband raised three children (now all young adults) while they both practiced medicine full time. She recently wrote a memoir called "So Many Babies: My life Balancing a Busy Medical Career and Motherhood." Susan enjoys recounting some of her best, and worst, experiences of being a working mother, and how she managed to stay resilient. She shares with other working mothers many things she learned along her journey as a busy mother and successful doctor.She attended medical school at the Medical University of South Carolina in Charleston and completed her pediatrics residency at University of Texas Southwestern Medical School and Parkland Hospital in Dallas. She completed her neonatology fellowship at Texas Children's Hospital and Baylor College of Medicine in Houston. She has special expertise in breastfeeding medicine, and human donor breastmilk banking. She previously worked for the American Academy of Pediatrics as a leader in the Section on Breastfeeding Medicine. Happy Friday Friends!My next guest has shared her story in the medical field juggling being a mother. I love her openness and her vulnerability about witnessing some very hard cases being in the Neonatal ICU. Please welcome PREMIERE guest Dr. Susan Landers. Susan has 34 years of experience in the neonatal intensive care unit (NICU). She practiced in academic medicine (on faculty of two medical schools) and in private practice. She found her work in the demanding environment of the NICU rewarding & managed to postpone burnout until the end of her career. She and her physician husband raised three children (now all young adults) while they both practiced medicine full time. She recently wrote a memoir called "So Many Babies: My life Balancing a Busy Medical Career and Motherhood." Susan enjoys recounting some of her best, and worst, experiences of being a working mother, and how she managed to stay resilient. She shares with other working mothers many things she learned along her journey as a busy mother and successful doctor.She attended medical school at the Medical University of South Carolina in Charleston and completed her pediatrics residency at University of Texas Southwestern Medical School and Parkland Hospital in Dallas. She completed her neonatology fellowship at Texas Children's Hospital and Baylor College of Medicine in Houston. She has special expertise in breastfeeding medicine, and human donor breastmilk banking. Breathing in Indian cities has been equated to being a regular smoker. WHO tells us that air pollution kills about 7 million people worldwide each year. In India alone, there were as many as 980,000 deaths in 2019 because of particulate matter. But these are numbers. Statistics don't touch us. Data doesn't shake us. We hear numbers, but we are moved by stories. She started her nursing career in the Neonatal ICU where she worked while she pursued her Masters in Family Nurse Practitioner. The Pacific Northwest National Laboratory is moving past energy efficiency research and into researching better life indoors. One of their projects was looking into ‘realistic’ lighting in an Neonatal ICU. Andrea Wilkerson, senior lighting researcher, shared her findings for sun-like colour and brightness automation. Beyond what happened during the test we explore some of the broader implications of circadian entrainment and it’s ability to signal behaviour and mood in subtle ways. We also settle the question of whether or not we can meaningfully or honestly label something as ‘Circadian-Friendly’ and continue to probe into why we see so many women in lighting research.

My guest for The Happy Hour # 238 is Jennifer Welker. Jennifer is a former Neonatal ICU nurse turned founder and designer of the jewelry brand, Golden Thread. Jennifer then opens up to share how deep wounds in her childhood led her to live life with a self savior mentality - one where on the outside everything looked "normal", but on the inside she was empty. She tells us how this spiraled downhill fast as she entered into an affair that would last years. I appreciated so much that Jennifer gives us the raw truth about the darkness, codependency, and toxic addiction that the affair was. I know you will be so moved as you hear Jennifer speak of when she hit her lowest, and the moment she tangibly felt God remove the scales from her eyes. Carly Kelly was born with autosomal recessive polycystic kidney disease, a disease that not only affects the kidneys, but also can lead to liver problems. She spent the first month of her life in the Neonatal ICU at Mayo Clinic, where physicians told Carly's family she would eventually need a kidney transplant. "I was the first one to register as a donor," says Tim Kelly, Carly's father. "And I was so blessed to be a match. Carly and I have the exact same blood type: AB negative. There are so many people out there that are waiting for kidney and other organs. It is such a long waitlist, and to be able to give my daughter a kidney right off the bat was so fortunate for both of us."At age 8, Carly had her first organ transplant, receiving a kidney from her father. In 2019, Carly went into liver failure she received a liver transplant on May 2, 2019. 10 months later, Carly received her second kidney transplant. All three transplants have taken place at Mayo Clinic in Rochester, Minnesota.Now 27 years old, Carly is doing well and hasn't let her transplants slow her down. She and her father will compete in the 2022 Transplant Games of America, which will take place July 29-Aug. 3 in San Diego. The games are open to transplant recipients, living donors and donor families in different categories.Carly and Tim will be part of Team MN-DAK, which is sponsored by LifeSource. This will be their third time competing in the games, taking part in cycling, pickleball and cornhole. They've been preparing together for the event."Every day, we'll bike, or we'll go for a walk, or we'll play pickleball or we'll throw some bags," says Carly. "Being together and preparing, it's something just my dad and I do, which is really cool. It's not really about competing. It's more about honoring your donor." There are nearly 106,000 people in the U.S. waiting for a lifesaving organ transplant. Tim and Carly both say that competing in the games is not only about honoring the donors, but also raising awareness. "Please registered to be a donor," implores Tim. "One person can save up to eight lives. And it's easy thing to do.
