I'm here with Abel Sandmann from Amsterdam University Medical Center, and we just finished recording a full-length podcast about some exciting findings related to cerebral cavernous malformations and the risk of seizures and epilepsy. Abel, can you give our listeners a rundown of the most exciting findings and how it can change practice? Dr. Abel Sandmann:  In our paper, we show that patients with a cerebral cavernous malformation who have a first unprovoked seizure should be diagnosed with epilepsy and considered for anti-seizure medication, as most of them achieve long-term seizure freedom with medical therapy alone. These findings are based on a prospective population-based cohort study in which we analyze long-term follow-up and assess the rates and risk factors for: one, a first-ever epileptic seizure; two, seizure recurrence to evaluate the updated ILAE definition of epilepsy; and three, seizure freedom over two years and five years among patients with epilepsy. We found that among patients who had never experienced a seizure before, the 10-year risk of a first-ever seizure was only 6%. This supports current recommendations against prophylactic anti-seizure medication in patients who are incidentally diagnosed with a cerebral cavernous malformation. However, following a first unprovoked seizure, the 10-year risk of recurrence was 80%, which exceeds the 60% threshold defined by the ILAE. This justifies diagnosing epilepsy after the first and provoked seizure in this population. Given that the risk of recurrence was lower in patients treated with anti-seizure medication after the first seizure, this supports early initiation of therapy, although these treatment analyses were non-randomized and should be interpreted cautiously. Most patients who met the definition of epilepsy became two year and five years seizure-free with medical management alone. But some patients with cerebral cavernous malformations develop medically intractable seizures and might benefit from surgical treatments. Dr. Halley Alexander: Excellent. Thank you so much, Abel. These are examples, yet choices are endless.   •  brussel sprouts: cut off tough end and cut in half   •  bell peppers: cut into chosen pieces; discard pith and seeds   •  cauliflower: cut into 1/2 inch slices - OK if you have small pieces and larger pieces and even crumbs   •  carrots: cut on diagonal about 1/2 inch slices   •  tomatoes: cut in half if large and put flat side down, if cherry - leave whole   •  purple cabbage: cut off tough end only and slice roughly 1/2 inch slices   •  onions: any color, stem and root end removed, quartered and peeled   •  summer and winter squash, peeled if needed and 1/2 inch slices•  1/4 - 1/2 tsp Kosher salt•  1/4 tsp pepper•  1 - 2 tsp Olive OilFor Green Sauce:•  4 pressed cups washed greens and hard stems/veins removed (cilantro, kale, parsley, spinach, arugula as examples.•  3/4 cup olive oil•  2 tsp lemon zest (from about 2 lemon)•  2 Tbsp lemon juice (from about 2 lemons)•  1 Tbsp red or white wine vinegar•  1/4 - 1/2 tsp Kosher saltFor topping (optional)•  1/2 cup Pomegranate seedsMethod:Heat oven to 425FPlace all vegetables in a large bowl. Drizzle vegetables with measured Olive Oil and rub with hands to make sure well coated. Sprinkle with 1/4 tsp salt and pepper.  For roasting: Place vegetables on baking sheets lined with aluminum foil or parchment paper, single layer Roast for 10 minutes, and then set timer at 5-minute intervals until desired doneness (goal is some brown bits/caramelization, and heartier vegetables such as cauliflower, cabbage, carrots, winter squash and brussel sprouts to be fork tender on inside.) In today's episode we talk about the Food is Medicine Movement.  My guests are Drs. Jaclyn Albin and Minako Abe.  Food Is Medicine is the provision of healthy food to prevent, manage or treat chronic disease within our health care system by closing the gap between a medical nutrition prescription and the ability of a patient to fill and consume it on a regular basis. We reference the 2023 paper published in Circulation: Food Is Medicine: A Presidential Advisory From the American Heart Association by Volpp KG et al Jaclyn Albin, M.D., CCMS, DipABLM, is an Associate Professor in the Departments of Internal Medicine and Pediatrics at UT Southwestern Medical Center. She specializes in internal medicine, pediatrics, lifestyle medicine, and culinary medicine.  Minako Abe, MD, BCEM, DipABLM has dual board certifications from the United States in Emergency Medicine and Lifestyle Medicine. She has had 15 years of clinical experience in Emergency Medicine in New York and New Jersey.  Since 2014, she has worked with the Hakushin Koseikai Medical Foundation and as a director of the International Society for Personalized Medicine. She is the vice-president of the Tokyo Cancer Clinic and is actively engaged in research and activities to further the fields of Lifestyle, Cellular Immunotherapy, and Regenerative Medicine.
