Wow! There have been so many crazy things going on in our world! How do we stay positive when so much negativity surrounds us? KNOW! We have HOPE in Him! Isaiah 12:2 tells us “God is my salvation; I will trust and not be afraid. The LORD, the LORD himself is my strength and my defense; he has become my salvation.”   I hold tight to this verse. When feelings of overwhelm and uncertainty arise, I go to Him and say this verse aloud. As I recite this verse a feeling of peace surrounds me and I know that He has me!   Let me encourage you today! Stay positive and KNOW that God is in control. No matter what is going on around me, I will sing His praise. In good and in bad I will praise HIM. When I notice that I am feeling anxious or am beginning to worry, I recognize the thought and then I proceed to say aloud everything that I am thankful for. I am surrounded by abundance! As I say aloud all of the things that I do have going for me; I am reminded how truly blessed I am and how abundance is everywhere. I was reminded this morning during my meditation time that God is Good, ALL the time. Whether we choose to recognize this is really up to us. We can choose abundance, joy, wealth, it's all there we just have to claim it!   Some Tips for Staying Positive: Take time to meditate – Taking time to meditate in the morning really gets the day started off right! Recite “Morning Affirmations” – Each morning I recite approximately 10-15 affirmations. It takes about 5 minutes but it reminds me that I am made for MORE! (You can find affirmations by googling or even on Pinterest) If you need help, email us and we would be glad to help you with this. Surround yourself with other positive people – Having the right support group is essential for living a stress-free, positive lifestyle. A high value man is a provider, someone who makes you feel safe mentally, emotionally and physically. A high value man is financially stable, makes sacrifices, makes you a priority in his life and puts family first.   What's the key to finding a high value partner? The key to finding a high value partner is finding yourself first. You must know yourself so that you can easily identify partners who can give you what you want and communicate to them clearly and accurately your specific needs. The following are steps to finding a high value partner:   Step 1: You must recognize your own patterns. Most of your insecurities stem from past traumas and/or childhood. You MUST commit to doing the inner work required to fix yourself and heal your past traumas so that you can attract a whole, healed and supportive partner.   Step 2: Make yourself Available Physical Availability-- Online dating is a great place to meet people. You may also need to go out more, specifically to places where you can find the type of partner you want to attract. Some Tips for making yourself more available: When online dating, set your location to where you believe your potential partner might be. For example, set your location to the more affluent area of town, if that's where you think your potential partner lives or hangs out. When you go to the grocery store or Target, go to the fancy target or grocery to increase the likelihood of finding the man that you're looking for. Wear clothes that you wouldn't mind your future potential partner seeing you in.   Emotional availability—You must make yourself emotionally available to receive the love you want to attract. This means doing your inner work to heal yourself and cultivating a positive outlook on dating in general by being mindful of the content you consume and the people who you allow to poor into you.    Step 3: Set Boundaries and Communicate Them Know your deal breakers so that you can communicate those and allow men who cannot meet those expectations with opportunities to de-select themselves.   Step 4: Allow Men to be Men If you treat a man like a child, even a high-value man, he will act like a child. Don't get in the habit of doing and being everything for your partner. Allow them the space to do and figure things out on their own. This will help them to be a better, more supportive partner to you when you need help. Meditation is something that we're constantly being encouraged to use. Every self-help guru, every highly successful individual and even many athletes trumpet its many benefits and the research too seems to back up its value. So why don't more people practice it? The main problem for most of us is that it's really rather daunting, obtuse and complicated. Meditation is ultimately about reaching enlightenment and inner peace, right? Sounds a bit heavy for a Friday evening! The real question for many people then is where to start. This article will provide you with a good starting point and help you with your first meditative experience. From there, you should feel a little more confident to try it again in future… Some Tips to Begin With The first tip is to set yourself a timer for 10 minutes. 10 minutes is a short enough amount of time that most of us will be able to fit it into our busy schedules and by setting an alarm you prevent yourself from having to keep checking the clock to see how much longer you have – this is not conducive to meditation as you might imagine. The next tip is to sit comfortably in a chair or cross-legged. You don't want to lie down for fear of falling asleep, but you should be comfortable. Focus The next thing you're going to do is to focus. This can mean focusing on your breath or repeating a mantra (a word of your choice) over and over. This will be your ‘anchor' and you will come back to this whenever your mind starts to wander. If you struggle with these anchors, another option is to watch a flame. Lighting a candle and watching it can be a surprisingly effective form of meditation. Just ‘Be' Now just ‘be' for 10 minutes. The mistake many people make here is to try and force themselves to have a ‘still mind' devoid of thoughts. This is almost impossible for a beginner and will lead to nothing but stress. Instead, we'll take the mindfulness approach of simply letting the mind wander. When it does, make a note of it and simply focus back on your anchor. This removes the stress and gives you a safe environment in which to practice directing your attention inwards. The same goes for itching and coughing – just let it happen and then return. Try to repeat this three times a week for a couple of months and see what happens… You'll be glad you did!

Lady Change Makers with Jessica Bargenquast One of the most challenging things for women business owners is getting their business in the spotlight. While we talk a lot about women supporting women, it's much difficult to apply in practice in a men-dominated industry. Having platforms amplifying women's voices is critical to even out the playing field in the competitive world of entrepreneurship. With the help of these platforms, we can open up more opportunities for women to build success. In today's episode, Jessica Bargenquast talks about the ins and outs of Lady ChangeMakers. She discusses the reason behind building this directory for women business owners. She also takes us through how she dealt with its technical aspects. Jessica also shares how she navigates the extroverted world of entrepreneurship as an introvert and her favorite parts of running the platform. If you want to support and amplify the voice of women business owners, this episode is for you! Here are three reasons why you should listen to the full episode: Find out how to support women business owners through an online directory. Discover how you can start a website and app for your business that's not from scratch. Learn how you can get out of your comfort zone as an introvert while staying true to your nature. The 9 Enneagram Types AppSumo WordPress Squarespace Mighty Networks Powerhouse Women Mind Your Business by Ilana Griffo Episode Highlights Supporting Women Business Owners Lady ChangeMakers started with Jessica's desire to support women business owners but not finding an easy way to do it. That's when she thought of creating an online directory that people would want to use. 83% of women want to support women-owned businesses; 62% don't know where to find them. More Than a Directory Lady ChangeMakers is community-focused. It aims to get women-owned businesses potential customers and help them connect to other women business owners. Jessica's goal in the platform's listing is to make people want to learn more about the businesses. Building the Directory Jessica does not have a technical background. It took her around six months to figure out how to get the directory running. Her tip: Take advantage of available templates and customize them accordingly. Making the Platform Engaging There are a lot of questions to answer when filling out the form for the directory. This is to let potential customers get to know the business. The questions in the form aim to better cater to the needs of different women business owners. They have also launched a Facebook group to interact and connect one-on-one with the people in the community. Navigating the Extroverted World as an Introvert Jessica admits that she does better digitally than in person.  Jessica created the brand so she didn't have to show her face. However, she learned that she needs to show up to make a connection. Showing up requires consistency. As an introvert, showing up is difficult; it can be draining. That's why Jessica's coach gave her advice to create content ahead of time.  Listen to the full episode to learn about the challenge that made Jessica start taking action and showing up on video! Jessica's Background Jessica came from marketing and advertising.  Her goal was to be a CEO by 30.  With this in mind, she started an agency. However, it failed.  Jessica likes being behind the scenes, looking for solutions, and building things.  When she learned to show up and speak up, she realized she could be an entrepreneur. Jessica's Favorite Parts About the Business Because the directory gets feedback from users, the team can constantly add new features to it. Through this, Jessica learns something new to do in the directory.  She also enjoys learning about the business that goes through the directory.  The Businesses in the Directory Anyone who supports women can be in the directory.  One man, who is a fitness trainer for women, currently has a listing in the directory.  There are now almost 700 women business owners in the directory.  There are also listings in Canada, the UK, and Australia. How to Submit a Listing Fill out and submit the listing form at Lady ChangeMaker's website. There are three listing options available: A free listing and two upgraded options.  These options have different benefits attached to them. Listen to the full episode to find out more.  Some Tips on the Technical Side of the Business A PWA (progressive web application) is a website for phones that act like an app without being an actual app.  PWA clones the website for phones. All Jessica had to do was create thumbnail images for different types of phones. Jessica recommends Squarespace for creating websites unless you want to customize. So I never want there to be that barrier for women to join.” [28:50] “My best advice I can give you is don't feel like you have to do all the things… What's the one platform you can do really well? Last year, I chose Instagram. So that was the one thing I was going to commit to every single day… And then just try and grow from there.” [31:10] “I think a lot of history for women has been kind of just glossed over... A Guide to Your First Meditative Session for Beginners

Meditation is something that we’re constantly being encouraged to use. Every self-help guru, every highly successful individual and even many athletes trumpet its many benefits and the research too seems to back up its value. So why don’t more people practice it? The main problem for most of us is that it’s really rather daunting, obtuse and complicated. Meditation is ultimately about reaching enlightenment and inner peace right? Sounds a bit heavy for a Friday evening! The real question for many people then is where to start. This article will provide you with a good starting point and help you with your first meditative experience. From there, you should feel a little more confident to try it again in the future… Some Tips to Begin With The first tip is to set yourself a timer for 10 minutes. You don’t want to lie down for fear of falling asleep, but you should be comfortable. Focus The next thing you’re going to do is to focus. This can mean focusing on your breath or repeating a mantra (a word of your choice) over and over. This will be your ‘anchor’ and you will come back to this whenever your mind starts to wander. If you struggle with these anchors, another option is to watch a flame. Lighting a candle and watching it can be a surprisingly effective form of meditation. Just ‘Be’ Now just ‘be’ for 10 minutes. The mistake many people make here is to try and force themselves to have a ‘still mind’ devoid of thoughts. This is almost impossible for a beginner and will lead to nothing but stress. Instead, we’ll take the mindfulness approach of simply letting the mind wander. When it does, make a note of it and simply focus back on your anchor. This removes the stress and gives you a safe environment in which to practice directing your attention inwards. The same goes for itching and coughing – just let it happen and then return. Try to repeat this three times a week for a couple of months and see what happens… You’ll be glad you did!

Welcome back to the Neuroscience Meets Social and Emotional Learning Podcast EPISODE #109 with a special episode where I want to take a minute to thank anyone who has ever downloaded an episode or come on a guest to the podcast. Without you (the listener) or our guest speakers, there is no podcast, and there would be no chance that we would have hit our milestone of surpassing 50,000 downloads since our launch in June 2019. I want to take a minute to share why hitting 50,000 downloads is such a big deal and why we couldn’t have done this on our own. It took a year and 8 months to attain this goal (that I’ve had my eye on since last summer) and if you are not a podcaster, these numbers don’t have as much meaning as those out there who create and release content and wonder—will anyone actually find what I have just created interesting, helpful and meaningful? When starting anything new, you are building something out of nothing, and the only way to do this is to have a vision of where you are going. For me, it started with “I hope I can get 100 people to listen to this podcast. That would be incredible!” Our first month we had 27 downloads (probably my good friends and family—thank you!), month 2 was 204 downloads, month 3, 248, then 396, jumping up to 1,648 in December 2019. What’s crazy is that if I look back to February 2020, or this time last year, our monthly numbers started to hit the thousands each month, but we were still just at 7,800 downloads in total. I’ve included a graph in the show notes so  you can see these results. If you are listening on iTunes, go over to Podbean to see these graphs. https://andreasamadi.podbean.com/  So, How Did We Go From 7,800 Downloads to 51,800 Downloads in Just One Year? When we launched the podcast, we did one episode a week, except for the beginning of the Pandemic last summer, we released 3 episodes in a week a couple of times, just due to the fact that we had extra time, most people we wanted to interview were available, and we knew the more episodes we released, the higher the downloads, which is exactly how the numbers increased so quickly.  If we want quantum leap results, it takes effort, focus, persistence and hard work.  There’s actually a formula that I learned in the late 1990s when I was given Price Pritchett’s book “You Squared: A High Velocity Formula for Multiplying your Personal Effectiveness in Quantum Leaps.”[i] Who doesn’t want to achieve exponential improvement in their performance, or multiply their personal effectiveness? “You don’t have to be content with improving things incrementally or gradually—You Squared implies an explosive jump in your personal performance that puts you far beyond the next logical step.” (Price Pritchett) So of course, I was looking for that exponential leap with our numbers, and not sure what the secret was. Since I always have the You Squared book on my desk, one day I looked up Pritchett and contacted his offices to see if I could have him on the podcast to discover what I was missing and share it with our listeners at the same time. He isn’t doing interviews anymore, but I will have one of his team members on this year and we will dive deeper into the principles of his book. The tips I’m going to share with you on this episode today are for anyone who is building something from nothing. You might have an idea in the back of your head, and aren’t sure where to begin, or you’ve been given a new sales territory, and you just can’t see how you will accomplish your goal. Whatever it is that you are looking to do, there are some secrets to achieving your goals, and I highly recommend reading Pritchett’s book, You Squared to begin with. Here’s Some Tips for Accelerating Your Rate of Achievement, and they all involve a shift in your mental mindset. Tip #1 Learn from Those Who Have Already Paved the Path Seek out those who have risen to the top, and model what they are doing. They had to have started where you are at some point. Whatever you are doing, there is always someone who has gone before you, and achieved your goal with ease. How did they do it? Ask them, and then be prepared to do exactly what they say. I watched other podcasters to see how they hit this milestone, and I remember that Lewis Howes, the host of one of the top podcasts, The School of Greatness, said something along the lines of “to get more downloads, I knew I had to release more episodes.”[ii] Since its launch in January 2013, the School of Greatness podcast has grown rapidly to be one of the top-ranked Business and Self-Development podcasts in iTunes. It regularly appears in the Top 50 of all iTunes podcasts, and has over 150 million downloads. He said “I built up my podcast through consistently providing quality valuable content and constantly being open to my own growth without being attached to the end result. In the early stages of the podcast, I was only doing one episode per week, and then I started adding two episodes, and then I added in my 5 Minute Fridays, and then I added video. This was a slow and gradual growth and has taken years to get to where I am today.”[iii]  Lewis Howes describes what it took for the Quantum Leap results he has achieved. So How Did This Work? Lewis Howes suggested doing 3 episodes/week to improve his downloads. To move away from releasing just one episode a week, we did try 3 episodes in a week but that almost killed me. My life was 100% focused on each guest, preparing and editing, but I did it because I was given access to high level speakers and didn’t want to miss that opportunity. Other projects were put aside, and it was an incredible learning opportunity, but I couldn’t sustain this level on my own. I was reminded of something I heard Brendon Burchard, the author of the book High Performance Habits talk about with how “High performers have mastered the art of prolific quality output (PQO). They produce more high-quality output than their peers over the long term, and that is how they become more effective, better known, more remembered. They aim their attention and consistent efforts toward PQO and minimize any distractions (including opportunities) that would steal them away from their craft” (Burchard, 2017)[iv] To release more than one interview a week would compromise the quality of the work I’m doing (unless I decide to outsource some of it, which I haven’t done yet).  So, I’m sticking to one interview a week, and will be adding Brain Fact Fridays, which will be short brain tips, to reinforce the strategies offered within the interviews.  Stay tuned, as we will begin Brain Fact Fridays next week. Who Can You Learn From? If there is someone that you know in your field who has a track record of success, look them up, and contact them. With 100% certainty, I can tell you that they will answer any questions that you might have for them. Ask them what they did to achieve their high levels of success, write it down, and follow what they did. You might have to adjust, like I did, but you will find your rhythm, and what works for you, once you begin to implement the ideas you are given. Just remember to take the advice they give you and try it. “If you want to accelerate your rate of achievement rapidly, you must search out and vigorously employ new behaviors.” (Pritchett, Page 7) Tip #2 Act As if Your Success is Certain—Expect It. “If you doubt something, doubt your limits.” (Pritchett). When starting something new, it’s normal, I think, to wonder if you will attain success with what you are doing. Just keep doing the work, and day by day, you will begin to see small wins that stack up, building your belief system. When you have true belief and faith that you will accomplish your goal, something inside you shifts. I have launched quite a few ideas that never took off, or never reached the heights I was hoping for. Even with all the belief in the world, some things just don’t work. But when something begins to turn the corner, and the momentum builds, you will see it. Never look back to the doubts, fears and uncertainty that you might have had at the beginning.  Even if it completely shocks you, just hold that belief. So How Did This Work? When I saw the numbers going up last summer around the time we were doing 3 podcasts each week, I remember feeling shocked, but also thinking “it’s about time.” The belief started to come, and it never left at that point. Do you believe in what you are doing? How can you strengthen your belief? What do you say to someone who tells you what you are trying to do is impossible, or that past results have proven it will fail? Tip #3 The How is Not Important, You Just Need to Know What You Want “A quantum leap is the unexpected that comes to you with sudden grace. You can open yourself to such a breakthrough, you can even invite it, but you can’t make it happen by struggling harder.” (Pritchett, page 17) I always remember my mentor, Bob Proctor[v] talking about forgetting about “how” I was going to attain my goal but focus on what I wanted. I remember saying “But Bob, how am I going to do that?” and he would look at me and say, “You don’t need to know how” and I would be left standing there, puzzled. Lewis Howes mentioned it when he said he remained focused on his growth. Just keep learning, working, moving, and growing, and putting one foot forward. Ideas and opportunities will come your way that you might have never seen or expected before. So How Did This Work? When our numbers started to accelerate, it was like a perfect storm of activity. I had just interviewed my mentored, Bob Proctor, and he offered to send out my podcast to his entire database. That really helped. Then Podbean (our podcast host) featured our podcast on the front page of their app, and we went from 32 followers to almost 4,000. This means that every time we upload an episode, 4,000 people are notified. Then we were contacted by Corwin Press, with 8 authors who they thought would be a great fit for the podcast. These authors all led me to other opportunities outside of the podcast, and the list can go on. If you are crystal clear on where you are going, the way will be shown to you. It might not look like the pathway you expect, but with faith and belief, it will come to you, and when your quantum leap appears, it won’t shock you, because you saw it all along. To close out this episode, I want to say thank you again to our past and future guests and listeners. I’m so grateful for the opportunity to provide this content, and that you are finding value with this podcast, from whatever part of the world you are in.  As I watch the statistics of who is listening where, I have to give a shout-out to the top 4 countries who tune into the podcast: The United States (where I live) Australia (where I have many friends), Canada (where I grew up) and the UK (where I was born).  We are proud to be showing up in the TOP 100 charts on iTunes all over the world in the Education (how-to) Category[vi] currently #55 in the US and #34 in Great Britain[vii] among other countries. 007: A Relaxation Breathing Technique for Freediving or for Life in General

The Renee Blundon Podcast

Play Episode Listen Later Jan 27, 2021 17:40

Relaxation breathing is a powerful tool for freediving and for life in general in order to decrease your heart rate and help calm the body and mind. In this podcast episode, I'll go over a relaxation breathing technique, also known as the 'Branko Petrovic Technique', which is used in freediving to help you relax deeply before your freedives and breath-holds. It slows down your breathing and intake of oxygen, therefore relaxing your nervous system and bringing your body back to its natural restful state. Because your heart-rate is lowered quite quickly and less oxygen is consumed, this means you'll be able to hold your breath much longer. And even if you are not planning to do any freediving or breath-holding, the breathing technique can help anyone who wants to relax more and help to quite the mind. Some Tips! While practicing this technique it's important to breathe as relaxed as possible without tensing any part of your body and without straining your intercostal muscles or diaphragm in order to keep the heart rate low. This will help your mind and your muscles to actively prepare you for a period of non-breathing, and be chill about it. The pause after each inhale and exhale helps to decrease your breathing rate even more, and this relaxation effect is enhanced even more when you do this breathing exercise in the water while breathing through a snorkel face-down on the surface. If there are waves, you can allow the waves to relax you even more (!) by keeping the body loose and de-contracting while allowing the water to move you in whichever which way, imagining you're like a rag-doll or just a piece of seaweed just floating along on the surface of the water. Lowers Stress and Anxiety This relaxation breathing technique helps lower stress and anxiety by quickly slowing down the heart rate and helping the body settle in the present moment. A study in 2013 proved that freedivers had a significantly lower level of stress, anxiety and negative affectivity compared to non-freedivers. And I believe this is mostly due to relaxation breathing / training and lifestyle elements. Feel Better The relaxation breathing can also help you feel better overall. Not only can it help lower your stress, but it can also boost your mood, because during the breath-work, feel-good neurotransmitters like dopamine and noradrenaline, are enhanced during the breathing exercise and also afterwards. The relaxation breathing technique also helps increase your energy and focus your mind. So you'll be able to focus better on your goals and have more energy to do the things you like. Also because you'll be using oxygen much more efficiently after doing this breathing, this also leads to better respiration, lower heart rate, increased blood flow to the vital organs and better overall health. So even though you may have never thought about changing your relaxation breathing for freediving or to practice relaxation breathing in general, it's time to give it a thought. Because the many benefits of this relaxation breathing exercise are super beneficial for your relaxation for freediving as well as for your physical and mental health in your everyday life. Hello MADAS, Is there a way to become a zero waste cannabis user? Is there some zero waste products you would recommend? Have you and your family ever thought about going zero waste? Thank you so much In advance!!! You guys are the best talk about my desires to go zero waste as a family in the past, but it is very hard for a big family like mine. COVID really put a damper on it to, with myself feeling very pressured to stock up and consume. It's a goal I will always work towards and try to model, but doubt we'd ever get there at this stage of our lives. Talk about Lizzy and the green team and how it really inspired us for awhile. There is a lot of packaging and waste that goes into commerical world of cannabis, there is no denying it. One of the first things you realize when you start regularly frequenting a dispensary is just how much damn packaging you will accumulate. Every preroll, every gram of weed put in a plastic container, every concentrate jar, edibles wrapped in tons of heavy printed packaging. We have a hemp company, Happy Flower Company, and for awhile we were using single plastic tubes for preroll sales. Once we started getting customers that ordered twetnty at a time we realized we just did not feel good about sending out that many plastic tubes. We switched our packaging. Talk about hemp plastic packaging, and how it's really cool but expensive. We ordered some hemp plastic tube samples and liked them but like the box thing we have going on better. Some Tips reuse packaging as much as possible Learn about and Use your dispensary recycle program or box. Return your weed jars and canisters for reuse or recycling. Avoid proprietary cartridge systems, or cartridges in general if the ability to recycle them is a concern. Some methods of consumption are just less wasteful than others. Smoking a joint thats gone at the end probably has the least impact on the enviorment, depending on the papers used I guess. Something like a propritary single use cartridge systems might have the most waste, especially if the cartridge is not able to be recylced. Avoid proprietary cartridge systems, or cartridges in general if the ability to recycle them is a concern. Opt for methods like homemade edibles, smoking flower, or having a single high quality and long lasting dry herb vaporizer as your main forms of consumption to avoid waste. Grow your own as much as possible and embrace making your own medicine! no packaging to deal with no harmful pesticides make your own concentrates like us use every part of the plant use the leaves to make cannabis tea use left over stems and leaves for mulching On vaporizers- Obviously a vaporizer isn't zero waste, eventually it will go into a landfill, so look for one that will last you a long time, with parts you can access and replace. Contact the vaporizer company or check their website for disposal of any devices. Some companies have programs in place for device disposal.

[03:11] Scenario of a Heart Murmur in a 60-Year-Old Male Case: A 60-year-old male is undergoing evaluation for a heart murmur. He's asymptomatic and his physician discovers a holosystolic murmur at the cardiac apex. The frequency of that murmur is increased when he expires and an echo confirms that there's a diagnosis to be found. Question: Compared to a normal patient, which of the following hemodynamic changes would be most likely present? Note: This involves multi-steps where you have to figure out from the murmur and it's confirming there is something so you could look at the murmur and decide what you think the diagnosis is. From there, you go one step further and say what physiologic effect that would have. [04:10] Knowing the Types of Murmur First, you have to know the types of murmurs out there. Holosystolic means that the murmur is present during the entire systole phase. There are some that fade off before the end or would just be a click sound at the beginning of systole. Holocystolic refers to the sound that is present the entire time. It can be one consistent sound for all systole or maybe it's something that starts at an increased volume or decreases. So you're left with different options. In this case, it says it's in the cardiac apex. You have to look at other components like the student's age to determine what exactly is causing the murmur. But here we're lucky since they're telling us here that it's right in the apex. [06:35] Mitral Valve Regurgitation When you're hearing this whole systolic murmur at the cardiac apex, you immediately start to think of mitral valve. When you look at the diagram, you will find different areas where you can picture where the apex of the heart is and what would be causing a sound in that direction. If you're visual, you can picture the mitral valve in the direction of the flow. If it weren't functioning properly, it would be right to the apex of the heart. So this would lead you to suspect mitral valve regurgitation. Another thing you can think of is mitral stenosis, however, that's diastolic so it would be heard in a similar area at a different time. Other things to think of when you have mitral valve regurgitation is that it kind of fits with the patient. If the question had said something about an irregular pulse or displaced apex, that would be the first thing that would fit with mitral valve regurgitation as well. Although the patient here is asymptomatic, common symptoms would be rhematic fever, palpitation, fatigue, shortness of breath, and it can go as far as having signs of heart failure. So any of those components in any combination could be present in the question that would lead to the same answer in the end. [09:25] Answer Choices A Increased after load B Decreased pre-load C Increased ejection fraction D Decreased ejection fraction E Decreased contractility Now, you have to understand what all of those components mean. In most cases, mitral valve regurgitation would be easily heard during expiration. This specific murmur isn't one that is always so strongly correlated. But this is something to keep in mind that expiration is more positive pressure down on the heart, more pressure on the ventricle and potentially easier to have a bit more of regurgitation. [11:11] Understanding the Terms: Afterload Afterload is the pressure against which the heart is working to eject the blood during systole. It's a systolic murmur so this is the one we're hearing here. And it's the component where the answer option is asking whether there would be increased afterload. In this case, that would not be correct since we're having regurgitation. Afterload does have an effect on the stress volume because the maximum pressure of the heart that can develop becomes smaller when there's volume inside the ventricle. During the systolic phase of the heart, this afterload is the pressure that's coming on the blood way out of the heart. As you're contracting, that mitral valve cannot hold itself shut. And you're having that regurgitation coming back in the opposite direction. [13:04] Understanding the Terms: Afterload and Ejection Fraction The preload is the filling pressure of the heart at the end of diastole. Once it's filled, the left atrial pressure at the end of diastole. We imagine Starling's Law which states that the heart is going to eject to a greater volume if it's filled to a greater volume at the end of diastole. That relationship can be modified by contractility in the afterload that would affect what ends up resulting. Ejection fraction is just the percentage of the blood left in the ventricle after a contraction. So if you have systole, the ventricles contract. 50% remains and that's the ejection fraction. [14:30] Causing Ejection Fraction After you've determined that it's mitral valve regurgitation, you can look at the electrocardiogram. We have shown that since the patient has no symptoms, probably compensating for this is a chronic disease that's why he's not coming in with any specific symptoms other than the murmur heard. Most likely, he has dilated his left atrium or left ventricle to compensate. Most patients with chronic mitral valve regurgitation have that. Then you have this retrograde flow happening over that mitral valve. So you would probably see a slight decrease in his afterload because of the resistance across that valve. The most likely though is you would see increased ejection fraction. [16:20] Starling's Law Think of the volume of the heart at the end of the diastole phase. That's going to give you a filling pressure, which is the preload. You would see that if you have that increased volume and pressure of blood in there to start with, that's going to lead to increased volume in systole and it affects contractility and afterload. But it's just sort of a volume and pressure play and less to do with the actual muscle fibers. If you look overall the spectrum of clinical cases, you can see mostly that it all comes down to volume and pressure. [17:45] More Options to Think Through This question is great because it brings up not just all the different types of murmurs that you have to run through but it also gives you all those different scenarios to think through for all the different murmurs. So you would have to go through all these options and assess each. If you just think through where is blood flowing during what phase of the cardiac cycle, you can almost figure out which answer you can immediately eliminate. This is the nice thing about cardiac physiology is that once you can think through oeach of the different causes of the murmur and think about afterload and preload and each one, it's something you probably won't forget because you've made sense of it, and not just memorized it. [19:55] Some Tips to Help You Study Effectively There are mnemonics you can use for the different types of murmurs so it's easier for you, like to know which sound and during which phase can lead to whether mitral stenosis or mitral valve regurgitation or aortic regurgitation. So if you're someone who can easily figure out how to memorize this then this is something you can jot down for sure. Moreover, rheumatic fever can also be something to consider that can cause aortic stenosis as well. If you know the basics and you know how to recognize mitral regurgitation and understand the physiology behind it, then you'll be able to lead yourself to any of those types of questions. Again, understand the causes of any of these murmurs. Know the signs. For instance, in aortic stenosis, that slow rising pulse is going to be in the question almost definitely, since that's one of those really unique cardinal signs of aortic stenosis, or the opposite is the collapsing pulse in aortic regurgitation. Knowing some of those clinical signs is super helpful in these types of questions. Links: Board Vitals (Save 15% off any of their QBank packages by using the promo code BOARDROUNDS at checkout.)

Shopping at Amazon For the Holidays? PLEASE visit www.logicalloss.com/support The holidays. How many of you just went "ugh!" or "sigh...." We have so much pressure, so much baggage, so many emotions and that's why we often head straight to the fridge for a fix of sugar. After all you HAVE To eat one of Aunt Jill's peanut butter sugar cookies covered in frosting, whip cream and chocolate! No you don't. But we need to step back from the emotions (easier said than done) and take a long hard look at the holidays. Most of the articles I looked at about "Surviving the holidays" gave out great advice like "have another cookie (glamour)" or basically get drunk. We stress out about the holidays because what the holidays are supposed to be if your "normal" is jammed down our faces.  There are all sorts of normal things we are supposed to be doing. Jewelry commercials are the worst. Attractive happy people so in love. Ugh. "This is what everyone is doing," you think. I want to see the Jewlery couple a year later when they are arguing over what type of lights to put on the tree, and whose house they are going to first on their Christmas tour of inlaws. "The woman of your dreams is now treating you like a child as you carry the pumpkin pie, as she insists it goes into the trunk and NOT the backseat" the diamond from last year is almost paid off thanks to the 18% interest on the credit card you opened to purchase it. For example, ladies if you are not sitting in the living room sipping wine and giggling, you're not normal. Apparently, women can't do anything without wine. It is the equivalent of beer commercials with men. Men drink beer. Women drink wine (apparently when they are doing everything). Flip Your Thinking Have you ever decorated a tree alone? It is one of the most depressing things you can do. Why? Because on TV everyone decorates with their family. So when I decorate my tree alone last year, I felt horrible. I felt abnormal. I felt like an outsider looking in. The reality of tree decorating is there is usually a discussion on what ornaments go in the front (because your heartfelt ornament is considered ugly by others). Do we put icicles on the tree? Flashing lights, old style lights, LED, so many things to argue over. The flip side of this is you get to decorate YOUR tree with all the ornaments YOU want. Enjoy the power. You get to decorate HASSLE free. How many times do you do something and feel like "Let's just get it over with" when it comes to traditions? Hate gift exchanges with strangers at work? Don't participate. Why go through another pointless activity to get a gift card from someone? Gift cards, the gift that says "I don't know you, and didn't feel like asking the person who sits next to you what you might enjoy. Happy Shopping. I hope you like Kohls." You have to weigh the lesser of two evils. Are you going to be seen as the Grinch, or have to put on a fake smile for 90 minutes. Holiday Parties with Families Let's face it, we are glad we only see some of these people once a year. Apparently, bathing is optional for some people. Who knew? I can't wait for someone to asking me about my dating life. If I'm lucky, they will rub my shoulder and give me the pity look. "Hang in there, you'll find someone" they say. Have an answer ready for, "So are you dating anyone?" This could be, "Im so glad you asked Aunt Mildred, I'm sleeping with five different people" just to see the reaction. You could also state that you've been having too many adventures to squeeze someone in (then tell them what you've been up to). You may avoid the pity face. Get them talking about themselves (if they are parents, ask about their kids) when they are done, go for more punch before they ask about your dating life. Offer to help out with whatever is going on in the kitchen. Speaking of families, my cousin (who has never married) once told me he would rather want what he doesn't have, then to have something he didn't want. That one made me think. What Is Up With All the Kissing? Put scissors in your pocket and cut down all the mistletoe Realize that New Years Eve is alot like prom: highly overrated. I remember one year I didn't even stay up to watch the ball drop. When I woke up, the new year was here anyway. Ever notice that cold and flu season coincides with the holidays? It's all the kissing. Seriously Snoopy Just Made Me Cry Why do we get depressed? Because its the most wonderful time of the year. Andy Williams said so. The bottom line is when you expect something to happen, and it doesn't, you feel lousy. Start a new tradition. There is a lot of pressure to smile, look good, and be happy. Eliminate some of the pressure by saying No. The holidays are about being thankful and giving. Be thankful for who you are and give yourself a break. You don't need to be scrooge, but you don't have to say yes to everything. Then there are those people who remind you about the true meaning of Christmas, and the baby Jesus. When you are lonely, there are times the story of a baby getting perfume don't cheer you up. There may be times when you're just blue. If you feel like crying, do it. It's an orgasm for your soul in some cases. Quit fighting it and let it out. If you find yourself in a "real dark place" there is nothing wrong with going and getting some help. Another way to let your feelings out is to start a journal.  The idea here is to let it out. Sometimes writing our feelings allows us to step outside of ourselves, and look at them differently. I use penzu. Here are Some Tips to Make You Feel Better Downsize and donate. Go through the closet and all those things you only wear when everything else is dirty? Give it to a charity. It will make you feel better (and make room in your closet) Go the the gym. Instead of arguing over the holidays with someone , go to the gym and get a jump on those new years resolution. I've never felt bad about going to the gym. I've felt sore, but not filled with regret. Plus in a month it is going to be packed at the gym, you might as well go there now while it's empty. Forgive someone . Some of the stress in our lives is based on grudges and stuff that happened in the past. Take advantage of this time when people think of family and friends and reach out and maybe patch things up with someone. In the event you don't want to get back to a place where this person is in your life, see if there is a way to get some closure on a situation. Start the new year with less regrets. Focus on what you DO have instead of what you don't. So much of the holidays are spent remembering the past. Maybe its time to start a new tradition. So many people put down other people to feel good about themselves, go a different route.  Quit talking about working in a soup kitchen, and go do it. I guarantee you will walk out of their feeling thankful for everything you have. To Sum It Up Acknowledge your feelings. If you recently lost someone, it's OK to cry and miss them as you try to adjust to the new normal. Reach out to organizations if you need help. Realize the holidays don't have to be perfect. Trying to make them perfect will only bring pressure. Accept your family or who they are. Don't binge on food, shopping, or alcohol. Don't throw away any good habits you have. If you can plan ahead, it might help eliminate last minute scrambling. Try to plan some "down time" even if its 15 minutes to just sit and be quiet. Most people who care about you will understand if you just say,"I'd love to, but I've got too much going on already." In the end, the holidays are a mental game. With pressure from the media of what we are "supposed" to do, throw in relatives and we may be outnumbered. So with this I say, if you feel a little blue it's OK. Take solace in knowing you're not the only one, and thus is it normal to feel this way. Realize then that you are normal. The holidays are three days (Thanksgiving, Christmas, and New Years) 72 hours.  You can make it. If you need to hear a voice, I found this place Samaritans Crisis Response Hotline When You Need Someone to Talk To (212) 673-3000 As always, if you need emergency help, call the National Suicide Prevention Lifeline at 1-800-273-TALK (8255), or call 911. Welcome to Episode 26 of Mastermind.fm! This week our masterminds Jean Galea and James Laws tackle hosting! Hosting can be a nightmare of trial and error to find what works for you in the often weird world of WordPress hosting. Jean and James have been through the wringer on this one, and have come out on the other side ok. Come listen in to the decades of hosting experiences that they've racked up between the two of them and make your own hosting decisions better! As always, show notes are below. Types of Hosting Basic hosting Managed hosting Managed WordPress hosting What's the difference? Typically, basic hosting simply gives you a place to run your website from, and leaves everything else to you. Managed hosting takes the responsibility of maintenance, security, updates, etc on their shoulders.  Managed WordPress does all of that too, but they specialize in WordPress hosting management. Some Tips to Keep in Mind Research your host! Make sure they are actually the people running the hosting software, not just someone reskinning a control panel with their own branding! A good host doesn't always have to manage their own hardware, but they should definitely be in charge of their own hosting software, database management, etc. Is Managed Hosting Expensive? It can be, but it isn't always. It's often done in tiers just like regular hosting. You can get managed WordPress hosting within almost any budget, you just have to shop around. What Host is Best? There isn't an answer to this question that isn't unique to your needs. Jean uses Digital Ocean because they fit his unique needs. James uses Pagely because they meet his unique needs. What host you want should also be based on your needs. How About a Shared Host? On a shared host, you share hardware resources with other websites on the same hardware. It is a great way to start and is really cheap. You should consider it a starting point only for any business though, something that you're going to grow out of. As your traffic grows, you're going to quickly outgrow the shared hardware resources. There's also a security risk, as other websites that share the hardware with you can potentially compromise your own security. Email Email often is either not covered in a hosting plan or not reliable through the host, but is an essential consideration for your website. Jean prefers Gmail, Zoho Mail, and Sendgrid as alternatives. James mentions Mandrill and Mailgun. Essentially, if you're looking for a service that will allow you to send bulk email, you need to find a transactional email service. If you're not sending in bulk, WordPress SMTP plugins are a good alternative. Backups? Yes. Do them. Jean strongly recommends you take responsibility of website backups for yourself. Even with a very reliable host, disasters can happen. Make regular high quality backups on your own and in addition to your host. Do be mindful that some hosts ban backup plugins that make whole site backups and store them on their servers.
