„Die Diagnose einer Hautmykose ist ganz einfach!“, erklärt Prof. Dr. Dietrich Abeck, Professor für Dermatologie und Allergologie an der TU München und Leiter des Hautzentrums in Nymphenburg: So fällt eine Pilzerkrankung vor allem durch eine randbetonte Schuppung und am Körper durch ein asymmetrisches Bild auf. Kinder stecken sich oft zuhause an, etwa durch ein Familienmitglied mit einer Zehennagelmykose. Tierpathogene Mykosen spielen im Kindesalter ebenfalls eine große Rolle. Eine starke Entzündung deutet darauf hin, dass vor allem Katze, Meerschweinchen und Co. als Überträger in Frage kommen. Dann heißt es: Schuppen gewinnen, Erreger bestimmen lassen, Kind behandeln und mit dem Tier zum Tierarzt. Weitere gute Nachrichten: Alle Dermatophytosen sind heilbar. Resistenzen gibt es so gut wie keine. Die Faustregel für die topische Behandlung lautet: in der Regel 2x täglich bis zum Abheilen plus eine Woche länger. Check out today's questions:    Anonymous: Hello I'm sure you've had clients before who have not got the results from the detox and CBO they expected. I just finished with CBO and have been taking the CBO finisher for 10 days now.. but I got a yeast infection in the vaginal area (I think I've had it for a looong time now) and in the last month of the CBO it just got a lot worse. The smell is very pungent and overwhelming and there's a lot of itching in the area. On some days it gets worse and I get UTI pain. It's very frustrating because I thought I would be feeling a lot better but overall it's all still the same.. Skin rashes, fibromyalgia symptoms, RA, anxiety, chronic pain, bloating, etc.. What would you recommend? Thank you so much, I trust your advice over anything!   Erica: Hi Dr. Cabral I wanted your advice on an issue I've been having for 5 years now. I have this dizziness that is constantly there (rocking on a boat) and gets worse under stress. I'm seeing a ND currently that tried increasing my potassium, but didn't help. I also checked out my upper cervical spine and that's fine. My ENT suspects it has something to do with my central nervous system based on her tests. Knowing that info, what would you do? I did all the big 5 labs, and I have SIBO, h pylori, 2 high 2 low minerals, high cortisol, DHEA, testosterone, estrogen dominance, high mold, viral issues. Also had chronic lyme in the past. I feel like I keep cycling with doing this functional medicine work to no avail. I've improved a lot but the dizziness hasn't gotten better. Tinea pellionella, the case-bearing clothes moth, is a species of tineoid moth in the family Tineidae, the fungus moths. This species has a cosmopolitan distribution, occurring nearly worldwide. The larva eats mainly fibrous keratin, such as hairs and feathers. It can become a pest when it feeds on carpets, furs, upholstery, and woolen fabrics. It also consumes detritus, cobwebs, bird nests (particularly of the domestic pigeon), stored vegetable produce and wallpaper. It stays inside a snug case it constructs from debris such as fibers and hairs.

Our program is affiliated with UCLA, and it's sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home.What is onychomycosis?-Onychomycosis is a fungal infection that resides in the finger and toenails. The nails become discolored, have onycholysis (painless separation of nail bed), splitting of nail bed, thickened. There are various causes of onychomycosis and examples include the following: dermatophytes, yeast, non-dermatophyte molds.-Onychomycosis occurs in 10% of the general population. Microbiology:Dermatophytes such as Tinea rubrum, account for most onychomycosis infections (~60-70%) while candida account for most of yeast causes of onychomycosis. Non-dermatophyte molds include fusarium, aspergillus, acremonium, scytalidium, Scopulariopsis brevicaulis. The type of organism involved has an association for the type of infection it causes. Yeast infects fingernails preferentially while the dermatophytes prefer to infect toenails. Diagnostic testing including culture, KOH preparation and PAS staining can help with confirming fungal infection, but culture not required for empiric treatment with oral terbinafine. Severity of onychomycosis:-Mild-moderate: ≤50 percent involvement of the nail and sparing the matrix/lunula-Moderate-severe: involving >50 percent of the nail or involving the matrix or lunula, including further spread throughout nail.-It's common to have multiple nails affected at the same time. Toenails and fingernails can both be affected. Remember to check all nails in your patients. Nails can show signs of local but also systemic diseases. Risk factors:-Health conditions: Diabetes, immune suppression, venous insufficiency, peripheral artery disease, or even just having slow growth of the nails. This makes sense because there is decreased blood flow to those areas resulting in decreased immune surveillance of that area. Patient s with PAD are at risk for onychomycosis. Nails normally grow slower in male. Hormones play a role in that growth.-Exposure: smoking, trauma to the nail, sports, wearing sweaty shoes, being barefoot in communal areas such as swimming pools, college showers, jail house showers, and gyms.-Dermatological diseases: tinea pedis (athletes' foot), excessively sweaty hands (hyperhidrosis), psoriasis-Other factors: old age, having family members whom the patient shares a living space with, bunion (hallux valgus). Effects on mental healthUnfortunately, the infection takes a toll on the patient because the infection is unsightly it results in psychosocial disturbances. The patients may not want to wear sandals, get pedicures, or shower during gym class if they are school age. These types of feelings can cause patients to not want to go to work or do things they enjoy due to feelings of embarrassment. ManagementTreatment of dermatophyte onychomycosis is guided by causative organism, severity, treatment availability, and cost.Oral agents-Oral treatment is generally the gold-standard for onychomycosis due to shorter course and greater efficacy compared to topical. -Oral terbinafine is the preferred oral agent. Itraconazole can be used in patients not able to tolerate/respond to terbinafine.-Terbinafine and itraconazole both work by blocking important enzymes in fungal synthesis.-A randomized double-blind trial showed that terbinafine is more effective outcomes and better long-term cure rates than itraconazole.-Adult dosing of terbinafine: fingernail onychomycosis =250 mg per day for 6 weeks. Toenail onychomycosis= 250 mg per day for 12 weeks.-Some side effects of oral terbinafine include headache, dermatitis, GI distress, taste disturbances, and liver enzyme abnormalities. Adverse effects of Itraconazole include headache, GI disturbances, liver enzyme abnormalities.-In patients receiving continuous therapy, monitoring of transaminase levels is typically performed at baseline and repeated at six weeks if therapy will continue beyond six weeks. A medication interaction check is recommended before starting treatment with oral agents. -Mycotic cure rates of 76% for terbinafine, 63% for itraconazole with pulse dosing, 59% for itraconazole with continuous dosing, and 48% for fluconazole, topical cure rate is about 40%.-Recurrence of infection ranges 10-50% (reinfection or persistent infection). Patients need to wait for up to 1 year to see full effect of treatment. Treatment is highly recommended in patients with diabetes, treatment in other patients is cosmetic.CompliancePatient compliance is difficult because while taking oral medications, you cannot drink alcohol, and this becomes a problem due to the length of the treatment.Topical agents-Efinaconazole, Amorolfine, Tavaborole, Ciclopirox-Patients who have contraindications to systemic antifungal therapy, who are at risk for drug-drug interactions with systemic antifungal drugs, or who prefer to avoid systemic treatment can be treated with topical therapy. Similarly, to oral agents, these medications work by blocking important processes in fungal synthesis. These agents come in solutions or nail lacquer. Possible side effects include local skin irritation or ingrown nails.Alternatives-Less common therapeutic interventions for onychomycosis include oral antifungal agents other than terbinafine and itraconazole, laser therapy, photodynamic therapy, and surgical nail removal.-Patients with pain or discomfort from infected nails may benefit from removal of hyperkeratotic nail debris. Application of topical urea under occlusion can help with debridement of the nail and symptom improvement.-Recurrence after treatment of onychomycosis is common.Prevention:Now that we have gone over a lot of material about onychomycosis, we should discuss how we can prevent these types of infections from occurring.  Having good “foot hygiene” can help reduce the of infection and re-infection.Wash your hands and feet frequently, especially after encountering someone who is infected.  Clip nails straight across and file afterward making sure to sterilize clippers before and after each use.  Do not share nail clippers with others.If you have a history of sweaty feet, consider using sweat absorbing socks or wearing “breathable shoes” to prevent sweat from accumulating.Throw out old shoes or disinfect them using antifungal powders.Wear sandals in communal shower areas and at the pool.Pay attention to the cleanliness of your nail salon.Joke: Do you want to know how a person with toenail fungus feels? _Health officials are concerned about the threat of an epidemic known as TINEA in Sri Lanka amid the corona. Hema Weerakoon, a doctor at the Anuradhapura Teaching Hospital, said the skin disease had been reported in several areas, including the North Central Province. He noted that the disease, identified as TINEA caused by a fungus, is spreading rapidly. He points out that environmental exposure, allergies to drugs, self-medication and social activities can cause infections. He noted that men, women and children of all ages are all affected by the disease. He insisted on recovering from the disease and seeking treatment under medical advice for about six months. He warned that otherwise the infection could worsen and adverse effects could occur.

This is episode four in our holistic care for skin series. Fungal conditions of the skin not only affect how your skin appears but can also be extremely painful, itchy and debilitating. Today we are talking about what the most common fungal conditions are and how we can treat them naturally. Fungal conditions are something we call mycosis, which is an external fungus which is bigger than a bacteria and takes over your body. We normally know the fungal condition called Candida (or yeast) which makes many females panic because it can be a really tough issue to get on top of, especially because it tends to show up in the vaginal area which is very moist making it the perfect area for Candida to grow. In today's episode however, we are not talking about internal presentations of Candida like this, but we will address it as a skin presentation.  Forms of fungal conditions on the skin: Pityriasis Versicolor - Trunk, neck and/or arms Tinea Corporis - Most common type, known as ringworm. Corporis Means in your body or comes from your core.  Tinea Pedis - Presents mostly between the toes but can also be seen between fingers.  Tinea Cruris - Presents in the groin area, between thighs and genitals.  Candida - Presents in the genital region (jocks itch) and also under the breasts There are other forms as well but as these are the most common this is where we will focus our attention today. For a fungal condition to take hold of the body it must have the right body conditions. The biggest factor is a low body pH (acidic). We have spoken about being too acid a lot in previous episodes. If you missed it I highly recommend going back and learning how to alkalise your body. Moisture and darkness are two other conditions which encourage fungal growth on the body. Tinea Corporis (Ringworm) lives in soils, pets and water with the perfect conditions described above. If/when we come in contact with it and we have a low pH it will be able to grow and spread, using our body as a host. You can see the effects of ringworm on the skin where you get patches of light red circles with defined edges. They are almost flat and slightly flaky. These spots will not necessarily itch.  Tinea Pedis shows up more like a cut in between the toes. It can be very painful, especially when you sweat, due to the rawness and blistering it can create. Candida varies from Tinea in that it is a lot more red and itchy. Pityriasis Versicolor is a different family or variety of fungus from Tinea. It prevents the sunlight from hitting the melanocytes (sun receptors on the skin which create pigment) and thus creates patchy, white areas over the body. Pityriasis Versicolor does not create flakiness or have a well defined edge. What should you be aware of if a fungal condition has taken hold? Look at what you are eating and drinking that could be contributing to a low pH. Reduce stress levels - High cortisol can definitely make you acid.  Supplement to help neutralise the pH If it is on the toes, keep them dry.  Avoid synthetics and polyester in which you will tend to sweat and trap the sweat in. Cotton is both breathable and cooling so it is the best option. Some people tend to use talcum powder to help with Tinea, but it is very toxic so it is not recommended. However, arrowroot powder is a good substitute and if you have essential oils you can blend a few drops of Lavender (not Lavandin as that will burn) with the arrowroot before applying.  For the other types where they are covering large areas (that aren't necessarily moist) we need to use an ointment. Sometimes we also need to exfoliate, especially with tinea corporis, as we need to remove the scale as the fungus dies so the new skin can come through. Exfoliation is best done twice a day as the skin regenerates about 3 times per day.   Support the immune system as a low immune system leaves the door open for any number of parasites, bacteria, viruses etc and fungus is no exception. A good starting point is to supplement with some Zinc, Vitamin D and Vitamin C. Tinea versicolor is a mild, superficial Malassezia infection of the skin. The condition has a high reoccurrence rate after treatment. Patients often report the malady after discovering an area of their skin that will not tan and, mistaken the disease as well as the resulting hypopigmentation for vitiligo. A few patients will also present with itching and velvety, tan, pink, or white macules or thin papules. Large, blunt hyphae, and thin walled budding spores will appear on the KOH. The patient can be treated with selenium sulfide lotion (applied from neck to waist every day and left on for five to fifteen minutes for seven days). This treatment is repeated weekly for a month and then monthly for maintenance. Ketoconazole shampoo (1% or 2%) lathered on the chest and back and left on for five minutes can also be used for treatment. Two doses of oral fluconazole (300 mg) fourteen days apart is the first-line treatment. Tinea pedis (commonly known as athlete's foot) is an acute or chronic dermatosis. Most infections are caused by the Trichophyton species. The patient will present with itching, burning, or stinging. Pain here may indicate secondary infection with complicating cellulitis. Interdigital tinea pedis is the most common predisposing cause of lower extremity cellulitis in healthy individuals. On the sole and heel, tinea may appear as chronic noninflammatory scaling occasionally with thickening or fissuring. This may extend over the sides of the feet in a moccasin distribution. The KOH preparation is usually positive. Tinea pedis can also present as scaling or fissuring in the toe webs with maceration. As the web spaces become more macerated, the KOH preparation and fungal culture have a lower tendency to return positive results because bacterial species begin to dominate. The malady can be treated with aluminum subacetate solution soaks for 20 minutes twice daily. Broad spectrum antifungal creams and solutions will help combat dipheriods and other gram-positive organisms. If topical imidazoles fail, one week of topical allylamine treatment will often result in clearing. I think all of my adversities are what pushes me to be as ambitious as I am. If not for my condition, I certainly would not have started Lemonayde.What would you like people to know about your daily life? *My daily life, especially when it includes any activity that forces me to take my shirt off, is full of a lot of explaining. Our culture is pretty hostile towards skin conditions, in general. The first question they need to know is, "Is it contagious?". Also, the assumption is usually that skin conditions are a result of poor hygiene, which, of course, is just not so. I take these moments to educate.What would make living and moving in the world easier for you? *More communities where people feel comfortable to be vulnerable in regards to their health without the expectation of judgement. There's a sense of relief and liberation that comes with being around people who you don't have to explain your health to.Do you have any life hacks? *More like mantras: 1) Your disadvantages are actually advantages. Find a way to use them as such. 2) The only two opinions I care about are that of parents. Everything else is noise. 3) Set your priorities and stick to them. Your health should be #1.What kind of support do you get from family or friends? *I love my family, they;re incredibly supportive, in that they don't treat any differently. And I pick my friends wisely. They don't see me any differently either. My family and friends know that I'm very independent and if I don't ask for help, it's because I DON'T want it. But if I do ask, I probably really need it.Have you ever had someone not believe you have an invisible illness because of your appearance and if so are there any examples that stand out? *I've had moments when people were convinced that my skin condition was due to poor hygiene even though I explained it has nothing to with hygiene. But whatevs.How has your invisible illness affected your relationships? *Woo. My skin condition started to develop in High School, which we all know is the most unforgiving place known to man. And with the emergence of a skin condition, that I had convinced myself was unsightly, came the insecurities and self-doubt. I put up a whole lot of walls when it came to social and dating activities. For me, being intimate was pretty much off the table. I didn't want to have to find myself in the excruciating situation of having to explain my skin condition and having the whole world know about it. My first relationship started in college and it was with someone who also had a skin condition (psoriasis). This may sound weird now but I initially fought off this relationship only because I knew she also had a skin condition. I don’t know why, but I guess I just didn’t want to convince myself that I’m only supposed to date people who were “like me” (whatever that means). But I was also keeping myself away from something great. It was just a strange confusing paradox. Fortunately, I lost that fight and we started dating and it was a wonderful relationship. We bonded about things only we would be able to understand. We skipped the whole “worrying about my condition” phase and just went straight to the fun part. We dated for quite a while and eventually moved on, as with most relationships. I did explore other relationships after the first, but it was the first that gave me that initial boost of confidence; validation of my awesomeness; and the relief that all ISN’T lost after all.Is there anything you are afraid to tell people in your life? *Very little. I used to be afraid to talk about my skin condition, now I don't care at all. I also used to be afraid to tell people about my undocumented status, now I couldn't care less.Does the fact that your disease is invisible change how healthcare professionals treat you? *The first time I saw a doctor about my condition, the first thing he recommended was Selsun Blue Medicated Formula. However, I told him that I've been using Selsun Blue for quite some time with no results and I asked him to prescribe something else. And this guy was just so unbelievably insistent. He told me I had to try it. But I didn't want to spend money on this doctor visit knowing that it would be a waste. And he did not let up. And it was extremely frustrating.What is your best coping mechanism? *Mashed potatoes and cookies. Also, I'm a self-healer. I like to do all of my healing and thinking on my own. So I meditate at least once a day.What are you the most concerned about and the hopeful for in the future? *This is on a grander scale but I do believe that our society is coming to the realization that empathy is the greatest healing power that's ever existed. My concern is that we will know that and consciously choose not to use it. However, my hope (and belief) is that we will. Dry skin It is usually due to your skin not having enough of its own natural oils (probably strips from soaps, cleansers, chemicals).  Moisturizing lotions and creams are "greasy" to try and match the oil of your skin.  The oil make the skin waterproof - keeps water out, but also keeps water in (prevents evaporation) Eczema - dry skin with an immune/inflammation component.  The deeper layers of the skin will release inflammation chemicals that irritate the other cells in the area and lead to redness, pain, and swelling.  It's common for kids to have eczema but then grow out of it, but other conditions can cause eczema to flare (in my case, pregnancy). Rash  Rash is one of the most ambiguous symptoms to try and figure out what the root cause is.  Rashes can be round or splotchy, symmetrical or asymmetrical, raised or flat, big or small.  Rashes are caused by some type of inflammation process (most commonly histamines).  Rashes are a sign that your body has been offended by something - either from the inside and then the chemicals reach the skin to cause rash, or from the outside that touched your skin and caused chemicals to be released. *Hives - a very distinctive rash with raised, irregular borders that may be red, but can also maintain their "flesh color".  Most commonly involve histamines, thus antihistamines (like benadryl can help them go away).  Can possibly be triggered by stress (the stress hormones cause a chain reaction of chemical release which may lead to hives). *Viral exanthem - "exit rash"; a rash that kids will get when their body has finally beat a virus.  Some start at the top of the body and then moves down to their feet.  Others start on the torso or core and then move out to the extremities.  Antihistamines do not help it, but they rarely hurt or itch or bother the kid in any way. Acne There are different pores in your skin (sweat glands, oil glands, hair follicles).  The oil on your face helps to trap bacteria and dirt that could get deeper in your skin and possibly hurt you.  The goal of cleaning your face, is to cleanse off the "dirty" oil, but not all the oil (because the result is either dry skin or over-production).  So the dirty oil plus the shedding dead skin cells can clog up any of these pores.  Blackheads are specifically when hair follicles are clogged - and they look black b/c the melanin proteins that get built into hair can be seen.  Generally small blackheads and whiteheads are not painful, unless a lot of bacteria are involved and they begin to fester. Cystic acne is when a larger area that involves multiple pore or hair follicles get clogged and infected with bacteria and then the spot can become swollen and painful (and unsightly). Hormones regulate the oil production, that's why puberty increases acne, stress increases acne, all of the transitions surrounding pregnancy can increase acne.  As long as you have hormones, you're going to have a risk of acne.  Also, working in really dirty environments can increase the amount of dirt that could clog up pores. Infections Ringworm = tinea corporis (there's no worm involved, it's a fungus).  Classic circular rash that is red and flakey and possibly flesh-colored in the middle. If it's on your head (tinea capitis = cradle cap).  If it's in your groin (tinea cruris = jock itch).  If it's on your feet (tinea pedis = athlete's foot). Tinea vesicolor = the fungus causes the skin to lose pigment (this is NOT vitiligo, which is the death of melanocytes and they don't grow back, Michael Jackson). Candida is the yeast that is a part of your natural skin flora, but can lead to rash if it gets out of balance. Bottom Line If there is anything on your skin that bothers you, see your doctor! Alopecia - hair falls out in the same area all at one time.  Kind of like crop circles. 3 Stages of the Hair Follicle life cycle - Anagen phase - active phase, cells are multiplying, hair is growing - Catagen phase - resting phase, "cat nap" - Telogen phase - shedding phase Hair thickness is determined by 2 things: the density of hair follicles (how many follicles in one area) and how large a single hair follicle is (thick vs fine hair). male patterned baldness - receding hairline from front to back, and the loss of the hair on the crown of the head. Female patterned baldness - the overall thinning of the hair. Hirsutism - females get male-patterned balding due to testosterone imbalance Why does all your hair fall out after you have a baby?? Telogen effluvium - a mass exodus of hair follicles This can also happen when the body goes through extreme shock or trauma. There is a condition called post-partum alopecia - this is temporary, and hair grows back afterwards. Itchy Scalp Dandruff is caused by the inflammation of the scalp.  Inflammation causes skin to be dry and itchy.  The topmost skin layer releases in larger amounts, thus the pieces are visible, opposed to when the skin naturally sheds, it's microscopic so you don't usually see the dead skin cells you lose. Tinea capitis - ringworm on your scalp, aka cradle cap.  Usually requires an antifungal to get rid of it. Tidbits Hair can actively grow for up to 6 years. Since hair is dead cells, hair hangs onto the DNA and it can be used to identify people (missing persons) or for paternity tests (who's your daddy)? Dying and perming affect the proteins that give it color or keep it connected. If someone has had prolonged exposure to toxin (carbon monoxide, heavy metals, smoking), the hair shows markers of that damage from the toxins.  It can also hang on to markers from being exposed to illicit drugs.  It's like a timeline of what you've been exposed to. Terbinafine, a synthetic allylamine, exerts fungicidal activity against dermatophytes, the causative pathogens of tinea pedis. As proven in numerous clinical trials, tinea pedis can be effectively and safely treated by topical terbinafine. In fact, a 1-week application of terbinafine 1% cream eradicated fungal pathogens at least as effectively as 4-week treatment courses with topical azole derivative antifungals and showed lower relapse rates. A new innovative single-application formulation of terbinafine 1% in a film-forming solution produces a high concentration gradient on the skin surface and enables a prolonged (up to 13 days) exposure of the skin to terbinafine. High drug penetration into the skin results in an otherwise not obtained drug reservoir in the horny layer, the location of dermatophytes in tinea pedis. Although azole antimycotics can also effectively penetrate into the horny layer of the skin, short-term therapy might not be feasible due to its primarily fungistatic activity against dermatophytes. Thus, we conclude that the high efficacy of short-term treatment with terbinafine in patients with tinea pedis is possible due to its fungicidal activity coupled with a distinct reservoir formation in the upper layers of the epidermis.
