TRANSCRIPT Melis Canturk: Hello, and welcome to the JCO Article Insight. I'm your host, Melis Canturk, and today we will be discussing the JCO article, "Atezolizumab With Bevacizumab and Nonplatinum Chemotherapy for Recurrent Ovarian Cancer: Final Results From the Placebo-Controlled AGO-OVAR 2.29/ENGOT-ov34 Phase III Trial." While integrating immune checkpoint inhibitors has revolutionized the treatment of various gynecologic cancers, these agents have historically shown limited single agent activity in ovarian cancer. Despite a strong biological rationale for combining immunotherapy with chemotherapy and bevacizumab to enhance T-cell infiltration and normalized tumor vasculature, several phase III trials have failed to demonstrate a significant survival benefit in this setting. The AGO-OVAR 2.29/ENGOT-ov34 trial was launched to definitely evaluate whether adding the PD-L1 inhibitor atezolizumab to this combination could improve long-term outcomes for patients experiencing early relapse. This international, double-blind, randomized phase III trial enrolled 574 patients with epithelial ovarian, fallopian tube, or peritoneal cancer. Eligible participants had to be in their first or second relapse within 6 months of completing platinum therapy or in their third relapse regardless of the treatment-free interval. All patients received bevacizumab and an investigator selected chemotherapy backbone, either paclitaxel or doxorubicin. They were randomly assigned to receive either 840 mg of atezolizumab or a placebo every 2 weeks until disease progression or for a maximum of 2 years. The study population was an all-comer group, though patients were stratified by their PD-L1 status, previous bevacizumab use, and the number of prior treatment lines. The trial did not meet its primary end points, as the addition of atezolizumab failed to significantly improve overall survival or progression-free survival in the intention-to-treat population. For the primary end point of overall survival, the median was 14.2 months with atezolizumab compared to 13 months with the placebo. Progression-free survival was similarly insignificant, with a median of 6.4 months in the experimental arm versus 6.7 months in the control arm. Furthermore, the objective response rates were nearly identical between the groups, recorded at 40% for atezolizumab and 44% for the placebo. Interestingly, exploratory subgroup analyses revealed potential signals of benefit in specific populations, even though the overall trial was negative. Patients who had been previously treated with bevacizumab appeared to derive a greater benefit from the addition of atezolizumab than those who were bevacizumab-naïve. Additionally, outcomes seemed more favorable for patients receiving a paclitaxel chemotherapy backbone compared to those receiving doxorubicin. However, PD-L1 status did not appear to be a predictive marker for success, as hazard ratios for survival were similar regardless of whether the tumor was PD-L1 positive or negative. The safety profile of the triple combination was consistent with the known toxicities of the individual drugs. Grade 3 or higher adverse events occurred in 73% of the atezolizumab group and 70% of the placebo group. While the experimental arm saw higher incidences of immune-mediated events, such as thyroid-related issues, these were generally manageable. Serious adverse events were more frequent in the atezolizumab arm than in the placebo arm, but discontinuation rates due to toxicity were relatively low and comparable between the two groups. In conclusion, the AGO-OVAR 2.29 trial confirms that adding atezolizumab to bevacizumab and nonplatinum chemotherapy does not provide a statistically significant survival advantage for patients who receive nonplatinum chemotherapy for recurrent ovarian cancer. This study contributes to the growing body of evidence showing that immune checkpoint inhibitors have yet to find a definitive role in the standard treatment of recurrent ovarian cancer. Future research will likely focus on more sophisticated molecular stratification and the use of novel agents, such as bispecific antibodies, to overcome the challenging tumor microenvironment of low-grade serous ovarian cancer. Mehr als 2 Stunden Stau. Und dann kam auch noch der Wintereinbruch und die Nacht. Wir haben uns durchgekämpft, auch als der Wagen auf eisglatter Fahrbahn einmal ausbrach – trotz Allrad und guter Winterreifen – konnte meine Frau ihn gekonnt abfangen.Anderswo ist es nicht so glimpflich abgegangen. In ganz Deutschland jede Menge Unfälle. Aber das ist ja jedes Jahr so. Bei Extrem-Wetter das Auto stehen lassen – wer tut das schon?Die USA erleben einen dramatischen Wintersturm. Das gab es früher auch – aber selten in dieser Häufigkeit und in dieser Härte. Studien deuten darauf hin, dass sich Winterstürme durch den Klimawandel in ihrer Intensität und Häufigkeit verändern können. Wärmere Luft kann mehr Feuchtigkeit speichern, was zu stärkeren Niederschlägen während Stürmen führt, während veränderte Jetstream-Muster die Zugbahnen von Stürmen verschieben und extremere Wetterlagen begünstigen. Wir haben uns das selbst zuzuschreiben. Viel zu lange haben wir die Erderwärmung nicht ernst genommen. Aber als ob das nicht reichen würde, benützt Putin auch im 4. Kriegswinter die Kälte als Waffe gegen die Zivilbevölkerung und greift erbarmungslos die Infrastruktur der Ukraine an. Es ist niederträchtig, dass einem dazu eigentlich nichts mehr einfällt. In der Haager Landkriegsordnung heißt es unter anderem: Zivilpersonen, andere Nichtkombattanten und zivile Einrichtungen sind so weit wie möglich zu verschonen. Here they attempted to begin again—but their refuge would all too soon become a trap.For the first time, Laura Hobson Faure brings to life the experiences of these children, and the Jewish and non-Jewish organizations who helped them. Drawing on survivors' testimonies as well as children's diaries, letters, drawings, songs, and poems, Who Will Rescue Us?: The Story of the Jewish Children who Fled to France and America During the Holocaust (Yale UP, 2025) re-creates their complex journeys, including how some of them eventually found safety in America.Hobson Faure paints a moving portrait of these children and their escape, uncovering their agency in the flight from Nazism—and knits together the network of the many who aided them along the way. Laura Hobson Faure is professor of modern history and chair of Modern Jewish History at Université Paris 1-Panthéon-Sorbonne. She's an expert on French-American Jewish history and the author of The “Jewish Marshall Plan”: The American Jewish Presence in Post-Holocaust France. Geraldine Gudefin is a modern Jewish historian researching Jewish migrations, family life, and legal pluralism. She is currently a Visiting Scholar at the Centre for Asian Legal Studies at the National University of Singapore, and is completing a book titled An Impossible Divorce? One of the toughest challenges in military human performance is how to implement programs that work for reserve service members. With only a couple days a month of direct interaction, how do you improve their health and fitness behaviors the rest of the month? Who better to workshop solutions to this issue with than the Chief of the Army Reserve? Lieutenant General Robert Harter was an H2F skeptic at first (we discuss why), but now he's a believer and trying to mobilize the right leaders and resources to improve human performance for his part time soldiers. LTG Harter was confirmed by the U.S. Senate on August 1, 2024, and assumed the dual role of Chief of Army Reserve and Commanding General, U.S. Army Reserve Command. As the Chief of Army Reserve, LTG Harter is the principal advisor on Army Reserve matters to the Secretary of the Army and the Chief of Staff of the Army. As Commanding General of U.S. Army Reserve Command, he leads a community-based force of more than 174,000 Citizen Soldiers and 11,000 Civilians – with a footprint that encompasses all 50 states, five U.S. territories, and more than 23 countries across the globe. His previous General Officer assignments include Commanding General of the 81st Readiness Division, Deputy Chief of Army Reserve, Chief of Staff for Army Material Command, and Commanding General, 316th Expeditionary Sustainment Command (deployed in support of Operation Enduring Freedom). After graduating from Virginia Tech in 1988 with a Bachelor of Arts degree in communication, LTG Harter received his Active Duty commission as a Field Artillery Officer. He also holds a Master of Science degree in National Security Strategy from the National War College. As his initial Active Duty assignment, LTG Harter served as a platoon leader in the 11th Armored Cavalry Regiment, Bad Hersfeld, Germany (deployed to Desert Storm). After branch transferring to the Ordnance Corps, he completed Active Duty assignments with the 101st Corps Support Group, Fort Campbell, Kentucky, and the U.S. Army Munition Center and School, Redstone Arsenal, Alabama. LTG Harter then transferred from Active Duty to the Active Guard and Reserve (AGR) program in 1999. His AGR assignments include Support Operations Officer, 55th Sustainment Brigade, Distribution Management Center Chief, 316th Expeditionary Sustainment Command (deployed in support of Operation Iraqi Freedom), and Assistant Chief of Staff, Office of Chief of Army Reserve. LTG Harter and his wife Erin have been married for over 35 years and have three children. They currently live in Stafford, Virginia.

This episode of The WildStory is all about the southernmost point in New Jersey—beautiful Cape May, known to beachgoers as a summer destination. But for nature lovers, September is migration season and the very best time to head to Cape May. Which is exactly what many of us from the Native Plant Society of New Jersey will be doing, for a special trip to Cape May the weekend of September 27th through the 29th. The retreat sold out faster than we anticipated, but we wanted to share some of the wonders of the area with you in this episode, which includes an inspiring roster of guests from South Jersey.  Our featured poet is Penny Harter (4:00), who has lived and written in May's Landing for the past dozen years. Penny and Ann spoke about memory, grief, and the everyday creatures and objects that become the imagery that fills her poetry. Penny also offers a mini-lesson on haibun, a beloved Japanese form near to her heart, a form that allows ideas to ripple and expand, like rings upon a lake,   In Ask Randi, Dr. Randi Eckel (0:32:00), a native plant expert for NPSNJ and owner of Toadshade Wildflower Farm, talks about entering fall, when the many species of goldenrod come into their glory. She clears up the confusion between goldenrod and ragweed and shares why Seaside Goldenrod, which grows along the seashore, is one of her favorites.    We then hear from naturalist Mark Garland (0:44:54) the Director Emeritus of the Cape May Monarch Monitoring Project. Mark has lived and worked in Cape May for more than 20 years and is the author of Watching Nature: A Mid-Atlantic Natural History. Mark talks with Kim about the Monarch butterfly and why its migration is unlike any other that we know of on this planet.    In our final segment, Kim and Ann catch up with naturalist Pat Sutton (01:09:04), who has lived and worked in the Cape May area for over 40 years. Pat shares her knowledge of Cape May Island, one of the top spots in the world to see butterfly and bird migration.
