In today's podcast we talk with Eric Wong, geriatrician-researcher from Toronto, and Thiago Silva, geriatrician-researcher from Brazil, about the comprehensive geriatrics assessment. We spend the first 30 minutes (at least) discussing what, exactly is the comprehensive geriatric assessment, including: What domains of assessment are essential/mandatory components of the comprehensive geriatrics assessment? Who performs it? Is a multidisciplinary team required? Can a geriatrician perform it alone? Can non-geriatricians perform it? Who is the comprehensive geriatrics assessment for? Who is most likely to benefit? Eric Widera suggests not as much benefit for very sick and very healthy older adults, more benefit in the vast middle. Why do the comprehensive geriatrics assessment? What are the interventions that it leads to (we cover this more conceptually, rather than naming all possible interventions) How does the comprehensive geriatrics assessment relate to the 4Ms (or 5 Ms)? How long does it take to conduct a comprehensive geriatrics assessment? What's the evidence (BMJ meta analysis) for the comprehensive geriatrics assessment? What are the outcomes we hope for from the comprehensive geriatrics assessment? That final point, about outcomes, bring's us to Eric Wong's study, published in JAGS, which evaluates the cost effectiveness of the comprehensive geriatrics assessment performed by a geriatrician across settings (e.g. acute care, rehab, community clinics). As an aside, as the editor at JAGS who managed this manuscript, I will say that we don't ordinarily publish cost effectiveness studies at JAGS, as the methods are opaque to our clinical audience (e.g. raise your hand if you understand what ‘CGA provided in the combination of acute care and rehab was non-dominated' means). We published this article because its bottom line is of great interest to geriatricians. In Eric's study, geriatricians performing CGA were more cost effective than usual care in Every. Single. Setting. And of course cost effectiveness is only one small piece of the argument for why we do the comprehensive geriatrics assessment in the first place (no patient in the history of the world has ever asked for a test or treatment because it's cost effective for the health care system). I'll close with a couple of “mic drop” excerpts from Thiago's accompanying editorial: Finally, it is instructive to compare the cost-effectiveness of geriatric services and CGAs with other interventions. A recent analysis of lecanemab for early-stage Alzheimer's disease found that gaining one QALY would cost approximately $287,000 (USD). In contrast, Wong et al. estimated that adding community-based CGA would cost about $1203 (CAD) per quality-adjusted life month (QALM) (equating to roughly $10,105 (USD) per QALY, using $1 USD = $0.7 CAD), making geriatrician-led CGA nearly 30 times more cost-effective. Put simply, for each dollar spent to improve quality of life for a year through CGA, one would need to spend almost $30 to achieve the same benefit with lecanemab. Ultimately, the question is not whether geriatricians represent a worthwhile investment (they are) but how healthcare systems can ensure that every older adult requiring specialized, comprehensive care can access it. Wong et al.'s modeling study provides a valuable contribution by showing that geriatricians placed in acute and rehabilitation settings offer the most cost-effective deployment given current workforce limitations. Despite some caveats, the overarching message remains clear: geriatric expertise not only enhances care quality but can also align with health-economic objectives, especially in high-acuity environments. However, we cannot allow an inadequate geriatric workforce to become a permanent constraint, forcing painful decisions about which older adults and which settings will miss out on optimal geriatric care. Instead, we should continue to strive to increase the number of geriatricians through robust training programs and payment model reform to ensure that cost-effective care can be provided for this large and growing vulnerable population. -Alex Smith

Welcome back to SEASON 14 of The Neuroscience Meets Social and Emotional Learning Podcast, where we connect the science-based evidence behind social and emotional learning and emotional intelligence training for improved well-being, achievement, productivity and results—using what I saw as the missing link (since we weren't taught this when we were growing up in school), the application of practical neuroscience. I'm Andrea Samadi, and seven years ago, launched this podcast with a question I had never truly asked myself before: (and that is) If productivity and results matter to us—and they do now more than ever—how exactly are we using our brain to make them happen? Most of us were never taught how to apply neuroscience to improve productivity, results, or well-being. About a decade ago, I became fascinated by the mind-brain-results connection—and how science can be applied to our everyday lives. That's why I've made it my mission to bring you the world's top experts—so together, we can explore the intersection of science and social-emotional learning. We'll break down complex ideas and turn them into practical strategies we can use every day for predictable, science-backed results. As we kick off Season 14, I've been revisiting past episodes and reflecting on the incredible insights our guests have shared since we first launched in 2019. I mentioned last week that this reflection started when a young filmmaker, Vishal Sharma[i], reached out to me and offered to create YouTube Shorts[ii] from past interviews. When I saw his first example, something clicked. As I reviewed older episodes, I realized I had only scratched the surface of the lessons within each interview. So as we dive into this review of past conversations, know that this review isn't just for you—it's for me as well. I've learned that slowing down helps us to uncover even more wisdom, and I'm excited for us to walk this path together. For today's Episode 367, we're going back to Episode 66[iii] with the late Bob Proctor—the first person who challenged me to think. Last week we covered our first YouTube Short[iv] where we picked out three important nuggets of wisdom that I had missed until re-listening to this episode. We covered in PART 1: ✔ Top Lesson from Episode 66 – Where we refined the question: “What do you really want to do with your life?” hoping that midway through 2025, we can refine our goals, and see if we are on track. ✔ Sage Advice Tip #1: “We have been given the mental faculties to create our own environment. “We explored how to go beyond our five senses and tap into the six higher faculties of our mind (further developing our reason, intuition, perception, will, memory and imagination.” ✔ Sage Advice Tip #2: “There's only one mind.” We looked at how our actions matter because we are all deeply connected. ✔ Sage Advice Tip#3: “We are a soul—we don't have one.” We reflected on the perfection within each of us, and how this perfection continually drives us toward bigger and better possibilities. Today, we are going to cover 2 short video clips and dive into the wisdom contained within each, making the connections with each nugget of wisdom learned. And since I'm always looking to connect the science to our learning, there are some age-old success principles we will cover today, that are proven to work time and time again. While science doesn't call out the success principles exactly as we will cover them today, Stanford University neuroscientist and tenured professor of neurobiology, Dr. Andrew Huberman does discuss “the science of imagination and its impact on creativity and learning” on his recent podcast episode “The Science of Creativity: How to Enhance Creative Innovation.”[v] Keep an open mind, as we discover how we can rise to new levels of success by applying the timeless principles we explored in Episode 66. For today's EP 367-PART 2 of our review of EP 66, we will cover: Sage Tip #1: In the first clip we learned that “If we want to improve our RESULTS, we must focus on the six faculties of our mind—reason, intuition, perception, will, memory, and imagination.” The key is to choose one of these faculties and dedicate time to studying and applying it until you see real results from your deeper understanding. You could even spend an entire year on just one faculty, and it would be time well invested. Sage Tip #2: In the second clip we learned that “Goals are not set to get; they are set to grow.” Sometimes goals don't unfold the way we expect, and that's okay—as long as we keep moving forward and growing. To do this, we must keep studying, learning about ourselves, clarifying what we truly want, and seeking specialized knowledge in our field from those who have gone before us. VIDEO 1 Watch by Clicking Here Click on the link in the show notes to watch video clip 1 where I asked the question “what are some of the common reasons why people don't reach their full potential” as I've always wondered why many of us get stuck. It really has boggled my mind over the years why there is such a puzzle around reaching high levels of success and achievement. Myself included. Bob replied with “well, the answer to that is pretty obvious. We were not taught anything about ourselves” and he went right back to how we were never taught how to live beyond our five senses, and most of us haven't fully developed the six faculties of our mind that we went into on EP 294[vi] and he said that we let the outside world control us. That rang true to me. It's a rare individual who moves forward, towards their goals, past the many obstacles they face. Something always seems to get in the way, and delay progress, but those who achieve these high levels of success, have become masters at moving forward, despite what they face. Sage Tip #1: If we want to improve our RESULTS, we are going to have to narrow in on the six faculties of our mind (our reason, intuition, perception, will, memory and imagination). You can review the 6 faculties on EP 294[vii] where we covered each faculty, with examples on how to develop them. We could spend years working on each faculty, to really sharpen the saw. I remember when I first found a book on how to develop my intuition (one of the 6 faculties of the mind). It was my early days of living in the US (after moving from Toronto to Arizona in 2001, sometime after September 11th had happened) and I was cleaning someone's house (after the work I was doing in the seminar industry came to a fast halt without the ability for people to travel by air) I found work that brought me more money than I imagined possible. It was a humbling time, that's for sure. I noticed this book on a book shelf and asked the owner if I could read her book, and bring it back when I returned. I remember she looked at me and said “keep it!” and took it off her book shelf. I must have given that book to someone else to read, (following her example) but I still have the next one I bought after that, called Developing Intuition by Shakti Gawain.[viii] Take your time learning these six intellectual factors. Each one, will serve you well once you learn how to use them, and developing these faculties would be one of the main differentiators that Bob mentioned separated those who achieved their goals, from those who did not. The biggest lesson I learned from diving into my intuitive factor (from reading these books) was learning to trust myself with the insights that would flash on the screen of my mind, (as learned from The Silva Method EP 261),[ix] or listening to my body with what neuroscience calls interoception and finally, applying this intuition to my work life, something that 20 years ago, was not a concept ever spoken about in our corporate workplaces. I'll never forget when Greg Link, who took the 7 Habits Book to great heights with the Covey Organization, mentioned this book on Intuition, on EP 207[x] on “Unleashing Greatness with Neuroscience, Trust and the 7 Habits.” If Gawain's book on Intuition was on Greg Link's book shelf, I think that's enough to say it comes highly recommended. To sharpen your saw on these higher faculties, go back and review EP 274, and make it a mission to work on one faculty at a time, until you are using each one, effectively, in your daily life. This will be time well spent. And sometimes if the goals don't work, that's ok. As long as you keep going, and growing. The main thing is that you keep growing, and become more of what you are designed to become. We have to keep doing greater good, and expand on what we are doing.” Then he went back to Thomas Troward's quote that we covered last week, saying that “spirit is always for expansion and fuller expression” which explains that we will want to do bigger and better things all the time. To do this, he says, “we will need help, because specialized knowledge is so important.” Keep Growing There is a lot to think about with this tip. How do you keep growing? I think the answer to this is to keep studying and learning about yourself. I sometimes hear my kids say they want to do something (can we go to the mall) because they are bored, and I think, “why not read a book” and take your mind on a trip inside the pages of a book. I want to hand them Shakti Gawain's book on Developing Intuition and tell them to come back to me in a year and tell me what they have learned, but in order to study and do the work needed, it has to come from within each person. You'll agree with me if you have ever tried to force someone to read a book that you are enjoying. It's much easier to transfer knowledge to others just by living by example. Do your own work. Keep studying, learning, growing, and when asked what you are reading, share what you are doing. You won't be surprised when others come back and tell you that they have bought the book you are reading, and share with you what they have learned. Learning is contagious, but it must come from within each person, when they are ready. Specialized Knowledge We covered the importance of this topic on PART 3 of our Review of Napoleon Hill's Think and Grow Rich book back on EP 193[xi] as this book has a whole chapter dedicated to Specialized Knowledge. Just to review, and further hone in on what it is that we want, chapter 5 of Hill's book reminds us that an understanding of specialized knowledge will separate you from everyone else, making you special with the knowledge you have acquired. In chapter 5 Hill teaches us that knowledge (general or specialized) must be “organized and intelligently directed” (CH 5, page 79, TAGR) “and is only potential power. It becomes power only when, and if it is organized into definite plans of action and directed to a definite end.” While we are working on whatever it is that we want, and we are studying and learning, and growing as a result of this, we now add the understanding of asking others who have knowledge that we need (specialized knowledge) that we will take, organize it, and use it in pursuit of whatever it is we are working on. REVIEW and CONCLUSION To wrap up this week's Episode 367—where we revisited two powerful clips from Episode 66, our interview with Bob Proctor—we explored: Sage Tip #1: In the first clip we learned that “If we want to improve our RESULTS, we must focus on the six faculties of our mind—reason, intuition, perception, will, memory, and imagination.” The key is to choose one of these faculties and dedicate time to studying and applying it until you see real results from your deeper understanding. You could even spend an entire year on just one faculty, and it would be time well invested. Sage Tip #2: In the second clip we learned that “Goals are not set to get; they are set to grow.” Sometimes goals don't unfold the way we expect, and that's okay—as long as we keep moving forward and growing. To do this, we must keep studying, learning about ourselves, clarifying what we truly want, and seeking specialized knowledge in our field from those who have gone before us. I hope that these tips that I wanted to revisit have helped you to continue to sharpen your saw, and bring you closer to whatever it is that you are working on this year. What I'm learning is that there isn't an end to this process. We will always seek “fuller and greater expression” and to dedicate time to studying and learning about ourselves, we separating ourselves from those who struggle to reach their goals, to the ones who attain them, and then reach out to help others do the same. Simply put, it's usually cheaper to make new plastic than to recycle old plastic, even the plastic you put in the recycling bin. Because it takes plastic centuries to break down, this means for each one of us you could build a mountain of plastic from all the packaging we use over the course of our lives. But what if plastic didn't have to take centuries to break down, and could actually biodegrade in a matter of days or weeks? That's exactly the vision Luna Yu is bringing to life as founder and CEO of Genecis Bioindustries, a biotech company using fermentation to transform food waste into high-performance, truly compostable bioplastics. The first ambush occurred on the evening of Saturday, August 9 in North Kivu Province. The Muslims targeted a specific vehicle and abducted its passengers. The second, more deadly attack occurred during the night of Sunday, August 10 in the Ituri Province. They killed three people, set ablaze several vehicles and houses, and abducted many residents who remain missing. Nearly 10,000 sign petition demanding Toronto mayor end taxpayer funding to homosexual ‘pride' events A petition signed by 10,000 people has been submitted to Toronto Mayor Olivia Chow demanding an end to taxpayer funding of homosexual/ transgender “pride” events, reports LifeSiteNews.com. Earlier this month, Gregory Tomchyshyn, the Canadian campaign director for Citizen Go, dropped off a petition to Mayor Chow calling for her government to end its lavish spending of taxpayer dollars on the promotion of sexual perversion, including the “Toronto Pride” parade. For years now, LifeSiteNews has documented the event's crude floats, public nudity, and public displays of sexual exhibitionism. The petition said, “Mayor Olivia Chow and Toronto City Council just handed $350,000 in taxpayer money to Pride Toronto, despite the event's history of public nudity and indecent displays in full view of families and children.” According to Canadian Broadasting Corporation News, “Pride Toronto” will see its taxpayer funding hiked by 26 percent since fewer corporations want to promote sexual immorality. Venezuelan Evangelical Christians march for Jesus Thousands of Evangelical Christians mobilized across Venezuela during the March for Jesus, reports The Christian Post. The event featured preaching, public worship, biblical reflection, and prayers for Venezuela. According to the Latin Evangelical Alliance, this year's theme was: “Jesus, the nations belong to You.” Social media was filled with images of the march, which was held not only in cities but also in rural towns. Pastor José Piñero, executive director of the Evangelical Council of Venezuela, addressed the crowd in Cumaná with a message in Spanish focused on grace. Trump-Putin meeting in Anchorage today about ending Russo-Ukrainian War Anchorage, Alaska Mayor Suzanne LaFrance, an Independent, has announced that the city is “prepared and ready” for President Donald Trump's high-stakes meeting today with Russian President Vladimir Putin about ending the Russo-Ukrainian War. The meeting is set to be held at the Joint Base Elmendorf-Richardson. Trump seeks to extend federal D.C. takeover as ‘sanctuary city' policies put on ice In an effort to eradicate violent crime in our nation's capital, President Donald Trump is preparing to ask Congress to extend the 30-day limit on his federal takeover of Washington, D.C. When asked by a reporter Thursday if he intends to seek congressional approval to prolong his federalization of D.C.'s Metropolitan Police Department, the president said this in reply. TRUMP: “Well, if it's a national emergency, we can do it without Congress, but we expect to be before Congress very quickly. We think the Democrats will not do anything to stop crime, but we think the Republicans will do it almost unanimously.” The president began moving federal law enforcement agents into D.C. late last week, following the vicious beating of a Department of Government Efficiency staffer, 19-year-old Edward Cristine, who intervened in a carjacking to protect the female victim. On August 11th, President Trump officially took federal control of the D.C. police department and quickly mobilized the D.C. National Guard. Between August 7 and the morning of August 13, the federal crackdown yielded 103 arrests on charges ranging from suspected homicide and narcotics to firearm offenses and even illegal immigration. Woman sues Marine who allegedly poisoned her drink with 10 abortion pills And finally, Liana Davis, a Texas woman, has become the latest victim of a disturbing case of the abortion kill pill being used against her, and she's taking her pain to court, reports The Washington Stand. Her lawsuit alleges that Christopher Cooprider, a 34-year-old U.S. Marine, impregnated her and repeatedly pressured her to “get rid” of the baby, despite her firm refusals. According to the federal lawsuit, Davis has accused Cooprider of dissolving at least 10 misoprostol pills into her drink, without her knowledge, at her Corpus Christi home while she was eight weeks pregnant with his child. It only took 30 minutes after drinking the spiked beverage for Davis to start “hemorrhaging and cramping.” She asked for his help, but Cooprider fled the scene and was unreachable. In a text at 12:30a.m., she wrote, “I am gushing blood. Please hurry.” But he never responded. Tragically, their baby died. The first half of Proverbs 28:13 describes Christopher Cooprider well. “He who covers his sins will not prosper, but whoever confesses and forsakes them will have mercy.” The wrongful death lawsuit seeks unspecified damages from Cooprider and Aid Access, the company which sold the abortion kill pill to the randy Marine. According to NBC News, “The Corpus Christi Police Department said there are no active investigations involving Cooprider.” The Marine declined to comment as well. According to a new study from the Journal of the American Medical Association, Aid Access shipped 118,000 chemical abortion pill packs into the U.S. between July 2023 and September 2024. On X, Family Research Council President Tony Perkins wrote, “The Trump administration must take action now. 1 in 10 women suffer severe complications from the abortion pill. Traffickers and others use easy drug access to control women and kill unborn children.
