On this week's BFG Podcast, we welcome in our friend Richard Rushfield, who takes time away from his deli-going schedule from time to time to stop by to talk to host Neal Pollack about issues and trends in Hollywood. Richard is recently back from Cinemcon, the annual Las Vegas convention for theater owners, who always have one question: "Does your movie star Dwayne Johnson, or does it star Ryan Reynolds?" Headier artistic questions don't concern them.For the first time in many years, Richard says, major franchises had no representation at Cinema Con. There was no Fast and Furious movie to tout. Even superhero offerings seemed kind of muted. The idea of a 90-day theatrical window went out the, well, window during COVID, and now they're trying to claw back a 45-day theatrical window. That seems highly unlikely. Finally, the conversation turns to what all of America has been waiting to hear about: Richard Rushfield's sleep apnea. It's a major problem for, we assume, his wife, but also for theatergoers around him when he takes his traditional nap an hour into a turkey. Neal and Richard try to find a workaround.BFG has also given near round-the-clock coverage of 'Drop,' an unserious thriller about cell phone misuse and bad dates that has been bombing at the box office since it released two years ago. Pablo Gallaga joins Neal to talk about 'Drop,' as the two of them continue to try to sell the public on the fact that this is the genre of the moment, the blood-soaked violence picture with a bit of cheeky comedy. We will write retrospectives about this genre, if not songs. Drop is pretty bad, they both agree, and it also accelerates a trend in modern pictures of extreme violence against women played as entertainment. We dip into cinematic classics of the silent age (before people learned how to talk) with 1923 classic SALOMÉ, co-directed by married couple Charles Bryant and Alla Nazimova. The latter stars as the titular SALOMÉ, a Bible character who has achieved historical infamy despite not being officially referred to by her first name. The story is provocative: a teenage princess dances seductively for her stepfather King Herod who promises her anything in reward; she picks the head of prophet John the Baptist, who is summarily executed and presented to her on a platter. Bryant and Nazimova's SALOMÉ takes cues from the Oscar Wilde play on the subject, adding extra-historical depth to the characters to portray Herod's throne in the throes of self destructive decadence. Male characters are simpering and devilishly gay (note that Alla Nazimova herself was openly bisexual, so this feels like intentional camp rather than homophobia.) Fashions are extravagant and imaginative, seeming both historical and futuristic. Physical acting is taken to extremes: SALOMÉ is a truly exciting watch, showcasing how much depth of character is possible from a silent film. To watch SALOMÉ is to not only experience an avant-garde take on Bible story, but also to see human ingenuity on full display with no boundaries to self expression despite limitations by emerging technology. I have the same issue, because I lived in Sweden during X years I am not able to donate blood here in the USA. What are some other means to remedy this? I'll add my values below from InsideTracker testing. 128 to 275 ug/dl range over 5 tests from the last 3 years. 34 year old fit male, diet is paleo + rice + whiskey. Protein sources are whey casein powder, chicken and red meat. Thank you! Garrett   Optimal Glucose level Tracie writes: Greetings from Spokane, WA! I reread the Paleo solution after Thanksgiving and have been pretty convinced I'll be sticking with the changes I made for life. I feel fantastic. Most of my digestive issues are gone, mood has improved, and I'm lifting more weight for more sets! I do have some anxiety/IBS that runs on my mom's side and I often feel pretty turned up, so I decided to give one of the Dexcom Stelo continuous glucose monitors a try. I was particularly stressed at work in December hiking around the woods in the snow as a forester. I noticed I'd wake up like clockwork at 11 pm and 1:30-2 am having trouble getting back to sleep, which seemed like a blood sugar low issue. Turns out I do dip down to 71 mg/dL but I'm very much in their recommended target range 99% of the time (with a little dip below 70 and so far not above 129 mg/dL post breakfast when there are tubers involved. When I wake up I'm just under 100. My question is what would you guys recommend is a more optimal glucose range? I feel like my fasted glucose in the morning should be in the 70s, and that having my blood sugar be more like 110 max after a meal would be more ideal. However, if you look/perform/feel great does it really matter? I know from your previous guests that bio individuality makes a difference and we shouldn't try to always be an A+ student. A “B” in OCHEM gets you the credit! My glucose level throughout the day is very steady around 100, I eat every 4 hours max. Breakfast at 6, a venison epic bar and satsuma tangerine at 10, chicken soup at 11:30-12:30, another venison bar and tangerine at 2:30, and dinner at 5. Breakfast and dinner is 4 ounces lean animal protein, 1/4 cup each color of fruit/vegetable, a little fat and salt for flavor. For some reference: I am a 34.5 year old female, 153 lbs, my sister is on an SSRI inhibitor/we have a family history of anxiety on moms side, familial hypercholesterolemia on dad side (so glucose control is important for avoiding heart disease) I eat 125-145 grams of protein from flank steak/smoked salmon/chicken/epic venison bars, 75-110 grams carbs from fruits/tubers/veggies, and 50-70 grams fat from animal proteins, some nuts, olive/avocado oil…and then a few times a month I stress eat some mostly paleo compliant junk snacks at work like eat a whole bag of coconut chocolate chip gluten free hippie cookies, or eat a whole Hu chocolate bar and a small bag of kettle sea salt and vinegar chips. I'm not a perfect angel, but I am 85% there. I sleep 11 hours on the weekend (cortisol induced likely), but mostly 8-8.5 with the occasional 7.8 hours. I have a lot of things going on: day job, trying to start a business, chasing moose through the woods, some of its great fun and the excitement seems to get me going too. So I have some dietary and personality factors that might drive my glucose a little higher than ideal.   Lead Testing Mari writes: Hello from North Idaho, I'm a long time listener and long time enjoyer of LMNT. I've recently become more concerned about heavy metals in what I'm consuming. I wanted to not pay attention to the chocolate situation a couple years ago because my HU bar addiction is not something I want to part with, but more keeps coming out. Recent testing done by “Clean Label Project” found a bunch of lead and cadmium in protein powders, with organic and chocolate flavored protein powders testing particularly high. I get that its a soil the chocolate in grown in thing (right?), and maybe also where/how its stored during processing? Anyways, to my point- I have an all winter long, morning LMNT hot chocolate routine. I'm also currently trying to get pregnant, so I thought I'd try to look into testing and I saw this immediately: LMNT Raw Unflavored Electrolyte Drink Mix Tests Positive for Lead: November 2024 Laboratory Report Trustworthy? No idea, which is why I was hoping you could speak to it. Thanks always for your courage and wisdom, Mari Lead Testing Hazel writes:  Hi Robb and Nicki,  I've been meaning to write to you on this topic for quite some time. I remember a few years ago there was a big to-do about heavy metals in chocolate - specifically lead and cadmium. The assertion was made, by whom I don't remember, that virtually all chocolate contained dangerous amounts of lead and we were all destined for lead poisoning if we kept eating our daily dark chocolate square. Much internet ink was spilled on the subject. Robb, you mentioned at that point that you were planning to write a piece about the bioavailabilty of that lead and whether we were really in danger. Did that ever get written? If so, please share the link! This question has come up for me again, but this time in a much broader context: there is a person on the internet who goes by the name Lead Safe Mama whose website contains pages and pages of highly alarming claims about the lead content of virtually any consumer good you can imagine - from children's and prenatal vitamins to pantry staples and cookware. She gathers this information by sending the products to a third-party lab for testing (related question: how reliable is your average third-party lab for this kind of stuff?). I don't doubt that we do have a lead contamination issue with any number of our consumer products, the same way we have a serious microplastics issue. And indeed, the LSM has been responsible for several recalls of glass products over the years based on her testing, so she's on to something. But I'm hoping you can provide more nuance to the claim that such a vast array of our products contain dangerous quantities of lead that our bodies are unable to detoxify. Do I need to add this to my ever-growing list of things to worry about? I'm Cynthia Rowland, and episode 109 is the third in a series of technical episodes describing the basic principles of the tax on unrelated business income generated by organizations described in Internal Revenue Code Section 501(c)(3). That includes public charities and private foundations. Most listeners are probably aware that 501(c)(3) organizations are generally exempt from income tax, but this does not mean that all income generated by the organization is free of income tax. For most organizations, unrelated business income is defined as income from a trade or business regularly carried on that is not substantially related to the charitable, educational, or other purpose that is the basis for the charity's exemption. In episode 107, I reviewed these defined terms. In episode 108, I explored the modifications to unrelated business taxable income that provide exceptions to income items that would otherwise fall into these definitions that are taxable but instead are categorically excluded because, generally, they constitute passive income. In this last episode in the series, I'll cover the exception to the exceptions for income that is debt-financed and thus generally taxable. I'm Cynthia Rowland, and episode 108 is the second in a series of technical episodes describing the basic principles of the tax on unrelated business income generated by organizations described in Internal Revenue Code Section 501(c)(3). Most listeners are probably aware that 501(c)(3) organizations are generally exempt from income tax. This does not mean that all income generated by the organization is tax-free. For most organizations, unrelated business income is defined as income from a trade or business, regularly carried on, that is not substantially related to the charitable, educational, or other exempt purpose that is the basis of the organization's tax exemption. In episode 107, I reviewed these defined terms and provided several good examples. In this episode, I'll explore the modifications to unrelated business taxable income that provide exceptions to income items that would otherwise fall into these definitions that are taxable but instead are categorically excluded because, generally, they constitute passive income. In the third episode of the series, I'll cover the exception to the exceptions for income that is debt-financed and thus generally is taxable income. Happy Monday! Sam and Emma speak with Mike Elk, founder of the Payday Report, to discuss the recent news out of Brazil that former President Bolsonaro has been indicted. Then, they speak with Jacob Silverman, co-author of his latest book Easy Money: Cryptocurrency, Casino Capitalism, and the Golden Age of Fraud, and writer at the AM Quickie newsletter, to discuss Trump's nomination of Howard Lutnick to be Commerce Secretary, and his associations with crypto. First, Sam and Emma run through updates on Israel's destruction of Lebanon, Israel's sanctioning of Haaretz, the Biden-Trump transition, Trump's developing cabinet, Trump's attacks on trans people, and Trump's plan of attack on Jack Smith, before diving a little deeper into the growing tension between the US-Israel alliance and the international order upholding the ICC's warrant for the arrest of Netanyahu. Mike Elk then joins, diving right into the major developments in Brazil's criminal case over the far-right (and Bolsonaro-administration-) backed coup and recently-revealed assassination in the wake of Lula da Silva's reelection in 2022, with the indicting of a massive portion of Bolsonaro's military advisors occurring last week in the leadup to the indictment of Bolsonaro himself this week, all at the hands of the Brazilian Supreme Court. Elk then walks Sam and Emma through the development of the investigation, with incredibly poor criminal operations UNRELATED to the coup allowing the Court to easily swing snitches over to their side, before stepping back to unpack the evolution of the fascist attacks on Lula and the Brazilian left, from the corrupt imprisonment of Lula and his successor, through the backlash to said political imprisonment, and into Bolsonaro's coup attempt in 2023. Mike expands on Bolsonaro's clear connection to the 2018 assassination of Rio de Janeiro city council member Marielle Franco, before wrapping up by noting the stunning impact this entire affair (and, more importantly, the Court's willingness to act on it) has had on completely decapitating the threat of the Brazilian far-right. Jacob Silverman then walks Sam and Emma through the major influx in crypto spending in the 2024 election cycle, despite the industry's recent stutters, and touches on the potential issue of an industry dominated by criminal conspiracy and transactions shaping US politics, before zeroing in on the particular role of one Tether, perhaps the largest cryptocurrency on the international level and a central investment of incoming Trump Commerce Secretary Howard Lutnick. After expanding on the incredibly shady, and fully not evidence-backed dealings of Tether, Jacob wraps up by unpacking the ultimate goal of the crypto industry in completely liberating the movement of money, freeing it from the shackles of absurd concepts like “fraud” and government regulations, and the role the Trump administration can play in making this goal a reality. In this JCO Article Insights episode, Alexandra Rojek provides a summary on "Post-Transplant Cyclophosphamide–Based Graft-Versus-Host Disease Prophylaxis Attenuates Disparity in Outcomes Between Use of Matched or Mismatched Unrelated Donors" by Schaffer et al published in the Journal of Clinical Oncology July 17th, 2024. TRANSCRIPT Alexandra Rojek: Hello and welcome to JCO Article Insights. I'm your host, Alexandra Rojek, and today we will be discussing an original report published in the October 1st issue of JCO titled, “Post-Transplant Cyclophosphamide–Based Graft-Versus-Host Disease Prophylaxis Attenuates Disparity in Outcomes Between Use of Matched or Mismatched Unrelated Donors,” by Shaffer et al. The CIBMTR registry study set out to compare outcomes of patients undergoing allogeneic stem cell transplantation hematologic malignancies by HLA antigen matching status as well as by the type of GVHD prophylaxis regimen received either calcineurin inhibitor-based prophylaxis or post-transplant cyclophosphamide or PTCy. This study included patients reported to CIBMTR from January 2017 to June 2021 with AML, ALL or MDS, and required that they have undergone allotransplant with either a calcineurin inhibitor based so tacro or cyclosporine, GVHD prophylaxis, or PTCy, which included a calcineurin inhibitor or sirolimus with or without MMF and ATG. Matched unrelated donors were defined as an 8 out of 8 HLA match. And mismatched unrelated donors were defined as HLA mismatched at any single locus or 7 out of 8. The primary objective of the study aimed to compare overall survival or OS and GVHD and relapse-free survival (GRFS) within and between matched unrelated donors versus mismatched unrelated donors separated by calcineurin inhibitor versus PTCy based GVHD prophylaxis. GRFS was defined as survival without grade 3 to 4 acute GVHD, moderate to severe chronic GVHD requiring systemic therapy or relapse. 10,025 patients were included from 153 centers, with a median follow up of over 36 months. Mismatched unrelated donor recipients were made up of 22% minority ancestry patients as compared to just 8% of patients receiving a matched unrelated donor allo transplant, showing an enrichment for patients of minority ancestry in the mismatched unrelated donor group. Just under 10% of patients were of minority ancestry in the study overall, reflective of challenges in transplant care for these patients, which may include inferior access to care, fewer available and suitably matched donors, among other factors. 54% of all patients were transplanted for AML and 29% for MDS. 45% of patients received myeloablative conditioning, 25% received regimens containing ATG, and 23% overall received PTCy with either a calcineurin inhibitor or sirolimus as well as MMF. Among patients receiving PTCy, the authors did not find differences in overall survival by degree of HLA matching, whereas among patients receiving calcineurin inhibitor-based prophylaxis, there remained survival differences by HLA matching status. When comparing matched unrelated donor calcineurin inhibitor patients with PTCy matched unrelated donor patients, the PTCy arm had better OS, and the mismatched unrelated donor group who received PTCy had similar OS as well. For GRFS, matched unrelated donor and mismatched unrelated donor PTCy patients had no difference in GRFS, similar to the trend the authors see with overall survival. But these patients also had better GRFS than matched unrelated donor patients receiving calcineurin inhibitor-based prophylaxis. Within each prophylaxis arm, there was no difference in GRFS by HLA matching status. HLA mismatched patients receiving PTCy were less likely to experience GRFS than HLA mismatched patients receiving calcineurin inhibitor-based prophylaxis. The authors saw similar differences in comparative trends when subgrouping patients based on conditioning intensity and additionally did not find differences in GRFS and OS by ATG exposure. When looking at patients with minority ancestry, those patients who received a match unrelated donor or mismatched unrelated donor with PTCy had comparable outcomes to non-Hispanic white patients. Additionally, among minority ancestry patients, there was a significant benefit in both GRFS and OS in the PTCy groups as compared to calcineurin inhibitor-based prophylaxis. When examining other specific toxicities included in the composite GRFS endpoint, such as GVHD rates among PTCy patients, the authors note that patients receiving a matched unrelated donor had similar rates of grade 3 to 4 acute GVHD but lower rates of moderate to severe chronic GVHD requiring systemic therapy. There appears to be signal that among PTCy patients, HLA matching reduced rates of moderate to severe chronic GVHD compared to mismatched unrelated donor patients receiving PTCy. These same trends also held when the authors looked at non relapse mortality with no significant differences within the PTCy groups by HLA matching status but reduced non relapse mortality compared to both calcineur and inhibitor-based groups. However, notably, there was a greater risk of relapse among matched unrelated donor PTCy patients than matched unrelated donor calcineurin inhibitor patients, although this risk was comparable between mismatched unrelated donor patients by type of prophylaxis. The authors note that this has also been observed in other retrospective cohorts and may be confounded by differences in conditioning intensity between these cohorts of matched unrelated donor patients, affecting the risk of relapse. Finally, the authors also evaluate whether expansion of donor search criteria to mismatch donors from full HLA matching would increase availability of young donors from minority ancestry patients, and the study noted striking increases for all subgroups examined. This study fits nicely with the BMT CTN 1703 trial published in the recent past, which has showed the superiority of PTCy with the calcineurin inhibitor and MMF when compared with conventional calcineurin inhibitor based immune prophylaxis for reduced intensity matched related donor and matched unrelated donor allotransplant. Of note, very few patients with one HLA antigen mismatch were enrolled on that study. However, others have shown the feasibility of PTCy in the mismatched unrelated donor setting, which has led to its adoption in practice. Although less than a quarter of patients included in this current study received PTCy overall, the findings clearly are aligned with the BMT CTN 1703 study, which is likely to change clinical practice in the longer term in this field. As the accompanying editorial in JCO, written by Dr. Chakravarty nicely lays out, the differences between this study and the EBMT registry study, also published in this issue of JCO are subtle but worthy of note. While both studies show that mismatched unrelated donor patients had worse OS and GRFS than those receiving matched unrelated donor transplants, and then among matched unrelated donor patients the addition of PTCy improved GRFS and OS, there is discordance between the studies whether the addition of PTCy abrogates the effect of HLA mismatching on GRFS and OS. As this editorial points out, there are strikingly different rates of T cell depletion with ATG between the US and Europe, which may account for differences in comparator arms that lead to this discordance. There are several very exciting clinical trials ongoing that will aim to answer some of these outstanding questions regarding comparisons of PTCy and T cell depletion, which the field eagerly looks forward to reviewing. In summary, this registry study of patients receiving allo transplant with matched unrelated donor or mismatched unrelated donor and calcineurin inhibitor or PTCy based GVHD prophylaxis, most notably shows that for patients who may not have a matched unrelated donor available, the addition of PTCy to a mismatched unrelated donor allo transplant allows for improved outcomes after transplant in toxicities and survival. This is most significant for patients of minority ancestries who usually have fewer matched unrelated donors available in registry searches. Improving the transplant options available to these groups of patients is of critical importance in improving equitable access to care for all of our patients. And this study, although retrospective in nature, provides an important understanding of our progress to date and suggests directions for future investigation may indeed be very feasible to continue to close these gaps in care for patients in need of an allo transplant for hematologic malignancies.
