Most people believe that strokes only affect adults, but children can experience them too. Although rare, strokes in kids can lead to catastrophic outcomes if not treated properly. Seizure or migraines are often misdiagnosed in this population, making it crucial to have proper protocols in place. While many never think of stroke as something that can impact kids, it's still in the top 10 causes of death in children.  To understand the scope of pediatric stroke and the strides being made to prevent and treat it, we are joined by two experts. Timothy Bernard, MD, is the Director of Education for the section of Child Neurology, and the Director of the Childhood Stroke Program here at Children's Hospital Colorado. He is also a Professor of Pediatrics and Neurology at the University of Colorado School of Medicine. Ethan Rosenberg, MD, is the Director of Inpatient Childhood Stroke and Neurocritical Care. In this episode, we cover the recent approval of Avadel's sodium oxybate formulation (Lumryz) as a treatment for pediatric patients with narcolepsy aged 7 years and older. The new indication expands on its previous one, granted in May 2023, which included the the treatment of cataplexy or excessive daytime sleepiness in adults with narcolepsy. It's initial approval was based on data from the phase 3 REST-ON trial (NCT02720744), a large-scale study in which the therapy met all 3 of its primary end points of change from baseline in mean sleep latency on the Maintenance of Wakefulness test, Clinical Global Impression Improvement, and weekly cataplexy attacks. Following its new indication, NeurologyLive sat down with Anne Marie Morse, DO, FAASM, a pediatric sleep disorders expert, to discuss the significance of the new approval. Morse, director of Child Neurology and Pediatric Sleep Medicine at Geisinger Janet Weis Children's Hospital as well as the program director for Child Neurology Residency Program, provided commentary on the clinical considerations behind this once-nightly formulation of sodium oxybate, and how it should be used going forward. Jennifer Bain, MD, PhD, is an assistant professor of neurology and pediatrics at Columbia University Medical Center.  Dr. Bain completed both an M.D. and PhD. as well as general pediatrics residency at Rutgers – New Jersey Medical School in New Jersey.  She then trained in Child Neurology at New York Presbyterian – Columbia University Medical Center in New York City and is a board-certified neurologist with special certification in Child Neurology seeing both inpatient and outpatient pediatric neurology patients. Her clinic focuses on diagnosis and management of autism, cerebral palsy and neurodevelopmental disorders in addition to genetic disorders associated with such conditions. Her early research career focused on spinal cord and brain development after injuries such as spinal cord injury and perinatal hypoxic ischemic encephalopathy.  During her residency training, her clinical research focused on studying autonomic dysfunction in children with autism spectrum disorders and neurological complications during extracorporeal membrane oxygenation. She currently works as a physician scientist at Columbia University specializing in general pediatric neurology with expertise in development, behavioral neurology, autism and cerebral palsy.  Her clinical research has focused on studying the genetics of neurodevelopmental disorders including autism and cerebral palsy. The genes she has worked closely on include HNRNPH2 and related disorders, GRIN disorders, KIF1A. She is interested in understanding clinically meaningful measures in families affected by neurodevelopmental disorders and measuring longitudinal trajectories in such disorders. She has been working closely with several patient advocacy groups, researchers, and Simons Searchlight to continuously move forward in the understanding of the developing and aging brain. Chanel grew up in Detroit and adopted a love for writing at an early age. As young as 8 years old she participated in various writing and oratorical contests to hone her skills. As Chanel advanced through school she continued to participate in writing contests, published poems and participated in creative writing programs.Chanel graduated from the University of Michigan-Dearborn with a double major in Psychology and Criminal Justice and a minor in Communications. She had the dream of pursuing a law career but life took a different turn. Instead, she worked in the social services field, working with underprivileged youth in group homes and independent living programs. Chanel eventually came to the realization that she wanted to reach children much earlier in life. The goal being, to help alleviate some of the issues and problems that plagued the children within, her then, current position. It was at this time Chanel started working at the Georgia Institute of Technology – Child Study Lab, as a research Assistant.In her position at Georgia Tech, Chanel assisted with research to help in the early detection of Autism and other developmental delays. While working as a research assistant, she was co-author on scholarly articles that were published in journals such as: The Journal of Developmental Medicine and Child Neurology, Nature Communications and the Journal of Autism and Developmental Disorders.At Georgia Tech Chanel also became a co-instructor for the Universities freshman seminar course, for incoming first year students. During this time, she started to pursue her Masters in Public Policy at the Andrew Young School of Policy Studies, at Georgia State.Shortly after starting schooling for her Masters Degree, Chanel got married to her husband, Justin Miller. After getting married and having children, her career goals shifted. She put her masters degree on hold and left academia to become a stay at home mom. Once her children grew, she started the blog, Them 3 and Me, where she wrote about life as a stay at home mom who also homeschools. The blog led Chanel to start self publishing her own literary works, the first being, Kid De Cuisine: Hadasah's Breakfast Delights. Dr. Devorah Segal was born in Baltimore and grew up in Brooklyn, NY. She graduated from Prospect Park Yeshiva and went to BJJ seminary, followed by Brooklyn College. She attended Mount Sinai Medical School, where she earned her medical degree and a PhD in neuroscience. She then completed a residency in child neurology at Rutgers New Jersey Medical School and a fellowship in pediatric neuro-oncology at NYU. Dr. Segal practiced general child neurology and neurogenetics at Weill Cornell for several years, where she was also the director of the Child Neurology residency training program. She then returned to NYU, where she practices pediatric neuro-oncology and is the Associate Director of the Comprehensive Neurofibromatosis Center at NYU and the director of the pediatric neuro-oncology fellowship. Dr. Segal is the principal investigator or co-investigator of several clinical trials looking for new treatments for nervous system tumors. She is also the site principal investigator of a clinical trial studying a treatment for metachromatic leukodystrophy. Dr. Segal lives in Passaic, NJ with her husband and 5 children, all of whom were born while she was in medical school.

He is part of the North American Mitochondrial Disease Research Consortium (NAMDC) and the Primary Investigator for the Pearson Syndrome Natural History study. He is an invited lecturer at national meetings and hospitals. He completed his residency in pediatrics and fellowship in child neurology at the Children's Hospital of Pittsburgh and received additional training in genetics and metabolism at Cleveland Clinic and Centers for Inherited Diseases of Metabolism. Dr. Parikh has had the privilege of having Bruce Cohen, Charles Hoppel and Marvin Natowicz serve as his teachers during that time. He joined the Cleveland Clinic in 2004. Since 2007, Dr. Parikh has been selected as one of "America's Best Doctors." Learn about current treatment options. About the Speaker Dr. Anselm is a member of MitoAction's medical advisory committee and practices in Child Neurology at Children's Hospital Boston. Her research is focused on clinical presentation of children with mitochondrial disorders and their response to therapy with different medications and vitamins/ supplements. Dr. Anselm is currently participating in several clinical research studies. She is a co-investigator in a study using Dichloroacetate (DCA) to treat children with lactic academia. She sees children enrolled in the protocol when they are admitted to CH for DCA initiation and follows them in the Mitochondrial Program while they are being maintained on the DCA. Dr. Anselm also is Principal Investigator in a study entitled "Indirect calorimetry and nutritional assessment in patients with mitochondrial disorders." Children enrolled in this study undergo evaluation of their resting energy expenditure by indirect calorimetry and nutritional assessment. They also undergo metabolic evaluation by measurement of the levels of micronutrients/vitamins in their blood. Dr. Anselm also is involved in a multicenter study of patients with Angelman syndrome.

Some talking points will include: Primary mitochondrial disease (PMD) is ideally diagnosed by a known or indisputably pathogenic mitochondrial or nuclear DNA mutation. Secondary mitochondrial dysfunction (SMD) can be caused by genes encoding either function nor production of the oxphos proteins and accompanies many heriditary non-mitochondrial diseases. Secondary mitochondrial dysfunction (SMD) can also be caused by enviornmental factors. In the absence of the ability to diagnose a primary mitochondrial disease (PMD), mitochondrial dysfunction can be effectively treated with standard treatments for PMD. When the etiology of mitochondrial dysfunction is unknown, re-evaluation for genetic and other causes should be revisited on a regular basis. About The Speaker: Dr. Richard Frye is a pediatric neurologist and Chief of the Division of Neurodevelopmental Disorders at Phoenix Children's Hospital. He received his MD/PhD from Georgetown University in 1998. After those 7 years, she trained in pediatrics at Westchester Medical Center. To further her expertise within the field, Dr. Dallara-Marsh completed fellowships in Neurology with Special Qualification in Child Neurology and Epilepsy at New York Presbyterian Hospital-Columbia. Some of Dr. Dallara-Marsh's interests include child neurology, epilepsy, and palliative care. She currently works in a private-practice setting in New Jersey. The Monarch external Trigeminal Nerve Stimulation (eTNS) System, a new device recently approved by the US Food and Drug Administration offers that possibility. Some parents on social media say they're ready to sign up their child for testing. Is it too soon for people managing ADHD to jump ship from their current treatment regimen?  We will hear from Dr. Max Wiznitzer, an associate professor of pediatrics, neurology, and international health at Case Western Reserve University. He serves on CHADD's board of directors and the editorial advisory board for Attention magazine, as well as the editorial boards of Lancet Neurology, and the Journal of Child Neurology. Dr. Wiznitzer lectures nationally and internationally about various neurodevelopmental disabilities.
