We recently published a podcast on palliative care for kidney failure, focusing on conservative kidney management. Today we're going to focus upstream on the decision to initiate dialysis vs conservative kidney management. As background, we discuss Manju Kurella Tamura's landmark NEJM paper that found, contrary to expectations, that function declines precipitously for nursing home residents who initiate dialysis. If the purpose of initiating dialysis is improving function - our complex, frail, older patients are likely to be disappointed. We also briefly mention Susan Wong's terrific studies that found a disconnect between older adults with renal failure's expressed values, focused on comfort, and their advance care planning and end-of-life care received, which focused on life extension; and another study that found quality of life was sustained until late in the illness course. One final briefly mentioned piece of background: John Oliver's hilarious and disturbing takedown of the for profit dialysis industry, focused on DaVita. And the main topic of today is a paper in Annals of Internal Medicine, Maria first author, that addressed the tradeoffs between initiating dialysis vs continued medical/supportive management. Turns out, in summary people who initiate dialysis have mildly longer lives, but spend more time in facilities, away from home. We also discuss (without trying to get too wonky!) immortal time bias and target emulation trials. Do target trials differ from randomized trials and “ordinary” observational studies, or do they differ?!? Eric is skeptical. Bottom line: if faced with the decision to initiate dialysis, waiting is generally better. Iako je njen posao neodvojiv od telefona, kompjutera i ekrana u svakom smislu, Aleksandra je uspela da svoju decu, u prvim godinama života, drži podalje od digitalnog sveta, pa čak i od crtanih filmova, koje samo povremeno gledaju, ali im nimalo ne nedostaju u realnom svetu igre i kreativnosti. „Meni je telefon često u rukama jer bez njega ne mogu da radim i deca to razumeju. Pružamo im i organizujemo razne igre u realnom svetu, toliko da im igrice, craći, mreže nisu potrebne. To ide dotle da u dnevnoj sobi imamo penjalicu iznad garniture, pa im doslovno dozvoljavam da mi se penju na glavu“, priča uz osmeh Aleksandra, i dodaje da je u prvim godinama života ključno da deca što manje budu pred ekranom, zbog razvoja govora i koncentracije, koju telefon oduzima. Ona naglašava da i pored brojnih fotografija i videa, koje možemo videti na njenim profilima, nikada nije objavljivala slike dece. „Desilo mi se da me ćerka pita zašto nigde nema njihovih slika, pa sam joj objasnila da ne želim da joj nepoznati ljudi prilaze u parku i javljaju joj se po imenu, jer su je prepoznali sa mog profila. Moj posao je moja stvar, ne želim njih da uvlačim u to, posebno jer su još mali, a i dosta sam zaštitnički nastrojena po tom pitanju“, objašnjava Aleksandra. U razgovoru za Mamazjaniju istakla je da je u metodama vaspitanja najvažniji model koji pružamo deci, ali i da smo neminovno svi, bar delimično, kopije naših roditelja, jer ono što smo slušali tokom odrastanja prenosimo dalje. Aleksandra se osvrnula i na odnos sa svojom majkom koji, kako kaže, nije drugarski, ali jeste blizak. Iako se često sukobljavaju u stavovima, sada joj je zahvalna za neke zabrane i pravila, koja su joj bila nametnuta dok je bila tinejdžerka. „Bila sam jedna od retkih u društvu kojima je bilo zabranjeno da idu na splavove. Mama to nije dozvoljavala i ja sam bila očajna i strašno ljuta zbog toga. Svi bi posle nekog pića u gradu produžili na splav, a ja - kući. Međutim, danas sam joj duboko zahvalna za to i smatram da zato ponekad treba i da naljutimo svoju decu, jer će nam kasnije, kad odrastu, reći hvala“, kaže Aleksandra. Na Sajmu knjiga ove godine, Aleksandra će biti prisutna kao autor sa knjigama „Nešto zeleno“ i „Nešto zdravo“. Research at Boston University has led to the discovery of a non-invasive method to diagnose Alzheimer's. This opens the door in the coming years to possibly detecting the disease in its early stages, decades before real symptoms appear. Manju Subramanian, MD and her team found that proteins in eye fluids are providing this window to the brain. These eye fluids are confirming pathological brain conditions like dementia in the Alzheimer's form. Until now, MRIs and lumbar punctures were the tools to aid the clinical diagnosis of Alzheimer's, but that has meant late detection when the disease is already in place. Alzheimer's is not actually confirmed until after death and a post-mortem examination of the brain is done. "We know that patients with eye disease tend to be an at-risk population for dementia. Patients with macular degeneration, glaucoma and diabetic retinopathy, those are the three big ones," says Subramanian. The potential of an eye fluid exam at an optometrist's office is ideal as it's non-invasive and not expensive. But, it is still several years out before potentially becoming commonplace. More research is needed. Still to be determined in future research is just how early eye fluid proteins become abnormal when dementia is developing. "As they say, the eye is the window to the soul. It is also very much the window to the brain," says Subramanian. ***** Manju Subramanian is an Associate Professor in Ophthalmology and Vice-Chairman of Faculty Affairs. She is an ophthalmic surgeon specializing in Vitreoretinal Disease and Surgery, and is in academic practice at Boston Medical Center. She also sees patients at the Dedham Ophthalmic Consultants. Her primary areas of clinical interest include medical and surgical management of diabetic retinopathy, age-related macular degeneration, retinal detachments, hereditary retinal diseases, ocular inflammation, and ocular trauma. Dr. Subramanian graduated from the University of Missouri School of Medicine and completed her residency at the University of Kansas Medical Center in 2002. She completed a fellowship in Vitreoretinal Disease and Surgery at Tufts University School of Medicine and Ophthalmic Consultants of Boston in 2004. Dr. Subramanian's research interests include the study of eye-based biomarkers for Alzheimer's Disease (AD), age-related macular degeneration, diabetic retinopathy, and the role of anesthesia in eye surgery. She was Principal Investigator for the first head to head clinical trial comparing the use of bevacizumab and ranibizumab in the treatment of age-related macular degeneration, and she is currently the Principal Investigator for a study assessing the role of oral sedation in eye surgery. She is also a recent recipient of an R03 Grant Award by the National Institutes of Aging as the Principal Investigator of a study looking at protein biomarkers for AD in the eye. In her role as Vice-Chairman of Faculty Affairs at Boston University Eye Associates, she works in a supportive role in the professional and career development and engagement of the clinical faculty. Prior to 2017, she served as the Vice-Chairman of Clinical Services for 8 years. She serves on several institutional committees, including the Women's Leadership Advisory Council, the Boston University Medical Group (BUMG) Research Committee, the BU School of Medicine Promotion Criteria Working Group, and also serves as Chair of the BUMG Professional Development Committee. Uncomfortable Conversations Podcast The Untold Stories of the 3HO Kundalini Yoga Community
Manju Sadarangani was born and raised in India, surrounded by yoga. Her yoga teacher mother insisted on an upbringing informed by yoga and traditional medicine. In her teen years, her deeply spiritual brilliant father joined a Hindu cult, resulting in upheaval resulting in her immigration to the United States with her mother and brother. Manju found yoga in the US to be deeply puzzling. She would try yoga studios and styles of yoga in an effort to feel the spiritual solace of her childhood yoga practice, but felt extremely excluded at American yoga studios. Through most of her adult life, she avoided yoga, and practiced it only when she visited the country of her birth. In 2019, serendipity led her to a kundalini yoga class. She was intrigued by this unfamiliar style of yoga which she found absurd and amusing, but was soothed by the elements of Surat Shabd that she recognized in kundalini yoga practice. She enjoyed the element of play and creativity in her kundalini meditations, but found herself often flummoxed by the pastiche mantras and practices. She was offended by some practices (like excerpts of Japji inserted into kriyas) - but when she asked questions of kundalini teachers, they were not able to give her answers that felt logical or grounded in any yoga discipline. Her teachers encouraged her to take teacher training with KRI, telling her that was where she would get answers. Manju was understandably reluctant, especially after she read the book Premka. In 2020, she was approached by representatives of KRI, who recommended a brand-new global teacher training program. This new TT was advertised as trauma-informed, inclusive, egalitarian - a symbol of the new direction KRI and 3HO were embracing. Manju applied to this new teacher training global cohort. She was one of only three Americans, all women of color. It turned out to be one of the most traumatic and racist experiences of her life. Since then, Manju has relied on her command of languages, academic background in South Asian history, conversations with elders and teachers in India, and cultural background to deeply research, question, retrain, and reform her yoga practice. She connects deeply with the feminist, spiritual and historical foundations of yoga to counter problematic cultural appropriation. Her evolving yoga practice is grounded in a desire for equity, justice, emotional and mental well-being. Wellington Zoo is now home to a pair of snow leopard sisters. Asha and Manju have come from Melbourne Zoo and are the only snow leopards in New Zealand. Our visual journalist Samuel Rillstone went to see them just hours after they were let into their enclosure.
A Snow Leopard expert has travelled all the way from Nepal for today's opening at Wellington Zoo. Snow Leopard sisters Asha and Manju can be seen from today in their new habitat, which has taken 18 months to build.

Maybe you're doing something like making an English lesson video, and you might want to bring the lesson to a close when we get close to the four minute mark. So when you bring something to a close, it means you've decided you're going to end it.So to review, when you bring someone back down to earth, or when someone is brought back down to earth, it means that they're reminded somehow, something happens that reminds them that whatever they're doing or whatever they're involved in is not as amazing or won't be as amazing all the time as maybe they think it would have been. Manju Hedge, CEO, Uhnder, joined Grayson Brulte on The Road To Autonomy Podcast to discuss why 4D Digital Imaging Radar will enhance ADAS safety. The conversation begins with Manju discussing why he gave a talk at the Univeristy of Michigan College of Engineering called; “When We Should Trust Self-Driving Cars” on July 28, 2016. The talk revolved around the topic of trust and the critical role it will play in the adoption of autonomous vehicles. Trust is important. Trust is in fact, critical. – Manju HedgeWithout trust, there will not be autonomous vehicles. Consumers have to trust that the autonomous vehicle will get them to and from their destination, safely and on-time. Trust is a thing that you have to earn slowly. It cannot be accelerated. – Manju HedgeTo build and maintain trust in autonomous vehicles, the proper exceptions have to be set. To properly set exceptions with the market and investors, Uhnder has focused on the ADAS market because as Manju says; “as a start-up revenue rules”. This philosophy was adapted and implemented because Uhnder is a semiconductor company which by definition is capital intensive. Radar which first got its start in World War II has traditionally been a capital intensive business, but it has been a business that has saved millions of lives around the world. Building upon the first radar systems which were produced in 1935 by Sir Robert Watson-Watt, Grayson asks Manju how radar has evolved since World War II. It's the progress in electronics, because remember in the Second World War, the integrated circuit was not there yet. So that was a huge advance. Then the progress in communications, that's more recent. I would say that from 1940's to say the 1990's, it was a tremendous increase in electronics. – Manju HedgeToday, a majority of vehicles on the road have radar. Radar on vehicles increases safety due to their ability to detect objects on the road. The next evolution of radar is 4D Digital Imaging Radar. 4D Digital Imaging Radar has advantages over analog radar such as increased resolution.The first vehicle that the Uhnder 4D Digital Imaging Radar will be available on will be the Fisker Ocean.
