Today we're having a conversation with MA Family Nurse Practitioner Rose Cain about how we might encourage our loved one's facility to implement a medical cannabis program, or even natural, full spectrum, hemp CBD. Ep.66 Embracing Cannabis Treatment for Hospice Patients with Megan Mbengue BSN, RN, CHPN Have you ever considered cannabis as part of your loved one's treatment for their end of life? If not, this episode with Megan Mbengue has all the necessary information. Cannabis may have a role in hospice care for symptom management and improving the quality of life for some patients near the end of life. Products containing both THC and CBD have been used by some hospice patients to help manage pain, especially when other pain medications are not effective or cause undesirable side effects. However, Megan advises that this kind of treatment should be approached with caution, fully considering legal, medical, and ethical factors, and always under healthcare professionals' guidance.     Megan Mbengue, BSN, RN, CHPN, is the Founder & CEO of Trusted Canna Nurse. She started Trusted Canna Nurse when she realized there were very few medical professionals well-versed in cannabis and CBD, much less someone who knew how to navigate a dispensary. She has been a nurse for over ten years, specializing in oncology, Labor & delivery, and, most recently, hospice & palliative care. Megan has received national certifications in all three specialties and is currently working towards a Master of Science in Medical Cannabis Therapeutics. Megan has done various speaking engagements and received various awards (read about them here). Additionally, Megan is an active member of the following professional organizations: Society of Cannabis Clinicians, Cannabis Nurses Network, and American Cannabis Nurses Association.   Tune in!   Key Highlights from the Episode; [00:30] Episode intro and a quick bio of the guest, Megan Mbengue  [01:51] Meet and greet Megan Mbengue and know what she does in her space [02:47] Great resources to research cannabis treatment [03:42] Megan's history from hospital nursing to the cannabis space [06:40] How Megan helps patients [07:59] Her partners in running the program [09:14] Megan's top three health conditions she helps with [09:44] Cannabis benefits for hospice patients [11:55] How cannabis treatments help in combating end-of-life anxiety [15:23] Using cannabis and psychedelics for treatments [18:56] How micro-dosing could help hospice patients get a greater sense of peace  [20:26] Is cannabis treatment good for trauma-based patients? Susan Marks, aka Nurse Susan, is passionate about the therapeutic benefits of cannabis. She is a medical cannabis patient. She has a progressive neurological movement disorder, and cannabis (high CBD, low THC) has allowed her to stop anti-seizure medication and the intolerable side effects. She is also a highly qualified healthcare professional, holding a nursing degree and several nursing certificates. She works with medical patients who want to include cannabis therapeutics in their health management program.During this fascinating conversation, Susan gives a history of the hemp plant as a product and cannabis as a medicine. We hear how political and financial motivations forced this highly valuable plant to become labeled a dangerous drug and eventually become placed as a Schedule I narcotic. The three criteria for a drug to be included in the federal list of Schedule I narcotics are:1. No medical benefit2. Addictive3. Very unsafe and dangerousDoes it meet these criteria? Nurse Susan believes it does not, listen in to hear why she feels this way. What is the difference between THC and CBD? Nurse Susan explains that basically, CBD works from the neck down and THC works from the neck up. However, she feels strongly that using the complete plant and portions of all the cannabinoids is the optimal way to treat a medical condition therapeutically.Does this mean you're going to get high? Not necessarily as this can be completely modulated and controlled. But, you need both. Why? Because the CBD diminishes the psychoactivity of THC and the THC boosts the effectiveness of the CBD.Why do different people experience different responses to cannabis? Two reasons, the product and the individual. When it comes to product, Susan says, "There's a lot of crap out there" and you have to make sure it is lab tested. And individuals need to start with a low dose, pay attention to the response, and adjust their protocol based upon that response."Know the product, know it's safe, and know how you respond."PUBLIC SERVICE ANNOUNCEMENT: Cartels are now spraying cannabis with Fentanyl to make it highly addictive, BEWARE!What inspired Susan to become an advocate for medical cannabis? And what's the most badass thing she's ever done? This episode of the Plant Medicine Podcast features a conversation with Dr. Carey Clark, discussing delta-8 THC. Dr. Clark is a registered nurse and an expert in cannabis care nursing. She holds a PhD from the California Institute of Integral Studies and is the immediate past president of the American Cannabis Nurses Association. She is also the editor and author of the first nursing textbook on medical cannabis Cannabis: A Handbook for Nurses and has over thirty publications in academic journals. Currently Dr. Clark serves as a professor at Pacific College of Health and Science where she also developed the first college-level, academic certificate in medical cannabis. In this conversation Dr. Clark introduces delta-8 THC, discussing its chemistry, pharmacology, and legal status. She begins by sharing that delta-8 is a minor cannabinoid which exists naturally in small quantities within cannabis and hemp. Delta-8 THC is chemically very similar to delta-9 THC—the primary cannabinoid within cannabis which is known for its mind-altering and medicinal properties—and in fact, delta-9 naturally degrades into delta-8 under certain conditions. Due to the structural similarities, delta-8 produces similar effects to delta-9, and may have similar medical applications. Dr. Clark mentions that delta-8 may be synthesized from CBD, allowing for this compound to be readily produced from legal components. However, the delta-8 which results from these synthesis methods contains traces of unknown compounds, perhaps as a result of the particular solvents used in the reaction, so Dr. Clark cautions that consuming these products could have certain risks, though the delta-8 cannabinoid itself is thought to have a similar safety profile to delta-9 THC. Instead. Dr. Clark emphasizes that whole plant medicines are likely the ideal for patients seeking to treat medical conditions with cannabis, but since delta-8 may be more readily available in certain areas due to its murky legal status it may be the best option for some individuals.  While scientific research into delta-8 is currently lacking, Dr. Clark shares the results from a few studies which use animal models and one study looking at the effects of delta-8 in the context of pediatric oncology. In addition to these published studies, Dr. Clark also shares anecdotal reports from patients, saying that these experiences are crucial, preferring to think of these reports as qualitative data which has yet to be systematically studied.   In this episode: What delta-8 THC is and how it is made Potential safety concerns with delta-8 products The current research on delta-8 The legal grey-area of delta-8 THC and states which have banned the cannabinoid Differences in the doses and effects of delta-8 and delta-9 THC   Quotes: “My hope as well is that people who are able to access whole-plant medicine can really use whole-plant medicine safely and effectively and they have access to safe, effective medicines. That would be my recommendation—if somebody had a choice—over choosing a really synthesized medicine at this point because of the safety issues.” [15:30] “[T]there's still very little research and most of the patient experience we're hearing is really anecdotal.” [24:52]   Links: Dr. Clark's faculty page at Pacific College of Health and Science Psychedelic Medicine Association Porangui

Opioids play a critical role in managing cancer-associated pain, but they're highly addictive, and they don't always provide sufficient pain relief for cancer patients. Many patients therefore turn to cannabis or cannabis-derived products to help manage their pain, using them either as standalone treatments or as an adjuvant therapy to opioids. But does research support cannabis use for this purpose? The evidence is mixed, and some studies suggest there may indeed be benefits for cancer pain—but there's also reason for caution. How Much Research Is There? There's a relatively strong body of research on cannabis for the treatment of chronic pain generally, according to Eloise Theisen, RN, MSN, president of the American Cannabis Nurses Association and co-founder of Radicle Health, a cannabis education and advocacy organization. However, much of this research has focused on neuropathic pain in multiple sclerosis and spinal cord injuries, and comparatively few studies have examined pain in the cancer context. Among the studies that exist for cancer, many have small sample sizes, and study design is also an issue. “We have animal studies and some clinical human studies,” says Theisen, but there's “a lack of randomized controlled trials, which are considered to be the gold standard.” A further complicating factor in the research on cannabis and cancer pain is that most of the existing studies on this topic have relied on products that do not match those cancer patients actually use in practice. According to Hance Clarke, MD, PhD, associate professor of anesthesia at the University of Toronto and author of a recent review article on cannabis and pain,1 the products that have been studied most extensively are pharmaceutical-grade cannabis isolates, such as nabiximols (an extract containing THC and CBD in a precise one-to-one ratio), nabilone (synthetic THC), and dronabinol (also synthetic THC). However, cancer patients outside of the research context are much more likely to use dispensary cannabis and non-pharmaceutical-grade plant-based extracts. “We're kind of stuck without enough evidence on the products that people are currently using,” says Clarke. This is important, because patients may react quite differently to a dispensary product than to a pharmaceutical product, according to Clarke. For example, cannabis is said to help manage anxiety or symptoms of PTSD. However, if one takes too much cannabis it can cause anxiety and panic. Also, if one with PTSD obtains a cannabis chemovar that is high in pinene it can cause the symptoms of PTSD to become worse. Cannabis is also known to increase heart rate up to 50%.A recent study published in Forensic Science International this month declared “After exclusion of other causes of death we assume that the young men experienced fatal cardiovascular complications evoked by smoking cannabis.”These reports indicate that the need for education is paramount for the safety of the consumer. CBD is a great vasorelaxant and can decrease blood pressure as well as stabilize blood sugar levels. CBD does not activate CB1 receptors in the brain, and is devoid of the psychotropic actions of THC (meaning it will not get you high). Indeed, CBD may antagonize the psychoses associated with THC (meaning CBD decreases the effects of THC). The difference between cannabis oil and hemp CBD oil is the amount of THC that is allowed. Hemp CBD oil is allowed to have 0.3% THC along with all the other cannabinoids found in that particular plant. Cannabis oil has a lot more THC and very little CBD. A research study suggests that 96% of cannabis has no CBD in it at all. CBD is found mostly in the Hemp plant. CBD isolate is just that- only the CBD. This isolate does not have any other cannabinoids. CBD is the cannabinoid that has the most medicinal benefit- CBD alone is great at decreasing pain, inflammation, nausea, insomnia, and muscle spasms as well as many other symptoms too.  Also, CBD does not show on a drug screen test as they usually only test for THC. When you use full spectrum cannabis or hemp products you are getting the most medicinal benefit as they all work well to balance each other and create an entourage effect. It is important to remember that if you use a full spectrum hemp oil that you can fail a drug test as these products do have 0.3% THC. There are both pros and cons to the different methods of administration. Smoking the byproducts can cause serious diseases in a person. Vaping or dabbing can reduce those harmful byproducts by not combusting the product. Inhaling is the fastest way to get cannabis into your body-- it is also the shortest lasting-- meaning you have to use this method a lot more often to feel the medicinal benefits. Note: We caution you that vaping has been determined to be dangerous. Edibles or Medibles take a lot longer to feel the effects although the effects may last up to 8 hours using this method. When you eat cannabis it is important to remember that the Delta 9 THC converts to 11 Hydroxy THC which is 3-4 times more potent and psycho active than Delta 9 THC. 11 Hydroxy THC passes through the blood brain barrier easier and that is good news for Parkinson’s patients. Sublingual is the easiest to micodose and is often used by the pediatric patients the most. Topical products are not psycho active and work at the site where it is applied.Medical marijuana may affect the effectiveness of medications taken to control GVHD (tacrolimus, cyclosporine, steroids) as well as anti-depressants and potentially antibiotics and antifungals: What we do know from our knowledge is that it does have some effect on post-transplant medications so there is some effect on mycophenolate, tacrolimus, cyclosporine or steroids so many of the medications a patient might receive post-transplant, these agents may inhibit them and cause increased levels of the drug which may result in toxicity. Also, it can inhibit our antidepressants which would again increase our level of toxicity with those drugs. We can see some potential drug-drug interactions with our antibiotics, as well as our antifungals which many transplant patients will receive. We do worry about these because things like Voriconazole, Posaconazole, any of our azoles. Many of our mycins, erythromycin, clarithromycin, antibiotics may be affected by these. There are concerns about infection in transplant patients who use marijuana: From an infection standpoint, we do have concerns. Especially with smoking marijuana in our patient population. We have concerns with fungal infections. There are some case reports looking at patients who are smoking marijuana or who have smoked marijuana or who are immunosuppressed post-transplant who have developed severe fungal infections and have died from those fungal infections. That could be a result of mold spores that may be contained in the buds that are smoked, as well it could be a result of increase in inflammation and causing infection risk in the lungs. Again, when we have patients who are immunosuppressed, who have too few white blood cells, we really do advise them NOT to use smoked products. We really advise them to use edibles or tinctures of some other nature.Microbe contamination of marijuana products is a concern: There's no standard manufacturing, so we worry about microbe contamination just like we would worry about smoking those natural products with mold spores.Micro contamination, we worry about pesticide contamination from how the crops were raised. With that, in our immunosuppressed patient population this is a real concern because patients can be more susceptible to these types of toxicities. Then there is just an unmonitored chain supply. Again, with FDA approved drugs as they go through quality control - we do monitor where the drugs are and what temperatures they are and when they have been where. In this situation, there really is an unmonitored chain supply so we do not know where there might be potential places for contamination to occur as the product is being prepared for patient use.Small studies suggest that marijuana may help in control of Graft Versus Host DiseasePlease remember: this is an overall, general education. Again, we want to stress that with laws differing so much state to state regarding the use of cannabis or CBD, it is very important you learn the laws for YOUR state. We also are working with what is legal at the time of this recording, in June 2020. On this episode, we talk with CannMed 2020 presenter Eloise Theisen of Radicle Health. Eloise is a board-certified Adult Geriatric Nurse Practitioner with over 20 years experience in nursing. She started her own cannabis practice about 6 years ago and she has treated over 6000 patients using cannabis. There are very few healthcare providers nationally with the same level of expertise and experience. Eloise is the President of the American Cannabis Nurses Association and will serve as President through 2022. Eloise made her first CannMed appearance at our 2019 event in a presentation sharing her experience using Cannabis to treat geriatric patients. We talked about some of the unique challenges that come with working with seniors including the stigma older patients have towards cannabis, how the endocannabinoid system changes as we age, and navigating drug interactions.  We are excited to have Eloise as a presenter at CannMed 2020 this year, but that will not be her only contribution to the event. Eloise will also be an instructor at a full-day Medical Practicum along with Dr. Bonni Goldstein, Dr. Dustin Sulak, Dr. Kevin Spelman. This is a great opportunity for healthcare professionals to learn about the latest research into cannabinoid therapies and also tap into the deep clinical experience the instructors have to offer. When you consider that most medical schools don't teach the endocannabinoid system, courses like this are really the only way healthcare providers can learn how to properly administer cannabinoid therapies. Attendees will walk away with practical information about the different types of extractions and products that are on the market and optimal dosing.  Due to federal guidelines, CME credits for physicians are not available for the practicum, However, This course is approved by the California Board of Registered Nursing for 9.5 contact hours** Learn more at CannMedEvents.com/practicum.  Click the player below to listen, and thanks to our sponsor, Radicle Health SIGN UP FOR PODCAST UPDATES AND ENTER TO WIN 2 VIP DINNER TICKETS! We heard Eloise Theisen speak about how cannabis helps seniors at CannMed2019 - a huge, very serious medical cannabis conference where we also met Dr. Raphael Mechoulam, the Godfather of Modern Cannabis Research. Every one who is anyone in the medical cannabis space attends this conference! Eloise Theisen, is a board-certified Geriatric Nurse Practitioner who has cultivated a wealth of knowledge and data for using cannabis to treat a broad range of conditions and diseases, especially those that commonly affect seniors. She has helped patients use cannabis to help treat chronic and age-related illness and was one of the first healthcare practitioners to bring a clinical dosing regimen to the cannabis space. She's treated more than 5,500 patients using cannabis. Eloise previously served on the board of the American Cannabis Nurses Association and is now the President. She helped in the development of ACNA's first online core curriculum, providing over eight hours of educational content as well as continuing education credits for nurses. Her work has been featured in Newsweek and the SF Chronicle, among others. There are very few healthcare practitioners in the US with a comparable level of cannabis expertise and experience. She is available via telemedicine to consult globally, and can be a patients' clinician if they live in California. Sherri brings passion and purpose to her work teaching bio-psycho-social-spiritual healing using cannabis as a tool.In this episode, Sherri shares from the annals of intensely personal experience detailing how her entire life went from one of a busy full time nurse, working in hospitals in Boston, single mother of three to becoming completely bedridden, dependent on others for care. She states, "I'm going to be a little bit transparent today. I think it'll be really helpful to talk from my own personal perspective in working with cannabinoid therapeutics and mental health in regards to my own mental health. So before I got sick in 2012 I had a history of migraines and ADHD-attention deficit disorder. Both of those were managed very well with medications, working full time as a nurse in the big teaching hospitals in Boston, three kids. I was highly functioning. When I took a hit from eating a bacteria while I was working I ended up in the intensive care unit, with a mega colon, an infectious disease, and it was really bad. So I got sick in 2012, took a hit to my gut. I went from high functioning to not functioning at all"From here Sherri lays out the journey back and how a fellow nurse at a particularly low point after she'd been put on 16 pharmaceuticals and was "circling the drain of despair", encouraged her to try cannabis as medicine. "I was on the other side of the bed, like in a really big bad way, all of a sudden I'm the patient and having to advocate for myself. So not only was it the physical issues that I had, but I also had to deal with the system, the system that I worked in, how is that going to affect my mental health and how are they going to take care of me? The trauma that I experienced from this illness is so overwhelming high functioning to not functioning, home- bed bound in the hospital- couldn't take care of my kids -could barely eat. I'm just gonna say it like it is explosive diarrhea, and nausea, vomiting, abdominal pain, pooping your pants in public. So you can see how literally I can talk about anything. From running around the block naked to talking about poop and stuff that comes out your bum. I was so mortified, horrified that I had this gastrointestinal illness that was debilitating, complete loss of control, it was horrible. And then the issues with eating and being able to take in food. So that spiraled into, mental health issues, the developing anxiety and the depression. And over time, I wasn't getting better. I had agoraphobia, I was afraid to leave."And I'm going to tell you too, and this is the part of trauma that I'm really trying to help patients with now, when I'm working with them because I've been there. You know, I've been there. I call it medical trauma. And Pharmaceutical trauma. It was cannabinoid therapeutics that came into my life that really made the significant impactful change, that allowed everything that wasn't working to start working again and actually allowed me to start processing information from a different place because I was pulled out of the story of the sickness, the story or the trajectory of sickness. an illness. You know, it was the cannabinoids that actually opened the door that allowed everything that wasn't working to start working again."I was utilizing all of the other tools that existed. The traditional system, the holistic system, the functional integrative system, acupuncture, chiropractor, essential oils, homeopathy, you name it, and none of it worked. Why? Because my endocannabinoid system was completely 100% deficient. I was not making enough of my own internal endogenous cannabinoids to keep my system regulated including my brain""And so what it took was it was one of my nursing colleagues who had just retired. She was one of my patients because I do energy healing work. I do Reiki and Reconnective Healing and she was one of my clients from energy healing. She had been trying to get in touch with me and she had just turned 70 and I kept blowing her off. So she came to my house with a big giant, big fat joint. And she walked into my bedroom and I looked at her and I go, are you kidding me? I mean, is this a joke? Seriously, you're bringing me weed? Like, how is this gonna help me?"There's much more within this episode as Sherri goes into a lot of detail on various aspects of cannabis as medicine and how it works to restore homeostasis, what this means for so many conceivably impossible to treat conditions, how this has profound implications, for ushering in a new paradigm of modern medicine, bridging the gaps between traditional and alternative medicine with results, research and centuries of "anecdotal experience" evidencing profound efficacy and facilitating personal autonomy and decreased dependence on a broken healthcare system. For Sherri, these brutal series of experiences were the launching pad for what is now her business, Green Nurse Group and has led to her work as Medical Director of Irie Bliss Wellness. She has the opportunity to help others on their journey with cannabis. She discusses the personal stigma she had to overcome as well as challenges within her own family who were not initially supportive of her foray into the cannabis industry among other potential roadblocks so many of us experience as we advocate for what we know to be true about cannabis. It is a life saving plant that has the power to transform one's entire state of being. Sherri and Green Nurse Group as well as many other Cannabis Nurses, Doctors who are slowly starting to embrace cannabis and other professionals with whom Sherri inspires sit at the crossroads of this exciting yet also frustrating time in traditional medicine as the systems we have depended on for so long to help keep us alive and having quality of life are not leading to overall improved quality of life for more than not. One final quote from Sherri and then be sure to check out her links: "I honestly see this (cannabis as medicine) being integrated into mainstream medicine, but it's going to happen in the community first. So in other words, as healthcare professionals, a lot of cannabis nurses, including myself, the traditional healthcare system has failed us in multiple ways. Not only were surgeries denied, medications were denied. I had to fight for my for surgical procedures, the mental health system-completely broken. Bottom line is that a lot of systems that we medical professionals, especially cannabis nurses have come from, we've come from broken systems, and so we want to make it better. And how do we make it better? We start in our communities by educating and then there's going to be a critical mass eventually. It's going to happen and I've already seen it ...now I'm getting phone calls from more doctors asking, "Hey, what's the green nurse?" The trauma that I experienced from this illness is so overwhelming high functioning to not functioning, home- bed bound in the hospital- couldn't take care of my kids -could barely eat. I had agoraphobia, I was afraid to leave." And I'm going to tell you too, and this is the part of trauma that I'm really trying to help patients with now, when I'm working with them because I've been there. You know, I've been there. I call it medical trauma. And Pharmaceutical trauma. It was cannabinoid therapeutics that came into my life that really made the significant impactful change, that allowed everything that wasn't working to start working again and actually allowed me to start processing information from a different place because I was pulled out of the story of the sickness, the story or the trajectory of sickness. an illness. You know, it was the cannabinoids that actually opened the door that allowed everything that wasn't working to start working again." I was utilizing all of the other tools that existed. The traditional system, the holistic system, the functional integrative system, acupuncture, chiropractor, essential oils, homeopathy, you name it, and none of it worked. Why? Because my endocannabinoid system was completely 100% deficient. I was not making enough of my own internal endogenous cannabinoids to keep my system regulated including my brain" "And so what it took was it was one of my nursing colleagues who had just retired. Like, how is this gonna help me?" There's much more within this episode as Sherri goes into a lot of detail on various aspects of cannabis as medicine and how it works to restore homeostasis, what this means for so many conceivably impossible to treat conditions, how this has profound implications, for ushering in a new paradigm of modern medicine, bridging the gaps between traditional and alternative medicine with results, research and centuries of "anecdotal experience" evidencing profound efficacy and facilitating personal autonomy and decreased dependence on a broken healthcare system. It's going to happen and I've already seen it ...now I'm getting phone calls from more doctors asking, "Hey, what's the green nurse?" I had the honor of speaking to three accomplished women about the current state of birth both locally and nationally. We discuss the increasing interest in home births, the advantages and disadvantages of home versus hospital births and how those expecting, or expecting to be expecting, can best prepare especially with the extra consideration of Covid19. Corrine Pellard is an Empath, Psychic Medium and Reiki Master. She is also a mom of two and tells us about her experience of both a hospital birth and an at-home birth. Corrine discusses the lack of control over the birthing experience that she experienced with her first child. She discusses being considered high risk because of her age and weight and how the entire birth experience was centered around preconceived notions based on the masses and not her individually. She was induced, unnecessarily in her view, was sick from the drug used to induce and delivered by cesarean section. She shares, “It was traumatic, and my healing my first two months of being a mother was physically painful and my anxiety post, I had postpartum anxiety to a very severe degree.” When Corrine became pregnant again, she knew she wanted a different birthing experience. She found Birth Matters and decided to try a natural, vaginal delivery at home. Jodi Churchill Chapin has been a Registered Nurse for 32 years, most of which she has spent in Labor and Delivery. Jodi discusses bridging the traditional birth and hospital procedures with less traditional therapies such as Reiki. Jodi states, “And we incorporated, you know, you're there, you’re exchanging energy. It's how the patient uses her energy and is able to focus on the birth process. And I think what’s important is that being able to balance, you know, following policy and procedure, but also carrying out the patient (and) the family's wishes of how they envisioned birth. I mean birth is, you know, one of those things, it's like, even planning a wedding, you look forward to this one day you plan you plan you plan, you think it's going to go one way and then plans change on a dime. And how it's handled can make it or break it.” Jessica Petrone is a Certified Professional Midwife (CPM), RN and BSN who has been in practice for 13 years. Her practice saw a record number of home births last year and Jessica believes that number will continue to grow. She relates, “But I think specifically in the last couple of years, there seems to be a shift in people just taking charge of their health care even in the medical setting. There's more conscious awareness, practices and practitioners breaking away from the traditional systems and framework but yeah, we love home birth. We do a lot of water birth we work with a lot of different practitioners to enhance really healthy pregnancies. We absolutely consult with medical providers when it's necessary.” Corrine shared about her experience with Jessica and Birth Matters, “So, I will say one of the biggest differences that I noticed on my very first appointment with a Birth Matters in the home birth midwives is they asked my consent, which nobody asked my consent. I mean, sure I signed forms but before touching me, nobody offered before examining me, nobody asked permission to touch my body.” The conversation continues to the state of birth currently with the threat of COVID19. Jodi discusses the difficulty of taking the necessary precautions in the time of a pandemic while trying to connect with an expectant mom during the labor and delivery process. “This Covid19 has put a huge damper on healthy women wanting to come in to have their normal birth. Especially right now, today. Anybody and everybody walking into the hospital or around the hospital has to wear a mask. You cannot take that mask off anytime you are on hospital property that includes in the labor room, that includes the patient that includes the patient's one person that's helping them with birth, there are no visitors allowed, no children, family, nobody else. The only person allowed in the room is the one that will be wearing that second baby bracelet that's given out when a baby's born. I mean, labor, you're breathing with women, you're teaching. I mean, it's all about the closeness. How do you socially distance yourself from somebody who needs your support, your encouragement, your empowerment?” Jessica discusses the fact that more women are considering home births currently. “The influx of inquiries is so unexpected, I can't keep up. I'm getting probably 10 emails a day, which it is impossible for us to entertain the idea of taking all these women on. And that's true of a lot of midwives across the state, really. Not everybody is a good candidate. I mean most people probably inquiring last minute. We had yesterday, a woman inquire and she's due March 30. That's like, less than a week from now. If people are making choices out of fear and panic it is not what's happening, and whenever we get an inquiry that somebody is so afraid of the hospital and so afraid of other providers and afraid of needles, I mean, we all encounter that in patient care, but they may not be a great candidate. But that's not what's happening right now. These are women that are realizing that their birth plan, what they thought they were going to experience in the hospital, is drastically different and there is obviously a great concern about becoming sick and being exposed but a lot of the women are making the choice based on the fact that their support person might not be able to be present with them.” Jessica recommends that women do their research before opting for a home birth. I am grateful to these remarkable women for sharing their wisdom. Corrine Pellard is an Empath and a Psychic Medium. Even as a small child, Corrine has had a special insight into other people’s emotions. Beginning in her early teenage years, her intuition would alert her when something important was about to occur, either for herself or a loved one. At age 14, Corrine began using the wisdom of Tarot Cards to enhance her psychic connection. As years went on, Corrine became more aware of her ability as her readings became more accurate and her predictions began to materialize. At the age of 30, she began having premonition dreams. After seeking the guidance of her Shaman, she came to accept that she is an Empath & an Intuitive. Corrine is a natural healer, using her Empathy, Intuition, and Reiki to heal others emotionally, spiritually, and physically. Corrine is also able to mediate communication between people who are no longer living to those who still are. She offers private readings and hearings and in-home parties. She is a wedding officiant offering custom designed ceremonies. Her true passion is teaching others to honor their own abilities and helping others heal themselves. She has an extensive background in women’s health as a labor and delivery nurse and has also worked in oncology. She resides in Framingham with her husband of 25 years. Together, they have 3 children ranging from middle school age to college, 2 dogs, 2 cats and a bunny named Kiwi. Jodi firmly believes that vulnerability is to be met with compassion, respect, and active listening to ensure that her patients have the very best outcomes. She credits her extensive nursing career and healer background with these virtues and skills. Jodi is also a Usui Shiki Ryoho Reiki Level II Practitioner. Through this healing art, Jodi may be able to help people who are struggling with a wide range of health conditions such as headaches, insomnia, back pain, cancer, heart disease, chronic pain, depression, anxiety, and may even be used to help in the recovery from injuries and surgery. As an active member of the American Cannabis Nurses Association and a GNG team member, she helps advocate for people who choose to use cannabis as a complementary or alternative treatment, providing education, insight, guidance, and support in the process. https://www.greennursegroup.com/our-team https://www.emedevents.com/speaker-profile/jodi-churchill-chapin https://green-nurse-sandbox.iriebliss.com/green_nurse_staff/jodi-chapin/ Jessica Petrone, CPM, RN, BSN Jessica's experience in birth includes her work as a doula, her completion of a 3 year midwifery apprenticeship in a very busy home birth practice and being a mother of four, with the last being an unassisted home birth attended only by her husband and herself. Her own births led her to become a doula which led her to become an apprentice and, in turn, a midwife. But that’s the abbreviated version! Jessica has been attending home births since 2006. She is co-president of the Massachusetts Midwives Alliance, a member of ALACE (Association of Labor Assistants and Childbirth Educators), and a member of the Birth Year Network (a Southeastern Mass, Cape and Islands Birth Resource Network). She holds certifications in CPR and Neonatal Resuscitation. She completed her formal education remotely through the National College of Midwifery in Taos, New Mexico, where she earned a degree in the Science of Midwifery. Most recently, Jessica has completed a Bachelor of Science in Nursing, which included a 6-month preceptorship in a high volume/high risk maternity unit at Massachusetts General Hospital. https://homebirthmatters.com/about/jessica-petrone/ https://www.modernmamamidwifery.com/the-midwives http://bostonvoyager.com/interview/meet-jessica-petrone-sarafina-kennedy-birthmatters-llc-boson/ Supplemental Article: https://nypost.com/2020/03/23/pregnant-women-share-what-its-like-to-be-expecting-during-coronavirus-pandemic/

In this episode, my guest Sherri Tutkus, RN, BSN, Founder & CEO of Green Nurse Group, came on the podcast to share a powerful and inspiring mother son story that follows the timeline from receiving a diagnosis at birth of her son Nicholas with Branchio-Oculo-Facial Syndrome through current day. Sherri gives one of the most powerful illustrations over the course of the episode, of a mother's love, courage, compassion, resilience and tenacity that I've ever heard, and is a fantastic example of holding onto presumption of competence as a guiding light, against all odds. Sherri Tutkus is the founder and CEO of GreenNurse Group, Nursing Director at Irie Bliss Wellness and host of GreenNurse on the Go Radio Show. Sherri is a cannabis nurse, patient and advocate. She earned her Bachelors in Science and Nursing from Boston College. She is highly skilled Registered Nurse with 30 years’ practical experience in various departments within the hospital and home setting. She is utilizing her expert nursing skills as a medical center specialist, clinical nurse liaison and educator to bridge the gap between patients and the cannabis community. Sherri has been educating and implementing holistic integrative healing modalities within her practice for over 20 years. She educates on the endocannabinoid system and the safe utilization of cannabis at dispensaries, hospitals, clinics, patients homes and she regularly does pop up events, seminars and expos. Sherri is an international speaker and she has contributed to the writing of the first cannabis nursing textbook with her cannabis nurse colleagues that will be available in nursing schools across the country. Sherri is a member of the American Cannabis Nurses Association and founding member of The Cannabis Nurses Network and was nominated as one of two nurses for “Health Professional of the Year” for the 2020 New England Cannabis Convention. Sherri brings passion and purpose to her work teaching bio-psycho-social-spiritual healing using cannabis as a tool. In the first part of this podcast, Sherri discusses her own background and connection to cannabinoid medicine. The tools she learned in her own journey enabled Sherri to cope with frequent and ongoing surgeries, procedures, treatments for her son Nicholas and advocate vociferously on his behalf. "When I'm presenting to people, I use myself as a case study. Because basically, prior to me getting ill, I had a history of migraine headaches, ADHD, and anxiety, that was managed with traditional medicine. I was functioning, I was healthy, single mother of three, child with a disability that we're going to talk about, but knowing that I had migraine headaches and at ADHD symptoms should have been a clear indicator that I had an Endocannabinoid deficiency. So everything that we do or don't do in our lives as far as health and wellness affects this neurotransmitter signaling system called the endocannabinoid system, and that the job of that system is to bring balance." She goes on to explain, "By the end before I discovered cannabis. I was on over 16 pharmaceuticals. So, as I've learned over time, if you're taking more than 10 pharmaceuticals, there's 100% chance of having an adverse drug reaction. Polypharmacy. I fit into that polypharmacy category. I am not anti pharma. However, what is really essential for people to understand when they are being prescribed pharmaceuticals is to ask those critical questions and to look at what side effects there are to look at the blackbox warning Okay, how many people have died? You know, what are the side effects, what are the adverse effects, you need to have knowledge knowledge is power. The other thing that people People don't realize is that a lot of pharmaceuticals have a drug nutrient depletion" She goes on to explain, " I have have a history of Polycystic Ovarian disease which is very interesting. And that yeah, that is considered to be a clinical Endocannabinoid deficiency diagnoses as well. So I had issues with fertility all along. ... And then my third pregnancy... it's interesting, you know I gained all kinds of weight and wasn't happy and I started changing my lifestyle when the babies came. And all of a sudden my reproductive system started to auto regulate. I was using specific supplements and nutrition. I changed my diet, I became vegan, I did a raw diet, and all of a sudden, I'm a fertile Myrtle. And before we know it, I'm pregnant with my third" From here, Sherri goes on to discuss the birth of Nicholas and the immediate realization that he had multiple issues of which she was not aware of up until the moment of his birth. She explained to me in writing prior to the recording of this episode, the following, "All of my births were traumatic experiences however this pregnancy was the best and the easiest. I had an uneventful healthy pregnancy. I broke my water 6 weeks early and when my first boy was born he was not breathing and they had to resuscitate him and over the course of my first postpartum days I learned that my son had a rare genetic disorder called Branchio-Oculo Facial Syndrome." BOFS is inherited in an autosomal dominant manner. Each child of an individual with BOFS has a 50% change of inheriting the variant. Nick’s dad carried the gene and at the time of diagnoses the gene had not been identified. She discusses the realization of impact, "you have these craniofacial anomalies that required over 15 surgeries, but the things that they said about mental retardation was not true. The surgical stuff they were able to do . I engaged upon a journey with him that I would do anything for him. It was kind of like, I just got to do this. And so he became my full time job. " I asked Sherri the question, "How did you do it? How did you handle this huge weight of reality repeatedly?" She explained, "I got back to really the basics of self care and being present and grounded. I had already have a lot of those spiritual tools. My son opened up the door to the unseen worlds for me, which is a whole other show. But literally, I kind of felt like and I know it sounds crazy, but I'm sure other mothers can agree or associate or identify.... that I just felt a really strong connection with him and being able to understand what his needs were even before he could communicate." She goes on to add, "it gets back to what I can and cannot control you know, even if I can't control my emotions, even if I'm out of control, like hysterical or anxiety-ridden or sad, I still can have the ability to control what I'm doing and not doing in my life. And so what I knew to be true was in order for me to be the best version of myself, I needed to do everything that I could to take care of myself. And that included my mental health." She goes on to say, "But he's a true miracle. I gave him every opportunity. And he basically showed me through his own actions and his kindness and compassion and his grit and his ability. The kid had so much resilience and he wanted to heal, you know, he's not mentally retarded, he got set up with the Deaf Services as we found out later, he doesn't have sensory neural hearing loss, he has a bone conduction hearing loss. And so he basically told me one day Mom, I want to smile with the rest of the kids. And I was like, oh, of course you do. How do I make that happen? And boy, that was a scary journey. We literally embarked upon the journey of having my boy get a smile." "The amazing craniofacial team at Boston Children's Hospital did an experimental surgery that is called facial reanimation. And basically they didn't know. They said it would evolve over time, would the nerve take and would it connect and would it work? So here we are... I don't know here we are going. Through 16 hours of surgery 10 days in the hospital, not knowing if this is going to work or not. That was another faith based thing. Honestly, you know, it's all you have, and then literally one morning when I was waking him up for school, all of a sudden he sits up in bed. He smiles. So he's really he's come a long way. He's 14 years old now. When he was in sixth grade, he said, I want to go to public school. Oh my god, how is he going to be able to go to public school? So I was like, Okay, let's try to make it happen. And we embarked on the journey of integrating him into the public system." Bringing us to current day, Sherri explains, "You know, he is thriving. He's finishing up the eighth grade this year. And his freshman year, next year in high school, he will be in the public school. He still has an IEP, he's going to be in all honors and advanced honors classes is number one subject Spanish. And one of the other thing is that, you know, oftentimes as parents, we may feel sorry for our kid and you may feel bad. He doesn't feel sorry for himself. He doesn't feel bad." A final quote, "So what can I control? I try to control other people. And we can't control anyone. We can't. And that's the part that was the biggest lesson that I learned. And he taught me that, he taught me where my power lies and where it doesn't lie and how to choose and pick my battles and what's important and what's not important. And in the grand scheme of life, when you can pull back from that and see, what am I doing to be the best version of myself, so I can help my child thrive?" Susan Marks, aka Nurse Susan, is passionate about the therapeutic benefits of cannabis.
