Dr. Raphael Mechoulam was an organic chemist at the Hebrew University of Jerusalem, and the father of cannabis research. His team was the first to elucidate the structure of cannabidiol, or CBD. Dr. Mechoulam passed away in March 2023. This episode is a remastering of a conversation between Dr. Mechoulam and Dr. Meir Bialer, held in 2018 at the 13th European Congress on Epileptology. Moriah Barnhart is a Native Floridian whose daughter was diagnosed with brain cancer at the age of two in 2013. Moriah watched her daughter struggle to survive chemotherapy, which compelled her to seek help from experts in cannabis. As a result, her family received support from Dr. Raphael Mechoulam and Moriah went on to co-found CannaMoms as a means to provide support system for other parents like herself, and to organize their stories to change voters hearts and minds about the medical efficacy of marijuana. After the State of Florida passed a Medical Marijuana Program, she was appointed to the Florida Department of Agriculture Hemp and Medical Marijuana Advisory Boards, respectively. She wrote Florida's first CME program for physicians and continued to work on legislative policy reform, Co-Founding a new non-profit with a broader mission toward ending prohibition, the Women's Initiative for a Safe and Equitable (WISE) Florida. Ele foi responsável pela descoberta e isolamento do tetra-hidrocanabinol (THC) lá em 1963, sendo reconhecido como o pai do THC e pioneiro na pesquisa de cannabis. Ele também estudou a estrutura do canabidiol (CBD) e afirmou na década de 1980 que a cannabis poderia substituir até 20% de todos os medicamentos farmacêuticos se fosse legalizada. Dr. Mechoulam continuou seus estudos sobre canabinoides nas décadas seguintes e a notícia de sua morte foi lamentada por colegas da comunidade científica e também pelos ativistas pró cannabis. Dr Luis Otavio Caboclo nesse podcast detalha as formas da planta Cannabis: para uso recreativo a planta tem altas concentrações de THC (cannabis sativa) e para uso medicinal a planta tem altas concentrações de CBD (cannabis sativa com muito canabidióde) e baixas concentrações de THC conhecida como cânhamo. Existem mais de 120 substancias (terrenos e flavonóides, entre outros) na planta cannabis sendo as duas principais CBD e THC. A cannabis é estudada há mais de 2000 mil anos antes de Cristo para tratar certas condições clinicas na China. Em Bagdá (Mesopotâmia) no século VII um médico usou a Cannabis para fins medicinas. No século XIX um médico inglês usou a Cannabis Índica na Índia para tratar uma forma de epilepsia grave (síndrome de West). Esse médico levou esse conhecimento para o Reino Unido que incluiu a Cannabis na farmacopéia britânica e nessa corrente também foi incluída na farmacopéia americana. Porém, na década de 40 percebeu-se os efeitos psicotrópicos da Cannabis com alto teor de THC e dessa forma, o governo americano proibiu o uso de Cannabis para qualquer forma de uso e consequente interrupção das pesquisas. Na década de 60 em Israel, Raphael Mechoulam iniciou novamente as pesquisas com Cannabis a partir de uma porção de maconha apreendida pela polícia. Este cientista foi o primeiro a identificar as moléculas CBD e THC. Na década de 70 professor Elisaudo Carlini publicou um periódico com tratamento de crises epiléticas com CBD em animais sem efeitos no comportamento deles. Mais tarde esse mesmo professor publicou um artigo com 6 pacientes que foram tratados com ótimos resultados com CBD. Na década de 90 identificou-se o sistema endocanabidióide onde achou-se receptores para o CBD em varias partes do corpo humano (pele, olhos, sistema gastrointeistal, etc..) inclusive no cérebro que são os principais alvos de CBD. As principais medicações com Cannabis sativa: 1) CBD isolado e purificado, sem THC, 2) Extrato integral da planta com CBD, THC e flavonóides e no Brasil a concentração de THC é de 0,2%, 3) Extrato integral sem THC, 4) CDB com THC com concentração mais alta de THC. Na pratica clinica é mais utilizada CBD isolado e Extrato integral de Canbbis. Efeito Entourage é o efeito que quando todas as substancias da Cannabis atuam juntas elas melhoram os efeitos do CBD. Principais indicações: Eplepsias refratárias (síndromes de Dravet, West, Lenox Gastaut, entre outras), dor crônica, espasticidade em pacientes com esclerose múltipla e caquexia com anorexia em pacientes com AIDS. Estas indicações tem nível de evidencia forte. Ainda com níveis de evidencias baixos: Autismo, controle comportamental de pacientes com Alzheimer, entre outros. Acesso no Brasil: desde 2014 a Cannabis foi liberada para uso medicinal no Brasil. A primeira forma de acesso foi a importação direta para pessoas que precisam da medicação, porém este processo é demorado e trabalhoso. Recentemente as farmácias foram autorizadas a importar as medicações o que facilitou o acesso à Cannabis. Algumas ONGS conseguiram autorizações liminares para plantar Cannabis no Brasil e extrair o óleo da planta e fornecer para pessoa de baixo poder aquisitivo, pois o produto na farmácia tem alto custo; atualmente estado de SP vai fornecer a medicação custeada pelo SUS. The Israel scientist Raphael Mechoulam has been researching what's thought to be the world's most popular drug since the 1960s. In 1964, he isolated Tetrahydrocannabinol, or THC – the compound that gets cannabis-users high. Later, Professor Mechoulam discovered another compound called CBD, or Cannabidiol, which has medical benefits without any kind of psychoactive effect. Our latest guest is Professor Raphael Mechoulam. Professor Mechoulam is the first researcher to isolate THC and CBD. For that achievement and the decades of research into the endocannabinoid system that followed, Professor Mechoulam is often called the "Godfather of cannabis research". Since his research began in the 1960s, Professor Mechoulam has received numerous academic awards. Most recently he received The Harvey Prize from the Technion Israel Institute of Technology in the Human Health category. We heard Eloise Theisen speak about how cannabis helps seniors at CannMed2019 - a huge, very serious medical cannabis conference where we also met Dr. Raphael Mechoulam, the Godfather of Modern Cannabis Research. Every one who is anyone in the medical cannabis space attends this conference! Eloise Theisen, is a board-certified Geriatric Nurse Practitioner who has cultivated a wealth of knowledge and data for using cannabis to treat a broad range of conditions and diseases, especially those that commonly affect seniors. She has helped patients use cannabis to help treat chronic and age-related illness and was one of the first healthcare practitioners to bring a clinical dosing regimen to the cannabis space. She's treated more than 5,500 patients using cannabis. Eloise previously served on the board of the American Cannabis Nurses Association and is now the President. She helped in the development of ACNA's first online core curriculum, providing over eight hours of educational content as well as continuing education credits for nurses. Her work has been featured in Newsweek and the SF Chronicle, among others. There are very few healthcare practitioners in the US with a comparable level of cannabis expertise and experience. She is available via telemedicine to consult globally, and can be a patients' clinician if they live in California. Gregory D. Gorgas, President and CEO, explains what pharmaceuticals his company is developing to work with the body's endocannabinoid system. He touches on How our endocannabinoid system was discovered and how it works, The ways Artelo Biosciences approaches the development of pharmaceuticals, and What three products they are currently working toward releasing. Mr. Gorgas begins with how Dr. Raphael Mechoulam discovered our endocannabinoid system in Israel as he researched the effects of cannabis, trying to pinpoint what receptors it targeted. This allowed him to identify several receptors in our body, discovering this powerful system. Mr. Gorgas explains that the body makes chemicals that target these receptors and the cannabis plant mimics what the body already makes. He then discusses why his company is working on pharmaceuticals to address this system. He says that it's a powerful communication system throughout the body and handles responses to inflammation, stress, and any stimulus which the body would want to address. When the body is overwhelmed, introducing a chemical from outside the body may help this system. He then explains the three ways Artelo Biosciences is trying to target this system through pharmaceuticals: First, a naturally-occurring cannabinoid that's been altered to increase appetite yet avoid the euphoria or high state as a treatment for cancer patients; second, a cannabinoid called EBD with anti-anxiety and anti-inflammatory properties; and third, a fatty-acid binding protein-5 inhibitor originally researched as a cancer treatment. When Catherine Jacobson's three-year-old son had exhausted all of the treatments available for his rare, life-threatening form of epilepsy, she wasn't about to just give up.“I began hearing stories from families with children with severe epilepsy and that they were treating their kids with cannabis and they were reporting good outcomes,” Dr. Jacobson said on The Cannabis Enigma Podcast. “So of course, I was interested.”Those families, however, were making their own medicines from cannabis and there was no way to guarantee consistency. Introducing a new movement dedicated to growing fair trade, environmentally sustainable cannabis that also provides small farmers with the one thing they need to thrive: a lucrative cash crop.Before introducing this episode I must apologize to Dr. Raphael Mechoulam, who is, apparently, a follower of this podcast. In Episode 70 (“The Strange Cannabis Illness that No One Understands”) I reported that Dr. Mechoulam thought the likely cause of this odd illness was strain or dose dependent. While that was part of what he said, I failed to offer his full explanation, which he made clear to me in an email dated Jan 10, 2020. “Hi, I do not recall stating that cannabinoid hyperemesis is strain dependent. I have published that it is probably an immune-type reaction. The premise is this: as more US states become legal, the price of North American cannabis is going to drop – this is already happening. At the same time as more countries around the globe legalize (Thailand did. Mexico plans to do so this year. India and Colombia are also heading in this direction) it’s crucial to establish a more economically sustainable future for small farmers who have been tending excellent sun-grown heritage strains such as Alcapulco Gold, Panama Red, Thai Stick, for hundreds—maybe thousands—of years. The first step to this project is organizing a global cannabis supply chain and that is precisely what the Small Cannabis Foundation is designed to do. As more markets open, big industrial scale cannabis currently grown in “greenhouses on the tundra” will flood the market with high potency but not so interesting varieties. Prices will plummet and excellent old world varieties grown in the subtropics will begin to make their way to those who want them. “Big cannabis will become worthless the minute great Thai Stick comes back into the country,” Plamondon contends. “Cocoas of Excellence” is one such program devoted to preserving the diversity of indigenous cocoa strains and at the same time providing income to farmers. Small Cannabis has similar aims.Plamondon is part dreamer, an essential quality to all visionaries, and part student of history who relies on Adam Smith’s time tested economic insights. As he points out, the cocoa, cotton and vanilla industries were established during the rush to colonize 300-400 years ago. They were wildly successful at enslaving vast numbers of people who depended on subsistence agriculture for their livelihood. Cannabis is one of the few new supply chains -- the internet makes import and fair trade transparent, traceable and enforceable. A two-way interview between journalist Patricia Miller and Joe Dolce about the gut-wrenching illness called hyperemesis.A few weeks ago Patricia Miller, associate editor of Cannabis & Tech Today magazine, called to talk about hyperemesis, the strange and increasingly more reported illness that hits some cannabis users hard. Hyperemesis is defined as bouts of cyclical vomiting caused from combusting flower (i.e., smoking a joint), but trust me, what happens is way more painful than simple puking. It’s as if your stomach is being turned inside out – gut wrenching -- and it can last for hours, in extreme cases, days. The few scientific papers on it contend that it is followed by a compulsive desire to take hot showers. In my case I was so wasted after each bout I could barely crawl into bed, let alone stand in a shower. As one who always wants to be first on a trend, I was hit with hyperemesis while reporting Brave New Weed and about a half dozen times in the subsequent two years. I couldn’t understand how a plant that chemotherapy patients use to quell nausea would send me into paroxysms of puking. After months of investigating I never identified the cause but I did learn a lot about the chemistry and biology of the cannabis plant, and turned that into a book. There’s no consensus about what hyperemesis is or what detonates it. Dr. Raphael Mechoulam surmised it is dose or strain related. Others blame pesticides or other chemicals used in cultivation that have not been properly cleared from harvested bud. Others say it’s caused by today’s super strong weed that can clock in at 30% THC. My own experience elides all of these theories. I was smacked down after a fat dab as well as on the very first puff of a joint. As for the quality/variety of the herb, well, I’ve been privileged to inhale some of the finest, organic, sun-grown, pesticide-free weed grown in North America, so quality and variety aren’t the only culprits. Patricia proposed interviewing me about my experiences but I suggested we interview each other in hopes of informing more people who’ve been felled. I don’t think hyperemesis is necessarily a new illness; rather, I suspect more people are reporting it now that legalization has opened up the conversation. Some sufferers I’ve met have quit using completely to avoid any future misery. Personally, I’ve navigated a cautious path and have concluded that the only surefire way of inhaling flower without retching is through vaporization. My untested theory is that there is something in the plant matter itself that triggers the convulsions. Before this interview I asked a few chemists if they had come across any home grown remedies. The only response came from James Curran, who has appeared on Episode 67 of this podcast. He forwarded this study that identifies capsaicin, the fiery ingredient in cayenne pepper, as a possible antidote because it interferes with the biochemical triggering. The Discovery of CBD In spite of the fact that CBD was thoroughly used for a wide range of reasons over thousands of years across the world. There remains a substantial amount about hemp and cannabis that we still don't know about. Cahn published his findings on the structure of Cannabinol (CBN). A couple of years later, a chemist based in the US at www.ucgardnercenter.com known as Roger Adams actually managed to isolate cannabidiol (CBD). This is one of the times that history was officially made as Adams officially isolated CBD for the very first time. It would be more than 20 years until THC would be effectively isolated too. And the accomplishment went to Dr. Raphael Mechoulam of the www.ecnsweb.com website in 1963, when he identified CBD's stereochemistry, as well as THC in 1964. With a study conducted in 1978, he provided doses of 300 mgs of CBD on a daily basis for eight volunteers with medically diagnosed epilepsy. The drug, Epidiolex, treats seizures associated with two rare forms of epilepsy and was approved by the FDA only last year. But even as his work laid the foundation for the modern cannabis industry and for understanding how cannabis interacts with the human body, a white whale eluded research: cannabis acids, which are compounds that are produced in the plant when it is alive and may be more potent than their better-known derivatives, such as CBD and THC. That changed on Monday, when Mechoulam and a group of researchers announced at a medical cannabis conference in Pasadena, California, that they have developed a process for creating synthetic, stable acids that are found within the plant, and that the synthetic acids, which include acid versions of CBD and THC, are now available for licensing to companies for drug development. CBD helps cushion the psychoactive impact of THC on CB1 receptors, making for a less intense ‘high’. Close to 60 years ago, in the early 1960s, Marijuana AKA Cannabis wasn't exactly the friendly substance it is today. In pretty much every developed country, recreational use of the drug was totally illegal and medical research was limited. So as a junior faculty member of the Weizmann Institute of Science in Rehovot, Israel, Dr. Raphael Mechoulam had to jump through a few loops to get his hands on a sample for his research. Luckily for Dr. Mechoulam, the director of the Weizmann institute had an old army buddy in an opportune place - the head of the investigative branch of the national police. Dr. Mechoula, reach out to him and was provided with 5 kilos of seized Lebanese Hasish. This sample allowed Dr. Mechoulam and his research partners to discover cannabidiol in 1963 and THC the following year. In 2001 Prof. Mechoulam was awarded the Israel Prize, the country's most prestigious award for scientific achievement and today he continues his research at the Hebrew University of Jerusalem. However, there are still some skeptics regarding the effectiveness of this plant, and many others still regard marijuana as an illegal drug. My guest today, Dr. Karen Munkacy, will shed light on why medical marijuana is better than conventional medicine. Breast Cancer Survivor Dr. Karen Munkacy is a breast cancer survivor. Upon being diagnosed at the age of 40, she has undergone 28 radiation sessions and a double mastectomy. During the four months that she was undergoing aggressive chemotherapy treatments, a friend suggested taking medical marijuana. Munkacy also heard about medical marijuana’s benefits. But not wanting to break the law, Munkacy refused the treatment. Although Munkacy is grateful to be in remission, going through a plethora of surgeries, radiation and chemotherapy treatments were not easy. Hence, Munkacy resolved to find ways to help other cancer patients ease their pain and suffering. Medical Marijuana Research Not forgetting her friend’s recommendation to use medical marijuana, Munkacy researched about the medicinal benefits of cannabis supported by scientific evidence and medical support. Her medical background gave her the perspective to assess her findings. Munkacy also began to began to speak publicly about her experience with cancer. This way, it could encourage lawmakers and health care professionals to support legislation, allowing the use of medical marijuana to alleviate patient pain and discomfort. Passing the Bill Eventually, Munkacy was able to get the medical marijuana bill passed in Massachusetts in November 2012. The bill passed with 63% votes. That encouraged Munkacy to lead an organization creating the best medical marijuana medicines. With a pioneer staff of 16 employees, Munkacy’s primary aim was to uphold the highest standards to cultivate medical marijuana and educate patients as well. Investing a lot in lab equipment also assures patients that the extractions and dosings of the medical marijuana are exact. Primary Benefits There are many other benefits of medical marijuana. Aside from the alleviation of pain and discomfort, medical marijuana also works for nausea. “It’s the only anti-nausea medication that also stimulates appetite. It works for anxiety, depression, and inability to sleep,” said Munkacy. “Medical marijuana is very safe with no side effects. Hence, it is the perfect medication for people in hospice care.” Unfortunately, some hospitals remain to be hesitant in the use of medical marijuana. Munkacy recalls a female patient wherein using medical marijuana significantly eased her pain, anxiety, and depression. That patient was able to sleep at night, had an appetite, and her nausea decreased. However, when the hospice found out that she was using medical marijuana, she was forced to leave. Consequently, her family had no choice but to take over and oversee her treatment. Father of Medical Marijuana Dr. Raphael Mechoulam is known as, “The Father of Medical Marijuana.” The title was given to Mechoulam after he discovered the endocannabinoid system. Consequently, studies show that medical marijuana inhibits the vascularization of tumors. It binds to receptors in our digestive tract. Hence, it can help with inflammatory diseases and problems like Chron’s disease. Despite that, Munkacy acknowledges that it is still hard for doctors to write recommendations. Consequently, doctors rely on top medical literature to declare effectivity of a drug or treatment before recommending it to patients. However, Israel has a different take on medical marijuana. Munkacy reveals that the Ministry of Defense in Israel pays for soldiers with PSTD to get medical marijuana. Unfortunately, for those in the United States, medical marijuana is not readily available for American soldiers with PSTD. Indeed a sad plight for people who serve our country. How The Body Processes Marijuana According to Munkacy, if you use medical marijuana everyday, you can have withdrawal symptoms if you just abruptly stop it. So it is best to do it under a doctor’s supervision. “Medical marijuana is processed through the liver. There are different effects when you inhale it vs. when you swallow it,” Munkacy said. Munkacy says that when you inhale medical marijuana, it goes into your lungs and right into your bloodstream. On the other hand, when you swallow it, it gets absorbed in your digestive tract, and the blood flow from your gastrointestinal tract goes right through your liver. “The more educated you are about this medication, the more efficient it’s going to be for you. You’ll be able to harness its effects and use it to the way that works best for you,” said Munkacy. Facing Challenges There are currently over 40 states in the United States that have legalized marijuana in different forms. However, Munkacy says it is still difficult to get the right type of medical marijuana. Unfortunately, the University of Mississipi is currently the only place where you can get medical marijuana. Hence, researchers have had a hard time getting the types of strains that they want to study despite having several cases of success. “There was a study published in the New England Journal of Medicine. This study documented that CBD, which is one of the compounds in medical marijuana, treats intractable pediatric epilepsy,” shares Munkacy. “This compound doesn’t make you high, but rather it controls seizures.” Looking Ahead Despite the challenges promoting the use of medical marijuana, Munkacy remains unfazed in her mission to have more people use this treatment. Since pharmaceutical drugs have proven to be a failure for a lot of individuals, Munkacy offers her products to anyone who needs it. “Our most common patient problem is a severe chronic pain. You can keep using more of it until your pain is under control,” Munkacy said. “Unlike narcotics, there is no fatal dose in medical marijuana.” Furthermore, Munkacy assures that each patient is given immediate attention as well as ample time for consultation. Rest assured, Munkacy is confident that her life’s mission will eventually be realized one step at a time. Dr. Karen Munkacy’s medical experience is extensive. In addition to her background as a board certified anesthesiologist and pain management specialist, she is a former faculty member at UCLA and USC Medical Centers in Los Angeles and has worked as a researcher and international medical consultant. Munkacy was trained in pain management and received her medical degree from the University of Michigan Medical School. She is also an active proponent in the public arena of the use of medical marijuana. In this role, Munkacy is a member of Americans for Safe Access, the largest organization of patients, medical professionals, scientists and concerned citizens promoting safe and legal access to medical marijuana. She was a leader in the successful ballot campaign for the legalization of medical marijuana in Massachusetts and helped persuade the Massachusetts Medical Society to sponsor a physician education program on the medicine. The Copernicus of Cannabis, Dr. Raphael Mechoulam joins us to discuss the advent of cannabis research, it’s history and current landscape. He says it’s very difficult to work on compounds, that are under legal constraints. When he started there was some knowledge around cannabinoids but it was vague and not in modern terms. As a natural products chemist he knew that the first thing that needed to be done was to elucidate the chemistry as you can’t do research with an unknown extract. He read historical information on the plant in many different languages to set his baseline understanding. This eventually set him up to be the custodian of the world’s supply of legal THC in 1963 which at the time was 10 grams and the rest as they say, is history.
