Episode 55 – Dr. Alex Eastman R298 The ATO brings you a different perspective today with an episode all about Doctor Alexander Eastman. Oftentimes people find a level of success and become comfortable with their positions in life.  They rest on their laurels and allow their previous accomplishments to define them.  Alexander Eastman is not one of those people. Growing up outside of Washington DC, Alex began a life of servanthood working as a firefighter/EMT/rescue worker and even spent time in the political arena of DC.   Doctor Eastman navigated the education world of the University of Texas and George Washington University School of Medicine and achieved what few do, becoming the Chief of the Rees-Jones Trauma Center at Parkland Medical Center in Dallas.  He was also an Assistant Professor and trauma surgeon in the Division of Burns, Trauma and Critical Care at the University of Texas Southwestern Medical Center. As if those achievements were not enough, a lunch with Lieutenant Owens led to Alex joining the Dallas Police Department in 2004 working alongside the Dallas SWAT team as the Lead Medical Officer, responding to every event the SWAT team would for years to come.  He and Doctor Jeffrey Metzger started the Tactical Medic program that has provided the SWAT team with an actual trauma surgeon on scene.  The Tactical Medic program has proven its worth time and time again saving lives in less than desirable conditions. He is the Chief Medical Officer of the Dallas Police Department and has reached the rank of Lieutenant in the Dallas Police Reserve Officer program that allows volunteers to work hand in hand with active duty officers. Doctor Eastman is now serving as the Senior Medical Officer at the Department of Homeland Security's Office of Health Security. Listen as we reminisce through his distinctive journey of almost 20 years of service. Carlton Marshall was shot and paralyzed while executing a federal search warrant. Dr. Eastman and Metzger performed tactical field surgery on the side of the house to save Marshall's life. The suspect received 45 years in prison for her actions.

Terry Rogers has been a practicing physician for over 56 years. He was the director of respiratory and  critical care at Seattle Swedish Hospital,  served  a Senior Medical Officer and  CEO for King County Blue Shield.  He was also CEO of the Foundation for Healthcare Quality in Seattle.  He retired as Medical Director of Lakeside-Milam Recovery Centers and still sees patients on a weekly basis. Terry has given his life to the good health of other people. Terry is also the author of Faces of Recovery,  a remarkable book that features before and after photos of patients who have been in a 28 day residential treatment program.  The visual alteration that occurs during treatment is stunning. Terry's photography provides these people with self-evidence that they can live a healthy life. Dr Dan Pronk served on over 100 combat missions in Afghanistan as a frontline special forces combat doctor, where the casualties he treated were his fellow SAS soldiers and commandos, local civilians and even the enemy. The thrill of adventure and the challenges of battlefield medicine brought out the very best in Dan; he discovered a sense of purpose in pushing his medical skills and courage to the limits. But there was a cost. Dan Pronk has written his memoirs 'The Combat Doctor' and chatted to Eoin about the messages in the book and the powerful learning in the line of duty. Royal Canadian Army 30 years – Chief Psychologist Was in Europe Then became Senior Medical Officer (7 months) – Israel peace keeping. The following year, genocide in Rawanda. Mass graves. Flight surgeon. Solidified interest in psychological trauma. Post Grad in Psychiatry , Univ of Toronto Wen to a Navy base in Halifax. Many sailors had PTSD from recovery program. Built out programs. 2006-7. Evidence based treatment still isn't solving the issue. Kandahar – 2006-7. IED's. NATO level meetings – virtual reality, psychedelics, biomarkers. Retired last year. To be a trauma therapist – need distance. Life Guard principal. Can you have too much empathy? Trauma/loss is the root of a lot of mental illness. Early on as a kid, India / Pakistan – partition. What men can do to each other. How do we disconnect ourselves morally, emotionally, to allow this to happen. It's both with the perpetrator and the victim. How powerful the mob can be. Nobody takes responsibility, no soul. Intellectually as a psychiatricist it's. Medicine quiets the noise but the psychotherapy helps you make sense of the trauma. Can anyone have PTSD.... is the extreme reaction to a trauma. Overwhelms my ability and gets stuck....comes back as nightmares and flashbacks....set up avoidance behaviors, then I become hypervigilant. Lifetime prevalence is 7-8%. Every single person has a lifechanging event. You can't unring the bell. In his POV, it makes you disengaged... after trauma you can still develop depression, or abuse drugs, Moral injury, what you did or didn't do. First responders – Imagine me with 400 people behind me – Army What are the things we can do as self care. Empathy and Kindness – When you shut off your emotional ties because you don't want to feel pain. But you can still have kindness....but being absorbed in the other person's could make you drown as well. Vulnerability – Only way to treat the trauma Best thing and worst thing in your life – memories become fuzzy. But PTSD memories don't. Combat veteran won't remember what he had for breakfast but remembers every detail about the event. Bring them out, process them, it's difficult therapy, but confronts it, Psychedelics – new networks and new meaning given to the trauma. Medicine associated psychotherapies. Smoking cessation – tried quitting, 6-7 times. They know it's bad. How does one does of psyilocibin stick, even if it's not an original Demistify the mystical experience. Certain networks in the brain, seems to have changes that last weeks to months.....there's an opening where our perception of self changes. Ego loss and ego death. If I feel guitl and shame from sexual abuse and that trauma has defined who I am, the trip allows you to build a new ego. How the parts of the brain talks to each other is more important. Loss doesn't have to be Every generation tries to be better. But how do we compartmentalize and not allow those events to define us.

He obtained his bachelor's degree from the University of California, Irvine, and his medical degree from Tulane School of Medicine on a full Navy Scholarship. Following medical school, he was commissioned in the U.S. Navy, completed residency training in family medicine at the Naval Hospital Camp Pendleton, and served seven years on active duty. Following medical school, Dr. Donnelly Wilkes was commissioned in the U.S. Navy, completed residency training in family medicine at the Naval Hospital Camp Pendleton, and served seven years on active duty. Wilkes served two combat tours in Iraq in 2004 and 2008 and was awarded the Navy Commendation Medal with Valor for his actions in the battle of Fallujah in April of 2004. He finished his Naval career as the Senior Medical Officer at Port Hueneme Naval Clinic, where he was responsible for the medical oversight of active duty members, their families, and local Veterans.  On the first day of his first tour, a firefight injured a Marine. Unfortunately, Wilkes and his team were unable to save him. Deeply saddened by this loss and their inability to treat him, they circled around the fallen soldier and prayed for him. A photographer captured the moment. The photo went on to become a Pulitzer Prize winning piece and a Life Magazine cover. Claudia also shares some of the simple yet powerful teachings from community elders.Claudia is currently working as Senior Medical Officer and Lead Clinician at Yulu-Burri-Ba Aboriginal Corporation for Community Health - serving the communities connected to Minjerribah (NSI), and the bayside communities around Capalaba and Wynnum.She is also a current Training Advisor for the Queensland Rural Generalist Pathway, navigating career pathways for rural primary care registrars across the state of Queensland.She has been living in Brisbane since 2018, but working within the Indigenous Health sector in regional and rural QLD since 2013 – as both an ACRRM registrar and fellowed Rural Generalist. She is a passionate advocate for Indigenous Health: particularly with regards to providing culturally appropriate gold-standard care, and working towards health equality for our First Nations People. She has had cultural mentors during her career that have been instrumental in challenging and fostering her understanding of the landscape and challenges faced by Aboriginal and Torres Strait Islander People, particularly in relation to health, and continues to be mentored as she progresses through her career.Starting her medical career in Central Queensland at Rockhampton Hospital, Claudia has worked then worked as a registrar in Longreach, and then as a senior Registrar and fellowed GP for Nhulundu Health Service in Gladstone (5yrs), and then as a locum GP for Bidgerdii Health Service in Gladstone, and Gidgee Healing in Mount Isa. When first moving to Brisbane, she came for an opportunity to work for the IUIH network at Moreton ATSICHS in their Caboolture and Deception Bay clinics, before making the move to Senior GP and then Senior Medical Officer at Yulu Burri Ba.Nhulundu Health Service was instrumental in fostering her passion for working within the ACCHO sector, and she had strong cultural mentors who helped to shape her viewpoints and drive for providing quality care to First Nations People, whom she also now considers as friends. He moved on to become a medical coordinating officer, then Senior Medical Officer, and is now the Chief Medical Officer at Healix, with an additional responsibility as Director of Medical Communications. Dr. Hyzler uses his expansive knowledge of medicine as it applies to travel and aviation. He plays a critical role in the medical management of patients who fall sick or are injured overseas. Graduating with a Bachelor of Medicine and Bachelor of Surgery in Dec 2004, he continued at Flinders Medical Centre for his intern and first residency year. Dr Pronk entered into General Practice training in 2006, and subsequently completed his FRACGP in Jan 2011.In 2007 Dr Pronk posted to his first Army unit in Darwin as a Regimental Medical Officer. In 2008 Dr Pronk posted to 5 RAR in Darwin, and continued to serve with high-readiness infantry units in Sydney and Perth for the remainder of his full-time military career from 2009-2013. Dr Pronk deployed on operations on five occasions with the Army, once to Timor and the remainder to Afghanistan.During his military career Dr Pronk took a particular interest in the pre-hospital management of penetrating trauma, and had the privilege of representing the Australian Army at the NATO Special Operations Forces Medical Expert Panel.In 2014 Dr Pronk discharged from the full-time Army and commenced an MBA through University of South Australia, which he successfully completed in December 2016. In November 2014 Dr Pronk also successfully completed Early Management of Severe Trauma instructor training through the Royal Australian College of Surgeons, commencing instructing in September 2015.For the period of July 2014 - June 2015 Dr Pronk worked primarily in an Occupational Medicine role at BHPB's Olympic dam mine site in South Australia, whilst also working periodically offshore.In July 2015 Dr Pronk accepted a Senior Medical Officer role at a regional hospital in Queensland. This talk was recorded live during the Prolonged Field Care Pre-Conference Lab during the Special Operations Medicine and Scientific Assembly(SOMSA).  Dr. Geir Strandenes is a founding member of the THOR(tactical Hemostasis, Oxygenation and Resuscitation) Group, the Senior Medical Officer of the Norwegian Naval Special Operations and a Researcher in the Department of Immunology and Transfusion Medicine at Haukeland University Hospital in Bergen, Norway.  He has worked hand in hand wit the US Army Institute of Surgical Research and the US Armed Forces Blood Program.

   Matthew Minson, MD is an author, inventor and clinician and holds a joint appointment as the Senior Advisor for Health Affairs for the Texas Engineering Extension Service, and Asst. professor at the Texas A&M University, School of Public Health. He is an editor for Health and Science at the Texas A&M University Press and the Texas Press Consortium, served as the Medical Director for Texas Task Force One, FEMA-USAR at Texas A&M University. He is the Author of Prepare to Defend Yourself; How to Navigate the Healthcare System… and Escape With Your Life, TAMU Press: 2014 and “How to Age Gracefully and Escape with your Dignity.” TAMUpress 2016, which was nominated for a National Book Award, Pulitzer, and chosen “Best of” by Library Journal. Dr. Minson was the Senior Medical Officer for Strategic Initiatives at the US Dept. of Health and Human Services/Office of the Assistant Secretary for Preparedness and Response, and served in the creation of the National Health Security Strategy, United States Agency for International Development, and deputy coordinator for the health and medical humanitarian response to Haiti. He has responded to disasters- the World Trade Center, the Columbia Shuttle Recovery, Hurricanes Katrina and Rita, the 2010 earthquake in Haiti, the Gulf of Mexico Oil Crisis, and the West, Texas Explosion. Dr. Minson obtained his Bachelor of Science in Biology from Abilene Christian University, his medical degree from the University of Texas Medical Branch, and his residency in Anesthesiology at the University of Texas Medical School, Houston. Dr. Harrison Spencer is the President and CEO of the Association of Schools and Programs of Public Health (ASPPH). He previously served as Dean of the London School of Hygiene & Tropical Medicine.  Prior to going to London, Dr. Spencer was Dean of the Tulane School of Public Health and Tropical Medicine in New Orleans.During a career with the Centers for Disease Control and Prevention (CDC), he served as an epidemic intelligence service officer and at the field station in El Salvador. He founded and directed the CDC research station in Nairobi, Kenya for 5 years and then served as Senior Medical Officer at the Malaria Action Program of the World Health Organization in Geneva.  Before going to Tulane, he was Chief of the Parasitic Diseases Branch at CDC. A graduate of Haverford College, his medical degree is from Johns Hopkins, and he has a Masters in Public Health degree from the University of California at Berkeley and a Diploma in Tropical Medicine and Hygiene from the University of London. He is board certified in internal medicine and preventive medicine; is a Fellow of the American College of Physicians and of the American College of Preventive Medicine. Dr. Spencer was elected a Founding Fellow of the United Kingdom Academy of Medical Sciences in 1998 and to the U.S. Institute of Medicine in 2003.
