Kalea Wattles, ND, IFMCP, is a naturopathic physician and functional medicine practitioner specializing in fertility optimization. Currently serving as the Clinical Special Projects Manager at The Institute for Functional Medicine, where she has been on staff since 2017, she is committed to the advancement of the field through research and education. She is the host of IFM's podcast Pathways to Well-Being. Dr. Wattles earned her doctorate in naturopathic medicine from Bastyr University, where she developed a keen interest in functional medicine. LEXINGTON, Ky. (Dec. 14, 2023) — A photo captures an instantaneous moment in time — allowing us to preserve a memory. Yet, oddly, the meaning of a photograph is never fixed. Its significance has the ability to change over time. A 4x6 frame holds a celebratory photo of Madona Elias, her mom and fellow graduates.  Elias is dressed in a black cap and gown. A golden tassel, signifying the commemorative closing of one chapter and the beginning of another, falls in front of her face — where Elias is also wearing a proud smile. It's 2004 and she has just graduated from the University of Aleppo in Syria. It's a feeling of accomplishment unlike any other — walking across the commencement stage, outstretched hand reaching for that coveted diploma. “Never did I imagine I would have to do this again nearly 20 years later,” Elias said. **** After receiving a degree in clinical mental health, Elias established herself as a mental health specialist and researcher in the global health community — serving several refugee populations (Syrian, Iraqi and Palestinian) worldwide. As a student who often excelled, she continued to chase her desire to be a catalyst for change through education. In 2010, Elias completed a Fellowship in medical anthropology at Newcastle University in the United Kingdom and was nominated three years in a row by the European Union as a key focal researcher from Syria in the RESCAP-MED project. “The main purpose of the project was to build sustainable research capacity for public health and its social determinants in low and middle-income countries in the Mediterranean Region,” she explained. With nearly two decades of experience in the Middle East, Europe and the United States, Elias implemented and oversaw complex research studies and evaluation projects with the World Health Organization (WHO), Catholic Relief Services (Caritas-Jordan) and the European Union. On top of a successful career, Elias was nearing the completion of her doctorate and felt as if her future was falling into place. “My life was much like anyone else's,” she said. “I had a career, a family and a home.” **** In the backdrop, unrest was spreading across the Arab world. When the Syrian war broke out in 2011, the only home Elias had ever known was no longer safe. “I was forced to flee my beloved country.” Elias' international work status allowed her to seek refuge in the U.S. with her 6-year-old daughter. But many family members were left behind. “Even though I escaped the violence, part of my heart remains there,” she said. Elias was forced to start a new life in a new country — where she would ultimately face language barriers, cultural differences and difficulties finding work. While Elias' education and career were not top of mind, she asked herself, “what's next?” That's when a deep uncertainty began to settle in. “English was my third language. And much of my education was not recognized in the U.S.,” Elias said. “I had to spend the next 10 years rebuilding my life from the ground up. And I had to start my career and education again from scratch.” **** Pain has a way of inspiring purpose. Elias remained resilient — determined to rebuild a bright, hopeful future for herself and her daughter. That's when, in 2016, she began working at UK HealthCare's Psychiatric and Behavioral Health Care Clinic in the College of Medicine at the University of Kentucky. “I loved the charming campus and learned how highly ranked UK's graduate programs in health care are nationwide,” she said. “While I was working, I enrolled in graduate school to earn a certificate in global health.” Elias continued working full-time to support her family, while earning exemplary grades. In 2019, she graduated with a certificate in global health with a focus on mental health. But that wasn't the end of her educational story or her journey at UK. In 2021, Elias was accepted into the online Master of Social Work (MSW) program in the College of Social Work (CoSW). In the years that followed, she pursued the ultimate balancing act — applying herself in the classroom, in the community and at home. “When choosing to complete my MSW as a single mom, while working two full-time jobs, the college offered me a flexible online program with a high quality of education and mentorship support that encouraged me to follow my dream,” Elias continued. “UK also has a wide array of programs that have allowed me to excel in multidisciplinary studies and foster an environment of cooperative engagement across all colleges, programs and research endeavors.” The rigorous MSW includes options for an advanced program (30 credits) and a regular program (60 credits) to give experienced social workers and/or passionate newcomers the practical skills needed to be effective in the field. The program is open to all undergraduate degrees and may be completed full-time or part-time, in either a hybrid or online format. MSW students also have the option to earn specialized certificates in clinical social work, child welfare, military behavioral health, rural population well-being, integrative behavioral health and substance use disorder, among others. The MSW graduates have the highest clinical licensing pass rates in Kentucky and routinely score above the national average — making those who complete the program uniquely poised to enter a competitive job market. And in so doing, students will positively impact the lives of individuals, groups and communities around the world. Through the CoSW, finally, Elias found a second home — and a second family. “It's been an arduous, lengthy, challenging journey. But I managed to reinvent myself and establish roots again here in Kentucky,” she continued. “My mentors, Kalea Benner, Katherine Crabtree and Peter Polatin, believed in me when I lost faith in myself.” **** What makes a photo powerful? On a sunny December afternoon, Elias finds herself back in front of the camera, beaming with pride as she poses in graduation gown — only this time it is blue, and her daughter, now 18, is standing by her side. It's been 20 years since Elias' first graduation. Now, both photos serve as a symbol — reminding her she discovered new happiness not despite challenges, but because of them. “I worked tirelessly, regardless of my scars of war PTSD and survivor's guilt, to become whole again as a refugee.” Elias is aware she is one among many who are forcibly displaced because of conflict, war or natural disaster. Through her initiative, “Global Mental Health Intervention Program During Crises and Wartime,” she hopes to not only help others like her but affect policy change surrounding mental health services for refugees. As Elias crosses the Commencement stage — for the second time — she hopes her story of resilience encourages others to embrace the best from the worst and keep moving forward. “Life is hard and challenging. But I am a war survivor, and now I can see why I'm still here,” she said. “Unfortunately, war is beyond our control. But refugees should have a better future and need help healing. The Wells Collective was born out of both necessity and the right amount of mimosas. After six Black women, who worked individually as public and private school educators, researchers, educational administrators, and psychologists, gathered together for a bubbly brunch, the kinetic energy that manifested itself at their table confirmed that by coming together, they could have a greater impact in promoting inclusion in their communities. These women rallied together to identify ways in which they could leverage their individual talents to develop a unique framework that would equip organizations with the tools  to be successful as change agents and architects for liberation. I was lucky enough to hear them speak at a conference last March at Mohonk Mountain House. I knew immediately, that I wanted to share their message on the Debrief. If you want to work with them or learn more, please reach out! Kalea Selmon, Areas of Expertise: Restorative practices, teacher coaching, social-emotional wellness, assessing school culture for inclusion and anti-racist practices. Kalea hails from the Washington DC area. She is a proud alumna of Howard University where she earned her Bachelor of Science in Psychology. After undergrad, she began her career teaching and facilitating a youth employment program. Kalea returned to school attending Johns Hopkins University where she earned a Master of Science in Counseling. Later, she transitioned into Independent Schools where she served as a school counselor and then a Director of Diversity and Inclusion where she focused on developing equity programming, providing professional development, curating curriculum, and hiring and retention of diverse staff. Jenifer Moore, Areas of Expertise: Anti-racist & anti-bias curriculum, teacher coaching, assessing school culture for inclusion and anti-racist practices. Jenifer is the Director of Diversity, Equity, and Inclusion at the Key School in Annapolis, MD. Previously, she has served as the Head of School and Director of Curriculum and Instruction at Sela Public Charter School. Jenifer has served children of Washington, DC since 2003 as a teacher, tutor, and school leader. She earned a Master's degree from Trinity Washington, DC in Educational Administration and a Bachelor's degree from Howard University. Akailah Jenkins McIntyre, Areas of Expertise: Anti-racist & anti-bias practices, employee and teacher coaching, assessing organizational culture for inclusion and anti-racist practices, public speaking. Akailah is a native of Charleston, SC who began her career in education at Vanderbilt University. She went on to contribute to multiple education research projects at the institution, resulting in two published works. In this time, she discovered that her areas of interest were racial justice in education, and exploring Black girl joy and freedom in educational spaces. She earned both her Bachelor's degree, and Master's Degree from the institution. She now serves as Chief of Staff at a nonprofit. Dr. Shari Baker, Areas of Expertise: Anti-racist & anti-bias curriculum, teacher coaching, assessing school culture for inclusion and anti-racist practices Shari began her career in education at Duke University. While there, she joined a women's scholarship group, the Baldwin Scholars, in which she continues to be an active member. Dr. Baker obtained her social studies teaching certificate and went on to teach social studies in Prince George's County Public Schools for a decade. She has also had extensive training on cultural responsive/sustaining pedagogy and restorative justice in schools. Her approach to helping women conceive and maintain pregnancies is based on the functional medicine philosophy. Key Takeaways: Focusing on reproductive longevity It's a human-specific phenomenon to experience a decline in fertility potential around middle age. This could be attributed to the "grandmother hypothesis," where women lose their fertility at a young age to allocate time and energy to taking care of their offspring. Nevertheless, losing reproductive potential has broader implications for various bodily systems, such as reduced bone mineral density and cognitive function, and an increased risk of cardiovascular disease. Therefore, it's essential to prioritize reproductive longevity to not only ensure successful pregnancies now, but also promote long-term health and vitality. Understanding lifespan, healthspan, and fertility span Lifespan is the total duration of a person's life, from birth to death. Healthspan, on the other hand, refers to the period of life during which an individual is in good health and free from chronic diseases or disabilities. It's a measure of function rather than a measure of time. The fertility span is the period during which an individual is capable of reproducing, and it refers to the health of the ovaries, regular ovulation, hormone production that protects your bones and brain, and the nervous system. Normal versus pathological ovarian aging Women's ovaries naturally age, leading to a decline in function that culminates in the menopausal transition, usually occurring in the early fifties. This process is a normal part of aging and occurs regardless of a woman's overall health. However, some women experience an acceleration in ovarian aging, resulting in a decrease in ovarian reserve, which can lead to infertility. This can occur even if the woman is still having menstrual cycles and normal hormone levels. Premature ovarian insufficiency is another condition in which the ovaries exhibit perimenopausal or menopausal symptoms, such as irregular periods, even in younger women. Oxidative stress and inflammation Oxidative stress and inflammation are the two biggest contributors to ovarian aging. Oxidative stress is when the body experiences a buildup of compounds that can damage DNA. It can be from having a high sugar diet, environmental toxin exposures, chronic inflammation, or a low intake of dietary antioxidants. Inflammation can come from lots of different sources. Periodontal disease is a significant contributor to inflammation in the reproductive system. But it can also come from food sensitivities, intestinal hyperpermeability, or leaky gut. It's crucial to acknowledge the interconnectivity of all the systems in our body, as each one can impact the others. Therefore, when a woman intends to get pregnant, she must ensure that all her bodily systems are in proper working order and balance. Slowing ovarian aging Be mindful of your diet and reduce sources of advanced glycation. This means cutting back on fried and baked foods and focusing on consuming more antioxidant-rich foods. In addition to dietary changes, it's also important to consider lifestyle factors such as exercise, stress management, and adequate sleep. Also, regularly test. She cleared her throat and began "reading" the story without even looking at her book. "When Hansel and Gretel woke up, the forest was very dark. 'How will we find our way home?' asked Gretel. She began to cry. Hansel picked up a pebble lying on the pathway. 'Look, Gretel, here are the pebbles I dropped. We can follow them back home.""I know the story," said Kalea with a grin. "When Mom left for the store, she said to remind you to memorize your Bible verses for church. Have you done that already?""Those verses are too hard!" said Nani. "Besides, what good is learning a bunch of verses anyway?""Well, for one thing, the Bible shows us who God is and how much He loves us. It tells us what Jesus did to save us and how we should live as we trust and follow Him." Nani continued to pout, so Kalea sat on the sofa beside her little sister. "Think of it like this," she said. This episode might be a tough one to listen to. The "baby blues" or "postpartum blues" affects 60-80% of all new moms. Postpartum depression affects between 10-20% of all new moms. Dayna is one of those 10-20%. Although Dayna and Kalea have been best friends for years, there is a lot Kaela doesn't know about Dayna's struggle with depression. This episode includes some laughs, but also a lot of tears. We hope that this honest, raw, and real conversation helps those who are also struggling or even those of us who think we might know someone struggling, find the help they need.
