Our guest today is Jesse Gould, founder and president of the Heroic Hearts Project, a nonprofit pioneering psychedelic therapies for military veterans. “I would love for the psychoanalytic world to re-embrace some of these adjunctive treatments that get to non-ordinary states of consciousness in order to enhance psychoanalytic treatment, and that includes psychedelics. The other thing I'd like to see is, I think psychoanalysts are extremely well suited to use psychedelic-assisted therapy in a non-harmful way. I really believe that without an ongoing treatment relationship that these medicines are not going to be quick fixes. There's an article in the Substack blog, Ecstatic Integration, about an Israeli man who had an MDMA treatment for chronic post-traumatic stress disorder and it's a very interesting read, and it really does speak to the crucial aspect of having an ongoing therapy relationship while we use these medicines. I want people who are in the analytic field who are trained right out of the box to provide these containers to be more involved in the psychedelic field. For the psychedelic field I think sometimes I wish they would be a little more humble, a little less zealous about the efficacy, and a little more concerned about what could happen that's harmful. I also think the psychedelic field, for some reason, has not embraced psychoanalysis as a major tool to enhance the medicine."    Episode Description: Charis begins by discussing her inspiration, attributed to Maimonides, for always seeking new understandings to enhance her care of patients. We also begin with a caution - any time we introduce a frame change in clinical work, we must carefully attend to our countertransference to determine the factors that are contributing to our actions. That said, we should be careful to not use our carefulness to rationalize inhibitions to thinking and acting creatively. Charis describes her thinking underlying her decision to introduce ketamine with a particular patient as well as the process of the ongoing psychotherapy. We discuss the practical aspects of this procedure, the risks, the changes in the patient, and the importance of an ongoing psychotherapy to serve as a productive holding and processing space for this work. Distinguished Audience, welcome to a very special episode of The Just Dumb Enough Podcast!   The show is officially 2 years old as of today.  So, I figured a great was to celebrate how far we've come is to compile some clips of the guests we've heard from over the last two years. But these weren't picked at random, they are the top 10 most downloaded and streamed episodes from the first two years.   I will note that the competition was very tight between the top ten with only a couple dozen streams separating #1 and #10. Plus, she gave so much insight on a highly desired job, which was an original intent behind the podcast.   #5 Psychedelic Medicines with Jonathan de Potter: I was worried that the audience who shows up for very professional topics might not receive the topic of drugs particularly well, but it's place in the top 10 shows that there is a broad interest to learn and grow amongst the audience, despite the perception of any given topic.   #6 Modern Caveman Brains with Dr. Betsy Holmberg: Psychology will never not be fascinating to me, and the thought that an outdated brain process might lead to our early deaths earned it's spot on the community's list.   #7 The JFK Assassination with KW Zachary & Sara Peterson: While usually I shoot for the highest audio quality, I love that this true crime slash conspiracy episode has that very on brand audio like I was taking a call from a safehouse.   #8 Blindness with Kevin Lowe: With such a warm personality and a great show of his own, this whole interview was either educational, surprising, emotional or all of the above.   #9 Near Death Experiences with Chris Jankulovski: For a man who has always been on the verge of death, Chris doesn't let it slow him down. And his willingness to push forward carries a lot of lessons. It was honestly hard to cut this one down for the compilation because it's all so compelling!   #10 Reframing Discrimination with Dr. Frank Douglas: I'm a bit surprised to see this recent episode competing so well with others that had more time, but I'm also glad to see that people are coming to the show for information about key issues in society.   And that's the whole top 10 most downloaded!    What do you think? "I don't think anybody can truly describe the [psilocybin] experience. One can use phrases and words that are common - ineffable, mystical, powerful. All I can say is it was the most powerful experience of my life that I have gone through ever."     "We had another CAT scan in September of last year, in 2021, and it showed another small nodule. My oncologist said, ‘Don't worry, we don't know what it is, it's very small.' But a part of me as a physician and as a cancer patient now thought ‘On no, we've  got another nodule - that means I am stage 4 again'. Before my experience with psilocybin this would have been terribly devastating - to have been given that hope six months ago with no nodules, and now again Stage 4. But now I say ‘Ok, if I have cancer again, it's alright we will deal with it'. While I was somewhat anxious and I was a little bit low in spirits but it wasn't a gut punch."     Episode Description: Pradeep shares with us his journey from initial symptoms of what was diagnosed as a stage 1 cancer to its return as a stage 4 metastatic cancer. Faced with a disturbing depression he entered a clinical trial using one dose of psilocybin in a carefully monitored research setting. He shares with us the "indescribable nature" of his 8-hour psychedelic experience. He then reports on the profound life changes that have remained with him over one year later. His approach to life is characterized less by highs and lows and more by a dependable sense of equanimity even in the face of his ongoing cancer. Listen In!mckenna.academyDennis Mckenna Bio Dennis Jon McKenna is an American ethnopharmacologist, research pharmacognosist, lecturer and author, who has studied plant hallucinogens for over forty years. He earned his Master's degree in botany at the University of Hawaii, and his Doctorate in Botanical Sciences from the University of British Columbia. Since that time, he has conducted extensive ethnobotanical fieldwork in the Peruvian, Colombian, and Brazilian Amazon. Dennis is also a founding board member of the Heffter Research Institute, a non-profit organization concerned with the investigation of the potential therapeutic uses of psychedelic medicines and was a key investigator on the Hoasca Project, the first biomedical investigation of ayahuasca. From 2000 to 2017, he taught courses on Ethnopharmacology and Plants in Human affairs as an adjunct Assistant Professor in the Center for Spirituality and Healing at the University of Minnesota. He emigrated to Canada in the spring of 2019 together with his wife Sheila, and now resides in Abbotsford.Since 2019, he has been working with colleagues to manifest a long-term dream: the McKenna Academy of Natural Philosophy, a non-profit organization founded in the spirit of the ancient Mystery Schools and dedicated to the study of plant medicines, consciousness, intelligence in nature, preservation of indigenous knowledge and a re-visioning of humanity's relationship with Nature. The renaissance of psychedelics in the field of clinical research has caught widespread public attention. This podcast would provide listeners an opportunity to understand why this resurgence has occurred, what is can offer to the field of clinical psychology, and the current scope of the research to date. Dr. Amanda Khan Dr. Amanda Khan is a licensed clinical psychologist and researcher specializing in the assessment and treatment of trauma sequelae. Dr. Khan is a staff psychologist at Sage Integrative Health, a holistic psychedelic clinic, where she provides ketamine-assisted psychotherapy, depth oriented trauma work, and post-psychedelic integration. She served as an independent diagnostician on the Multidisciplinary Association for Psychedelic Studies (MAPS) on phase II and III trials of MDMA-enhanced psychotherapy for PTSD for four years. Dr. Khan has trained in MDMA-Assisted Therapy with MAPS and Ketamine-Assisted Psychotherapy with Polaris Insight Center, and serves as a Psilocybin Therapist on the UCSD clinical trial for phantom limb pain. Dr. Khan's program of research aims to elucidate how characteristics of trauma, emotion regulation, and biology impact the etiology, maintenance, and treatment of PTSD, moral injury, and depression/suicidality. She is especially interested in developing novel approaches, such as using psychedelics, to heal treatment-resistant PTSD. Dr. Khan is the Chair of the Moral Injury Special Interest Group (SIG) for the International Society of Traumatic Stress Studies (ISTSS). She has published numerous peer-reviewed journal articles, presents regularly at conferences, and is lead author on a chapter exploring psilocybin for trauma-related disorders. She also writes op-eds for Psychedelics Today, Stress Points, and Medium. Dr. Khan gives regular invited talks on moral injury and working with gender and sexual orientation diverse people. “The autobiographical stage [in a psilocybin experience] is where a great deal of important work happens. One of the most important things that happens during this phase is the emergence of what is truly valued by the individual. Consider when there is a cancer diagnosis - it seems that at that point everything about them falls away except their body, their cancer, their chemo, their vomiting and so forth. Their values and relationships often suffer, and the importance of this, the need for this returns [during this autobiographical stage] in a very vivid dynamic almost forceful way.”     Episode Description: We discuss the history of plant medicines over the millennium as well as the recent research into their medical benefits that took place in the 1950s and 60s. We review the current studies on the role of psychedelic therapies with cancer related anxieties, severe depression, addictions and PTSD as well as with eating disorders and obsessive-compulsive conditions. Dr. Guss describes the various levels of transcendence that may occur while using psilocybin and how it may deepen an individual's awareness of the interconnections among living beings. Finally, we review how the careful clinical use of these substances may be usefully applied adjunctively to an ongoing depth psychotherapy.   Our Guest: Jeffrey Guss, MD is a Clinical Assistant Professor of Psychiatry at NYU School of Medicine. He is a psychiatrist and psychoanalyst who has been practicing for 30 years. He is active in numerous psychedelic research projects with the NYU School of Medicine and Yale University. He is a graduate of the NYU Postdoctoral Program in Psychotherapy and Psychoanalysis and is a Lead Trainer with Fluence (www.fluence8.com) a teaching center specializing in psychedelic integration.
