Sonya Gabrielian, MD, MPH, is a psychiatrist and health services researcher at the VA Greater Los Angeles and an Assistant Professor at the UCLA David Geffen School of Medicine. At the VA, she is an investigator with the Center for the Study of Healthcare Innovation, Implementation, & Policy (CSHIIP) and the Mental Illness Research, Education, and Clinical Center (MIRECC). She is also an Affiliated Researcher at the National Center on Homelessness Among Veterans and a current Fellow with the Implementation Research Institute at the Washington University in St. Louis. Dr. Gabrielian's research interests focus on improving housing retention and community functioning among homeless adults; she has a VA Career Development Award to use implementation approaches to improve housing acquisition and retention for homeless Veterans with serious mental illness. She is also a practicing psychiatrist in the VA's homeless program.

So what exactly does a Sex Therapist do? In my exploration of trying to identify opportunities for patients and providers to best communicate about sex, what I'm finding is that there's a lot more to understand than the stigma component. Kimberly shared that on the surface, doctors don't talk about sex with patients because of time, but what we learn is that that's just an excuse for a much larger issue. Kimberly Resnick Anderson, LCSW, is an AASECT Certified Sex Therapist, speaker, writer, and professor, with more than 30 years of experience treating individuals, couples and groups. Many parents hesitate to jump to a medication treatment for their child's ADHD and ask about alternatives to medication. On this podcast, we have talked about different medication options but also behavioral therapies, mindfulness, and classroom accommodations, but today we're talking about an empirically supported non-medication treatment for ADHD and the results look very promising. Today's guest is Dr. Sandra Loo who researches Trigeminal Nerve Stimulation and its impact on ADHD symptoms in children ages 7-12 at UCLA. In today's episode, she describes what Trigeminal Nerve Stimulation (TNS) is and how it works to generate brain activity. She also shares the benefits of the treatment and provides updates on how the study is going. It is a very exciting advance in non-medication treatment alternatives for children and you can reach out to Dr. Loo through the links below to participate in the study as well.   Show Notes: [2:44] - The work Dr. Loo is doing with this new treatment is exciting and hopeful. [4:06] - Trigeminal Nerve Stimulation is the first FDA cleared treatment of ADHD of kids ages 7-12 years old. It's a mild stimulation that generates brain activity. Dr. Loo describes the process. [5:15] - After about 4 weeks of nightly stimulation, Dr. Loo has seen improvement in ADHD symptoms. [7:18] - Children on the older side of the 7-12 age range tend to feel comfortable putting the device on themselves. Dr. Loo explains what it might feel like for a child to wear. [9:16] - This isn't medication and results are slower than medication treatment. [10:22] - Improvements show the greatest results in inattention. [11:35] - Dr. Loo is also working on predictors that will help determine what treatment would be best for individual children. [13:08] - One observation from parents is that ADHD symptoms are better and that their child is less emotionally dysregulated. [15:45] - After 4 weeks in the study, they found that stopping the treatment did see an increase in the symptoms again. [17:37] - There are other goals for this treatment that are still being researched. [18:15] - There have not been any adverse side effects of this treatment and everyone responds differently. [20:47] - What are the comparisons to more common medication treatment results? [23:33] - All presentations of ADHD are welcome to participate in this study. My guest on this episode is Reiko Joseph, LMFT. Reiko is a therapist who has credentials in Psychedelic Somatic Interactional Therapy. She is also Clinical Instructor, UCLA David Geffen School of Medicine, Certified Instructor, Intensive Short-Term Dynamic Psychotherapy (ISTDP), approved by the International Experiential Dynamic Therapy Association (IEDTA), and a member of the California Association of Marriage & Family Therapists (CAMFT). Reiko is a masterful therapist and colleague in Los Angeles who practices/teaches ISTDP therapy and has in recent years ventured into the realm of psychedelic assisted therapies. The jury is still out for me on psychedelics for a number of reasons-- but I'm cautiously curious about their clinical application. I have many many more questions, but this discussion is solid intro for anyone who is curious about, skeptical of, or sold on psychedelics in clinical practice. Hopefully in a follow up episode we can discuss the difference between learning about our defenses consciously vs. under psychedelic influence, the mechanisms of change, and what models of the mind (if any) can best explain aspects of psychedelic experience. Lung cancer is one of the most commonly diagnosed type of cancer and so it is fitting that we start the first of our disease-specific oncology series with this diagnosis. This week, we sit down with Thoracic Surgeon, Dr. Jane Yanagawa to discuss surgical considerations in treatment of NSCLC. * How do you choose what type of surgical resection to do?- Considerations: --Lung anatomy --Location of the nodule within lung--Lymph node involvement-Options: --Pneumonectomy: removal of whole lung --Lobectomy: remove a whole lobe--Segmentectomy/sublobar resection: part of a lobe* What does “adequate margins” mean? And how do you know if it's enough?- If you're removing the whole lobe, it does not matter as much - If you're doing a segmentectomy, you want to have samples evaluated while in the OR because if there is signs of more disease that initially thought, you would take this one step further and do a lobectomy. - Need to consider the patient's situation - how good is their status * Why does preoperative workup matter?- Pulmonary function tests: Surgeons are looking at the %FEV1 and %DLCO to then predict what their function would be AFTER surgery. After surgery, they want to ensure patient has %FEV1 or %DLCO >40%. - Lung anatomy: In patients with COPD and endobronchial lesions, sometimes they also get V/Q scans to evaluate ratio- Cardiac echo: Important in pneumonectomy where removal of lung tissue will also remove a significant amount of blood vessels. Want to rule out pulmonary hypertension pre-operatively. - Pulmonary hypertension can also affect someone's survival while they're ventilating with only one lung during the procedure (“single lung ventilation”). - Smoking status: Smoking can increase complications by ~60%. - Pre-habilitation: Encouraging patients to be fit prior to surgery with walking, nutrition, +/- pulmonary rehabilitation* What is “VATS”?- VATS stands for video-assisted thoracoscopic surgery; it is not, in itself, a procedure. But a VATS allows for minimally invasive surgery through the use of a camera. - It involves three incisions (axilla, lowest part of mid-axillary line, one posterior)* In what scenario is a mediastinoscopy warranted? - Needed after EBUS if there is still high index of suspicion for cancer involvement in lymph nodes, even if lymph nodes are negative from EBUS* What is “systematic lymph node sampling”?- An organized way to sample lymph nodes, including all lymph nodes that are along the way, not just the ones that may be involved * As a surgeon, how do you determine if a patient is okay for surgery if the mass is invading another structure?- Need to take the anatomy into consideration - are there major blood vessels or nerves there, for instance, which can impact outcome and recovery.* When should we consider induction chemotherapy from a surgeon's perspective?- Lots of changes in this sphere coming; lots of discrepancy between institutions when there is N2 disease - In Dr. Yanagawa's opinion, anyone with N2 disease should get neoadjuvant therapy * If there is neoadjuvant chemoradiation given, how does that effect your surgery?- Radiation increases scar tissue in the lung tissue. But what is worse is that radiation neoadjuvantly may make wound healing more difficult. She does not prefer radiation pre-operatively- Chemotherapy also adds scar tissue*How does neoadjuvant IO therapy affect scar tissue formation?- The hilum and lymph nodes are more swollen, but does not translate to more complications - She has even seen patients who had gotten IO for another cancer and then get lung cancer, she can still appreciate swollen nodes!* How long after surgery is it safe to start adjuvant therapy?- If patient has a complication from surgery, would not start right away. It is important to discuss with the surgeon about when it is okay to proceed with adjuvant therapy. - If patient has good recovery/without complications, okay to start about 4 weeks after- There is no good guidance yet about when it is safe to start IO after surgery About our guest: Jane Yanagawa, MD is an Assistant Professor of Thoracic Surgery at the UCLA David Geffen School of Medicine and the UCLA Jonsson Comprehensive Cancer Center. He is currently a Clinical Professor of Radiology, at the USC Keck School of Medicine with an academic focus on the applied science of longevity. He is also Chief of Metabolic Imaging at a large medical network in southern California. In addition to being a practicing physician, he is the author of over 200 peer-reviewed scientific papers, 32 book chapters and 14 books that are available in six languages. He studied computer science at Brown University and completed his medical degree at the University of Virginia School of Medicine. Robert has given invited lectures/keynotes around the world and was named one of the ‘100 Most Creative People in Los Angeles' by Buzz Magazine. In our discussion with Dr. Lufkin, we discussed the faults in our medical system that have led so many people astray and some actionable steps people can take to reclaim their metabolic health. Dr. Daniel Geschwind Bio: Dr. Geschwind is the Gordon and Virginia MacDonald Distinguished Professor of Human Genetics, Neurology and Psychiatry at UCLA. In his capacity as Senior Associate Dean and Associate Vice Chancellor of Precision Health, he leads the Institute for Precision Health (IPH) at UCLA, where he oversees campus precision health initiatives. In his laboratory, his group has pioneered the application of systems biology methods in neurologic and psychiatric disease, with a focus on autism spectrum disorders (ASD) and neurodegenerative conditions. Dr. Geschwind is a pioneer in the transcriptomic and functional genomic analyses of the nervous system. His laboratory showed that gene co-expression has a reproducible network structure that can be used to understand neurobiological mechanisms in health and disease. He led the first studies to define the molecular pathology of autism and several other major psychiatric disorders, and has made major contributions to defining the genetic basis of autism. He demonstrated the utility of using gene network approaches to discover new pathways involved in neurodegeneration and new approaches to facilitate neural regeneration.  More recently, his laboratory demonstrated how knowledge of 3-dimensional chromatin structure can be used to understand the functional impact of human genetic variation. Dr. Geschwind has trained over 70 graduate students and post-doctoral research fellows, and is among the highest cited scientists in neurology, neuroscience and genetics (H index > 140). In addition to serving on several scientific advisory boards, including the Faculty of 1000 Medicine, the Scientific Advisory Board for the Allen Institute for Brain Science, the NIMH Advisory Council and the NIH Council of Councils, he currently serves on the editorial boards of the journals Cell, Neuron and Science. He has received several awards for his laboratory's work is an elected Member of the American Association of Physicians and the National Academy of Medicine.   What can big data and genomics offer the individual regarding medical treatment? Dr. Daniel Geschwind is here to explain exome sequencing data analysis as well as how this field might expand and progress. Listen and learn What makes up our genome, how to understand the terms for each part, like exon, intron, and exome, and what parts are effected epigenetically; Why genomics testing still focuses on that 2 to 3 percent of the genome called the exome that codes for proteins; and How sequencing will broaden and change, making preventative care that much more effective for certain patients and more. Dr. Daniel Geschwind is the Gordon and Virginia MacDonald Distinguished Professor in Neurology, Psychiatry, and Human Genetics; the Senior Associate Dean and Associate Vice Chancellor for Precision Health at UCLA; and the director of the Center for Autism Research and Treatment at the Semel Institute at UCLA David Geffen School of Medicine. He opens up the world of genomics for listeners, explaining why an exome sequencing test is the center of precision medicine currently. He explains how the exome is composed of the 3% of your genome that actually codes for protein sequences. What's the rest of the genome up to? Well, scientists believe that remainder of your genome is busy regulating those sequences, determining levels and turning gene expressions on and off.  He explains how scientists use the exome sequencing process. In fact, the majority of people who say they've been sequenced mean, in fact, that their exome has been sequenced. Whole genome sequencing costs about three times as much but offers scientists the same information at this point. However, he thinks this will soon change. As the exome sequencing project continues and our knowledge accumulates, the benefits will increase from whole genome sequencing. The costs are likely to drop as well.  Once it gets inexpensive enough and we have sequenced hundreds of millions of patients with a variety of disorders, we will have much better predictive power.  For more information, he suggests UCLA's Precision Health website. Tatkin is a clinician, researcher, teacher, and developer of A Psychobiological Approach to Couple Therapy® (PACT). He has a clinical practice in Calabasas, CA, and developed the PACT Institute for the purpose of training other psychotherapists to use this method in their clinical practice. Dr. Tatkin also teaches and supervises family medicine residents at Kaiser Permanente, Woodland Hills, CA, and is an assistant clinical professor at the UCLA David Geffen School of Medicine, Department of Family Medicine. Dr. Tatkin is on the board of directors of Lifespan Learning Institute and serves as a member on Relationships First Counsel, a nonprofit organization founded by Harville Hendrix and Helen LaKelly Hunt. Dr. Tatkin was a primary inpatient group therapist at the John Bradshaw Center, where among other things, he taught mindfulness to patients and staff. He was trained in Vipassana meditation by Shinzen Young, and was an experienced facilitator in Vipassana. He was also trained by David Reynolds in two Japanese forms of psychotherapy, Morita and Naikan. Dr. Tatkin was clinical director of Charter Hospital’s intensive outpatient drug and alcohol program, and is a former president of the California Association of Marriage and Family Therapists, Ventura County chapter. He is a veteran member of Allan N. Schore’s study group. He also trained in the Adult Attachment Interview through Mary Main and Erik Hesse’s program out of UC Berkeley.

GUEST STAN TATKIN: Dr. Stan Tatkin, PsyD, MFT, is a couple therapist known for his pioneering work in helping partners form happy, secure, and long-lasting relationships. His method—called PACT (Psychobiological Approach to Couple Therapy®)—draws on principles of neuroscience and teaches partners to become what he terms “secure-functioning.” Together with his wife, Tracey Boldemann-Tatkin, PhD, Dr. Tatkin founded the PACT Institute to train psychotherapists and other professionals how to incorporate his method into their practices with couples. Therapists from all over the world are being trained in this breakthrough approach. Dr. Tatkin has a private practice in Calabasas, CA, and is an assistant professor at the UCLA David Geffen School of Medicine, Department of Family Medicine. He is the author of several books, including the bestselling WIRED FOR LOVE and WIRED FOR DATING published by New Harbinger. STAN TATKIN TALKED ABOUT: “Secure Functioning” is being in a relationship that is fully collaborative, fully mutual, and based on justice, fairness, and sensitivity. How a couple protects each other and how they handle stress together. Environmental threats may bring stress to the individuals and the couple, and helping couples regulate the stress through connection and relationship. Family culture – is relationship a priority? If relationship is not important and prioritized, than children often develop some level of insecure attachment. Cultural messages can be misleading and confusing (i.e. “Love yourself before you can love another.”) Couples often lack purpose in their union…why they are together? What is their partnership all about? What are their shared values, agreements, and principles that keep them committed? People with insecure attachment styles tend to behave in ways that are good for the self, but are not good for the relationship. How our neurobiology impacts our perception and memory, which in turn impacts the way we perceive and experience our partners in relationship. Naturally, we tend to have a negativity bias. Why are relationships so hard? If you're anything like I am you've spent immeasurable time and effort clawing away at romantic relationships that just never seem to work out. For me personally, it's been by far the biggest supplier of grist for the mill. When the offer came across my desk to have Stan on the podcast I immediately jumped on it because I not only wanted to bring his wisdom to you but I also wanted to get into the deep end of my own issues. I got so much out of this one. We talked about the patterns within relationships, our nervous systems, gender roles and history, neurobiology and why that plays a role and techniques for dealing with conflict. Dr. Sung-Jae Lee is an Assistant Professor-in-Residence in the Department of Psychiatry and Biobehavioral Sciences at UCLA David Geffen School of Medicine and Core Scientist for CHIPTS Methods Core. Dr. Lee is an epidemiologist whose research has included adaptation of family-based interventions for HIV-affected families in Thailand, examination of HIV vaccine acceptability, assessment of HIV-testing preferences, and assessment of pre-exposure prophylaxis acceptability. He has conducted HIV research in Thailand for the past 14 years. His current ongoing research project focuses on designing a family-focused intervention for People Living with HIV in Thailand to assist them with their HIV disclosure decisions and pathways.
