What if the fastest way to grow your healthcare organization is to slow down? In this episode of The Disrupted Podcast, Scott Middleton returns from Scotland straight into the thick of a merger — and what he's learning is reshaping how he thinks about change itself. Scott takes us inside the integration of TCPA and Providence Care into Your Health, where two very different models are colliding. One organization built 640 billing codes last year; the other built 40. One puts a single nurse practitioner in a building with no support; the other surrounds providers with nurses and community health workers. The opportunity is enormous — but so is the risk of moving too fast and scaring everyone away. What if the fastest way to grow your healthcare organization is to slow down?   A lot of people don't like to face getting older, but it is an important thing cities and states need to address. According to Deseret News, Utah's aging population has grown by over 50 percent between 2010 and 2020, surpassing the national rate. Holly and Greg speak to article author Lois Collins from the Deseret News.

The Disrupted Podcast

What does it actually cost when a doctor writes a verbal order over the phone instead of seeing the patient? Scott Middleton has the receipts — and the answer is going to make you rethink everything about how American healthcare spends its money. In this episode of The Disrupted Podcast, Scott announces a landmark three-way merger bringing Your Health together with Transitional Care Professionals of America (TCPA) out of Georgia and Providence Care, a hospice organization in South Carolina. The combined organization will serve approximately 55,000 active patients — not patients on a list, but people being seen regularly — and Scott lays out exactly how he's going to run it. What you'll hear in this episode: Why Scott's family owning 80% of the merged company changes everything about how decisions get made — and who they get made for The difference between fee-for-service and value-based care, and why the ACO model means every unnecessary hospitalization literally comes out of Your Health's pocket How Your Health's risk-adjustment-based visit model (16 visits per year per risk point) was independently validated by a new government study — and why it works The three things Scott is asking every new employee to do in the first weeks: align with a nurse practitioner, track every minute of care management, and recruit like their livelihood depends on it — because it does Why Scott's new management philosophy is six words: "Keep them out of the hospital and see your damn patients" This isn't a corporate announcement. It's a playbook for how healthcare can actually work when operators run the company, providers see their patients, and every minute of care gets counted. www.YourHealth.Org

Canada's population is getting a makeover, and it is changing the way we live. We are digging into new demographic data with Patrick Charbonneau, Section Chief in Demography at Statistics Canada, to see how big boomer and millennial generations, plus fewer younger people, are shaking up demand for housing, schools, and healthcare. We also look at the rise of the sandwich generation, the caregiving squeeze that comes with longer lives and smaller families, and the many ways older Canadians keep things running through volunteering, childcare, and family support.

Australians are living longer and longer, which is on one hand a beautiful thing. But on the other, prolonged old age is wreaking havoc. So how might we respond to this new demographic situation we find ourselves in?Lucinda Holdforth is a writer who specialises in looking at what makes good societies flourish, everything from manners to politics and equality. Most recently, she's set her sights on the unintended negative consequences following the extraordinary increase in life span around the world, particularly in Australia. In the past 50 years, human life expectancy across the globe has jumped from 46 years old to 73, and in Australia that number is even higher -- an Australian born today is likely to live until they are 84 years old.On the surface, living longer is a very good thing. It means more time spent with our loved ones, looking at the stars, feeling the sun, living. But prolonged old age can also be very lonely and painful, and, as Lucinda argues, it is costing society as a whole in many ways. She has seen this firsthand, as a daughter who supported her own parents in their long old age, and has some surprising suggestions about how we could do things differently to ease the impact on our economy, our medical system, our elderly and our youth.GOING ON AND ON: Why our longevity threatens our future is published by Simon & Schuster. This episode was produced by Meggie Morris. This episode explores how Denmark's 2024 health reform is accelerating an already mature digital health ecosystem, with a special focus on Copenhagen's municipality-led elderly and community care services. Speakers Anders Elken Sønderby and Rikke Saltoft Andersen from City of Copenhagen, explain how the reform responds to demographic pressures: a growing elderly population, increasing chronic disease burden, and workforce shortages. Rather than representing a radical shift, the reform acts as an acceleration layer on top of long-established digital health infrastructure. The discussion dives into how municipalities support home care, nursing, rehabilitation, prevention, and care homes, all digitally connected with hospitals and general practitioners through Denmark's long-standing MedCom interoperability framework. A strong emphasis is placed on care continuity, ensuring data follows citizens across hospitals, care homes, and home-based services. A standout theme is Copenhagen's effort to include relatives and informal caregivers in care planning through digital dialogue tools and telemedicine, improving health equity and patient support. The conversation concludes with the city's AI ambitions, particularly a proof-of-conceptwhich uses speech-to-text, summarization, and structured categorization to reduce documentation burden for care workers and improve data quality across 10,000 staff members.

It's expected that more than 13 million people will be diagnosed by 2060. Beyond Alzheimer's, roughly 10% of U.S. adults over 65 have dementia.  On this podcast we talked with Dr. Ronald Petersen, a neurologist at the Mayo Clinic and an expert in Alzheimer's disease and other cognitive disorders, and Jennifer Rosen, vice president for state affairs at the Alzheimer's Association. Joining the podcast to offer the legislative perspective were Senator Laura Murphy of Illinois, a Democrat, and Senator T.J. Shope of Arizona, a Republican.  Dr Petersen explained the remarkable progress that has been made in recent years in the diagnosis and treatment of Alzheimer's and the importance of early intervention. Rosen outlined the enormous financial impact of the disease on society, and the role state legislatures can play in supporting early detection and treatment.  Senators Shope and Murphy each laid out the legislation enacted in their states and how that legislation will aid both people with the disease and their families.  Resources Alzheimer's Disease Treatment Clinic, Mayo Clinic Alzheimer's Association Comprehensive Policy Approaches to Support the Aging Population, NCSL State Policy Approaches to Improving Alzheimer's and Dementia Care, NCSL State Strategies to Reduce Costs and Provide Quality Care for Older Adults and People with Disabilities, NCSL

The main goal is to make tasks like care coordination, document management, medication adherence, and detecting changes in behavior and physical abilities easier. Chase explains, "AgeTech is more around the people in the population. So if you look at aging, it is not one point in time, but is truly a continuum and impacts so many people. It's not just the person who's aging, but also the family members and the entire circle of care around them. So, AgeTech is that kind of emerging field, which is how I try to differentiate it. Again, it's not product-centric, but it is really human-centric over a continuum of time."   "What we felt was the biggest opportunity was not only creating a solution for older adults, but creating a solution for the circle of care, the people every single day who are helping those older adults. And in parallel, we really started to lean in on this employer front with the workforce. So in the workforce environment, it's pretty interesting, 25% of FMLA is for caregiving. And right now, there's not a ton of solutions that can support them.

Pat took the reins in 2024 and brings both global perspective and hands-on experience from the World Economic Forum's Longevity Economy Initiative, which aims to address the demographic and financial challenges of aging societies. "Having fewer workers as people age and … a lower birth rate will impact employers and will impact business and will impact society as we think about how to grow GDP," Pat says. Pat explains how retirement savings and healthcare systems need to evolve to meet the needs of the aging global population. And he outlines creative solutions that employers and governments are considering to address these challenges — including through public-private collaboration, AI, reskilling workers and flexible working arrangements.   Read the research: S&P Global's Top 10 Sustainability Trends to Watch in 2026 | S&P Global  Copyright ©2026 by S&P Global   DISCLAIMER       By accessing this Podcast, I acknowledge that S&P GLOBAL makes no warranty, guarantee, or representation as to the accuracy or sufficiency of the information featured in this Podcast.
