As an experienced CISO, Dr. Adrian understands the difficulties of a cyber security career. With this in mind, he shares the day-to-day obstacles of the profession and what aspiring CISOs can expect from the job. Listen to the episode to learn more about the pressures CISOs face, the psychology of insider threats, and how to work past life's challenges. Listen to Steve and Dr. Adrian discuss how to get through difficult life hurdles and manage cyber threats: The “Superhero” CISO (1:44) Host Steve Moore reintroduces our guest today, Dr. Adrian Mayers. They acknowledge the stress and pressure a CISO may feel to play a superhero role, stopping every cyber threat. Although no one can prevent every obstacle, Dr. Adrian insists that every CISO must consistently attempt with high motivation to stop every threat that comes your way. Taking A “Bad” Job (5:26) Steve presses Dr. Adrian if someone should ever take a “bad” CISO job.  Dr. Adrian brings up that every CISO needs their eyes wide open with every gig, but that early in your career, you may have to take less than ideal positions in exchange for experience.  The Bad Day Factor (9:53) When asked about his worst day on the job, Dr. Adrian reflects that there is always something you can learn from your most challenging moments.  Insider Threat Management (13:01) Dr. Adrian shares that his affinity for investigating insider threats first developed from his love of video games. After extensive research on counter-intelligence, he understands that specific triggers in people's lives can lead to unattended consequences or malicious intent.  Evaluating The Insider Threat (15:35) Steve questions why an insider psychologically may want to compromise the security of their company.  Dr. Adrian states that every insider who goes against their company has one thing in common: a desire to deviate from the norm. And determining that motivation helps the CISO manage their investigation.  How Far Should The Staff Go (20:46) Dr. Adrian states that your team needs to understand exactly what their doing before talking to vendors or others. By discussing with your team the boundaries for their current investigation, you can gain additional insights that will put everyone on the right path. The “Why” For Education (22:31) Years ago, Dr. Adrian decided to get a doctorate in business administration specializing in international security. He then decided to get additional certificates in the security field. Ultimately, his desire for further education came from his immense curiosity but also was prompted by the grief of losing his daughter. Defining Quality Intel Programs (28:49) “Threat intelligence is full-spectrum intelligence,” according to Dr. Adrian. By leveraging the information from your intel program and applying context around it, every security team should be able to determine the motivation for the threat and paint a more holistic picture.  Surprising Information (32:00) Steve presses Dr. Adrian on the most surprising things he's learned from his background in threat management. Dr. Adrian reflects on the amount of data vacuumed from our adversaries. Another shocking piece is the amount of data our allies gather on the U.S. Though, of course, the reasoning for gathering that information varies. Ph.D. Proud (35:54) Dr. Adrian always puts his doctorate before his name for several reasons.  As a Black man from Canada living and working in the United States, he realized he would experience more hurdles than others in the field. Since fifth grade, Dr. Adrian Mayers has had a passion for computers. Now a CISO, he shares the role computers play in a security professional's day-to-day life. Listen to the episode to hear more about Dr. Adrian's advice for aspiring CISOs, the relationship between human behavior and tech, and his thoughts on the transition to automation. Listen to Steve and Dr. Adrian discuss how to find the right security team and solve human problems with technology: Meet Dr. Adrian (1:56) Host Steve Moore introduces our guest today, Dr. Adrian Mayers, who shares a bit about his life before Premera Blue Cross and his childhood interest in computers. The Power of Story-telling (5:43) Dr. Adrian explains his love of narrative-based gaming and how escapism provides relief after difficult work days. He also shares how video games give him strategy ideas he uses in his current role. The Character of a Leader (8:19) When asked about his characterization of a leader, Dr. Adrian reflects that a leader is someone who has integrity at their core. He expresses the importance of evaluating who you are and ensuring you bring your values into a leadership position. Advice For Aspiring CISOS (9:25) Steve asks Dr. Adrian his advice for those interviewing for CISO positions.  Dr. Adrian shares why you should communicate how security plays into your day-to-day life and ask questions about the team's previous history when tackling security problems. The main thing is to be comfortable with who's in front of you because you would build relationships with this team if offered the position. Looking For Your Next Position (16:08) There are different considerations if you're courting a government position than evaluating a job at a start-up company. Ultimately, it depends on each security company's process and context when navigating the interview stages. Solving Human Problems (18:25) Steve asks Dr. Adrian about his thoughts on tech solving human problems.  Dr. Adrian reminds the listeners not to get so wrapped in the technology that they forget what they're trying to do: tackle human problems. Ultimately, tech helps CISOs do this work, but focusing on the human elements will keep you centered and effective. Not Trusting The Tech (21:44) Dr. Adrian recognizes that many security professionals wonder if they can trust data platforms versus the insights of actual human beings.  He also understands that there is a difference between installing programs and implementing them. Overall, if you take the time to understand the tools, you can see how tech makes effective security decisions regarding human problems. Defending Automation (26:27) Steve presses Dr. Adrian on ways to convince security professionals to automate low-level tasks.  Dr. Adrian assures the listeners that these changes are being made daily in the security field. By clarifying to security professionals that they will not be replaced by automation but will have more space for high-level problem-solving, the transition will be easier for teams to accept. The Definition of Good (29:49) Dr. Adrian explains that the definition of “good” for security programs stems from people. If security professionals have a sense of purpose to show up every day and learn how to use the tech, then that is the measure of a quality program. If you build a dialogue with your security team and understand their concerns and issues, they will have a sense of ease when moving in a technological direction. Final Advice (38:45) To Dr. Adrian, the most vital thing when interviewing for a CISO position is always to do your homework. If you are clear about your experience and how you can benefit the security team, you will be able to communicate why you are suitable...

We heard it again a few weeks back.  Big Tech is pulling back from healthcare, it's just too hard for them.  Do we really believe that?What if I asked a healthcare company to build out a next day delivery service for just about everything that exists on the planet, think they would be successful?  Are we really smarter than big tech because we are an entrenched incumbent in one of the most complex labyrinths of payment models, special interests, and regulatory requirements.  Probably not, we just have a map of the mine field.  And so they persist.  FTANow, Amazon is looking to bring both telehealth and in-person care to Philadelphia, Chicago, Dallas and Boston in 2021, people familiar with the deal told Insider.Additionally, the company plans to bring in-person Amazon Care to 16 cities in 2022.In July, Insider reported that Amazon Care approached several big health insurers in an effort to expand coverage of its services. The healthcare venture reportedly talked to Aetna, Premera Blue Cross and Blue Cross Blue Shield of Massachusetts.Securing health insurance coverage or joining an insurer's network would be a big step for Amazon Care, as it would allow the company to expand services to more companies and patients. We're excited to bring you a closer look at one of our top stories and other announcements every Tuesday, Thursday, and Saturday.    Today - The city of East Wenatchee is in good financial shape, according to its latest financial report. According to the presentation to the City Council earlier this week, the city's cash balances for all funds at the end of June were at $11.2 million.   Before we jump in, we're excited to let you know that Wenatchee Wine & Food Fest is back this year on August 28th at the Town Toyota Center! We'll see you there!   Now our feature story...   The city of East Wenatchee is in good financial shape, according to its latest financial report.   The report, which compares revenue and expenditures from June 2021 to June 2020, paints a positive picture of the city's finances.   East Wenatchee Finance Director Josh DeLay said that they're in a great financial position. It'll allow Wenatchee to continue to upgrade and modernize city infrastructure.    DeLay noted that people can expect to see increases in expenditures in the next few years for these upgrades, but these are all planned and that's the reason why cities build up their fund balances.   During a presentation of the financial report to the City Council on Tuesday night, DeLay said the city's cash balances for all funds at the end of June were at $11.2 million.   He explained that he doesn't think the city has ever been over $10 million.    A lot of that increase is thanks to $1.5 million in federal American Rescue Plan funds, but tax revenues have also contributed.   So in all, DeLay had a positive outlook on tax revenues during his City Council presentation.   He said sales tax is up 24.1% from last year, which is incredible. The numbers are just rolling in.   Gambling taxes are also up - 104% from last year.    Revenue from operating funds is up 59% compared to June 2020 and non-operation funds increased by 38%.    The revenue increase is more than just recovery from the pandemic. Operating funds revenue in June 2020 also increased from the previous year. Non-operating funds revenue did too - it rose by a whopping 722.42%.   DeLay said the increases are a combination of what appears to be a recovery from the pandemic, federal dollars received for the American Rescue Plan and a limited tax general obligation bond the city took out for capital purchases and improvements.   To read more on this story visit us at wenatcheeworld.com   Also,    Local artist Dan McConnell will have an exhibit displayed in the Pybus Market Board Room through August. So check it out!   The show, which is called Images in Grids, pulls together a variety of topics, dating back to watercolor paintings that McConnell painted in the 1970s. The show's name references McConnell's process when he was fresh out of college and would paint with watercolors and draw using an underlayment of gridded pencil drawings. They are Grateful | They are Confluence Health.  Learn more by visiting them at ConfluenceHealth.com   Next,    Over 36,000 patients with Premera Blue Cross health insurance will be considered out-of-network with Confluence Health in February of 2022… that is unless the two sides settle a price dispute before then.   Confluence Health began negotiations back in April by informing Premera that they would not renew their contract. Why? Because of disagreements about Premera's pay rate.   Dr. Peter Rutherford, Confluence Health CEO explained that Health insurance contracts typically last between two to three years. And after months of negotiations, there was no real progress.   But David Condon, Premera vice president of Eastern Washington, doesn't believe that negotiations are at an impasse.   He said that they still have quite a ways on the runway, and they really do believe that they can come to an agreement. They want Confluence to come back to the table - it's in the best interest of members and for the community.   Still, Confluence Health says the issue is that Premera refuses to a pay rate consistent with other commercial insurance payers in the market.   For now, Confluence Health is not allowed to discuss any details of the contract, but Premera is the outlier among the many commercial insurers Confluence Health works with.   What we do know is that in 2020, Premera paid Confluence Health $167 million in claims, up from $160 million in 2019. The increase is attributed to rising hospital costs.   And if Confluence Health does not renew the contract, more than 36,000 patients across North Central Washington will be “out-of-network” which could mean higher deductibles and other fees for patients.   These Premera-affiliated health insurance providers will also be “out-of-network” or affected in some way starting in February of 2022…. Suzanne Delbanco chats with Rick Abbott, VP of Product and Market Solutions at Premera Blue Cross. Premera Blue Cross is a health plan in the Pacific Northwest, serving about 2.2 million members with customers ranging from large tech companies to family-owned grocery stores. Suzanne and Rick discuss if narrow networks, also known as high-performance networks, represent a viable way to lower prices in the employer-sponsored health insurance market. Historically, employers have demanded broad access PPO networks that include the vast majority of providers and hospitals in their region. This trend has somewhat impeded health plans from using their volume to negotiate steeper discounts from providers. Rick describes how creating narrow networks based on provider quality provides a real opportunity to lower costs by both reducing wasteful spending on unnecessary or harmful care and by negotiating discounts from higher-quality providers in exchange for higher volumes of patients. Suzanne and Rick also discuss the opportunities and obstacles for employers interested in pursuing alternatives to the incumbent health plans, like alternative third party administrators or group purchasing initiatives. Rick points to Premera’s 85-year history as an important value-add for customers, allowing the health plan to implement strategic initiatives at scale. For instance, Premera Blue Cross recently launched a “virtual-first” health plan that allows members to designate a virtual network of providers as their primary care providers.

In this episode I talk to Torben Nielsen about creating new products and systems in health insurance. We touch on the tension between insurer's well-founded risk aversion and trying new things, the process of insurance companies working with startups, and how to even know if things are working. Torben runs programs at Premera Blue Cross with both internal teams and external startups to build new products and systems. Premera is one of the largest health insurers in Alaska and the northwest US, so even small changes can impact many people. Torben spent many years working in healthcare and built his tech chops at Xerox and Lego. Much to my chagrin, we spent zero time talking about the latter because of time constraints. Dr. Hanscom deals with chronic pain patients on a daily basis year after year. After seeing new data that showed patients were worse after surgery Dr. Hanscom decided to look deeper into the issue of pain.  He is no stranger to pain because he too had a horrible pain syndrome with no answers that almost made him take his own life.  He saw first hand how living with pain affected his family and marriage.  He now has hundreds and even thousands of paitents healing their pain by following his amazing work!   Dr. Hanscom believes that each human being has a deep drive to thrive and give back. This force is frequently compromised by chronic pain, which has been documented to have a similar impact on your quality of life as terminal cancer. Life becomes one of survival. Through his own personal ordeal, he discovered that chronic pain is curable by systematically using established medical practices in a self-directed manner. The key is to connect to your own capacity to heal.   Dr. Hanscom is an orthopedic complex spinal deformity surgeon who now understands that most chronic pain from any source is solvable – usually without surgery. He is the creator of the DOC (Direct your Own Care) program, which is the subject of his book, Back in Control: A Surgeon's Roadmap out of Chronic Pain. He also publishes a weekly blog and maintains a website, www.backincontrol.com, to provide resources for chronic pain patients and their families. Dr. Hanscom distinguishes himself from many spine surgeons in that he strongly encourages non-surgical solutions for his patients. Working from his practice at the Swedish Medical Center in Seattle, Washington, Dr. Hanscom witnessed scores of patients recover, usually without surgery. Through these experiences and his own personal victory over a severe chronic condition, he came to understand that chronic pain, rather than being a structural problem, was a neurological condition; and that most chronic pain, whatever the cause, duration, or degree, is solvable.   As patients apply Dr. Hanscom's DOC program (Direct your Own Care), in which they choose and perform activities that have been documented to be effective by extensive medical research, the results are often miraculous. Not only have hundreds of people become pain-free using DOC, many of Dr. Hanscom's surgical patients have opted out of surgery after trying the program for a few weeks. The key to alleviating chronic pain, Dr. Hanscom discovered, is the patient's control of his or her own treatment plan. He didn't always believe this way. He spent the first eight years of his practice willingly performing spinal procedures to cure his patients' discomfort–until research data revealed that the overall success rate for surgery performed for low back pain was less than 30%.   Meanwhile, he was struggling with chronic pain and was suffering from an additional 15 symptoms caused by his stressed nervous system. They included unrelenting anxiety and tinnitus that nearly put him over the edge. He inadvertently discovered an effective (non-surgical) tool that began his path to healing over a mere six months. The experience opened his eyes to the fact that the source of most people's chronic pain was in the nervous system, not the skeletal system.   Determined to raise the odds for successful spine surgeries, Dr. Hanscom began teaching surgical residents and fellows; partnered with his hospital to establish a database that identifies patients who are at a high risk for negative surgical outcomes; founded the Puget Sound Spine Society, of which he is current president; and, as orthopedic consultant to Premera (Blue Cross of Washington), helped establish best practices for orthopedic surgery.   Today Dr. Hanscom's mission is to re-introduce true healing into medicine. He feels that doctors should be given the time to understand their patients and the life situations surrounding their medical problems. Currently, we are prescribing simplistic solutions for a complex problem. Bob also teaches in the Harvard Negotiation Institute and the Harvard Program on Negotiation’s Senior Executive Education seminars. As a professional facilitator and conflict resolution consultant, Bob works with individual and corporate clients across a spectrum of industries. He specializes in dispute systems design and in assisting individuals and groups seeking to manage conflicts in highly sensitive, emotional, or difficult situations. Getting people to think about dying is not the first health behavior that comes to mind when employing incentives. However, incentives can be used anywhere in the healthcare continuum --- including end of life --- to influence behaviors, notes Neal Sofian, MSPH, director of member engagement at Premera Blue Cross. Sofian describes the barriers individuals face at this time in their lives and how the use of incentives can result in exactly the kind of care these patients want and need.
