What if the best decision is to not decide at all? We waste valuable mental energy overthinking simple choices, especially when the outcomes are nearly identical. That kind of cognitive drain reduces our capacity to think clearly when decisions actually matter. In this episode, we explore how to reduce cognitive load, identify low-risk choices that can be automated or ignored, and recognize when deliberation is just noise. Finally, we break down how framing, values, and the right question can make even complex decisions frictionless.Guest bio: Dan Dworkis MD, PhD is an emergency physician who is a clinical professor of emergency medicine at USC Keck School of Medicine. Our immune system operates like a finely tuned symphony, yet many of us find ourselves out of harmony, vulnerable to persistent infections, autoimmune conditions, and chronic disease. Rather than merely suppressing symptoms, a Functional Medicine approach seeks to identify and address the underlying disruptions driving immune imbalance. Central to this dysfunction is compromised gut health, which undermines immune regulation, while mitochondrial impairment and chronic inflammation further erode the body's capacity for resilience and repair. By restoring balance at the root level, we can cultivate a more robust and adaptive immune system. In this episode, I discuss, along with Dr. Elroy Vojdani and Dr. Leonard Calabrese, how cleaning up our diets, improving gut health, removing toxins, and decreasing stress can do wonders for our immune systems. Dr. Elroy Vojdani is a pioneer in the field of functional medicine and research and is the founder of Regenera Medical, a concierge functional medicine practice in Los Angeles, California. He graduated from USC Keck School of Medicine, is a certified Institute for Functional Medicine Practitioner. Dr. Vojdani has conducted medical scientific research for decades with more than 25 publications in multiple peer-reviewed journals. He is also world-renowned for his research and development of state-of-the-art lab testing in the field of immunology. He recently authored a book entitled “When Food Bites Back” which discusses the role of food immune reactions in the development of autoimmune disease. Dr. Leonard Calabrese, is an expert in immunology and rheumatology. In fact, he is a Professor of Medicine at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University and Vice Chair of the Department of Rheumatic and Immunologic Diseases. Dr. Calabrese is the director of the RJ Fasenmyer Center for Clinical Immunology at the Cleveland Clinic and holds joint appointments in the Department of Infectious Diseases and the Wellness Institute. Dr. Calabrese has made significant contributions to science in the fields of chronic viral infections and autoimmunity and vascular inflammatory diseases of the brain. He has received numerous awards and honors for his contributions to the advancements of immunology and wellness. This episode is brought to you by BIOptimizers. On a special edition of the BioCentury This Week podcast, BioCentury's editors deliver their takeaways from the debut Grand Rounds conference, which focused on whether biotech can write a more successful playbook for translating from target to product. Weaving together takeaways from the panels, fireside chat and keynote at the conference, the editors assess the tensions between generalizability and fit-for-purpose models, between having control and capturing complexity, and, in human data, between scale and robustness/reliability, particularly for longitudinal readouts. The editors also discuss BioCentury's Q&A with USC Keck School's Patrick Lyden, who explained how high-quality, reproducible preclinical science can be feasible.View full story: https://www.biocentury.com/article/65363100:00 - Introduction02:56 - Generalizability vs. Fit for Purpose10:03 - Control vs. Complexity17:57 - Scale vs. Robustness in Human Data26:25 - Hypothesis-Driven vs. Unbiased Research28:50 - Grand Rounds 2025To submit a question to BioCentury's editors, email the BioCentury This Week team at podcasts@biocentury.com.Reach us by sending a text

Shades of Life: Sublime Joy is in Living by Dr. Mohammad Akmal and Vasundhara RaghavanFifteen-year-old Aditya was like any other teenager—busy in school, horsing around with a brother at home, pushing things around in the kitchen to try out daring recipes of his own, cheering lustily at games of cricket and, generally, being boisterous. His parents had begun to wonder whether he'd ever take life seriously. Would he toughen up to withstand the pressures of the outside world? In the winter of 1996, life suddenly got more serious than anyone had wanted. One day, his mother, Vasundhara, took him to see a doctor for a headache that refused to go away despite over-the-counter pain relievers. The ensuing prognosis revealed that Aditya's kidneys were headed towards complete failure. In this heart-wrenching account, Vasundhara Ramanujan shares more than that worst nightmare of all—a child being afflicted with a life-threatening condition—she relates a story of instinctive courage. She narrates how her family, instead of letting their circumstances devastate them, summoned every emotional and psychological resource to provide a young boy, and themselves, with hope. In their quest for the best cures available, they were guided by many well-wishers, one of them being Dr Mohammad Akmal, who lent his medical expertise to authenticate the treatments outlined in the book. The ultimate purpose of Shades of Life is to prepare others to meet such exigencies of renal failure, and to help them find a life-saving solution.Vasundhara Raghavan retired as general secretary of Mumbai-based Media Research Users Council. She and her husband Srinivas live in Dubai. They have two sons. Pablo Picasso once said “Every child is an artist; the problem is staying an artist when you grow up.” On this episode of the N&H Podcast my special guest Dominic Quagliozzi talks about his experience creating art that started from childhood while he was hospitalized with Cystic Fibrosis. We also talk about the importance Art in healthcare where Dominic will share his experience of creating art from illness which has guided him into a career as an artist. Through various media, with a focus on drawing, painting and performance, Dominic Quagliozzi's work merges his lived experience as a person with chronic illness and disability into art. Using medical materials common to hospitals, clinics and home healthcare; hospital gowns, IV tubing, clinic table tissue paper, to name a few, Quagliozzi hints at the presence of a body past, present and future within health care systems and sick-well-sick-well cycles. By repurposing and re-coding medical materials as art making materials, he explores the emotional and psychological space in those moments of vulnerability, anxiety, fragility and resilience. Parallel to his art practice, Dominic uses art as a method of teaching for medical students and health workers. Quagliozzi received an MFA in Studio Arts from Cal State University, Los Angeles and a BA in Sociology from Providence College. His work is in the permanent collection at the Rhode Island School of Design Museum and a collaborative work in the permanent collection at Museum of Latin American Art, in Long Beach, CA. The idea of staying fit, young and healthy for longer, is something many of us would love to achieve, but just how possible is it? Dr Robert Lufkin, who has a particular research interest in the science of longevity discusses the reality. He says the aim is not to live forever, or live longer when we are frail and ill, but to lengthen our healthy lifespan. When we age, the odds of us getting a range of degenerative diseases or life threatening illnesses increase, so the science of anti-aging is really all about discovering why this happens and what we can do to slow or stop these pathways. Lufkin reveals the role that he believes lifestyle, diet and exercise can play. And goes on to review drugs such as Rapamyzin that some people, including him, are now taking in the hope they can keep their bodies healthier for longer. So exactly what is the evidence that we can stave off the perils of aging? Dr Robert Lufkin is currently Adjunct Clinical Professor of Radiology at the USC Keck School of Medicine. He is also Chief of Metabolic Imaging at a large medical network in southern California. Previously, Rob has been president of the society of magnetic resonance imaging, president of the American society of head and neck radiology. As the burden of cardiovascular disease increases in the United States, the importance of enhanced screening tools, early risk prediction, and prevention strategies grows. Novel risk scoring methods, including polygenic risk scores (PRS), may help identify patients that benefit from early intervention and risk modification. In this episode, we discuss how a PRS is calculated, how to incorporate a PRS into clinical practice, and current barriers to the equitable implementation of risk scores. In terms of frontiers in clinical genetics we also discuss the burgeoning field of pharmacogenetics and how pharmacogenetics may be used to identify responders and non-responders to certain therapies. Join CardioNerds Dr. Jessie Holtzman (CardioNerds Academy Chief and Chief Resident and soon FIT at UCSF), Dr. Alaa Diab (CardioNerds Academy Fellow and Medicine Resident at GBMC), and student doctor Hirsh Elhence (CardioNerds Academy Intern and medical student at USC Keck School of Medicine) as they discuss frontiers in clinical genetics with Dr. Pradeep Natarajan (Director of Preventive Cardiology, Massachusetts General Hospital). Audio editing by CardioNerds Academy Intern, student doctor Akiva Rosenzveig. Pearls - Frontiers in Clinical Genetics in Cardiovascular Prevention For common diseases like coronary artery disease, rare mutations may confer a several-fold increased risk of disease – for instance, in familial hypercholesterolemia, a single rare mutation may confer as much as a three-fold increase in risk of coronary artery disease. However, for most common diseases, the overall cumulative impact of several common genetic variants may be greater than that of a monogenetic trait. Family history is a particularly coarse predictor of CV risk, highlighting the need for polygenic risk scores. In particular, younger patients with borderline cardiovascular risk may benefit from the use of a polygenic risk score in the determination of their overall cardiovascular risk profile. A polygenic risk score (PRS) is a weighted sum of several risk-conferring alleles. The weight assigned to an allele is determined by the strength of the association between the allele and CV disease, as determined by genome-wide association studies (GWAS). The data used for genome-wide associated studies in cardiovascular disease have historically included populations primarily of European ancestry. However, more data is being collected from diverse patient cohorts to increase the external validity and broader applicability of such studies. Pharmacogenetic polygenic risk scores may be used to predict drug efficacy and toxicity, as well as to identify biologically plausible drug targets for clinical trial design. Show notes - Frontiers in Clinical Genetics in Cardiovascular Prevention What is a polygenic risk score (PRS)? Monogenic conditions are those in which a variant in a single gene causes a pathological phenotype. For example, familial hypercholesterolemia is often the result of a mutated allele in the LDL receptor gene. In contrast, polygenic risk suggests that there are variants in multiple genes that all confer risk independently, each with a small individual effect size. By aggregating many variants,
"Your symptoms make sense." This is what Dr. Omid Naim tells his patients, many of whom have struggled with chronic mental or physical health issues for years. He also tells them that he thinks they can "get fully better." Dr. Naim's approach to psychiatry turns the medical model inside out by viewing illness through the lens of ecology and empowerment. He takes into consideration the contextual factors that contribute to illness such as childhood trauma, disconnection from community, and lack of meaning, while empowering and inspiring patients to achieve greater health by allowing their nervous systems to do what it is designed for: healing. In this episode, Dr. Naim and I discuss how he works with patients to reorganize their internal stories in order to initiate a journey of holistic healing. Dr. Omid Naim is an integrative psychiatrist and Founder of HOPE Psychiatry. He is also co-founder of La Maida Project, a nonprofit organization dedicated to rewriting the story of what it means to be healthy through an understanding of trauma and stress via an ecological lens, and to rediscovering our human nature and our innate capacity to heal. Dr. Naim completed medical school at the USC Keck School of Medicine and went on to complete residency at UCSF and a fellowship at the Andrew Weil Center for Integrative Medicine at the University of Arizona. In this episode we cover: Why Dr. Naim's goal is not just managing symptoms, but for patients to "get completely better." The underlying role of early childhood trauma in illness. The role of social factors (i.e., "social determinants of health) in disease and the importance of community. The sickness model versus the empowerment model. Why the "chemical imbalance" theory of mental illness lacks support. The role of food, lifestyle, and natural remedies in supporting the body's natural healing capacity. Why mental health symptoms go down in times of collective crisis. The ecological approach to mental health and why context matters, from family, to community, to society. Why health and resilience are founded on struggle and crisis. The factors of "salutogenesis" (health promotion) versus pathogenesis. Working with our internal stories about illness. Working with trauma and patterns of avoiding emotion and unresolved feelings. The role of meaning and purpose on both an individual and collective level. The hero's journey of making meaning about of our health journey. Dr. Bendikson is an OB/GYN and a nationally recognized leader in the fertility field. She has gone through fertility testing herself and now counsels clients about family planning decisions like egg freezing and other fertility treatment options. This conversation demystifies the scary and overwhelming aspects of egg freezing and fertility testing. We talk about all the things - fertility age ranges, the quality and number of your eggs, and the technology behind testing. Join us as we talk about how we can become more educated and proactive with our fertility and family planning! IN THIS EPISODE: [03:56] About Dr. Bendikson and her passion for fertility awareness. [07:11] The intersection of fertility and culture and navigating difficult conversations around fertility testing. [13:48] The egg freezing process explained and answers to common questions. [27:11] Technological improvements in egg freezing and the difference of embryo freezing. [33:00] Clarifying what fertility testing means for family planning. [39:29] Dr. Bendikson's own experience with fertility testing and making decisions based on outliers. KEY TAKEAWAYS: Women in their twenties are encouraged to start thinking about their long-term family planning goals. You don't have to make any decisions but it's helpful to consider fertility testing as a way to learn and understand what your options are as you think about having kids in the future. In the last 15 years, there have been advancements that have made the egg freezing experience easier. Improvements to medications have decreased the length of time that women experience being uncomfortable and it's also limited the amount of days of taking shots. You can't change the number or quality of your eggs. However, the number thing you can do is choose to freeze your eggs earlier rather than later. If you freeze your eggs in your late 20s and then wait to get pregnant in your late 30s, your chances of getting pregnant are that of your late 20 year old self - so that's really important to remember.
