Wisdom. There's real wisdom, godly wisdom. And then there's the false wisdom that many in this world hold out to be true wisdom. God yearns for you to be filled with real wisdom. Wisdom from above.   A Different Kind of Wisdom Let's be honest. Sometimes we do the dumbest things. We don't mean to, we just do. And there comes a point where you think to yourself: “Why am I making this same mistake over and over again?” I'm guessing you've been there. For some people, it's constant worry. Sure you've heard the bit where Jesus says “Who, by worrying, can add a single hour to their lives?” (Matt 6:27). Maybe you've heard it a hundred times before, but you still can't stop worrying. Perhaps you're someone who carries anger and bitterness around in your heart. Sure, you know that the Bible says - “don't let the sun go down on your anger” (Eph 4:26) … but that anger keeps bubbling away. Or maybe your someone who is constantly trying to impress other people. You know it's a pride thing. You know that pride comes before the fall (Prov 16:18) but, well, you know … There are so many things that hold us back in this life. There are so many things that we hang on to for dear life, that are in fact ruining our lives. We all have our Achilles heel. We all have blind spots. We all have circumstances in our lives where “doing it my way” – as much as that may be the way we've always done it – just doesn't work. If I asked you, in those circumstances, “Do you want to be wise?” I'm sure your answer would be, “Absolutely!”. But then, if I asked you, “So, are you prepared to make the changes you need to make to get that wisdom; to live out that wisdom?” Well, most of us, at that point, would hesitate; equivocate. “Change? Really? Change? Well, I'm not so sure about that.” If we're going to talk about wisdom then, we have to start at the beginning. The fear of the Lord is the beginning of knowledge; fools despise wisdom and instruction. (Proverbs 1:7) Now maybe that's a verse that you've heard many times before. But let's just get a grip on it for a minute. The fear of the Lord means an awe, a respect; a realisation of the terrible outcomes of the judgements of God. My father loved be and I loved him. But let me tell you, when I was a lad, I knew that if I did something wrong, there was going to be a reckoning with my dad. And if it was bad enough, I knew that it would involve punishment. We just toddle along through life as though anything goes. If it feels good do it. I wonder though, if we thought a bit more about the terrible judgement of God, if we questioned our thoughts, our motives, our actions, if we asked ourselves “What would God think about this, really?” … I wonder if that wouldn't open our hearts to God's wisdom. A few years ago, I recorded a radio program about the fear of God. Now, I remember thinking to myself “No one wants to hear about the fear of God these days.” But I just felt God nudging me towards that message. So, I did it. I wrote it, I recorded it. And a few months later, I received an email from a man in the Democratic Republic of the Congo. This is what he wrote: I was at a peace conference in Goma and in my hotel room I turned on the radio. I heard a program about the fear of God. And I realised that with no fear of God in the hearts of people there will be no peace! No peace at all! I don't know if this was a revelation from God … but my heart was beating, coming far away, God was speaking to me. I have been on the battle field for 13 years now and I don't know how many people have died because of my gun, but what I know is that I decided to lay down my arms in that hotel room. I've never forgotten that email. He gave his life to Christ and ended up fleeing to another country in fear of his life. The bit of his email that keeps ringing out for me is where he says, “without the fear of God there can be no peace, no peace at all!” You know what, he's absolutely right. When we just carry on as if anything goes, when we live our lives for ourselves without any respect, without any awe, without a right sense of the fear of God in our hearts, we are going to make a hash of it. A complete mess of our lives and a complete mess of the lives of the people around us. The people we hurt through what we say and do. Through the expression of anger and disapproval on our face. Through our failure to love them, the way they need to be loved. And the crazy thing about this 'fear of God' thing, is that once we yield our lives to God, once we say “Lord I'm sick of making mistakes. My way doesn't work. I want your way. I want your wisdom. I want your help. Once we do that, all the fear in fact goes away. There is no fear in love, but perfect love casts out fear; for fear has to do with punishment, and whoever fears has not reached perfection in love. (1 John 4:18) Yes there is a terrible judgement that awaits those who rebel against God. A terrible But when we come to a turning point, as Safari Murinzi came to a turning point in that hotel room, the fear goes away. When we choose to live in God's love, that perfect love casts out all fear. God wants to impart to you, His wisdom. The wisdom of the ages. My dad had a lot of wisdom. He wasn't perfect of course, but he took the time and made the effort to impart much of that wisdom into my life. And to a great extent that wisdom has made me the man I am today. Imagine though, God who is perfect, God who knows all things for all time, God who loves you beyond measure, wanting to impart His wisdom into your life. Surely, we have to stand back from that, you and I, and think to ourselves, “WOW! That's amazing!!” And it is. Totally amazing. Just think for a moment of the things that you have going on in your life right at this moment that are causing you grief. The things that are complicated, that you just can't sort out and fix. Imagine now, that God is prepared to speak His word of wisdom into that one situation that's driving you to despair. Would you want to hear that wisdom? Would you want to receive that wisdom? Probably out of frustration, out of a desire just to get that thing sorted, the answer is a big, fat “Yes!” But then let's say you open your Bible and you read this verse: The fear of the Lord is hatred of evil. Pride and arrogance and the way of evil and perverted speech I hate. (Proverbs 8:13) And you realise that actually it's your pride and your arrogance, the way you've been talking to other people, that's the biggest factor in this dilemma in which you find yourself. Now let me ask you again. Do you want to receive that wisdom? It's not easy is it. There's this thing inside each one of us that wants to blame everyone and everything else for our problems, except our selves. And yet so often, the getting of wisdom involves owning up. It involves being honest with ourselves. It involves making the changes that we need to make in order to bring peace. And that's where the fear of the Lord comes in. It's this heart attitude that says, “You know God is right. The alternative here is continuing in this mess. The alternative here is facing God's judgement for this one day.” Receiving the wisdom of God into our lives isn't always easy. In fact a lot of the time, it's incredibly tough. It's something that we don't want to do, because we want to wallow in our immaturity, in our pride, in our … let me call it for what it is … sin. But your Father in Heaven has, sitting ready and waiting for you, the wisdom of the ages. A wisdom that will utterly transform your life. A wisdom that, day by day, will set you free to be all He made you to be.   Yearning for Wisdom Wisdom is something that I think, we all desire. I mean, I don't know anyone on this planet, who, if asked, wouldn't want to be wise. But I wonder just how high it is up our priority list of wants and desires. We long after different things in our lives. Sometimes we yearn for love, acceptance and companionship. Other times it might by something more basic – like enough food to eat. There are plenty of people on this earth who simply don't have the basics – food, clean water, shelter. But in affluent places, we yearn, frankly, for silly things. A new pet dog – nice, but not essential. The latest hair style, the latest fashion accessory, a new dress in this season's colours, a new car – shiny and bright, just like the one on the TV add. Right now, given what's going on in your life, what do you yearn for? What's the object of longing that's dominating your heart and mind right at this moment. Does godly wisdom even feature in, say, your top three desires? If you're like most people, the answer is: probably not. When Solomon, the son of King David, was still a young man, he became the king of Israel. What a huge responsibility. Ruling a country, that young, that immature, with so little experience. He knew it was way beyond him, just like many of the things that you and I confront in life are beyond us. One night he had a dream. And in that dream, God said to him: Ask what I should give you. (1 Kings 3:5) I wonder … I wonder if God appeared to you and me in a dream one night and said that to us, what we would ask for. Well, here's what Solomon asked for: Give your servant therefore an understanding mind to govern your people, able to discern between good and evil; for who can govern this your great people? (1 Kings 3:9) And then, we're told in the very next verse that: It pleased the Lord that Solomon had asked this. God said to him, “Because you have asked this, and have not asked for yourself long life or riches, or for the life of your enemies, but have asked for yourself understanding to discern what is right, I now do according to your word. Indeed I give you a wise and discerning mind; no one like you has been before you and no one like you shall arise after you. I give you also what you have not asked, both riches and honour all your life; no other king shall compare with you. (1 Kings 3:10-13) So asking for wisdom, it seems, asking unselfishly for the right thing, is an act that pleases God and releases great blessing into our lives. Just imagine what your life would look like if you yearned for the wisdom of God. Imagine what your life would look like if, like King Solomon, you petitioned God for the wisdom to do the things that He has set before you, the things that in your heart of hearts, you know are way before you. It would please the Lord no end. It would bring you rewards that go way beyond anything you could ever have imagined. When we ask God for wisdom, we're asking Him for something that's not so much about us, as it is about serving Him and other people. Think about the things that you need wisdom for in your life right at the moment. What are the things that are going on in your relationships that are beyond you? What decisions do you need to make that are going to have an impact on your life and perhaps the lives of others, that you're just not sure about? The young and inexperienced King Solomon wanted wisdom to govern God's people. So when he asked God for an understanding and discerning mind, instead of wealth and honour as king, the longing in his heart was for his people, not for himself. He could have asked for great riches and honour, but instead He asked for wisdom. You know what I think. I think that all too often we ask God for things that are about us. Our needs, our Now, just as Jesus taught us to pray, “Give us this day our daily bread” there's nothing wrong with asking God to meet our needs. But if that's all we ask of Him, then it's a pretty selfish little prayer life that we're having isn't it? It's a pretty narrow life that we end up leading. Imagine if you could see your problems through God's eyes. Imagine if you could see your circumstances, the decisions that you need to make, through God's eyes. Imagine if you could stand on heaven's balcony and see the view from there. I think that's what wisdom is. God's wisdom gives us a completely different perspective. God's wisdom helps us see the bigger picture, rather than our own little narrow circumstances. A thousand or so years after Solomon, the Apostle Paul found himself in a Roman dungeon, chained to a guard, on death row. Imagine how easy it would have been for him to have a narrow view of his circumstances. But instead, he was able to write: I can do all things through him who strengthens me. (Philippians 4:13) There's a man filled with the wisdom of God. There's a man who can see his dire circumstances from the vantage point of heaven's balcony. Back to you. Back to your life. Your dilemmas. Your decisions. The things that are before you that you know are way beyond you. The struggles, the hurts, the situation that's confusing you and confounding you at the moment. Your God wants to show you the big picture. Your God wants to put His arm around you and show you the view from heaven's balcony. Now sure, He may not show you the whole picture, but if you'll just ask Him for His wisdom, it will please His Father's heart and He will show you just enough to get you through.   The Getting of Wisdom It's a humbling thing to come to the realisation that you don't have the wisdom, the where-with-all, the IQ, the emotional intelligence – call it whatever you want – to deal with a problem. Pride keeps a lot of people from asking for help. Pride whispers to us “If you ask for help, you'll be showing other people how dumb you are.” And so, many people make bad decisions because they don't seek help from others, when they need it. Without counsel, plans go wrong, but with many advisers they succeed. (Proverbs 15:13) The truth is that none of us has all the answers. When I was a young man, working as a pretty high priced IT consultant, I used to think that I had to show my clients that I had all the answers for everything. That there were no limits to my expertise. After all, look at what I was charging them. How stupid I must have looked, and, let me tell you, that attitude caused me a lot of grief in the end. No, we will need help. We all need the different perspectives that others can bring. We all need counsel from good advisers. And THE best adviser of all, without any shadow of a doubt, is God Himself. Especially, let me say, when we're in the middle of a crisis. Crises happen, often, without warning. They're like earthquakes. People are going about their ordinary, day to day business. Or perhaps they're asleep in bed and without any warning, the earthquake hits, shacking the very foundations of their lives. Reeking devastation. Throwing people into fear and panic. We tend not to make very good decisions under those circumstances. Suffering has a way of completely distorting our perspective. My brothers and sisters, whenever you face trials of any kind, consider it nothing but joy, because you know that the testing of your faith produces endurance; and let endurance have its full effect, so that you may be mature and complete, lacking in nothing. If any of you is lacking in wisdom … (James 1:2-5) Now I want to stop it just there, because that last bit is the bit I want to focus on. If any of you is lacking in wisdom … how easy is it to pretend you have all the answers? How easy is it to go on and make decisions when you're facing trials, when your faith is being tested, when things are hurting – as though you know it all? With all that you have going on in your life at the moment, is there a place where you haven't admitted to yourself, that you don't have all the answers? Because, listen to me here, admitting to yourself that you actually need help, is the essential first step, into God's wisdom. And once we come to that point, once we admit that to ourselves, well the flood gates open. God steps in, in power. In a way that we could scarcely have imagined. I wonder whether, when you go through difficult trials, through times of testing, whether you've ever felt as though God has deserted you. You pray and you pray and you pray, but it feels as though your prayers are just bouncing off the ceiling. You ask God for guidance, for wisdom, for advice – but it seems as though He's left you hear, alone in the wilderness, to your own devices. You realise that you don't have the power or the wisdom to deal with whatever it is, but God is silent, mute it seems … and so know despondency sets in. Ever been there? Sure you have. Perhaps you're even there now. There's some huge problem in your life and you know you need help, but there doesn't appear to be any help on the horizon. Would you like to know what's going on? Here it is. God has taken a step back to test your faith. My brothers and sisters, whenever you face trials of any kind, consider it nothing but joy, because you know that the testing of your faith produces endurance; and let endurance have its full effect, so that you may be mature and complete, lacking in nothing. (James 1:2-4) He does that, because your faith is precious. God knows that by testing your faith, He will grow it stronger. Is it fun, no. But perhaps He's brought us together today, just to remind you of what's going on here in your suffering, in your time of testing. And now … now I want to share a powerful promise with you into the middle of that despondency. Are you ready?! If any of you is lacking in wisdom, ask God, who gives to all generously and ungrudgingly, and it will be given you. But ask in faith, never doubting, for the one who doubts is like a wave of the sea, driven and tossed by the wind; , for the doubter, being double-minded and unstable in every way, must not expect to receive anything from the Lord. (James 1:5-8) WOW! So you've come to the conclusion that you need help. You've realised that you don't have the wisdom that you need. That's a good thing. That's what He wants, the realisation that: “I am lacking in wisdom.” A great step of humility. A step that God will honour. And look again at his Word, His promise is that He will give you His wisdom – generously and ungrudgingly. So stop doubting, stop being tossed to and fro, ditch the doubt, the double-mindedness, and just believe in what God says. If you need His help, if you need His wisdom in the middle of your suffering and the testing of your faith, ask, believe and His wisdom WILL be given to you – generously, and ungrudgingly. Hallelujah!

In this latest episode of the Personal Mastery Training podcast, we delve into the wisdom imparted by Franklin Covey: "You want to climb the ladder of success, but you want to make sure it's against the right wall." This principle is crucial for anyone striving for greatness, and today's discussion explores how to ensure your efforts and aspirations align with your true goals and values. Key Highlights Franklin Covey's Wisdom: It's essential to periodically reflect and ensure that your efforts are directed towards meaningful and aligned goals. Jim Rohn's Goal-Setting Philosophy: Set big goals not just for the end result, but for the personal growth and transformation that achieving those goals necessitates. Personal Reflection: Regular reflection (annually or quarterly) helps ensure that your actions are motivated by genuine passion rather than fleeting motivation. Real-Life Example: After 17 years in business, the host reflects on his journey from aiming to build multiple clinics worldwide to focusing on a single, specialized boutique clinic. Understanding Your True Nature: It's crucial to identify whether you're a business builder or an income producer and align your efforts accordingly. Lessons Learned: The importance of shifting your ladder to the right wall if the current one isn't yielding the desired outcomes, to prevent years of misdirected effort. Passion and Alignment: Ensure that your journey feels aligned and that your pursuit is worth the time, energy, and resources invested. Reflect on where your ladder of success is leaning. Take the time to ensure it's against the right wall, aligned with your true passions and goals. If it doesn't feel right, don't hesitate to shift your ladder. Garreth Wood is a Scottish philanthropist and advocate. He and his wife Nicola founded Kids Operating Room with the vision of ‘A world where every child has equal access to safe surgery.' He believes that generosity requires tenacity; the ability to hang on when letting go appears most attractive. He is inspired by the tenacity of the surgical teams KidsOR supports worldwide. The grit, perseverance, and resilience they show every day, often in impossibly difficult working environments, is an inspiration to him. David Cunningham is passionate about achieving Universal Health Coverage. This includes ensuring every child has access to high-quality healthcare when they need it – no matter where they were born. A strong advocate of the Sustainable Development Goals, David is a global leader in developing charitable programs for children's health. He has provided leadership to healthcare projects in the UK and across the world, impacting the lives of millions of children. David is a highly regarded charity regulation expert. He is a Board member of the UK Fundraising Regulator and a member of the regulator's Standards Committee, setting the rules for all fundraising in the UK. Garreth and David join me on the show to discuss Kids OR's mission and how they help hospitals in developing countries by providing the equipment and facilities they need to provide pediatric surgical services. Garreth and David share their individual journeys into generosity and how they came to work together at Kids OR. The pair also dig into the details of the artwork used in the operating rooms and explain why the bright, bold colors have such a positive impact on doctors, nurses, and, most importantly, their tiny patients.  “If you get the right people, it makes life very easy.” - Garreth Wood “Our goal should be to support countries to get to a place where they don't need our aid.” - David Cunningham “Our artwork is hugely beneficial in terms of the journey to the OR for children.” -  Garreth Wood This Week on The Wow Factor: Why Garreth moved from the restaurant and hospitality industry and became committed to helping children who need surgery in developing countries Some of Garreth's entrepreneurial failures and how they taught him to move forward What David learned from going in at the bottom at a cancer non-profit in Scotland How Garreth and David met and why they decided to work together at Kids OR The personal circumstances that made saying yes to a philanthropic project in Uganda easy for Garreth Godwin, the first baby they operated on in Uganda, and the impact of that moment on the mission and Godwin's family  How they help countries in the developing world to create working operating rooms by being respectful of their grassroots needs Why the artwork on the walls of an operating room is so impactful for both the clinical teams and their patients Garreth's perspective on generosity and the kind of people who are generous with their time or money The responsibilities of wealth transfer and how to foster generosity within that  David Cunningham's Word of Wisdom: Having empathy is vital in leadership — kindness is courageous. Garreth Wood's Words of Wisdom: It's so vital for leaders to share the success with their team and recognize them regularly and publicly. Reacting to why actor Seth Rogen says he and his wife don't want kids, Father Len explains the relationship between making commitments, happiness, love, and having kids. Support Wrestling with God Productions here: https://www.GiveSendGo.com/WWGProductions Highlights, Ideas, and Wisdom “It's not an easy haul having kids. You can't be narcissistic. You have to give yourself over to parenting. Kids have got to be the priority.” – Howard Stern God asks of us that our lives be life-giving. Living life for yourself, free from commitments, is an immature definition of happiness. Commitments lead to greater happiness. Lack of commitments lead to greater unhappiness. You'll never find yourself through your feelings and aspirations. What defines you is your commitments. "Without being bound to the fulfillment of our promises, we would never be able to keep our identities; we would be condemned to wander helplessly and without direction in the darkness of each person's lonely heart, caught in its contradictions and equivocations."-Hannah Arendt Being unable to settle on something means not being settled ever. People willing to commit to marriage and having children almost always test out happier than those who don't. The world tells us self-fulfillment comes from being free of commitments. Truth says fulfillment comes after commitments. People who avoid commitments and constantly pursue pleasure in pursuit of an easy life tend to end up bitter and unhappy. Christ challenges us to not to choose the easy way, to make a sacrifice and give ourselves away. Life is not supposed to be easy, but rather a great adventure. Pursuing pleasure as happiness always ends up in unhappiness. Each year since 1972, there has been a gradual decline in people's overall happiness in the United States. This despite the average person spending 22% more on eating out and entertainment, living in homes that have doubled in size, having huge screen TVs, access to the Internet, and social media. The moral question is not whether you should or should not have kids. The moral question is why you would not want to have children. Religion challenges us to look at everything in life and decide what kind of life we want to have. Once you fall in love, you can't help but want to give yourself away. That's what real love feels like. God's perpetual command is to make an offering of our lives, to give ourselves away. Those who don't know the power of commitment are yet to know what love is. He leads the Omaha Chartered Advisor In Philanthropy® Study Group—a group he founded in 2012 with the support of the Omaha Community Foundation. Mark is currently working with The American College of Financial Services to create legacy planning tools and micro-courses designed to reach beyond high-net-worth individuals and families.   Mark joins me on the show to discuss the turning point that made him transition out of the life insurance business and into helping people pass on their wealth with thought and meaning. He shares why he decided to take the Chartered Advisor in Philanthropy® (CAP) designation and encourage a group of colleagues to join him, and how that turned into ten years of sharing the course with others. Mark also explores the three major themes in his book The Legacy Spectrum, including the psychology of feeling that you have enough money to commit to helping others.   “A lot of people want to pass as much wealth as they possibly can to their families, but they don't think about what happens when they get it.” - Mark Weber   “A moral autobiography talks about the money you leave, it talks about who you were as a person, and what your legacy is going to be.” - Mark Weber   “I felt if I could change how advisors talk to their clients about philanthropy, I could have an impact.” - Mark Weber    This Week on The Wow Factor: How Mark's parents instilled a work ethic in him as a young man Why Mark decided not to pursue a legal career after finishing his law degree and how he got the experience to develop a career in the insurance industry Key skills that Mark learned from his years working in high-end life insurance The distressing experience that really highlighted the importance of taking out life insurance The typical focus of estate planning and where Mark saw the gaps to expand people's understanding of how they can help the wider community How a chance encounter with Warren Buffett changed Mark's perspective How Mark's book, The Legacy Spectrum, helps estate planning professionals to consider how to initiate a conversation about philanthropy with their clients The aim of a moral biography and how it acts as a legacy for your children The importance of teaching your heirs to be good stewards of the money you leave behind Why Mark wants to make Omaha a more generous and gracious city and what he's doing today to continue to push towards that   Mark Weber's Words of Wisdom It's important to educate your kids about the value of money. Grace's Table was born out of Lisa's struggle and prayers as a teen mama. When she entered motherhood at age 17, she felt alone, isolated, and scared, and there were times when she had to choose between keeping a roof over her son's head or keeping food on the table. Grace's Table is a hedge of protection around fragile families; their giving partners are defenders, shielding them as they grow and linking arms with them as they navigate the road to a fulfilling life and future for their family. Together, they preserve families.   I welcome Lisa on to the podcast this week to share her feelings about when she found out at the age of 17 that her life would change forever.  She discusses how she transitioned from hosting a bible study group alongside her full-time job in finance to inviting young moms in need to eat at her table, and when she felt that tap on the shoulder from the Lord to build something life-changing for such a vulnerable group of people. Lisa also shares how she came up with the name Grace's Table and tells the story of who she is honoring and how they helped her.    “Grace's table is a way of me ... extending God's grace to every mom that comes to the table.” - Lisa Anderson   “We really don't know what the person sitting across the table from us has carried, or is still holding.” - Lisa Anderson   “I knew that there was something more that needed to be done because we had moms without education, without stable housing, without food. And I want to always radiate Jesus in the things that I do, but I also want to meet tangible, practical, needs that can allow them to take their next steps.” - Lisa Anderson   This Week on The Wow Factor: What Lisa learned from her parents about serving others and serving in the church How her foundation was fractured at the age of 15 How she managed to survive when her son was born and the people who walked with her and shone God's light and love in broken places What led to Lisa and her two-year-old son couch surf for a time and how Lisa moved past that and found a stable job in the corporate world Why Lisa was so moved to support young moms who are lonely and isolated and how she built the community that embraces them How Lisa's son supported his mom's work with mothers and small children Examples of some of the knocks from young mothers that Lisa has had on her door and how she starts to walk alongside them Lisa's practical advice for families, parents, and grandparents to be on how to have challenging conversations  What's next for Grace's table and how they are extending their support to young families    Lisa Anderson's Words of Wisdom: It's so important to have dedicated time to invest in yourself — in getting quiet and praying. Great Bible Truths with Dr David Petts
103 1 Corinthians 1:18-2:16 - God's Superior Wisdom

Great Bible Truths with Dr David Petts

Play Episode Listen Later Oct 9, 2020 30:16

Talk 5       God’s Superior Wisdom   Today we’re continuing our study of 1 Corinthians 1:18 -2:16. Last time we discovered nine problems with human wisdom:   It is foolishness to God It does not recognise God It rejects the message of the cross It rejects Christ who is God’s Wisdom personified It is totally different from and inferior to God’s Wisdom It is useless as a means of winning others for Christ It is an insecure basis for our faith in Christ It gets you nowhere! It leads only to death It crucified the Lord of glory   Today we’ll be considering God’s superior Wisdom   One of the causes of division in the Corinthian church seems to have sprung from a human and worldly wisdom which was totally contrary to the wisdom of God. These two kinds of wisdom (God’s and man’s) are contrasted in 1 Corinthians 1:18 -2:16   But first it will be helpful to mention some other NT verses which refer to God’s wisdom.   Romans 11:33 33 Oh, the depth of the riches of the wisdom and knowledge of God! How unsearchable his judgments, and his paths beyond tracing out!   Romans 16:25-27 25 Now to him who is able to establish you by my gospel and the proclamation of Jesus Christ, according to the revelation of the mystery hidden for long ages past, 26 but now revealed and made known through the prophetic writings by the command of the eternal God, so that all nations might believe and obey him - 27 to the only wise God be glory forever through Jesus Christ! Amen.   Ephesians 1:7-10 7 In him we have redemption through his blood, the forgiveness of sins, in accordance with the riches of God's grace 8 that he lavished on us with all wisdom and understanding. 9 And he made known to us the mystery of his will according to his good pleasure, which he purposed in Christ, 10 to be put into effect when the times will have reached their fulfilment - to bring all things in heaven and on earth together under one head, even Christ   Ephesians 3:8-11 8 Although I am less than the least of all God's people, this grace was given me: to preach to the Gentiles the unsearchable riches of Christ, 9 and to make plain to everyone the administration of this mystery, which for ages past was kept hidden in God, who created all things. 10 His intent was that now, through the church, the manifold wisdom of God should be made known to the rulers and authorities in the heavenly realms, 11 according to his eternal purpose which he accomplished in Christ Jesus our Lord   Colossians 2:2-3 2 My purpose is that they may be encouraged in heart and united in love, so that they may have the full riches of complete understanding, in order that they may know the mystery of God, namely, Christ, 3 in whom are hidden all the treasures of wisdom and knowledge.   1 Timothy 1:17 17 Now to the King eternal, immortal, invisible, the only wise God, be honour and glory for ever and ever. Amen   Jude 25 24 To him who is able to keep you from falling and to present you before his glorious presence without fault and with great joy-- 25 to the only wise God our Saviour be glory, majesty, power and authority, through Jesus Christ our Lord, before all ages, now and forevermore! Amen.   From these passages we learn that:   Only God, who is eternal, is truly wise God’s wisdom is beyond our understanding God’s wisdom is a mystery The mystery is the message of Christ crucified – the gospel God’s wisdom is manifested in his church   With this in mind we can now turn to today’s passage and see what more we can learn about God’s wisdom.   The ‘foolishness’ of the gospel is described in 1:18-2:5. However, in 2:6-16 it is seen as God’s revealed wisdom. This is the passage we’ll be looking at today, but, as I am not attempting a verse by verse commentary in these talks, for those who are interested there are additional notes on 1:18-2-5 at the end of the notes on today’s talk.   We now turn to Chapter 2. Please have your Bible open there.   In the previous section Paul has been talking largely about human wisdom.  Now he turns his attention to the wisdom of God.  This is true wisdom.  The main point of this section is that those who are still ‘of this age’, who do not have the Spirit, do not understand the wisdom of God in Christ crucified.  Paul’s concern is to get the Corinthians to stop acting like non-Spirit people and understand who they are in terms of the cross.   v6 We do, however, speak a message of wisdom among the mature, but not the wisdom of this age or of the rulers of this age, who are coming to nothing.   Although man’s wisdom cannot lead him to God, God’s wisdom can lead man to him.  The very message that is foolishness to the unbeliever is wisdom in God’s eyes.    Mature here means spiritually adult.  The Corinthians were showing their immaturity by boasting in a wisdom that was not the wisdom of the cross.   v7 No, we speak of God's secret wisdom, a wisdom that has been hidden and that God destined for our glory before time began.   In this verse Paul sees God’s wisdom as eternal It was ordained before the world began but is hidden from the wisdom of man.  It is (literally) in a mystery, a secret once hidden but now revealed in the gospel.   v8 None of the rulers of this age understood it, for if they had, they would not have crucified the Lord of glory.   None of the political rulers of the day had possessed this divine wisdom or they would not have crucified Christ.  It was by their so-called wisdom that he was put to death.   The Lord of glory - Another example of a title clearly applied to Jesus but which in the OT was applied uniquely to Jehovah (Ps. 24:7-10).   v9 However, as it is written: "No eye has seen, no ear has heard, no mind has conceived what God has prepared for those who love him"   Human wisdom is based on the observation of the senses (Cf. modern Logical Positivism).   v10 but God has revealed it to us by his Spirit. The Spirit searches all things, even the deep things of God.   Divine wisdom is not perceived with the senses but is revealed by the Spirit.  It’s possible that some of the Corinthians were being influenced by an early form of Gnosticism. They supposed that they could by searching plumb the depths of God’s being.  But only the Spirit of God can know the thoughts of God. See next verse:   11 For who among men knows the thoughts of a man except the man's spirit within him? In the same way no one knows the thoughts of God except the Spirit of God.   The key to understanding God’s wisdom lies with the Spirit. How can we really know the thoughts of another?  A man can only know his own thoughts.  Similarly, only the Spirit of God can know the mind of God. True wisdom comes to us by revelation from the Holy Spirit   12 We have not received the spirit of the world but the Spirit who is from God, that we may understand what God has freely given us.   We have received God’s Spirit so that we may understand… We have not received the spirit of the world (Satan, the god of this age 2 Cor.4:4).  Any suggestion that a Christian could receive any spirit other than that which comes from God would be totally foreign to Paul’s thinking.    13 This is what we speak, not in words taught us by human wisdom but in words taught by the Spirit, expressing spiritual truths in spiritual words.   The revelation of the Spirit is what enables Christians to understand God’s wisdom.  It is also what enables us to preach it! This is what we speak…             Expressing spiritual truths in spiritual words   The gospel is a spiritual truth and must be proclaimed with spiritual words – words taught by the Spirit (not with human wisdom) There could even be a reference to spiritual gifts here (cf. 1 Corinthians 12:1 pneumatika).    14 The man without the Spirit does not accept the things that come from the Spirit of God, for they are foolishness to him, and he cannot understand them, because they are spiritually discerned.   Note that the ‘man without the spirit’ (psuchikos - natural) does not accept the things of the Spirit.  They are foolishness to him because he cannot understand them.   15 The spiritual man makes judgments about all things, but he himself is not subject to any man's judgment:   By contrast, ‘the spiritual man’ (pneumatikos) can make judgments about all things because he has the Spirit.   16 "For who has known the mind of the Lord that he may instruct him?"  But we have the mind of Christ.   Mind here = spirit.  ‘In the Greek Bible that Paul cites the word “mind” translates the Heb. ruah, which ordinarily means “spirit”’ (Fee pp 119-120).   Note again that Christ is identified with Jehovah (cf. Isaiah 40:13).    Finally, note the connection between the Spirit and the cross in this passage.    2 For I resolved to know nothing while I was with you except Jesus Christ and him crucified. 3 I came to you in weakness and fear, and with much trembling. 4 My message and my preaching were not with wise and persuasive words, but with a demonstration of the Spirit's power,   7 No, we speak of God's secret wisdom, a wisdom that has been hidden and that God destined for our glory before time began. 8 None of the rulers of this age understood it, for if they had, they would not have crucified the Lord of glory. 9 However, as it is written: "No eye has seen, no ear has heard, no mind has conceived what God has prepared for those who love him"-- 10 but God has revealed it to us by his Spirit   God’s wisdom is revealed in the cross God’s wisdom is revealed by the Spirit The things of the Spirit may be discerned by their relationship to the cross It is only by preaching the message of the cross with the power and wisdom given by the Spirit that we can hope to bring others to Christ who is the Wisdom of God.     Additional notes on 1:18-2:5   a) God’s ‘foolishness’ – the cross (1:18-25)   In v17 Paul reminds the Corinthians that the Gospel had not been preached to them with human wisdom lest it should detract from the cross of Christ 18 For the message of the cross is foolishness to those who are perishing, but to us who are being saved it is the power of God. 19 For it is written: "I will destroy the wisdom of the wise; the intelligence of the intelligent I will frustrate." 20 Where is the wise man? Where is the scholar? Where is the philosopher of this age? Has not God made foolish the wisdom of the world?   v18    but to us who are being saved   There are only two groups of people in the world as far as God is concerned, not Jew and Gentile but those who are being saved and those who are perishing.   Note the continuous tense here.  The message of the cross is foolishness to those who are on their way to destruction, but it is the power of God to those who are on their way to salvation.    v19    I will destroy   Man’s wisdom has been destroyed by the cross.  Any wisdom that man may seem to have is rendered meaningless by Calvary.    v20    age......world   The terms are used synonymously here.  The emphasis is that the world is passing, transient.             scholar   Fee (p 71) rightly points out that this should be translated expert in the law.   v21    The world though its wisdom did not know him   Paul rejects the claim that man can know God through wisdom (cf. Romans 1:18-31).              The foolishness of what was preached   It is not the act of preaching but the content that is referred to here (as NIV makes clear – cf. AV ‘the foolishness of preaching’).  It is through the apparently foolish message of Christ crucified that God is pleased to save those who believe.   v22    Jews......Greeks   Both Jews and Gentiles are looking in the wrong direction unless they look to Christ. The answer is not in miraculous signs, nor in human wisdom. The search for these is an expression of man’s rebellion against God.  The answer is the message of the cross.   Power and wisdom are still the two basic idolatries of our fallen world.   v23    but we preach Christ crucified   Christ crucified, by human standards, is the very opposite of what each group is looking for. Indeed a crucified Messiah is a contradiction in terms - ‘fried ice’!   The verb crucified is in the Perfect Tense which speaks of a past act the effects of which are still felt in the present.  Christ’s atoning death is still efficacious.   v24    to us who are called   Salvation is rooted in a divine, not a human decision - but this does not deny the necessity of a human response.             power..... wisdom   Note the different effects the preaching of the Gospel has.  Power contrasts with stumbling-block, wisdom with foolishness.  The cross was a stumbling block to the Jew because of Deuteronomy 21:23 - Cursed is everyone who hangs on a tree (cf. Galatians 3:13).   v25    the foolishness of God   Of course Paul is not ascribing foolishness to God.  He is saying that God’s truth which seems foolishness to the unbeliever is wiser than the unbeliever’s wisdom.  He uses the neuter of the adjective - to moron - instead of the noun - moria - here (literally the foolish thing instead of foolishness).  This points to a particular act of God’s foolishness (the cross).  Compare weakness where again the neuter of the adjective is used.   ‘Had God consulted us for wisdom we could have given him a more workable plan, something that would attract the sign-seeker and the lover of wisdom.  As it is, in his own wisdom he left us out of the consultation’ (Fee, p 77).   b) God’s ‘foolishness’ - the Corinthian believers (1:26-31)   v26    think of what you were when you were called   This verse has been much used as evidence of the sociological structure of early Christianity.  See Fee pp 80ff for a brief discussion.  However,   ‘The truly unique feature of early Christianity was its nonhomogeneous character, that it cut across all sociological lines and accepted as “brothers” slave and free, Jew and Gentile, male and female” (Fee, p 81).   vv27-28                foolish...... weak.......lowly   It is the things which the world considers foolish, weak, and lowly that God has chosen to nullify or render inoperative the things considered to be wise, strong and noble.  The purpose of this is in verse 29 - that no-one may boast before him. In Christ God has already set the future in motion, whereby the present age is on its way out.   v30    righteousness ...... holiness..... redemption   NIV gets the translation right here (AV is misleading).  Righteousness, holiness and redemption are three different aspects of God’s wisdom.  They’re all central to Paul’s theology and result from the cross.   v31    he who boasts.... let him boast in the Lord   The verse clearly refers to Jesus, but it is a quote from Jeremiah 9:23-24 where it is Jehovah who is referred to (cf notes on 1:2 and 1:8.   Boast here has the sense of trust.   c) God’s ‘foolishness’ - Paul’s preaching (2:1-5)   v1      I did not come with eloquence or superior wisdom   When Paul came to Corinth he did not try to persuade them with human philosophy.  He simply preached Christ crucified (v2).  The cross cannot be rationalised.             The testimony about God   Testimony or mystery? Texts differ (marturion or musterion).  Barrett prefers testimony because (a) mystery is in the context and could have affected a copyist subconsciously, and (b) testimony is more suited to the initial proclamation of the Gospel.   v2      I resolved   Paul’s determination here is probably not because of his apparent failure at Athens (Acts 17).  ‘He is not contrasting his evangelistic method with that which he employed elsewhere, but with that which others employed in Corinth’ (Barrett).  Cf Fee, p. 92.   v3      weakness, fear ...... much trembling   See note on Acts 18:1.  The weakness may refer to some observable physical condition.  Astheneia normally means sickness and this is by no means impossible - cf Galatians 4:13-14.   v4      not with wise and persuasive words but with ....... the Spirit’s power   Having contrasted human wisdom with the cross in vv 1-2 he now contrasts it with the Spirit’s power.  This does not necessarily refer to signs and wonders here - note the absence of reference to miracles in Acts 18.  Fee, p. 95, believes it refers to their actual conversion with the accompanying gift of the Spirit which was probably evidenced by spiritual gifts, especially tongues.

110: What Makes Geriatrics so Stimulating for This Doctor?

Session 110 Geriatric medicine is both stimulating and satisfying for Dr. Shannon Tapia. We’ll talk about housecalls, mortality, and the importance of having a sense of humor. Meanwhile, be sure to check out all our other resources on Meded Media for more help as you journey along this awesome field of medicine! Listen to this podcast episode with the player above, or keep reading for the highlights and takeaway points. [01:20] Interest in Geriatrics Having a father who's a geriatrician was Shannon's first exposure to medicine. Growing up, medicine was different back then but she got to witness how it was being a physician. She liked the cognitive aspects of medicine. She could do procedures but she just never really got stoked about it. Being exposed to it early on and realizing how cognitively challenging geriatrics is, she was essentially drawn to it. Shannon compares geriatrics with being the Sherlock Holmes of doctors. Aside from a huge kinetic variability if they live long enough, they also have a lifetime of choices. With geriatric patients, many of them could be suffering from dementia and other cognitive issues, making it difficult for them to express how they feel. So geriatricians have to get a collaborative history from their family and know the environment.  Shannon finds this to be very interesting, challenging, and satisfying. Half the time, it's med side effects from the specialists. They throw a med at them which they should never have been on. You will also realize there's not an answer so you need to be working with the patient and their family. It basically covers all aspects of medicine. You have to be constantly thinking of options and navigate it with your patients and their families. [05:00] Types of Patients The majority of 30-50-year-olds are rare diagnoses but most of them present pretty similar cases. They come in and the doctor asks appropriate questions and they give an accurate history for the most part. This excludes people who are actively psychotic. In the older population, you have to expand your differential in what they say because a lot of things present differently. They have dampened immune systems. They have neuropathy and they don't feel pain in the same way. Until you spend a lot of time with your geriatric patients, it's hard to truly describe the extent of how different it is. You're essentially dealing with a variety of factors when you're trying to approach a problem. Then there are a lot more limitations on what the achievable goals are. So you have to reconcile those to arrive at a realistic outcome and that people can be comfortable with. [07:19] Traits that Lead to Being a Good Geriatrician Shannon says that having a healthy sense of humor is good. You have to be patient and not afraid to get into the thick of things. You never know what you're going to walk into half of the time. Don't take things too seriously otherwise you're going to end up missing what the patient really needs and that of their family. Being empathic and being comfortable with mortality are two other important traits of a good geriatrician. Shannon believes that if you're not someone who can stop doing things to people, you should not be a geriatrician. There's this mentality in medicine where doctors intervene when there's a problem and they're going to fix it. As patients get older, the only truth is we all die. There's always more we could do but you have to be able to step back. Think about the quality of life and prognosis for the patient if you did it. How would it look like not only after they recover but also in two years down the road? Essentially, you have to take it one patient at a time and take their goals and preferences at a time. Have your opinions but separate yourself from that. Moreover, there's a lot of misinformation even for geriatric patients and their families as to what's achievable in medicine. You have to get to know both the patient and their family. Be honest with them about what you think and whatever intervention they're considering. If it's a treatment situation, you have to be able to take their goals and translate what the realistic prognosis would be for them, knowing what their wishes are. That's not easy always because there's a lot of misinformation about what the medical community can achieve at a certain point." [11:00] A Typical Bread and Butter Day Shannon explains how geriatrics is struggling in terms of how they're under Medicare plans. But it's a cognitive field. It's not a procedure that they can always do and can rack up reimbursement for.  Especially if you're in private practice, it's really hard to pay off your student loans and do it well.  Unfortunately, there are very few private practice geriatricians anymore because it's tough and the pressures in the private practice world are hard. Not to mention that there are only a few great academic institutions that have great geriatric support programs.  Shannon describes her typical day as being different from an academician. It's basically different depending on what realm you practice in. If you're in academic geriatrics, you're going to do a mixture of geriatric consult service at a hospital you're affiliated with. You will be on service with heavy clinic and lots of didactics. It's hard for academic institutions to do long-term care providing. It's a whole different set of private regulations that tend to be challenging in an academic setting. As a private practice geriatrician, your day is variable depending on whether you're clinic-based or when you're doing house-based care. When Shannon's new job starts, her practice will be in one geographic area with the goal to see 10 patients a day. Four will be in one assisted living facility and the other four from another assisted living facility, and then two independent homes within the same geographic region. Moreover, Shannon has done some expert witness and chart reviews. There is so much chat vomit in terms of what they're required to put in the medical records. So much of it is just completely useless information. [14:50] Doing House Calls Before Shannon moved to Denver, she used to do direct primary care house calls. She was fortunate in her geriatrics fellowship to get good exposure to it. The problem with geriatric fellowships is they're hugely variable. Some are more research-focused, some are more clinical. Shannon did it for a year but it was clinically focused on every level.  While Shannon loved her fellowship training, she also saw how bad what you walk into could get. But part of why she loves doing this is that because of the patient population they serve. Just because you're Medicare age does not mean you need a geriatrician. It's really based on your physiology and the individual patients. Shannon explains that doing house calls could be best for the majority of the patients. This way, you also get to figure out what's going on with them. Whether it's physical debility or even a mile cognitive debility, getting them to the doctor is a huge deal. This also gives Shannon as their physician so much more information. Plus, the relationship you're able to build with them goes to another higher level. Being invited to their home, you kind of become part of their family. There is a much more intimate relationship with your patients. There is a lot more trust involved. Whereas just receiving patients in your clinic and you only have to go on the face with what they say that they're taking their medicines or they're eating. But it's different when you walk into their homes and see how they're taking their medications. So you get this unique perspective when you get to go to their home. [17:45] Work-Life Balance and Geriatrics Being a Low-Reimbursement Field Shannon says there's a potential to have a very good work-life balance as a geriatrician but it depends on how much money you need for your life balance. Again, it's a low-reimbursement field. So if you are one of those people that wants to take extravagant vacations, it is not for you. As a single mom and not having to be tied to an office, Shannon says this is really huge for her. When you're doing house calls or going to a long term care facility or nursing home, there's a timeframe. There's that flexibility of time that comes with having children but still being able to go out and do clinical work. The only challenge is the documentation requirements that put a lot of pressure on you so you end up taking more work home. Medicare sets the fees even for private insurances. So everything in medicine is all based on trying to figure out how much they're going to pay you and how many RVUs (relative value unit) you've got for a visit. The way the system works out is that time gets very few RVUs unless you do a ton of volume. But procedures get a ton. It's inherent that it's highly cognitive and diagnostic but it's not like you should be ordering tons of tests because usually, those tests are bad for the patients. People who do nursing home care sometimes do pretty well because they can do high volume as they're able to see a ton of people in one place.  Also, the reimbursement system is different in nursing homes than in an assisted living or even in house calls. In nursing home care, how reimbursement works is that it's not just face-to-face visits but it works like at a hospital where it's billed for all the time you spent on the patient. If you're in an outpatient clinic or in a house call, your time is all face-to-face time with them.  [21:35] The Training Path You can either do internal medicine or family practice to become a geriatrician. She was originally going to do internal medicine. Then she had great mentors in family medicine that told her that geriatrics is really an outpatient field and that if your patients are in the hospital, it's not good. Because of this, Shannon wanted to go to a field that emphasizes outpatient and she got a sense in medical school that family medicine did this. So she decided to do family medicine the last minute. After residency, you have to go through either a one or two-year fellowship thereafter. Whichever one you choose, you have to take both boards.  Since she did family medicine, she would have to take a family medicine board every ten years and the geriatrics board. She also has to do a ton of things that family medicine requires that are focused on peds and women's health, which are not related to what she's currently doing. In terms of subspecialty, there really isn't that much. You can do geriatrics/psychiatry but you would have to also go into psychiatry residency. [24:04] Bias Against DOs There is apparently no bias against DOs. Geriatricians are badly needed. Unfortunately, there are people who do fellowships just for filler because they can but they don't really have intention of really practicing the specialty. [25:33] Working with Primary Care and Other Specialties Shannon wishes to say to internal medicine and family medicine physicians that geriatrics is a specialty. This has been her biggest frustration with some family doctors. There are some that think that they've got it all and they don't need help from geriatricians. But they are trained to recognize a lot of things that can help their patients out. Geriatricians are very good at recognizing and helping treat dementia, Alzheimer's, and most vascular dementias.  They're actually better suited to treat it than neurologists because they are also generalists in the whole area. They're able to take a more big-picture approach. They can stay involved with the families and get them connected to the resources they need, more than just do a cognitive evaluation once a year and give them medicine. For family medicine or internal medicine physicians that have patients that are of the geriatric population or have geriatric syndromes such as Parkinson's disease and certain types of early-onset dementia, you would benefit from a geriatrician. Other specialties they work the closest with include cardiology, ophthalmology, and neurology, and gastroenterology. She also works with nephrology but not as much. They also work closely with trauma surgeons because so much of the geriatric trauma are related to unrecognized geriatric syndromes. Getting a geriatrician involved can medicate delirium and help the patients get on the right track. [30:30] Special Opportunities Outside of Clinical Medicine One opportunity is being an expert witness. Shannon only does defense. There's a lot of physicians, especially academic physicians that are on the plaintiff side. For instance, if a geriatric person had a fall in a nursing home and a bad outcome, the family goes and sues. So she does the defense and expert witness if she's asked to. [31:45] Most and Least Liked Things What Shannon wished she knew about the specialty that she knows now is that the system is not great for geriatric patients. And Shannon thinks it's even gotten a lot worse. If she had to do it all over again, she would still have gone into geriatrics. She just wished she had known how things would change in the system for the worse.  For her, it would have been easier to accept things if she knew about it going in, sort of like an informed consent process. Just like when she didn't know she had to take the family medicine boards every ten years that doesn't even apply to geriatric patients. So she was really frustrated about it. And had she known it, it would have been a different story. There's a disconnect between the general population's understanding of how the medical system works and how the system actually works. What she likes the most about being a geriatrician is the patient population. It's fun to be dealing with people. You get to know them and their families. She likes how these people have a lot of wisdom. If you go into it with a sense of humor, you can have a lot of fun with it. On the flip side, what she likes the least about her specialty is the system. She really believes how detrimental the system is to the patient population. It's a growing population and it's costly.  Eventually, she wants to get involved in advocacy for changing the system because she really doesn't think we can sustain the way we do things now. [37:08] Major Changes in the Field Shannon admits that she stopped subscribing to the Journal of American Geriatrics Society as she didn't see it necessary. She doesn't think there are major changes in the field but she hopes that there will be changes in terms of the system and Medicare.  On a side note, Shannon also does hospice, which a lot of geriatricians do because of a similar mentality. And she really hopes politics will recognize the need for qualified geriatricians. [39:15] Final Words of Wisdom It's one of the most fascinating medical fields you can go into. You have to constantly use your mind and be an expert at pharmacology and psychiatry in some ways. The physiology is fascinating as well as the pathophysiology. So if you love to think and you love relationships, it's a great field for you.  But be aware that everybody is different in terms of their student debt burden and the kind of support systems. That being said, it's not the field that's going to make you the most money the fastest. Links: Meded Media

93: Academic Pediatric Hospitalist Shares His Career

Session 93 Dr. Potisek is an academic Pediatric Hospitalist. Today, he discusses the reasons he chose pediatrics, the different facets of his job, and the types of patients he sees. Also, please check out all our other episodes on MedEd Media Network. [01:10] Interest in Pediatric Hospitalist Medicine Dr. Potisek has always enjoyed being around kids. But it was during the third year of medical school that he realized there were so many things about pediatrics that he really loved. So by the end of that year, he was choosing between internal medicine and pediatrics. Ultimately, what drew him to pediatrics is the resiliency of kids. He also likes the incorporation of families as you also take care of, not just the child, but the family members as well. Before Dr. Potisek decided to go to medical school, his dad got very sick. He saw that the kind of communication a physician has with the loved ones, not only makes a difference for that individual but for the entire family as well. Additionally, Dr. Potisek describes himself as a good communicator. And so this was something he was looking forward to going into this career. [04:45] Traits that Lead to Being a Good Pediatric Hospitalist Pediatric hospitalists either work in a community hospital or in an academic setting. Oftentimes, you have to work well in a team. You have to be able to work well with your colleagues and be able to communicate well with patients and their families. Teaching is also an important skill, not only for learners but also for patients and their families so they can understand what's going on. [06:15] Hospitalist vs Outpatient Pediatrician What drew Dr. Potisek more to being a hospitalist over being an outpatient pediatrician is the acuity of care, which he likes more. He likes dealing with "sicker" children and some of the medical mysteries he deals with. He also likes working with numerous subspecialists as they try to figure out the problem. Hence, the two big things he likes about being a hospitalist is the acuity of care and the complexity of diseases he encounters. [07:30] Typical Patients During Fall through Winter, Dr. Potisek deals with a ton of respiratory conditions, with bronchiolitis as a heavy-hitter. He also deals with pneumonia (viral or bacterial) and other respiratory-related diseases that are more seasonal-dependent. He also takes care of neonates, infants 30 days or less. Other common cases would be skin and soft tissue, bone infections. They're also taking care more and more of medically complex children who are technology-dependent. [08:55] Typical Week Half of his time is geared towards pediatric hospitalist medicine while the other half is dedicated to teaching medical students and residents. He works seven days in a row. Mondays thru Fridays would typically start from 7am to 5:30 to 6pm. On weekends, he works for those same hours in the hospital and he'd just take calls from home for new admissions. He also works at night although this is not the majority of what he does. This only happens about 2-3 weeks of the year. When not doing patient care, he'd usually do curriculum development and other teaching activities. [10:19] Doing Procedures Dr. Potisek found that a lot of procedures are already done in the emergency department And if they aren't, they'd typically do a lumbar puncture. There are also pediatric hospitalists across the country that are trained in sedation, which they could incorporate into their practice. [11:12] Training Path After medical school, you would typically do a three-year pediatric residency. Then you can do a fellowship for 2-3 years. There are different options you can take such as additional master training, research, etc. So from the completion of medical, it takes around 5 years in total. That being said, Dr. Potisek has friends who are more outpatient-predominant. Currently, people are being grandfathered in. Because of the many pediatric hospitalists and the lack of fellowships, they see this as an opportunity. People can have a certain amount of hours leading into the board exam. [14:00] Subspecialty Opportunities and Bias Against DOs A lot of fellowships are actually allowing opportunities for pediatric hospitalists. As with bias against DOs in the field, Dr. Potisek hasn't really seen this. At their hospital, they have a number of residents who are DO-trained. [15:44] Working with Primary Care and Outside of Clinical Medicine Dr. Potisek wishes to tell primary care physicians that they reach out earlier to them. Moreover, there are special opportunities outside of clinical medicine that pediatric hospitalists can do such as outpatient opportunities. [18:10] Most and Least Like Things About Pediatric Hospitalist Medicine What Dr. Potisek likes most about his specialty is working with learners – students, residents, pharmacists, and nursing staff. On the flip side, what he likes the least about his specialty is that sometimes you don't necessarily think that a child needs to be hospitalized all the time. But at the same time, he respects his emergency department colleagues. But at the end of the day, if this brings peace of mind to the family, he'd just have to honor the initial decision of his colleagues. That being said, he still finds that sometimes it's unnecessary. "Sometimes, I don't necessarily think that it needs to be hospitalized but that seed has already been planted for them." [21:55] Major Changes in the Field and Final Words of Wisdom It would be interesting to see more fellowships popping up and so Dr. Potisek is excited to see a more developed curriculum. Ultimately, if he had to do it all over again, he would still have chosen the same field. He wishes to impart to students that whatever it is you want to do, just remember why you decided to get into medicine in the first place. Remember what you love about it and where can you maximize the things that you love. It can be challenging at times so having that understanding at the forefront of your mind is helpful. "Just remember the joy and the love you have about the things that you do." Links: MedEd Media Network

Happy Holidays Wonderlings, How’s your December been so far? Ready for the big day next Monday? Hard to believe that Christmas is a week from today. Got all your gifts bought and wrapped? Holiday cards all mailed out? Make any Christmas cookies yet? I am so excited for this episode I watch it every December to get into the Christmas spirit. Let’s jump into the fun filled holiday fun time with S2E3 Christmas which aired on December 14, 1988. In this episode Kevin and Wayne try to convince Jack to buy a color television for Christmas. Meanwhile Kevin tries to find the perfect present for Winnie after she unexpectedly gives him a present. I’m giving this episode 5 out of 5 Flower Power Petals: 1. For the allure of color tv. 2. For Kevin getting Winnie the snow globe instead of perfume. 3. For Paul being a good friend while he was dying of sinus issues at the perfume counter while Kevin figured out which perfume Winnie wore. 4. For Jack finally coming out of his sour mood when the family gets rained on during the Christmas caroling and Jack bursts out laughing and the family dries off in the kitchen drinking hot cocoa finally coming together as a family. 5. Kevin opening Winnie’s gift finding a four leaf clover. Time for my Wonderling Words of Wisdom: It’s Christmas time, it’s not the amount you spend on a person, it’s the love and thought you put behind the gift you give. We don’t always have to have the latest and greatest piece of technology as soon as it comes off the assembly line or the day it’s released. Jack waited two years to get the family a color tv and I’m sure the price came down at least a few hundred dollars by then. Here’s my quote of the episode: Narrator: I don't even remember what I got for Christmas that year. But Dad gave Mom a bracelet that knocked her socks off. Oh, yeah... and he did get us that color-TV... two years later. For me, that year Christmas stopped being about tinsel and wrapping paper, and started being about memory. At first I was disappointed. Until I learned that memory is a way of holding on to the things you love, the things you are, the things you wish to never lose. And I learned from Winnie, that in a world that changes too fast, the best we can do is wish each other Merry Christmas. [Kevin opens Winnie's present, which is a four-leaf clover] And good luck. I want to give a sincere thank to all you listeners out there whether you listen to this podcast on Itunes or Soundcloud or any other listening platforms that you may use to hear this podcast. I love giving you the episode recaps of The Wonder Years every week joining me as I go into full detail as we reminisce of days gone by with Kevin and his friends Paul and Winnie, and the rest of the Arnold family. as well as giving my occasional running commentary and childhood stories. Hey there Wonderlings, I'm your host Angela Bowen and welcome to a weekend edition of Looking Back On My Wonder Years A Wonder Years. Today I’ll be reviewing S2E16: Whose Woods Are These which aired on May 9, 1989. This is the last episode of Season 2 since I already covered S2E17 How I’m Spending My Summer Vacation during my June 21st 3 Episode Summer Special which you can find on Itunes or Soundcloud. In this episode Kevin, Winnie & Paul band together to save their childhood playground of Harper's Woods from becoming a shopping mall. The only problem is no one seems to care. Time for my Flower Power Episode Rating: I’m giving it 5 out of 5 Flower Power Petals 1. For activism the kids had their heart in the right place fighting for Harper’s Woods. 2. Showing the kids as children playing in the woods. 3. A call back to the pilot episode where Kevin and Winnie shared their first kiss. 4. Kevin, Winnie, and Paul working together. This is the first episode where the kids are shown as friends hanging out together. 5. The end shot of them playing hide and seek as the song in my life plays and then seeing their initials in the cement. My Wonderling Words of Wisdom: It’s good to have something a cause to fight for. Activism is important. I know that saving Harper’s Woods was a lost cause for them and they later realized it, but it was a place they loved and they didn’t want to see it die. At least they got to have a moment there one last time to feel young again and Harper’s Woods was their playground one last time. My line of the show: Maybe every human soul deals with loss and grief in its own way. Some curse the darkness. Some play hide and seek. That night Paul and Winnie and I found something we almost lost. We found our spirit. The spirit of children. The bond of memory. And the next day they tore down Harper’s Woods. Tells your friends. I will be taking a month long break in September and start Season 3 in October beginning with Season 3 Episode 3 Wayne on Wheels which aired on October 24, 1989 and focuses on Wayne finally gets his driver's license, but doesn't sit well with his new job of driving Kevin and Paul around. I’ll probably be doubling up on the episodes so I can finish Season 3 before December and take that month off and release the 3 Christmas episodes that month. I have two themed Wonder Years episodes coming out. September 4 is Labor Day so I’ll be reviewing Season 4 Episode 1 Growing Up with aired on September 19, 1990. And also September 5 I’m doing a Back to School Special with 3 back to school episodes Season 3 Episode 2 Math Class which aired on October 10, 1989. Then we have Season 4 Episode 2 Ninth Grade Man which aired on September 26, 1990. And the final of the three Season 5 Episode 2 Day One which aired on October 9, 1991. All right Wonderlings, have a great week!

This week I had the pleasure and honor of speaking with Sarah Aderson, CEO of Expand Your Heart. Sarah is the authority that spiritual and visionary entrepreneurs depend on to help them transform their services into digital and physical products. Sarah is an international speaker, bestselling author, marketing strategist and empire builder. When she’s not creating products or helping change makers build generational wealth, you can find Sarah in Atlanta, GA enjoying trail mix with her awesome son. You’re going to get a TON of value from my talk with Sarah! She shares the biggest block that entrepreneurs struggle with in the beginning and what they can do early on to prevent this block and work around it if it shows up. Sarah explains the difference between a business and an empire and why entrepreneurs should focus on building empires. Sarah then breaks down the steps to help you (even if you’re brand new) start building your very own empire. You’re going to learn simple, easy-to-follow tips for getting started with video marketing. There’s a lot more inside this short interview, but you’ll have to listen to get all the juicy details.   Quotes and Highlights: “If you’re building an empire, then it should be based on a lifestyle.” “When you’re creating a product, you don’t have to marry it. Just get it out in the world so it can help someone.” “Your first digital product does not need to be labor intensive or long and drawn out.” “Your products are generating new leads and new potential clients that you may not have been able to reach had you not had a product on Amazon or other places that you can have them” Sarah Recommends that You Read: The Power of Now by Eckhart Tolle – this life-changing book talks about honoring the present moment and paying attention to the simple things in life. For this special Listener Series episode, I interview David from Texas who shares his debt story and how he got out of it by sticking to a cash-only diet. Long description: After listening to my Listener Series episode with Amanda from Ottawa, David from Texas answered my call out to be on my podcast, and I'm so glad he did. I feel like his story is so relatable, and yet I don't think I've ever interviewed anyone who paid off $40,000 in debt in only 14 months, then stuck to a cash-only diet for good after that. I'm serious in that after listening to our conversation again, I kind of want to try to live off cash only for a month or two to see how many spending and savings are affected. I do try to use debit for all my day-to-day spending, but I do lean on my credit cards from time-to-time. I hope you find this episode as inspiring as I did, but since David has so many good sound-bites, I've included a few gems below. Thanks again David for joining me on the podcast, and I mean it, you should start your own. Your voice and charm are gold! David's Words of Wisdom It is possible, if you believe it's possible...You have got to be focused and you have got to treat [paying down your debt] like it's a life and death situation. David's Top Money Tips for People in Debt Get serious about it Start with a budget Look after your taxes Track your spending Believe it's possible Prayer works Got a story that you think would be good to share on an episode on of Listener series?
