Transcending Weight Bias and Treating Eating Disorders using a Health at Every Size Model – Interview with Amelia MeachamDid you know that Eating Disorders affect at least 9% of the population worldwide? About 28.8 million Americans, will have an eating disorder in their lifetime. Eating disorders are also among the deadliest mental illnesses, second only to opioid overdose. About 26% of people with eating disorders attempt suicide. There are a variety of different ways of treating Eating Disorders. And today, joining me is Amelia Meacham, who is a Senior at The University of Puget Sound in Washington State. Amelia has a Psychology Major and a Religious Studies minor, she'll be graduating this May with her Bachelor of Arts Degree, and she is also my current practicum student. (Practicum = where Amelia and I create opportunities for her to learn about Eating Disorders and treatment using discussion, session observations, and other creative ways she can learn about individuals who struggle with Eating Disorders). And I have learned so much from Amelia! Today, Amelia will be sharing her thesis about how to treat eating disorders better using a “Health at Every Size” (HAES) model. She will also be my first podcast guest to illustrate what I hope to be a new, shorter format that I'll use with future guests in my next season of Calming the Chaos. Intro to Amelia – Tell us about yourself and about your studiesQuestions1. How did you get interested in treating eating disorders?2. What is a Weight based model of treatment?3. What is a HAES model of treatment? ASDAH has the trademark on HAES, so we'll use this definition in part from the ASDAH website:a. Definition: The Health At Every Size® Principles promote health equity, support ending weight discrimination, and improve access to quality healthcare regardless of size. More info at https://asdah.org/health-at-every-size-haes-approach/4. So what is a “Weight Bias?”5. What barriers to treatment happen when Doctors, Therapists and Dieticians have weight biases?a. Lack of perceived needb. Failure of clinicians to diagnose and refer appropriatelyc. Lack of treatment resources6. How does a HAES Approach to Eating Disorder Treatment help people transcend these barriers and move toward lasting recovery (vs. relapsing)?a. Weight neutral, holistic care …vs. Weight-based careb. Weight inclusive framework, including intuitive eating and self-compassion workc. Intuitive Eating Information can be found at http://www.intuitiveeating.org/d. A non-discriminative type of care that combats the weight-centered approach. The weight-centered approach utilizes the biased BMI measure and discriminates against those in bigger bodies, often intersecting with race, disability, and gender. HAES works to combat this discriminative care. Takeaway: Individuals and organizations that align with the HAES movement advocate against weight bias and discrimination in healthcare. This means that not only is HAES is helping individuals with eating disorders, but it is also combating the issues that influence the development of eating disorders.Resources: ASDAH = Association for Size Diversity and Health at www.asdah.orgNEDA = National Eating Disorders Association at www.nationaleatingdisorders.orgFEAST = Families Empowered and Supporting Treatment of Eating Disorders at www.feast-ed.orgANAD = National Association of Anorexia and Associated Disorders at www.anad.orgIAEDP = International Association of Eating Disorders Professionals at www.iaedp.com

Fiona Sutherland, APD  Food and eating and the way we regard our bodies can't be disconnected from the other parts of our lives.   Bring questions and a reflective spirit to supervision.   Supervision is a powerful burnout-protective mechanism   Burnout can carry with it feelings of shame, especially for new dietitians. But, it is not abnormal; many professionals experience burnout.   Eating disorder work requires a lot from practitioners. We need to recognize honestly the weight of the work we're doing and acknowledge the help we need to continue to do the work well.   It's not just our clients who are on a journey of healing- we are too. Our nervous systems are both in the room together, and we need to acknowledge this.   Being in a community of dietitians on Facebook is not the same as supervision and it cannot replace supervision.   When we elevate supervision, we set a higher standard of care to expect of ourselves and each other when doing this work.   I can be who I am right now and that is enough. This feeling can be “supercharged” by being in supervision.   Is it enough to just sit with someone and be present for a person as they're telling me something difficult? We are trained to “get to the good stuff,” that is, the action steps towards behavior change for our clients. But that does not always have to be the ultimate priority for time with our clients. We don't always have to be “fix it” people.    Fiona's “Seasonings”  Fiona Sutherland  Fiona & Tracy's  Trauma-informed care workshop  Fiona & Marci's Body Image Workshop   Sumner Brooks at EDRD Pro  Aaron Flores   Marci Evans    Bio:  Fiona is an Accredited Practising Dietitian (APD) and Director of The Mindful Dietitian, living on Wurundjeri Country in Melbourne, Australia. How boundary setting is generally challenging, but especially when it comes to food. Emotional regulation with teens & pre-teens, and getting consent before giving advice. A control model versus a trust model. How interfering with our teens' dietary intake breaks down their natural ability to eat intuitively.  When parents make it clear that we trust our children to listen to their bodies it is supportive and creates a sense of safety. Katherine Godwin, MD    In the medical field, there's a general assumption that our patients are well-nourished, and that the medications we give them will function in their bodies like they would in a nourished body and that's not true in a malnourished body. When a person is malnourished, meds don't work.  Our body assimilates nutrients like a lego system –when we eat, our bodies break down the nutrients to then build up serotonin, norepinephrine, dopamine (and more).  Listen in to hear more of this nutrition connection.  Medications can improve brain plasticity and enhance the hard work being done, but what to do if there's medical or medication ambivalence?   Listen in to hear about some of the fascinating research on float tanks, avatars, body image, MTHFR, and more.  Bio:  Katherine Godwin, MD, joined Laureate in 2005. She has served as medical director of outpatient services and of Laureate's independent living program for an outpatient eating disorder care, Magnolia House, before taking over as medical director and attending physician of the Adult Eating Disorders Program. We are Made to Move: Exercise and Eating Disorder Recovery    Join us as we speak to Dr. Brian Cook, whose research focuses on the etiological role and therapeutic potential of exercise in eating disorders. We look at the benefit of addressing exercise in eating disorder treatment and treatment tools that can be used.  We don’t have an upper limit to how much we exercise but should there be one? With eating disorders, this is probably a good point to address. Cook believes we must address exercise in treatment just like we do food. We have a professional and ethical obligation to help discern all of the issues that surround exercise and eating disorder treatment. Our bodies are built to move. Intervening on the compulsive aspect of exercise to be able to unlock the potential the benefit of exercise without disordered behavior. We do know that we can use exercise appropriately in treatment to strengthen the body and also the brain and how the brain connects to the body.   You Know He’s Legit Dr. Cook’s research focuses on the etiological role and therapeutic potential of exercise in eating disorders. Download Episode! Hey, lovely radicals... podcast time! This week on the "Life. Unrestricted." podcast, I get to talk to Erica Leon from Westchester County, New York. Erica holds a Bachelor’s degree in nutrition from Cornell University and a Master’s degree in clinical nutrition from New York University. Erica works with clients all over the globe - by all technical means necessary -, people who struggle with food, their body, and people who want to overcome years of dieting or being stuck in disordered eating. She meets people where they are at and guides them to a better relationship to food and their body with a non-diet approach. She has 25 years of experience under her belt and her career has included working in hospitals, corporations and colleges. Ultimately combining all of her skills and interests, she established a private nutrition practice over 15 years ago. As a Certified Eating Disorders Registered Dietitian (CEDRD) she also volunteers on the Board of the International Association of Eating Disorders Professionals, she has written professional articles on nutritions, she frequently gives talks, and she has often been quoted in the media on topics such as eating disorder prevention and childhood weight issues. Today, you’ll hear Erica talk about: – How she remembers being in a body felt like as a child – Where she internalized the message that her body was too big – What the role of her family was in the development of her relationship to food – What counting and tracking points/calories can do to our self-image – Why it is so important to curate your social media feed, and to look at bodies in all shapes and sizes – Why striving for "a flat stomach" is complete BS, and why having belly-fat is, in fact, important for a woman’s overall health as she gets older – Why it is crucial to realize that we are all so much more than a body – How it can be helpful to think about what we would love our legacy to be – How long she lived with her own internal diet-mindset – How she got into the field of nutrition – Why she was working in the marketing department for a weight-loss company (and what memories about her connection to one of Oprah’s many weight-loss incidents still make her cringe) – How she herself unknowingly contributed to diet-culture and its message that "thin is better" – What made her realize that something was not right – How working with eating disorders helped her on her own journey – What she would tell her younger self with the knowledge she has now – Why women often automatically objectify themselves according to what they see in the media and in ads – Why she loves the "Poodle Science" video of the Academy of Size Diversity and Health (ASDAH) – The problem with today’s celebrity worshipping culture and the unfortunate influence they have on our children’s self-esteem – How she’s helping one of her clients with exercise bulimia – What the key ingredient is in the journey to give up lifelong dieting, as well as recovering from disordered eating or eating disorders – Why it takes time, self-compassion, patience and community to heal our societally damaged body image and how we relate to food and weight – How she curated her social media feed and left all the groups (and unfollowed/unfriended people) that were repeatedly engaging in of diet- and weight-loss-talk – Why social media opened up a whole new world for her and made her realize that she wasn’t the only one AT ALL who was preaching the non-diet approach to health – Why it is so hard for the old paradigm-believers to change their minds – What kind of transformations she gets to witness in her practice... ...
