On the 5 September 1986, hijackers took over a Pan-American Airways flight during a stop-over at Karachi Airport in Pakistan. The armed Palestinian terrorists wanted to fly the plane to Cyprus and Israel, to free members of their organisation who were jailed there. But, after the pilots escaped the cockpit, the plane stayed on the ground for 16 hours with almost 400 passengers on board. When the power on the plane ran out – the hijackers began to shoot indiscriminately. 22 people were killed and more than 150 injured. "I know all the things."I hear this constantly. On calls, in my membership. She's read all the quit-lit. She's listened to hundreds of podcast episodes. She's done therapy, maybe twice. It is 50 years since a peace movement began in Northern Ireland spawned by the killing of three children. There had been seven years of sectarian violence. On 10 August 1976, Anne Maguire was walking along Finaghy Road North in West Belfast with her children when an out-of-control car plunged into them. It was driven by a member of the Irish Republican Army paramilitary group who had been fatally wounded at the wheel by British Army soldiers chasing him. Six-week-old baby, Andrew, who was in his pram and eight-year-old Joanne, who was on her bicycle, were instantly killed. Their brother John, just two-and-a-half, died the following day in hospital. The children's uncle, Gerry Grehan, tells Josephine McDermott about the life-changing tragedy and how it resulted in the Peace People, founded by Mairead Corrigan (later known as Mairead Corrigan Maguire), Betty Williams and Ciaran McKeown.It was a peace movement which attracted support from across the religious divide. The sight of Protestants from the Shankill Road and Catholics from the Falls Road in Belfast walking side-by-side drew international attention. There were dozens of marches in other towns and cities including Ballymena, Londonderry, Enniskillen, Newry and London. On 31st October 2004, Uruguayans voted to enshrine water as a human right in their constitution, the first country in the world to do so. The so-called water referendum was organised by an alliance of residents' associations, trade unions, scientists, and environmentalists. Campaigners on horseback criss-crossed the country, they held community meetings in towns and villages across Uruguay and collected 283,000 signatures, the number required to make the government hold a plebiscite, which they overwhelmingly won. I've got a love-hate relationship with research, and this week I finally get into why. I'll read a study, I'll watch a talk, I genuinely enjoy learning about it, but performing research myself has never been my thing, and I get into the whole story of how I ended up doing it in med school anyway purely to check a box. Then I break down the newest NRMP match data, and the numbers are insane. We're talking applicants matching into ortho and neurosurgery with over a hundred research experiences and dozens of publications, and I explain why I think this whole system has turned into a predatory arms race that has very little to do with becoming a good doctor. After that, I get back to some actual eyeball content. I dig into a fascinating study out of Portugal on wind instrument musicians and their eye pressure, and it turns out playing high-resistance instruments like trumpet, oboe, and French horn can cause real spikes in intraocular pressure, potentially even mimicking a condition called normal pressure glaucoma. I walk through some of the older research on this topic too, and honestly, it's the first time in ten years of practicing ophthalmology that I've even heard about it. If you're a serious wind player, this one's worth a listen. Takeaways: The number of research experiences, abstracts, and publications reported by matched medical residency applicants has climbed to levels that go far beyond what's needed to become a good clinician. Doing research as a medical student is not a requirement for becoming a skilled doctor. Wind instrument players, especially those using high-resistance instruments like trumpet, oboe, and French horn, can experience measurable spikes in intraocular pressure while playing. Chronic pressure spikes in musicians could potentially be misdiagnosed as normal pressure glaucoma if a patient's musical background isn't factored into their history. Professional musicians who play high-resistance instruments at a high level may benefit from routine glaucoma screening.
