This episode examines new evidence comparing omalizumab with multiallergen oral immunotherapy for multiple food allergies, finding that omalizumab may offer better tolerability and treatment completion. It also reviews the ICECAP trial, which found no neurologic benefit from extending hypothermia after out-of-hospital cardiac arrest, and the SOLARIS trial, in which high-dose vitamin D3 did not improve outcomes in metastatic colorectal cancer.

Lainy Sharpe began to feel numbness in her legs, followed by paralysis when she was 12 years old in the summer of 2023. Her diagnosis was Mesenchymal Chondrosarcoma, which as rare of a diagnosis as it gets. Lainy fought as hard as possible before passing away in January of 2025 and was very much a part of the Pediatric Cancer community during her fight, which was proven by her winning the Children's Healthy Hero Award for her dedication to helping others at Banner Children's Hospital in Tucson, Arizona.

Then He moves to the purpose of creation: this world was made beautiful as a test of who among us has the best deeds. Then it is destroyed. Something cannot come from nothing, and everything around us points to an uncreated Creator. When they were asked who created the heavens and the earth, they would say: Allah.Their problem was Yawm al-Qiyamah.Because belief in the Day of Judgement changes how you behave. It changes your priorities. If you believe in God but not in the reckoning, you can do whatever you want — you die, and that is the end of it. The rich and the powerful can steal from the poor, oppress the weak, bomb nations and schools, and walk away from it. If there is no ākhirah, what is there to worry about?Belief in the ākhirah takes that away. It forces a change.Ayah 9 — Do you find this the amazing part?أم حسبت أن أصحاب الكهف والرقيم كانوا من آياتنا عجباDo you think that the people of the cave and the inscription were, among Our signs, a wonder?Al-raqīm means inscription. The story of these young men was carved into stone — as the ancients would inscribe things — and so it survived. It was known among certain Christian communities, which is exactly why the Quraysh used it to test the Prophet ﷺ. Nobody in Makkah knew this story. They had to travel outside the city to find people of the book who could tell it to them.So Allah puts the question back to them. You accept that a group of young men slept in a cave for centuries and woke up alive — and that you find miraculous. But death and resurrection you cannot accept?This ayah is not saying the story of the people of the cave is no sign. It is a sign, and a great one. But it is not the greatest. Why is this the thing that excites you, while you ignore what actually matters — that you will die, be resurrected, and be held to account?Ayah 10 — The duʿāʾ on the way to the caveإذ أوى الفتية إلى الكهف فقالوا ربنا آتنا من لدنك رحمة وهيئ لنا من أمرنا رشداWhen the youths retreated to the cave and said: our Lord, grant us mercy from Yourself, and prepare for us right guidance in our affair.Awā means to seek protection — to move behind something and shelter there.Notice what the Qur'an leaves out. We are not told the city, the town, the house, or the cave. This is how the Qur'an teaches history: it omits what is not relevant to the lesson, because detail distracts. If Allah had named the city, people would argue about the city. What matters is that there was a group of young men.And Allah highlights that word — fityah. Young men.We tend to treat the youth as the weakest link in the chain of the ummah. Children come to the masjid because their parents bring them. The elderly come because perhaps there is nothing else. And the youth are absent. Allah is addressing exactly that.Look at the early followers of the Prophet ﷺ. The Prophet ﷺ was forty; Abu Bakr al-Siddiq رضي الله عنه was around the same age. Those are middle-aged men. But Sayyidina Ali was in his early teens. Zayd ibn Harithah, another young man. Gestalt Language Processing (GLP) and Natural Language Acquisition (NLA) have become extraordinarily popular in speech-language pathology, particularly in conversations about autistic children who use echolalia. But what exactly is a "Gestalt Language Processor?" How would we know if someone is one? And perhaps most importantly, what does the research actually tell us about the claims and treatment recommendations associated with GLP and NLA? In this episode, I'm joined by speech-language pathologist Kathleen Oppenheimer, who has spent considerable time examining these questions through the lens of language science and psycholinguistics. Kathleen describes how she first encountered GLP and NLA on social media during the pandemic and why some of the claims she encountered immediately raised questions for her as someone trained in linguistics. We unpack the distinction between language production and language processing, and discuss why observing the words and phrases a child says cannot necessarily tell us how that child is processing language. We also explore an important nuance in this debate: Many practices commonly associated with NLA are quite reasonable—and in some cases very well aligned with established, evidence-based approaches. Following the child's lead, building strong therapeutic relationships, recognizing echolalia as meaningful communication, and honoring multiple forms of communication are all valuable practices. But those practices aren't unique to NLA. Kathleen contrasts NLA recommendations with research-supported approaches such as Enhanced Milieu Teaching, and explains why recommendations involving prompting, single-word models, verbs, and grammatical language input matter for language development. Along the way, we discuss: What GLP and NLA actually claim about language development. Why defining and reliably identifying a "Gestalt Language Processor" is more difficult than social media might suggest. The difference between what a child says and how researchers determine what a child is actually processing. What psycholinguistic methods such as the visual world paradigm can tell us about real-time language comprehension. Why echolalia can be meaningful communication without accepting the larger GLP/NLA framework. The substantial areas of common ground between NLA proponents and research-aligned language interventions. Why following a child's lead and honoring their communication does not require adopting NLA stages. How Enhanced Milieu Teaching differs from NLA-based intervention. Concerns about recommendations to avoid or delay modeling single words, verbs, and certain grammatical forms. What we know—and don't know—about the empirical evidence supporting GLP/NLA. How social media can turn nuanced clinical ideas into much broader claims. The potential costs of implementing unsupported interventions, even when those interventions appear benign. How clinicians can have productive conversations with colleagues and families who have embraced GLP/NLA. Why clear, jargon-free explanations of what we're doing in therapy—and why we're doing it—matter. One thing I especially appreciated about this conversation is that Kathleen doesn't approach this as a simple choice between being "for" or "against" GLP/NLA. Instead, she encourages us to pull apart the individual claims and practices, retain the components that are supported by evidence, and scrutinize those that aren't. For clinicians navigating an increasingly complicated landscape of research, social media, and rapidly spreading treatment approaches, I think that's an incredibly useful framework. It can be even more difficult for children who have a hard time with change given trauma and attachment issues and their placement in foster care or being adopted.
