This April 2026 Heart Rhythm Journal publication reports a sub-analysis of the CHAMPION-AF trial evaluating left atrial appendage closure (LAAC) versus oral anticoagulation in patients with atrial fibrillation who previously underwent catheter ablation. The analysis demonstrated that LAAC with the WATCHMAN FLX device provided comparable protection against stroke, systemic embolism, and cardiovascular death when compared with NOAC therapy, while also reducing long-term non-procedural bleeding events. O Parlamento angolano aprovou, nesta quinta-feira, 22 de Janeiro, em votação final, a lei sobre o estatuto das ONG, com os votos contra da UNITA, que considerou que o diploma restringe a liberdade de associação. Em entrevista à RFI, Zola Álvaro, activista e presidente da Associação Cívica Handeka, refere que esta lei vai dificultar o trabalho das ONG e reintroduz o espírito de controlo, o que constitui uma violação do direito à liberdade de associação.   O MPLA, partido no poder em Angola, considera que esta lei “reafirma o princípio do Estado de direito”. Este diploma salvaguarda os direitos das ONG em Angola? Discordamos todos a esse nível. Tanto as organizações da sociedade civil como os 72 deputados da UNITA -que votaram contra - e as duas abstenções. Uma posição diferente da dos deputados do MPLA, que votaram a favor [do diploma]. Primeiro, há um conjunto de preocupações em torno da aprovação desta lei, principalmente quando é aprovada num contexto pré-eleitoral, com toda essa celeridade, e quando tenta ressuscitar normas já declaradas inconstitucionais pelo Tribunal Constitucional angolano. No Acórdão n.º 447/17, que revogou o Decreto Presidencial n.º 74/15, a [lei das ONG] tenta claramente introduzir este espírito de controlo através de uma lei ordinária, o que constitui uma violação material do direito à liberdade de associação. O Governo angolano refere que esta lei confere às ONG um quadro jurídico “claro, moderado e equilibrado”. Não era necessário preencher este buraco na lei? Claro que não. A lei estabelece um conjunto de barreiras burocráticas que asfixiam [as ONG], com obrigatoriedades. O artigo 19 contém um conjunto de exigências, como é o caso de relatórios mensais exaustivos. Essas medidas desviam os recursos e o tempo que deviam ser dedicados ao apoio directo às populações mais vulneráveis. Depois, há ainda o artigo 22, que fala sobre a não exportação do capital já doado pelas organizações. Isso retira toda a confiança do doador, porque, na definição, construção e arquitectura de projectos, pode haver excedentes, e esses excedentes têm de ser devolvidos ao doador, conforme as exigências contratuais. Ao existir uma lei doméstica que proíbe essa garantia, vai, certamente, retirar toda a confiança do doador nesse processo de aprovação de projectos locais. O executivo refere ainda que esta lei vai permitir o combate ao branqueamento de capitais e ao financiamento do terrorismo… Até agora, não há registos, nem qualquer decisão judicial, de que uma associação angolana esteja envolvida em branqueamento de capitais ou em financiamento do terrorismo. Essa preocupação expressa na lei pode ser legítima, mas não se trata de uma questão de prevenção. Não, é de facto, uma questão de controlo das organizações. Trata-se de uma forma de o Governo controlar a liberdade e a autonomia das associações? Há aqui um aproveitamento nesse sentido, porque Angola já dispõe de um conjunto de dispositivos normativos próprios que previnem o branqueamento de capitais, a corrupção e o financiamento do terrorismo. A UNITA, o principal partido da oposição no país, votou contra, alertando para o facto de esta lei restringir a liberdade de associação. A partir de hoje, o trabalho das ONG fica mais difícil? Fica muito mais difícil a partir de agora, porque temos de entender que a intenção fundamental desta lei é, de facto, impedir que as associações continuem a operar com o mesmo nível de autonomia. Esta lei confere poder ao Governo, principalmente no que diz respeito ao encerramento das organizações. Isso demonstra, de facto, o rosto e as pretensões da aprovação desta lei. Quais são as outras mudanças que serão implementadas com esta lei, relativamente ao trabalho das ONG? Os artigos 7.º e 34.º permitem ao órgão decisor -ou seja, a um órgão com poder administrativo - propor onde os projectos devem ser executados e exigir a manutenção de registos dos beneficiários efectivos e de outras pessoas que controlam ou estejam a gerir essas organizações. Ora, as organizações identificam, elas próprias, os espaços adequados para a implementação dos seus projectos. Se existe um órgão administrativo que vai redefinir onde os projectos devem ser executados, isso já não é uma questão de autonomia das organizações. Trata-se de a administração política decidir, efectivamente, onde as organizações devem realizar ou executar as suas actividades. Isso retira, de alguma maneira, a capacidade de decisão própria, a autonomia e a vontade das organizações. Outra grande preocupação prende-se com a obrigação de as organizações partilharem o registo dos beneficiários dessas mesmas actividades. Basta pensarmos, por exemplo, numa associação como a Kutakesa, com objectivos claros de protecção de defensores de direitos humanos, que tem na sua base de dados um conjunto de defensores perseguidos pelo Governo angolano. Já vemos aqui, de facto, uma situação extremamente delicada, em que a instituição que vai monitorizar o exercício das associações exige, efectivamente, uma base de dados desses beneficiários de protecção. Aqui há, de forma clara, má-fé, tanto do proponente como do legislador, ao retirar, de facto, às organizações a autonomia e a capacidade de preservar a identidade dos beneficiários, muitos dos quais têm sido alvo de graves violações de direitos humanos, sendo que o grande prevaricador tem sido o próprio Governo angolano. A proposta de lei sobre a disseminação de fake news (notícias falsas) foi aprovada na generalidade e será agora apreciada na especialidade. Esta lei ajusta-se à realidade de Angola? Não, não se ajusta. Na verdade, o que acontece é que estamos próximos de um processo eleitoral e, ao longo dos últimos anos, tem sido criada, de alguma forma, uma arquitectura, um conjunto de propostas de lei que visam restringir o espaço cívico, limitar as acções das organizações da sociedade civil e a actuação de pessoas individuais, activistas, defensores de direitos humanos e outras iniciativas. Isto faz parte de um pacote legislativo que visa limitar o exercício das organizações da sociedade civil e a iniciativa de cidadãos em torno do próprio processo eleitoral. Há uma tentativa de limitar a liberdade de expressão e de informação? Exactamente. O executivo procura garantir que, durante um processo eleitoral, sejam asfixiadas todas as iniciativas das organizações da sociedade civil, principalmente com esta lei das fake news. Serve também para restringir, de facto, o nível de actuação da imprensa privada e dos defensores de direitos humanos. No entanto, são conhecidas as ameaças que representam as fake news, as notícias falsas. Qual seria o caminho para lutar contra esta ameaça? No contexto angolano, o maior disseminador de informações falsas que circulam na imprensa é o próprio Governo de Angola, através da manipulação da imprensa pública, nomeadamente a TPA, Televisão Pública de Angola, ou a TV Zimbo, estação de televisão privada, que funciona como uma televisão alegadamente alternativa. A televisão pública e os jornais públicos são a maior fonte de desinformação existente e, não apenas isso, são também a principal fonte de manipulação do debate público nacional. Assim, a grande preocupação deveria incidir, de facto, sobre essas fontes já claramente identificadas de desinformação. Poderia existir um mecanismo próprio, assente num espaço de concertação efectiva com a sociedade, sobre a necessidade de legislar contra a desinformação e as fake news. Este tem sido um debate internacional, mas existem mecanismos específicos para os sectores que devem ser alvo dessa preocupação legítima. Que mecanismos seriam esses? Esse processo passaria necessariamente por uma transformação tanto da imprensa pública como da privada, bem como pelo envolvimento da academia e das organizações da sociedade civil. No entanto, tudo isso exigiria vontade política. A maior parte das disposições constantes da proposta de lei visam, na prática, silenciar a imprensa privada e todos os outros intervenientes nos espaços públicos. Nos últimos anos, em Angola, sobretudo através das redes sociais, surgiram fontes alternativas de informação. Vivemos num país onde o acesso à informação, especialmente a informação de interesse público, tem sido cada vez mais escasso. Os meios de comunicação públicos praticamente não informam sobre matérias de interesse público. Assim, os angolanos recorrem a fontes alternativas, que são precisamente aquelas que estão na base da preocupação do proponente da lei -neste caso, o Presidente da República- ao propor um diploma que visa restringir o exercício dessas fontes alternativas de informação. Silenciar essas fontes? O objectivo é claro: silenciar essas fontes, silenciar, de facto, também as pessoas de bem que têm, de alguma forma, tentado informar a sociedade.

The case-based review helps clarify some key concepts, such as when warfarin is preferred for anticoagulation, who may be a good DOAC (direct-acting oral anticoagulant) candidate, how to choose an appropriate DOAC agent, and how to manage anticoagulation therapy in patients already on antiplatelet therapies. Pearls and Quotes - Anticoagulation Pharmacology Avoid potentially fatal errors with this terminology tip for correctly referencing non-warfarin oral anticoagulant agents: it's DOAC (like, please DO use AntiCoagulation), not NOAC (imagine someone interpreting that as “NO AntiCoagulation for this patient” at discharge – yikes)! Sometimes, an oldie really is a goodie – warfarin is recommended over DOACs for patients with mechanical heart valves, moderate-to-severe mitral stenosis, anti-phospholipid antibody syndrome (APLS), left ventricular (LV) thrombus, higher INR goals, or DOAC failure. Patient preference and medication costs should also be considered – at the end of the day, “the best drug is the drug that a patient is willing to take!” Standard-dose rivaroxaban or apixaban may be considered for use in patients weighing >120kg or with BMI >40; use of other DOACs should be limited to pts weighing =/< 120kg or with BMI =/< 40. The pharmacists involved in this podcast promise they don't have stock in apixaban! It just often happens to be the preferred DOAC option in certain scenarios – think patients with severe renal impairment (including ESRD) or with an increased risk for bleeding events (including older adults, those with a history of GI bleed, etc). In general, dual therapy (DOAC or warfarin + P2Y12 inhibitor) is non-inferior to triple therapy (oral anticoagulant + P2Y12 inhibitor + aspirin) at preventing thrombotic events but is associated with a lower risk of bleeding events. Most patients can be transitioned to dual therapy after 7-30 days on triple therapy post-percutaneous coronary intervention. What's that on the horizon? Factor XI inhibitors may become the breakout stars of anticoagulation – multiple investigational agents are being studied for their potential to reduce thrombotic risk without significantly increasing bleeding risk in patients with indications for anticoagulation therapy…at least that's the theorize hope. Watch this space! Notes - Anticoagulation Pharmacology In which cases is warfarin preferred over DOACs in patients with atrial fibrillation? Long-term anticoagulation with warfarin is indicated in patients with atrial fibrillation and either a mechanical valve or moderate-to-severe mitral stenosis (i.e., valvular atrial fibrillation as defined in the 2019 AHA/ACC/HRS guidelines on atrial fibrillation [1]). The REALIGN trial [2] showed increased rates of thromboembolic and bleeding complications with dabigatran vs.
