#47 Living with NAION: Rachel Schreiman's Journey Through Vision Loss and Advocacy

It Happened To Me: A Rare Disease and Medical Challenges Podcast

Play Episode Listen Later Nov 4, 2024 56:02

In this powerful and educational episode of It Happened To Me, we sit down with Rachel Schreiman who turned her personal struggle with vision loss into a mission to support others. Rachel shares her story of resilience after experiencing two episodes of Non-Arteritic Anterior Ischemic Optic Neuropathy (NAION), a rare and debilitating eye condition caused by insufficient blood flow to the optic nerve. These episodes left her with significant central vision loss, but instead of giving up, Rachel embraced a new path.   Rachel Schreiman is a CPA and musician who holds an MBA from the University of California, Irvine. In 2019 while working as the Controller for a trade association in Washington DC, she suffered two episodes of Non-arteritic anterior ischemic optic neuropathy (NAION), rendering her legally blind with significant central vision loss. After receiving rehabilitation care and training she started a new "career" devoted to others with low vision. She now works for Dr. Suleiman Alibhai OD, a low vision rehabilitation optometrist, and as a Resource Navigator for the Prevention of Blindness Society of Metropolitan Washington (POB). Both of these positions allow her to give back to others with low vision by sharing information about resources and demonstrating how to use many of the devices, assistive technologies and applications available that allow users to live full and independent lives.   Key Topics Discussed:   The Onset of NAION:  Rachel recounts the initial episode of NAION in 2019, the symptoms she experienced, and her journey to diagnosis. She describes NAION as a "stroke in the eye," explaining how lack of blood flow to the optic nerve led to significant vision loss.   Risk Factors and Triggers for NAION: Rachel discusses possible risk factors, such as sleep apnea and cardiovascular issues, which can contribute to the development of NAION. She shares insights into lifestyle adjustments and precautions she now takes to help manage her health.   Navigating a Second Episode:   Five months after the first NAION episode, Rachel suffered another in her other eye. She reflects on the impact of this second event, the rapid response from her medical team, and the steps she took to prepare for further adaptation to vision loss.   Diagnosing and Treating NAION:   Rachel describes the challenges of diagnosing NAION, which can be easily mistaken for other conditions such as multiple sclerosis, brain tumors, or stroke. She also explains the diagnostic process and the types of specialists who are essential for accurate diagnosis and care.   Adapting to Vision Loss: Rachel opens up about the difficult decision to stop driving and the profound impact it had on her independence. She shares the changes she made in her home and daily routines, along with the support she received from her husband and family, which helped her navigate life with low vision.   Coping Strategies for Low Vision:   Rachel reveals the practical and emotional strategies she uses to cope with vision loss, from using assistive technologies to finding new hobbies and ways to stay connected with her passions.   Advocacy and Empowerment in Low Vision Care:   Through her roles with Dr. Alibhai and the POB, Rachel describes her work in educating others about low vision resources, providing hands-on training with assistive devices, and guiding patients and their families through the journey to independence. She also highlights the services POB offers for those with low vision and encourages listeners to seek support early in their vision loss journey. Episode 138 | "Feed Brooke cubes of cheese and she'll go right to sleep." In this thrilling episode of REMelations, Mindy delves into a bizarre and unnerving dream submitted by Lisa. What starts as a fun night out with friends quickly turns uncomfortable when Lisa's body becomes magnetized, attracting pins from everywhere. While the pins stuck in her body are horrifying enough, it's the ones embedded in her mouth that leave Mindy and Brooke squirming in their seats. In the Cat Nap segment, the duo shares a collection of wild and whimsical dreams from their listeners. From the frustrations of not being able to wash your hair to the haunting disembodied voice of a father, and from roofs on fire with no one to help to the chaotic mess of dog and baby poo, there's never a dull moment. One listener even dreams of two suitors vying for her heart: Will Ferrell and Adam Sandler. Who will she choose? The celebrity fun doesn't end there. Brooke dives into the darker side of fame, recounting how being a celebrity can sometimes turn into a nightmare, especially when the law gets involved. In this episode of Double Tap, Steven Scott and Shaun Preece dive into the latest updates from Voice Dream Reader and explore a new tool to enhance web accessibility. First up, they discuss the recent changes announced by Voice Dream Reader. A listener, Graham, shares his discovery that the app's update notes suggest a shift away from a mandatory subscription model for legacy customers. This news sparks a detailed analysis by Steven and Shaun, who weigh in on the implications of this decision for both the company and its user base. They delve into the community's reaction, including the overwhelming feedback Voice Dream received, which ultimately led them to retract their subscription requirement for adding new documents. However, Steven and Shaun express concern about the sustainability of this decision, pondering how it might affect the app's future development and the broader context of app monetization strategies. The second part of the episode introduces a conversation with Oliver Emberton from Silktide, recorded by Allison Sheridan at the CSUN conference. Oliver presents a new Chrome extension designed to aid web developers in making their sites more accessible. This tool offers features like a screen reader simulator, a focus order visualizer, and a colour contrast checker, all aimed at simplifying the process of ensuring web accessibility. Allison's enthusiasm and insightful questions highlight the potential impact of such tools in promoting inclusivity online. Many of us out there are using OCR apps for basic tasks like scanning mail or reading books but did you know how powerful the apps on smartphones are compared to desktop PC applications? On the podcast today Steven shares an archive review of his tests of both Kurzweil 1000 and Voice Dream Scanner. It was interesting to learn how the two software applications were so similar yet priced very differently. Ultimately this isn't a review that should make you think less of one tool vs the other but to highlight the incredible functionality of our smartphones against powerful desktop applications. We strongly believe knowledge is power so the more you have the better informed you can be. Here's where where to find more information: Kurzweil 1000 Voice Dream Scanner What do you think? The CCB recognizes that vision loss has no boundaries with respect to gender, income, ethnicity, culture, other disabilities or age.  The CCB understands in many instances vision loss is preventable and sometimes is symptomatic of other health issues.  For the 21st century, the CCB is committed to an integrated proactive health approach for early detection to improve the quality of life for all Canadians. As the largest membership organization of the blind and partially sighted in Canada the CCB is the "Voice of the Blind™".  GTT is an exciting initiative of the Canadian Council of the Blind, founded in Ottawa in 2011 by Kim Kilpatrick and Ellen Goodman.  GTT aims to help people who are blind or have low vision in their exploration of low vision and blindness related access technology.  Through involvement with GTT participants can learn from and discuss assistive technology with others walking the same path of discovery.  GTT is made up of blindness related assistive technology users, and those who have an interest in using assistive technology designed to help blind and vision impaired people level the playing field.  GTT groups interact through social media, and periodically meet in-person or by teleconference to share their passions for assistive technology and to learn what others can offer from their individual perspectives.  CCB National Office 100-20 James Street Ottawa ON  K2P 0T6 Toll Free: 1-877-304-0968 Email: info@ccbnational.net URL: www.ccbnational.net

Show Summary: Voice Dream Scanner just hit the market and for $5.99, this app from VoiceDream is another nice addition to the other apps developed by the VoiceDream Team. VoiceDream Reader is one of the most popular apps in the BVI community and VoiceDream Scanner seems like the perfect addition. With the ability to save scanned text as PDFs, text files and copy to the ClipBoard, VoiceDream Scanner packs a new punch entering the crowded ring of OCR apps now available to the BVI Community. Does it stand out enough to make an impact? Probably so. With an easy lay-out and great accessibility throughout, VoiceDream Scanner may just move onto some people’s front page of apps and not be buried in the ever-growing OCR folder on page 8.  With features like Batch mode and the Auto Capture Beta, Scanner makes one pay attention as well as paying 5.99 one-time. Not so bad for quite an intuitive OCR app. I think this app will be around for a while and with the addition of being able to import photos from your phone, maybe this Scanner app is all we are looking for when it comes to OCR? 21st Century Work Life and leading remote teams

Really connecting with our core consulting work at Virtual Not Distant, today is all about virtual teams – we talk about the idea of ‘virtual distance’, and how there are many ways that distance and closeness can be created. Also coming soon, look out for Pilar’s book, Online Meetings that R.O.C.K. – expanded and advanced and taking longer than expected due to fantastic insightful feedback from beta readers, find out what changed about the ‘C’! It’s going to be worth the wait. 07.15 What do you mean by that: Virtual Distance, with Dr Karen Lojeski  We really enjoyed this conversation with Karen Lojeski, founder and CEO of Virtual Distance International, and author of The New Rules of the Virtual Workplace. Virtual Distance describes what gets lost, when the human is translated through a machine. Karen’s original research found out that this could be measured and understood – and that it definitely impacts on relationships and collaborations to a measurable extent. But it is NOT dependent purely on physical distance – that is merely one dimension in a complex system, involving operational and affinity distance as well.  When virtual distance is high we damage innovation, trust and performance. And it doesn’t only apply at work… After all these are human factors, the technology is much less important. As Karen explains, "We often mix up the notion of the computer as a tool with the computer as somehow being smarter than the person and it just couldn't be further from the truth."  We are so much more complex and significant and interesting, the machines don’t come close. Check out both of Karen’s inspiring books on virtual team leadership.   39.05 Recommended tools: Voicedream A little while ago in episode 173 we discussed the medical affliction we affectionately described as ‘homeworkers bum’, a syndrome exacerbated by work which involves lots of reading and research.  It’s not easy to read whilst doing anything but sitting down, but Voicedream is an app which reads written text aloud. It’s not perfectly ‘human’ of course, but this is a technology which is rapidly evolving and will surely improve. But for now this is a nice app which can sync with Pocket (which we discussed in episode 172), so it helps you get through your reading queue more quickly, even when you’re out for a walk.   42.36 Oh No, my team’s gone remote, and I know we’ll grow more distant, but I don’t know how! Instead of our usual talking head format, Pilar gives us a little exercise to complete, to help us plan or reflect upon the virtual distances within our teams – and perhaps lead to improvements in how you work together. Here’s the diagram you will need, to work through the exercise whilst thinking about YOUR team.  You can find Pilar’s diagram for this at https://www.virtualnotdistant.com/podcasts/virtual-distance. How do the different expectations and behaviours impact on your work, and how interdependent are your activities? You can score yourself on the many different dimensions of virtual distance, to see how you’re doing. If you do this as individuals within a team, how do your ratings compare? How do you think they will evolve over time, if you are transitioning to new ways of working? And can you see how little of the whole picture has to do with physical distance? Remember to review, reflect and learn, from your exercise into virtual distance. Please let us know how it went for you, and make sure you’re subscribed for future episodes.

The new technique combines audio, visual, and attention propulsion. The words disappear from the page a fraction of a second before they're spoken by text-to-speech.  This forces the reader to visually scan ahead. With  practice, the results can be amazing. Finger reading: Some readers struggle to follow even the slowest speed, which also becomes distorted. With finger reading, the reader drags a finger under the text to hear each word. Quickly browse the text: On an iPhone 6s or 6s plus, you can press firmly on the fast forward button while the app is reading --  Force Touch -- to double the reading speed. Release it slightly to return to normal speed. What’s New in 4.0.0 release of Voice Dream Reader? A beautiful new user interface Grid view and cover images in the library. Improved library organization: filters, flags, and sort order. Visual appearance of the entire app uses the current color theme. Experimental rich text and image support for all documents. Library Synchronization: Your entire library, including folders, reading locations, bookmarks and annotations are synchronized across all your devices using iCloud Drive. New Reading Modes: Finger reading allows beginning reads to control the speed by running a finger under each word. Pac-Man mode. Harvard and MIT developed speed-reading method for everyone to read at 2x speed with no loss of comprehension. Sentence mode. Speech pauses at every sentence. Useful for beginning readers. Repeat mode. Option to rewind to beginning of the last sentence when you pause. New Voices: Alex, free, high-quality voice in iOS 9. New premium child voices from Acapela in US, UK, Australian English, US Spanish, and German. Acapela's new flagship German voice: Claudia. Emotive variations of Jeroen, Antoine, Margaux, Dimitris and Will. Other New Features: Set PDF margins to exclude text in header and footer. Up to 4X speed for recorded audio. Support for 3D Touch, split-screen multitasking, and Spotlight search. Safari extension to save Web pages and Web addresses. Evernote file attachments. Background download for files and voices. More accurate language detection. Option to hide header and footer in full-screen mode.

At the very tail end of August, Voice Dream Reader, the wildly popular book reading app, launched on Android. This opens up a huge potential market, ranging from students who want to read college coursework on-the-go, to professionals who drive a lot and need an easy way of reading materials with high-quality voices. Because of the excellent price range of Android devices, you can now own a book reader for under $100. We've interview Winston Chen who developed the iOS application, which has now grown into a comprehensive app. Getting this to work on Android from scratch was a unique challenge, as it requires a great effort and collaboration between features and platforms. Michael Scott developed the Android version of Voice Dream Reader, and it has many of the same functionality as found on iOS: Bookmarks, various high quality voices from Acapela (at the moment, Ivona is expected in a future update), along with bookmarking support, DRM-free Epubs, bookshelf views, Bookshare, Dropbox,One drive integration, visual adjustment of the document's contents, highlighting, built-in web browser... Just what does it take to develop an Android app anyway? How difficult was it to have all of these features migrate over from the iOS world? What are some future plans for the app? Ultimately, what goals might the team have in mind for its purpose? We sit down with both Michael and Winston to talk about the development and story of Voice Dream reader.

On this episode I take an introductory look at the iOS app Voice Dream Mail. This app is free and from within you can purchase a subscription to unlock further features. Read on to see what the developer has to say about the app… App Store Description Voice Dream Mail works by speech and touch, so you can efficiently go through your email inbox without having to use your eyes. How It Works The app reads your email out loud using text-to-speech. You control it using the entire screen of your iPhone or iPad as a touchpad. For example, swipe right anywhere to advance to the next email, and swipe down to trash. Why It's Efficient Once you learn the handful of gestures, you will be able to triage your inbox very quickly. You know what to do with most emails after hearing the sender and subject. Because all commands can interrupt reading, it only takes a few seconds to handle each email. Plus, you can gradually increase speech rate: most people can listen at a higher speed than they can read visually. Check out the app on the App Store to learn more and download.
