The latest episode of Unplugged, in partnership with BridgeHead Software, explores why the NHS must move beyond ‘analogue to digital' to ‘digitalised'. Digital Health Unplugged host Jordan Sollof is joined by Adam Coombes, product owner, HealthStore at BridgeHead Software, and Andy Meiner, founder of SI Squared, to examine the hidden barriers to interoperability. The pair argue that data only becomes useful when enriched with structure, standards and metadata, before discussing why paper is still so deeply embedded in NHS workflows. They then outline the practical strategies organisations can use to unify and operationalise their data to help deliver the ambitions of the NHS 10 year health plan. Lastly, Coombes and Meiner offer one mindset shift and one practical step that chief information officers and chief clinical information officers should take to move towards true digitalisation. Guest: Adam Coombes, product owner, HealthStore at BridgeHead Software Andy Meiner, founder of SI Squared

Cold plunges are everywhere, and the way people talk about them, you'd think they're a miracle cure for your brain, body, and soul. But in an age of algorithm-fueled evangelism, when a ritual becomes this ubiquitous and loud, we have to ask: how much of the buzz is backed by science… and how much is just marketing? In this episode, we explore the neuroscience of cold exposure: what's real, what's overstated, and why this "discomfort" has become a billion-dollar industry. We discuss: Why cold plunges went viral, and how wellness movements often devolve into identity-driven cultures The difference between cold exposure itself and the monetized "cold plunge movement" What constitutes a "cult" (and how pseudoscience forms around partial truths) The real physiological cold shock response Why the mental "high" after a plunge doesn't automatically equal long-term brain benefit The cardiovascular risks that rarely get discussed, especially for people with underlying heart disease What the research suggests about soreness, pain reduction, and muscle growth (including why cold immersion can blunt hypertrophy) The real story behind brown fat Who should avoid cold plunges altogether (asthma, arrhythmias, coronary disease, vascular conditions) Joining us for this conversation is investigative journalist and bestselling author Scott Carney (What Doesn't Kill Us, The Wedge), who has spent years inside the cold exposure world, first as a skeptic, then as a believer, and eventually as a critic of the culture that formed around it. His work reveals what happens when discomfort becomes identity, and when unfounded "social media science" outruns real science. — Bello! bello! Ritornai a casa e maltrattai i miei convitati, bastonai mia moglie, feci tutti i diavoli, ruppi quanto mi veniva in mano: cantai, gridai, insomma un finimondo! Mia moglie scappò in camera e rimase là rintanata. — Velenose sì, ma inutili forse no! – rispose Coombes. Fine. In this episode of the Epigenetics Podcast, we talked with Luca Magnani from Institute of Cancer Research and UNIMI in Milan about his work on epigenetic mechanisms of drug resistance and cancer cell dormancy in breast cancer. We start the interview by putting our focus on his significant contributions to the understanding of estrogen receptor-positive breast cancer. In a foundational study from 2013, Professor Magnani and his colleagues illuminated the role of genome-wide reprogramming of the chromatin landscape in conferring resistance to endocrine therapy. This research marked a departure from a purely genetic mutation paradigm, proposing instead that epigenetic modifications play a pivotal role in the development of drug resistance. A fascinating part of our conversation centers on the role of pioneer transcription factors, particularly PBX1, in regulating the estrogen receptor's transcriptional response. Professor Magnani explains how PBX1, typically associated with hematopoietic development, influences estrogen receptor activity, thereby shaping the cancer cell's fate and response to treatment. Continuing our exploration, we discuss the critical distinctions between primary and metastatic breast cancer through the lens of epigenetic reprogramming. By analyzing samples from women with breast cancer, Professor Magnani's work identifies specific enhancer usage that marks the transition to a drug-resistant state which was a breakthrough in linking epigenetic alterations to real-world patient outcomes. He emphasizes that the reliance on genetic mutations alone does not adequately explain the mechanisms of drug resistance, pushing the field to consider the epigenetic landscape more deeply. Our conversation also touches on the evolution of experimental techniques. Professor Magnani shares insights into the transition from traditional ChIP-seq methods to CUT&RUN, demonstrating the need for techniques that cater to the limited material available from clinical samples. This adaptability mirrors the dynamic nature of cancer itself, as cells continuously evolve under therapeutic pressure. As we traverse through the complexities of dormancy and reactivation in cancer cells, Professor Magnani enlightens us on the unpredictable nature of tumor behavior. He describes how cancer cells can enter dormant states and how their awakening is influenced by environmental factors, akin to an evolutionary response to stressors, thus revealing the intricate balance between survival and proliferation. In the latter part of the episode, we explore Professor Magnani's vision for the future of breast cancer research, which includes the need for better animal models that mimic human disease. His pursuit of understanding estrogen receptor behavior both in healthy and cancerous cells reflects a holistic approach to cancer biology, aiming to decipher the transition from normal tissue to malignancy. In the sweltering summer of 1895, two boys lived with their mother's decomposing corpse for ten days while playing cricket and going to the seaside — a Victorian crime that shocked even the most hardened of London's East End residents.Join the DARKNESS SYNDICATE: https://weirddarkness.com/syndicateTake the WEIRD DARKNESS LISTENER SURVEY and help mold the future of the podcast: https://weirddarkness.com/surveyIN THIS EPISODE: In the summer of 1895, two young brothers in London's East End harbored a secret so dark that when it was finally discovered, it would become one of Victorian England's most shocking criminal cases. The stench had become unbearable by the time anyone noticed something was wrong at 35 Cave Road. It was July in Plaistow, and the heat made everything worse — the smell of the docks, the horse manure from the market gardens, and whatever was rotting behind the locked bedroom door of the Coombes family home. Aside from that, the whole project is a magnificent work and for sure deserves viewing and attendance. Otherwise, Thomas goes in on a ton of his other roles, including The Acolyte, Boiling Point and Baby Reindeer, as well as celebrating the goodness of Stephen Grahame when the chance arises. A great chat and definitely something for all, especially those of actorial interest.PIP'S PATREON PAGE if you're of a supporting natureDEATH OF ENGLANDINSTAGRAMIMDBXSPEECH DEVELOPMENT WEBSTOREPIP TWITCH • (music stuff)PIP INSTAGRAMPIP TWITTERPIP PATREONPIP IMDBPOD BIBLE Hosted on Acast. This episode features a conversation with Thomas Coombes, Founder and Director of narrative change organization, Hope-based communications. It was recorded in January 2024.Hope-based communications is a simple, open-source tool for anyone to use, as well as a community anyone can join. It centres around a pledge to apply five shifts to any and all communications output. Those shifts are fear to hope, problem to solution, against to for, threat to opportunity, and victims to everyday heroes. It's a pragmatic approach to winning support for policies and advocacy positions by showing how they'll actually work, acting on the principle that we should be focused on building the world we want, setting the agenda with our values, our goals, our mission, rather than reacting to our opponent's frames and actions. Borne of learnings and epiphanies during Thomas' prior roles at Amnesty International and the European Commission, Hope-based communications offers a simple formula for telling stories of our own that contribute to the long-term changes in ideas, attitudes and behaviours, that we want to bring about. Amongst other things, Thomas and I discussed the concept of hope as both active and visionary, how flipping our intuitions inside out and focusing on particular aspects of a story over others can strengthen our output, and how our brains work to trip us up at so many points along the way.Additional links: Visit the hope-based communications websiteCheck out Thomas's TED TalkThomas' article "Hope is Action”Thomas's piece "“A New Green Wave of Hope”Follow Thomas on Substack or LinkedIn“A Message From the Future” from Alexandria Ocasio-Cortez
