Gelb, M.D. is the Director and Gogel Family Professor of the Mindich Child Health and Development Institute at the Icahn School of Medicine at Mount Sinai. He is Professor of Pediatrics and of Genetics and Genomic Sciences. Dr. Gelb completed a pediatric residency and pediatric cardiology fellowship at Babies Hospital of Columbia-Presbyterian Medical Center and Texas Children's Hospital at the Baylor College of Medicine, respectively. He joined the faculty at Mount Sinai in 1991 after fellowship and has remained there since. He developed and now oversees an extensive program in genomics/gene discovery for congenital heart disease. Dr. Gelb has received the E. Mead Johnson Award from the Society for Pediatric Research and the Norman J. Siegel New Member Outstanding Science Award from the American Pediatric Society. He was elected to the American Society of Clinical Investigation and the National Academy of Medicine (formerly, the Institute of Medicine). Dr. Gelb is the President for the American Pediatric Society, Immediate Past President for the International Pediatric Research Foundation and Treasurer-Elect for the American Society of Human Genetics. In addition to his research, he co-directs the Cardiovascular Genetics Program at Mount Sinai.   Dr. Eric Green is the director of the National Human Genome Research Institute (NHGRI) at the U.S. National Institutes of Health (NIH). As NHGRI director, Dr. Green leads the Institute's research programs and other initiatives. He has played an instrumental leadership role in developing many high-profile efforts relevant to genomics. Dr. Green received his B.S. degree in bacteriology from the University of Wisconsin - Madison in 1981, and his M.D. and Ph.D. degrees from Washington University in 1987. Coincidentally, 1987 was the same year that the word “genomics” was coined. Dr. Green's relationship with the Institute began long before his appointment as director. He served as the Institute's scientific director (2002 - 2009), chief of the NHGRI Genome Technology Branch (1996 - 2009) and founding director of the NIH Intramural Sequencing Center (1997 - 2009). Prior to that, he played an integral role in the Human Genome Project. Dr. Green is a founding editor of the journal Genome Research (1995 - present) and a series editor of Genome Analysis: A Laboratory Manual (1994 - 1998), both published by Cold Spring Harbor Laboratory Press. He is also co-editor of Annual Review of Genomics and Human Genetics (since 2005). Throughout his career, he has authored and co-authored over 385 scientific publications.    Dr. Green is a recurring guest on DNA Today, and he might hold the title as the guest who has been on the show the most times! He was featured on Episode #182 when we chatted about the Human Genome Project and the recent completion of the human genome sequence -- from telomere to telomere. Dr. Green was a panelist on the PhenoTips Speaker Series installment that our host Kira Dineen moderated about population genomics in clinical practice, this was also released on the DNA Today podcast feed as Episode #260. The speed with which a medical team responds to an emergency situation can make the difference between life and death, as well as impact the odds of liability. DePinho, M.D. is currently past president and distinguished university professor of The University of Texas MD Anderson Cancer Center. Dr. DePinho studied biology at Fordham University, where he graduated class salutatorian, and received his M.D. degree with distinction in microbiology and immunology from the Albert Einstein College of Medicine. He performed his residency and postdoctoral training at Columbia-Presbyterian Medical Center. Dr. DePinho's independent career began at Einstein as the Feinberg Senior Faculty Scholar in Cancer Research and an ACS Research Professor. He then joined the Dana-Farber Cancer Institute and Harvard Medical School where he was the founding Director of the Belfer Institute for Applied Cancer Science and a Professor of Medicine and Genetics at Harvard. As president of MD Anderson, Dr. DePinho conceived and launched the Moon Shots Program, a goal-oriented comprehensive effort designed to accelerate declines in cancer incidence and mortality. This initiative has yielded practice-changing advances in cancer and inspired the national cancer moonshot under President Biden. He also elevated MD Anderson's research and graduate programs, recruited many world class faculty including National Academy Members and its first Nobel Prize. To improve cancer care beyond MD Anderson, he launched and assembled a network of cancer institutions in 24 countries, reaching one-third of the human population. As a researcher, he has published over 400 articles, books and chapters that have advanced our understanding of cancer, aging and degenerative disorders which have led to clinical advances including new cancer drugs and diagnostics. His most notable discoveries include elucidation of the core molecular pathway for aging, determination of the basis for the intimate link between advancing age and increased epithelial cancer incidence, establishment of the central role of telomeres in cancer genome instability, discovery of Sin3 and linkage of sequence specific transcription factors and chromatin regulators, demonstration that aging itself can be reversed which led to new therapeutic strategies for aging and age-related diseases including Alzheimer's Disease. To translate these basic discoveries, he founded 9 private and publicly traded biotechnology companies and served as an advisor or director for biotech and large biopharma companies focused on oncology. His public service includes advisory and directorship roles for AACR and the NIH, as well as the Vatican and several countries. He is chairman and co-founder a non-profit, OPA Health (previously, Unite to Prevent Cancer) whose mission is disease prevention in underserved communities. As an advocate, he was a key instigator in the bipartisan bill raising the age of tobacco purchase to 21 (T21) and the bipartisan Cures Act. He is Vice Chair of the BOD for Act For NIH, a non-profit advocacy organization to garner bipartisan support for the NIH resulting in an additional $14.9B to the base budget and restoring the NIH budget following years of inflationary decline. For his fundamental contributions to cancer and aging and to healthcare, he has been recognized with numerous honors and awards. He was named one of the 100 most influential healthcare leaders, according to Modern Healthcare. His honors include the Melini Award for Biomedical Excellence, the American Society for Clinical Investigation Award, the Biomedicum Helsinki Medal, Albert Szent-Györgyi Prize, AACR Clowes Award, American-Italian Foundation Prize, among others. He is a member of the National Academy of Medicine and the National Academy of Science, and a fellow of the American Academy of Arts and Sciences, the American Association of the Advancement of Science and the American Association of Cancer Research. He received Fordham University's McMahon Memorial Award for Distinguished Public Service, honorary degrees from Harvard University and Hofstra University, and the Ellis Island Medal of Honor. For his work to improve the health of Portugal's people, Dr. DePinho was recognized by its president with the highest commendation, the Order of Saint James of the Sword. Ginny sits down with board-certified dermatologist & friend, Dr. Mohiba Tareen. In the fall of 2018, Ginny needed Dr. Tareen's help. She had a huge lump growing rapidly above her right eye and her face and eye were swelling terribly. She was concerned. Dr. Tareen was also concerned and was able to see her rather quickly. Upon entering the room, Dr. Tareen diagnosed Ginny with singles...on her face. This seemed unusual since she was only in her 30's, but Dr. Tareen asked right away, "Have you been under a lot of stress?" That's what sparked this episode and the important dialogue around the topic of how stress manifests in our hair, skin & nails. Who better to have this conversation with than an expert?! Ginny wondered if our bodies sometimes know we're under stress before our minds can catch up. Dr. Tareen unpacks how stress can impact the skin with things like shingles, acne, eczema, psoriasis, precancerous lesions...or even melanoma. Ginny also experienced significant hair loss & breakage during this time, as identified by her hair stylist. Dr. Tareen let us know that this is very common, especially in women. She breaks down the 3 different phases of hair growth and how they play a part: telogen, anagen & catagen. Dr. Tareen didn't just talk about the issues, but also provided solutions to all of these issues. In nails, you may also experience breakage and Dr. Tareen shared that picking cuticles & skin is also common when people are under stress. And of course, it wouldn't be a complete conversation with a board-certified dermatologist if we didn't talk prevention (your skin is your largest organ!) and sun protection. Dr. Tareen unpacked the best sunscreens and that you need to protect your skin from both UVA (aging) and UVA (burning) rays and the best way to do this is with a physical block, like a zinc. Barry E. LoSasso, M.D. was born in New York City and received his undergraduate degree from Yale University. He attended medical school at the University of Florida and completed his fellowship in pediatric surgery at Babies Hospital, Columbia-Presbyterian Medical Center in 1984. Dr. LoSasso started his pediatric surgical career practicing at Kaiser Permanente Hospital in San Diego, California. From there he opened his private practice in 1987, which continues today. In early 2017, Dr. LoSasso joined the Pediatric Surgical Associates, P.A. in Paramus, New Jersey. In addition, Dr. LoSasso served as the Director of Trauma Services at Rady Children's Hospital- San Diego from February 1998 until June of 2006, Chairman for the Safe Kid's Coalition in San Diego from 2001 until 2006, and Scientific Coordinator for the Fourteenth Annual Pediatric Critical Care Colloquium in San Diego in October 2002. Dr. LoSasso has spent extensive time working with world renowned, Dr. Donald Nuss, creator of the Nuss Procedure, at the Children's Hospital of the King's Daughter in Norfolk, Virginia. Dr. LoSasso has devoted much of his practice to the research and performance of the minimally-invasive Nuss Procedure for the correction of Pectus Excavatum( also known as sunken chest), developing the successful Center of Excellence for Pectus Deformities in 2006.

Dr. Wendy Chung is the Herbert Irving Associate Professor of Pediatrics and Medicine and Director of Clinical Genetics at Columbia University. As a human molecular geneticist, she seeks out rare and unexpected causes behind health problems in people. Her laboratory works to identify genes that cause human diseases and use this information to inform the creation of novel treatments in the future. Wendy spends most of her free time with her family. She has two sons, and they like to get outside to go hiking, swimming, and bicycling. They can also frequently be found working through puzzles and going on mystery scavenger hunts together. She received her undergraduate training at Cornell University and went on to receive her PhD in Molecular Genetics from Rockefeller University and her M.D. from Cornell University. Afterward, Wendy completed her Internship and Residency in Pediatrics, a Fellowship in Clinical Genetics, and a Fellowship in Molecular Genetics at the Columbia Presbyterian Medical Center. Wendy is the recipient of an American Academy of Pediatrics Young Investigator Award, the Charles W. As we spotlight this April Parkinson's Awareness Month, I am so excited to welcome, Dr. Rebecca Gilbert, Chief Scientific Officer, American Parkinson Disease Association (APDA)   Dr. Rebecca Gilbert joined APDA in 2018, bringing a wealth of practical experience in diagnosing and treating Parkinson's disease, as well as a strong background in fundamental and clinical science to the team. She is responsible for overseeing APDA's research portfolio in conjunction with APDA's Scientific Advisory Board, and also provides medical and clinical expertise to support APDA programming as well as print and web content.   Dr. Gilbert received her MD degree at Weill Medical College of Cornell University and PhD in Cell Biology and Genetics at the Weill Graduate School of Medical Sciences. She then completed her Neurology Residency training as well as Movement Disorders Fellowship training at Columbia Presbyterian Medical Center. In this episode, we interview Dr. Steven Styllianos. Dr. Stylianos serves Columbia University as the Rudolph N Schullinger Professor of Surgery and Pediatrics and Chief of the Division of Pediatric Surgery. He is currently the Surgeon-in-Chief of the Morgan Stanley Children’s Hospital/New York Presbyterian.A graduate of Rutgers University and the New York University School of Medicine, Dr. Stylianos completed his general surgical training at Columbia–Presbyterian Medical Center. He subsequently spent two years as the Trauma Fellow at the Kiwanis Pediatric Trauma Institute in Boston and then completed his formal pediatric surgery training at Boston Children’s Hospital. Dr. Stylianos joined the faculty of Columbia University College of Physicians and Surgeons and the Children’s Hospital of New York in 1992. He organized and directed the 50-member team of physicians and nurses who separated conjoined twins in 1993, 1995, 2000 and 2020. These conjoined twins separations attracted the attention of the national media, including Dateline NBC, CBS 48 Hours and Fox News.Throughout the years, Dr. Stylianos has served as Chairman of the Trauma Committee for the American Pediatric Surgical Association (APSA) from 1997–2002 and authored the APSA position paper supporting all measures to reduce the toll of firearm violence in children. He also served as the Co-Principal Investigator of the U.S. Department of Health, Maternal and Child Health Bureau’s grant to APSA “Partnership for Development and Dissemination of Outcomes Measures for Injured Children.”Currently, Dr. Stylianos is a site verification officer of the American College of Surgeons Committee on Trauma and was recently elected to serve on the American Pediatric Surgical Association Foundation’s Board of Directors. He is also Editor-in-Chief of the Journal of Pediatric Surgery Case Reports, Associate Editor of the Journal of Pediatric Surgery and served on the Executive Board as a founding member of the Pediatric Trauma Society. Dr Stylianos recently received the prestigious American Pediatric Surgical Nurses Association’s 2016 Champions Award and the American Trauma Society’s 2016 NY State Trauma Medical Director of Distinction.And most importantly, Dr Stylianos and his wife Joann are proud first-time grandparents to to their grandson Nico. What is Mitochondrial Myopathy? my·op·a·thy mīˈäpəTHē/ The word “myopathy” means disease of the muscle tissue. As the term implies, mitochondrial myopathy (MM) is a neuromuscular disease caused by damage to the mitochondria. Many patients with mitochondrial disease have a mitochondrial myopathy, either as their sole diagnosis or as an additional, descriptive co-diagnosis as part of their mitochondrial disorder. Mitochondrial myopathy may be present in adults and children, and may occur with or without a genetic mitochondrial disease diagnosis. Further, several clinical trials are currently examining the impact of various therapies or potential treatments for people with mitochondrial myopathy. Join us this month on Friday, November 20th at 12 pm EST with Dr. Bruce Cohen, Director of Pediatric Neurology and then the Director of the NeuroDevelopmental Science Center at Akron Children’s Hospital. Dr. Cohen is well-known and highly respected as an expert author, speaker, clinician and research investigator in mitochondrial disorders. Cohen attended college at Washington University in St. Louis and a BA in chemistry, graduating summa cum laude in 1978. He received his medical degree from the Albert Einstein College of Medicine in Bronx, N.Y. in 1982 and went on to his pediatric residency at the Children’s Hospital of Philadelphia, followed by a residency in neurology at Columbia Presbyterian Medical Center in New York. He then obtained a two-year American Cancer Society fellowship in neuro-oncology at the Children’s Hospital of Philadelphia. In 1989 Dr. Cohen joined the Cleveland Clinic’s department of Neurology, and served as chief of Pediatric Neurology from 1999-2002, with joint appointments in the Clinic’s Taussig Cancer Center, the Eepartment of Neurosurgery and the department of Pediatrics. In 2011, he joined Akron Children’s Hospital in Akron, Ohio, as Director of Pediatric Neurology and then the Director of the NeuroDevelopmental Science Center. He serves in many leadership roles within Akron Children’s Hospital. His specialty interests include adult and pediatric neuro-oncology, mitochondrial medicine, neurofibromatosis, neurometabolic diseases and pediatric neurology. He has served on a number of committees for the Children’s Oncology Group, the American Academy of Neurology (AAN), the Child Neurology Society. Anne Curtis, MD, is the Charles and Mary Bauer Professor and Chair of the Department of Medicine at the Jacobs School of Medicine and Biomedical Sciences and SUNY Distinguished Professor at the University at Buffalo. Dr. Curtis completed her medical school at Columbia University, her residency in internal medicine at the Columbia Presbyterian Medical Center, and then she went on to Duke University to pursue fellowships in cardiovascular disease and clinical cardiac electrophysiology. Dr. Curtis maintains an active clinical practice, with a focus on cardiac electrophysiology, and she has been involved in the development of national guidelines for the treatment of atrial fibrillation and ventricular arrhythmias. She has been involved in clinical trials for over 25 years with over 300 publications, and she serves as an associate editor for the Journal of the American College of Cardiology. At a national level, she has held numerous leadership roles, including serving as the President of the Association of University Cardiologists, President of the Heart Rhythm Society, and Chairing the ACC's Clinical Electrophysiology Committee and the FDA's Circulatory System Devices Panel. How can we become our own best advocate? Today, Dr. Anne Curtis explains that the best way to find more opportunities is to go after them with all the enthusiasm we've got. She shares stories throughout her career and recalls that she's always felt highly motivated to work hard, get things done, and prove herself in a career in medicine. She advises us to take the opportunities we are presented with, even if they seem daunting at first. It is when we prove ourselves to our mentors and show them that we're willing to do the work, that they build more trust in us—and offer us more and more opportunity. Dr. Curtis explains that—especially in these early years of training—our one job is to become good physicians. She leaves us with this: “If you want to do anything that involves patient care and research, you better be a darn good doctor.” Pearls of Wisdom: 1. Be your own best advocate. Opportunities may or may not come—it's up to you to seek them out. 2. Find role models for potential. Look for the mentors that you can see yourself in, and realize that their potential also exists in you. When you follow in their footsteps, you'll achieve greater and greater things. 3. The mark of a good mentee is when a supervisor trusts you when they're not there. Develop trust and prove that you can handle things on your own, that you know what you're doing, and that you're willing to do things the right way.

IN THIS EPISODE: It can be hard to make tough decisions even in the best of circumstances. It’s even more challenging in times of crisis when you or a loved one are suffering with a chronic or potentially life-threatening illness or injury.  It can be overwhelming and it’s easy to feel lost. It's the job of hospital chaplains to support patients and family members in navigating these trying times and helping them make what can be some of the most difficult decisions they will ever face. Today’s guest, Ted Taylor, is a chaplain and Director of Pastoral Care & Training at Robert Wood Johnson University Hospital Hamilton.  Ted supervises the spiritual and emotional care provided by the hospital’s more than twenty volunteer and intern chaplains.  He also supervises a clinical pastoral education training program affiliated with the College of Pastoral Supervision and Psychotherapy (CPSP).  Ted is a liaison to nearby religious communities and partners with area congregations to provide compassionate support to neighbors in need.  He also is a clinical resource for bioethical and end-of-life dilemmas. Ted holds a diplomate in pastoral supervision through CPSP, and is board certified as a clinical chaplain with CPSP, with a clinical fellowship in hospice & palliative care.  Ted is also a member of the Association of Professional Chaplains, the Association of Clinical Pastoral Education, and the American Society for Bioethics & Humanities. He details the many ways that oral health is overall health, in that what happens in the mouth can impact the entire body. He stresses the importance of taking care of teeth and gums and making regular appointments for care. Specifically, he discusses the various diseases that can arise and the treatments and procedures that can be implemented to combat them. He states that bleeding gums, when we brush or floss, can be an indicator that something is going on, for it is not natural for bleeding to occur. Additionally, he says that sensitivity issues to cold, etc. are a possible indicator, and should be considered for further review. Odors and bad breath are also indicators that perhaps one should schedule an appointment right away, or simply increase the regularity of their appointment scheduling. The cosmetic dentist provides an overview of the technical aspects of reviewing teeth, specifically x-rays and a newer process called transillumination. A dental transillumination test can reveal problems in teeth that could be difficult or impossible to see with the naked eye or even with x-rays. Decay will appear in the image as cloudy shadows. Healthy tooth structure will appear lighter, because light shines through it more easily. A dental transillumination might also expose fine cracks within the teeth. He explains how some of the newer viewing options provide greater detail and more accurate information, whereas traditional x-rays may sometimes make it difficult to clearly tell whether something is a cavity or just a shadow. Dr. Chase discusses new scientific discoveries that show that bacteria in the mouth varies greatly between people, and how some bacteria is extremely more destructive than other bacteria. He details how some newer mouthwashes can directly impact the flora in the mouth and make it less destructive. He stresses the importance of using mouthwash that contains no alcohol because alcohol dries out the mouth, and can reduce saliva, which is a natural protector of the teeth. The dental expert answers questions about his particular approach to dentistry. He talks about increasing frequency of visits, and how the increase can help curb potential problems. Additionally, he discusses products such as electric toothbrushes and more advanced mouthwashes. He underscores the importance of brushing for at least two minutes, which may seem like a long time but it is necessary for thorough cleaning. The benefits of electric toothbrushes are many, but some of the top points he discusses are: thoroughness of cleaning and proper time allotment. Dr. Chase points out that electric toothbrushes tend to give their users a better cleaning overall, and some of the models have timers that allow the user to know when it is time to move on to another section of the teeth. Further, he talks about teeth whitening and the products that have been proven to be safe, as well as those products that are questionable in regard to safety. Dr. Chase served as a clinical instructor at New York University Dental School where he was responsible for the training of undergraduate dental students, as well as doctors from many countries around the world, on the current techniques in restorative dentistry. Dr. Chase is a faculty mentor at Spear Advanced Dental Education Center in Scottsdale, AZ. He went on to complete his advanced specialty training in Prosthodontics at The Goldman School of Graduate Dentistry-Boston University.   Steven was born in New York City and raised, the oldest of four children, in Oradell, NJ (near the George Washington Bridge).   Steven enjoys spending time with Traci, his wife, and their kids. Listen NowAs a follow up to my April 1st conversation with Jim Gera concerning bundled payments, during this podcast Dr. Richard Morel discusses Medicare's other major payment reform program, Accountable Care Organizations (ACOs), or WESTMED Medical Group's three year experience as a Track 1 ACO.   The Medicare ACO program is a creation of the 2010 Accountable Care Act and participation in the program began in 2012.   Currently, there are 434 ACOs (over 90 percent participating in the "no risk" Track 1) caring for approximately 7.5 million Medicare beneficiaries.  The program to date has been a mixed success.  After two performance years (2013 and 2014) only 25 percent of participants have been successful, i.e., have earned shared savings.  (Performance year three or 2015 performance will be made known this September.)  CMS is currently in the process of revising how the agency calculates an ACO's reset financial benchmark.  It is anticipated these changes will improve program performance, or improve both provider interest in participating (or continuing to participate) in the program and participant success in earning shared savings.   During this 21-minute conversation Dr. Morel provides an overview of WESTMED, explains the organization's interest in becoming a Medicare Shared Savings Program or ACO participant in 2013, WESTMED's experience under their first three year agreement, what explained their success, challenges they've found with the program, how the program could be improved and their expectations now as a second agreement period Track 1 ACO.Dr. Richard Morel is the Co-Medical Director of WESTMED Medical Group in Yonkers, New York.  Prior to joining WESTMED in 2008 Dr. Morel was in private practice affiliated with Columbia-Presbyterian Riverdale Hospital for 12 years.  Dr. Morel is board certified in internal medicine.  He received his medical degree from Columbia University College of Physicians and Surgeons, did his postgraduate training at Columbia-Presbyterian Medical Center and  received his masters of medical management from Carnegie Mellon.  He is a fellow of the American College of Physicians and a member of the American College of Physician Executives.  For information regarding WESTMED go to: http://www.westmedgroup.com/. This is largely because Americans suffer more disease/disease burden throughout their life spans compared to individuals in other industrialized countries.   This therefore makes obvious sense since primary care is the foundation upon which an effective and efficient health care/medical care program is built.  When done well primary care promotes wellness, prevents disease onset, progression, exacerbation and prevents premature death.  Primary care also moderates the need for higher cost specialty care and improves population health.  For numerous reasons, not least of which is inadequate reimbursement, primary care delivery has been sub-optimal.   New models of primary care are emerging, one termed direct primary care (noted in the ACA under Section 1301 (A) (3) and now recognized in 13 states) is showing promise in improving quality, improving patient satisfaction and lowering cost growth.      During this 21 minute conversation Dr. Judge discusses moreover the impetus for the creation of Iora Healh, how Iora's primary care delivery model works or how it is different from traditional primary care delivery, how Iora's model is staffed, IT supported and reimbursed, with whom and how it contracts and what Iora's performance data demonstrates to date. Dr. David Judge serves as Iora Health's Chief Medical Officer.  Dr. Judge joined Iora in 2014 to continue his work in improving and redesigning of primary care.  Priorto , he helped found and was the Medical Director of the Ambulatory Practice of the Future at Mass.
