Nidhi Bouri, DAA at USAID Bureau for Global Health, joined us to speak to the U.S. response to the dangerous mpox outbreak (clade 1b) centered in the Democratic Republic of Congo, boosted by President Biden's commitment at UNGA to $500m in support, including 1 million vaccine doses. Much better data is urgently needed on the needs for diagnostics and vaccines. Tensions remain high among Africa CDC, WHO, and other key institutions with proven response capability, most notably Gavi, UNICEF and the Global Fund. Much is not known about modes of transmission, and the durability and efficacy of the Jynneos vaccine for clade 1b. As the virus inevitably lands in the United States, communications will be critical. Some important progress was seen in the High Level Meeting on anti-microbial resistance. The Marburg outbreak in Rwanda is of acute concern for multiple reasons: no vaccine, little testing, little knowledge of the pattern of spread. It is crunch time, as multiple replenishments converge. “Let's be clear, there is not enough money.”

Dr. Lisa Law and Dr. Randy Taplitz share the latest evidence-based recommendations from ASCO on vaccines in adults with cancer. They discuss recommended routine preventative vaccinations, additional vaccinations and revaccinations for adults undergoing HSCT, CD19 CAR-T treatment, or B cell-depleting therapy, guidance for adults with cancer traveling outside the U.S., and recommendations for vaccination of household and close contacts of adults with cancer. My name is Brittany Harvey, and today, I am interviewing Dr. Lisa Law from Kaiser Permanente and Dr. Randy Taplitz from City of Hope Comprehensive Cancer Center, authors on “Vaccination of Adults with Cancer: ASCO Guideline.” Thank you for being here, Dr. Law and Dr. Taplitz. Dr. Lisa Law: Thank you. Dr. Taplitz: Thank you, Brittany. Brittany Harvey: Before we discuss this guideline, I'd like to take note that ASCO takes great care in the development of its guidelines and ensuring that the ASCO conflict of interest policy is followed for each guideline. The disclosures of potential conflicts of interest for the guideline panel, including Dr. Taplitz and Dr. Law, who have joined us here today, are available online with the publication of the guideline in the Journal of Clinical Oncology, which is linked in the show notes. So then, to dive into the content, here first, Dr. Taplitz, can you provide a general overview of both the scope and purpose of this guideline on vaccination of adults with cancer? Dr. Randy Taplitz: Yes, so people with cancer often experience a compromised immune system due to a variety of factors. This includes chronic inflammation, impaired or decreased function of the hematopoietic system, and treatments that compromise their immune function. Because of this, people with cancer are at a higher risk for infection, including with vaccine-preventable diseases. Also, response to vaccines in patients with cancer may well be affected by this underlying immune status, and their anticancer therapy, as well as the severity of the underlying malignancy. The purpose of vaccination in this group of patients is to prevent infection or to attenuate the severity of the disease when infection cannot be fully prevented. This ASCO review builds on a 2013 guideline by the Infectious Diseases Society of America, or IDSA, and uses what's called a systematic literature review of 102 publications between 2013 and 2023. This includes 24 systematic reviews, 14 randomized clinical trials, and 64 non-randomized studies. The largest body of evidence in these studies, not surprisingly, addresses COVID vaccines on the efficacy and safety of vaccines used by adults with cancer or their household contacts. ASCO convened an expert panel to review this evidence and formulate recommendations for vaccinations in this population. Brittany Harvey: Understood. I appreciate that context, Dr. Taplitz. So then, next, Dr. Law, I'd like to review the key recommendations of this guideline. The guideline addresses four overarching clinical questions. So starting with the first question, what are the recommended routine preventative vaccinations for adults with cancer? Dr. Lisa Law: Thank you, Brittany. Before I start, I just want to wholeheartedly thank the first author of this paper, Dr. Mini Kamboj, Dr. Elise Kohn from the NCI, as well as the ASCO staff in putting this publication and guideline together. It is a very, very important guideline, and I echo everything Dr. Taplitz just said. So going back to your question, what are the recommended routine preventative vaccines for adults with cancer? As per this guideline, there are about 7 to 8 based on patient age and risk. Namely, they are: seasonal flu, RSV for those aged 60 or above, COVID-19, Tdap, Hepatitis B, Shingrix, Pneumococcal vaccine, and the HPV vaccine. These vaccines should ideally be given two to four weeks before therapy. However, non-live vaccines can be given anytime during or after chemo, immunotherapy, hormonal treatment, radiation, or surgery. Brittany Harvey: Excellent. Thank you for reviewing those vaccinations and the timing of them as well. So then, following those recommendations, Dr. Taplitz, what additional vaccinations and revaccinations are recommended for adults undergoing hematopoietic stem cell transplantation, CD19 CAR-T treatment, or B-cell depleting therapy? Dr. Randy Taplitz: Many studies have shown that stem cell transplant recipients essentially lose immunity from childhood immunizations, and we know that these individuals are very vulnerable to infection, particularly in the first year after transplant. Revaccination is critical to help restore their immunity. The optimal timing of vaccination is based on our understanding of adequate immune reconstitution with B and T-cell recovery so that the individual can mount a response to the vaccine. We know that a lot of factors influence this immune reconstitution, including the age of the stem cell transplant recipient, the source of the donor, the time from transplant, graft-versus-host disease prophylaxis, the treatment and severity of graft-versus-host disease, and the vaccine type and antigens used. There are a number of bodies throughout the world, IDSA as I mentioned, CDC, American Society for Transplant and Cellular Therapy, European Society for Blood and Marrow Transplant, and European Conference for Infections and Leukemia. All of these bodies have guidelines that approach vaccination in stem cell transplants. However, variation does exist in the use of a variety of things including whether to use immune predictors to help guide vaccination, and there is really not consensus on whether this immune predictor guided vaccination is more likely to produce a protective immune response versus a standardized schedule. In addition, the duration of protection is incompletely understood. The bottom line in these guidelines is that they recommend complete revaccination starting for most vaccines at 6 to 12 months after stem cell transplant, in order to restore vaccine-induced immunity. And I just want to go through a few of the particulars. For COVID-19, which is a three-dose series in the primary series, influenza - generally high-dose influenza - and pneumococcal vaccine, PCV20 in general, ultimately four doses, can be administered, starting as early as three months after transplant. Although there is really not much data to guide the use of the recombinant zoster vaccine in allogeneic stem cell transplant, the vaccine can be administered after the end of antiviral prophylaxis, which in general is 12 to 18 months after allogeneic and 3 to 12 months after autologous stem cell transplant. Some of the other vaccines, such as hepatitis B, Tdap, meningococcal vaccines, and HPV revaccination in those less than 45 are also recommended. I want to also spend the moment talking about the two recently licensed RSV vaccines, which were essentially studied in less compromised hosts and really without any immunogenicity data in stem cell transplant, and thus, there is no recommendation in this guideline for the use of these vaccines after transplant. Live vaccines, such as MMR and varicella – varicella would be in varicella-seronegative patients without a prior history of varicella – should be delayed for at least two years and only given in the absence of active graft-versus-host disease or immunosuppression. Moving briefly to CAR T, which is an immunotherapy that involves adoptive cell therapy, given the available data and after a review by the group, it was recommended that adults with hematopoietic malignancies receiving CAR T therapy directed against B-cell antigens should receive influenza and COVID-19 vaccines either two weeks before lymphodepletion or no sooner than three months after the completion of therapy. Administration of non-live vaccines preferably should occur before CAR T treatment or at least 6 to 12 months after, following the same timing as what we recommend for stem cell transplant. There is really little data to guide the safety and timing of administration of live vaccines after CAR T therapy. In terms of adults receiving B-cell depleting therapy, they are generally unable for time to mount an effective humoral response but may have at least partially intact cellular immune responses. They are encouraged to be revaccinated for COVID-19 no sooner than six months after completion of B-cell depleting therapy, and they should receive influenza vaccine approximately four weeks from the most recent treatment dose for patients on chronic therapy. For other non-seasonal immunizations, vaccines ideally should be given two to four weeks before commencing anti-CD20 therapy or delayed until 6 to 12 months after completion, except for the recombinant zoster vaccine, which can be given one month after the most recent dose of B-cell depleting therapy. Brittany Harvey: I appreciate you reviewing each of those vaccinations and when they should be given, and reviewing the available data – albeit, limited data – in these situations. So beyond these routine preventative vaccinations and revaccinations that you've both just described, Dr. Law, what additional vaccinations does the expert panel recommend for adults with cancer traveling outside the United States? Dr. Lisa Law: Good question. As per these ASCO guidelines, adults with solid or blood cancer traveling outside of the United States should follow the CDC standard recommendations for their destination. For the 2024 CDC Yellow Book, travel vaccines, in general, should be delayed until three months from the last chemotherapy or, and for those with solid tumors, ideally when the disease is in remission. Of note, hepatitis A, typhoid, inactivated polio, Hep B, rabies, meningococcal vaccine, and Japanese encephalitis vaccines are considered to be safe. In all cases of travel, patients should be counseled by their healthcare provider about the travel timing, with the additional attention to the regional seasonality of infections, for instance, influenza is more common in late summer in Australia, and also with attention to any outbreaks that may be occurring globally at the time of travel. Brittany Harvey: Absolutely. Those are key points for clinicians to discuss with their patients as they consider upcoming travel. So then, the final clinical question that the panel addressed, Dr. Taplitz, what vaccinations does the panel recommend for household and close contacts of adults with cancer? Dr. Randy Taplitz: Thank you. Yes, it is recommended that all household members and close contacts, when possible, be up to date on their vaccinations. And the only further thing I would say is that there are some special considerations for the use of live vaccines in household contacts, particularly in stem cell transplant recipients. Contacts of people who receive stem cell transplants should preferably receive inactivated influenza vaccines. As was mentioned, MMR and varicella vaccines are both safe to administer to close contacts. Vaccine strain transmission to immunocompromised hosts has not been associated with MMR use in family members. Eleven cases of the varicella vaccine strain transmission are described in the published literature, but none occurred in compromised hosts. Because the vaccine strain can cause severe and fatal varicella in profoundly immunocompromised people, precautions are advised to avoid close contact with a person with a vaccine-induced rash. For household contact travelers, MMR and yellow fever vaccines are considered safe. Oral cholera should be avoided. For smallpox vaccines, the second-generation ACAM2000 has rarely been associated with vaccinia transmission and should be avoided because of this. But the live replication-deficient MVA-based JYNNEOS vaccine is felt to be safe for household contacts of immunocompromised individuals. Brittany Harvey: I appreciate you reviewing the importance of vaccination for household and close contacts, and some of those precautions that individuals should take. I appreciate you both for reviewing all of these recommendations. So then in your view, Dr. Law, what is the importance of this guideline, and how will it impact both clinicians and adults with cancer? Dr. Lisa Law: In my opinion, this is a very important guideline that is long overdue in the oncology community and will have a huge impact on both clinicians and adults with cancer. Over the years, I have often been asked by my colleagues and patients, “Can I have the flu vaccine, and if so, when?” So this guideline really is going to be helpful. More importantly, our cancer patients are living much longer. They may have years of quality of life even with third or fourth line of treatment, especially, for instance, like CAR T for myeloma and lymphoma. However, we know that with additional treatment, that carries a substantial risk of infection complication among these immunocompromised patients. So it is of paramount importance to inform our patients and colleagues to be proactive in advocating preventive therapy ahead of time, meaning trying to get the patients appropriately vaccinated as early as possible to generate immunity. Another case in point is the Shingrix vaccine. I used to see lots of shingles, but ever since we have the recombinant Shingrix, I have fewer encounters. And this is huge because post-herpetic neuralgia robs a patient's quality of life. So, again, it is very important to recommend appropriate vaccines for our cancer patients. Brittany Harvey: Absolutely. It is key to ensure patients receive these preventative vaccines, and we hope that this guideline puts an emphasis on that for clinicians and patients. So finally, to wrap us up, Dr. Taplitz, what are the current gaps in knowledge regarding the vaccination of people with cancer? Dr. Randy Taplitz: There are a number of really important gaps in knowledge and really critical unmet needs that require research and other dedicated efforts. Among these are, and I think paramount, are really the participation of people with cancer with varied types of immunocompromise in vaccine trials. Where vaccine trials are only for cancer patients, obviously is ideal, testing vaccines in the appropriate population. But when that's not feasible, pre-existing cancer should not preclude eligibility, and inclusion of cohorts of people receiving anticancer treatment should be incorporated prospectively. So that's really critical because the quality of our guidelines is based upon the data. We use the data for developing guidelines and gathering more data in the particular patient population is really, really critical. Secondly, work for creating more immunogenic vaccines and research to understand the immune response to vaccines after immuno-depleting therapies, particularly with newer therapies such as CAR T and newer B cell therapies, bispecific antibodies, etc. is really critical. We need to really understand the immune response and have the most potent vaccines available to these people who may have impaired immune responses. Switching gears a little bit, we really need mechanisms to promote institutional commitment to integrate and sustain immunization best practices for people with cancer. This will largely be through multidisciplinary, team-based approaches, protocol-based vaccination standing orders, and leveraging data sharing so that we can all be on the same page with giving vaccines to these individuals. We also need education and evidence-based decision-making tools, emphasizing preventive care through immunization, the availability of educational resources to clinicians and patients to address commonly asked questions and also misconceptions about vaccination, that's absolutely critical. And finally, I think we need to develop strategies for addressing unique challenges and factors contributing to vaccine hesitancy during cancer therapy. We need to focus on patient and clinician communication, and very importantly, we need to consider health equity considerations in the development and approach to vaccines in these compromised patients. Second, more than 33 million children in Africa are missing a measles vaccine, highlighting the urgent need to increase immunization efforts to prevent outbreaks and deaths. Third, the JYNNEOS vaccine offers protection against hospitalization among people with mpox, including people with HIV. Finally, norovirus was the main cause of illness among more than two dozen hikers on Washington's Pacific Crest Trail.

Second, COVID-19 vaccines protect young children against COVID-19-associated emergency department and urgent care visits. This episode discusses four MMWR reports. First, although the number of mpox cases has decreased since the peak of the U.S. outbreak in August 2022, the risk for future outbreaks remains. Clinicians need to be alert for new cases and people at risk should continue to take prevention measures. Second, a new report looking at data from 12 U.S. jurisdictions shows the JYNNEOS vaccine is effective at preventing mpox in people at high risk for mpox. Third, a study of mpox patients in New York provides additional evidence showing the JYNNEOS vaccine is highly effective against mpox. Finally, a new report looks at which occupations had the highest, or lowest, percentages of workers with disability.

Vidcast: https://youtu.be/QqX4vyzngIk The shortage of the available Danish monkeypox vaccine Jynneos has driven the CDC to recommend vaccination with one-fifth the originally recommended dose injected between the layers of the skin rather than beneath the skin. Some scientists are pushing back on the plan claiming that the so-called intradermal vaccination route is not as effective. The literature pushes back on this notion. As early as 1980, a Swedish study on the typhoid vaccine showed that the intradermal route generated comparable protective antibody titers when compared with the subcutaneous route. The added benefits of intradermal vaccinations: 7.5 fold lower incidence of local skin and general febrile reactions as well as a 3.7 fold lower incidence of severe local skin reactions. More recently, in 2004, Dr. Anthony Fauci himself co-authored a New England Journal article about stretching available doses of flu vaccine using a smaller dose of vaccine administered intradermally rather than intramuscularly. He points out that intradermal vaccination may actually induce a stronger immune response than intramuscular vaccination when used for not only flu vaccinations but also for those against hepatitis B, rabies, and other infectious disease, If you need a monkeypox vaccination, get the intradermal jab. The Hawaii Department of Health on Thursday reported two additional cases of monkeypox, bringing the total number of infections in an ongoing outbreak to 18. The additional cases include an Oahu resident whose case is still being investigated and a non-resident who was diagnosed on Oahu and had a recent history of travel outside Hawaii. “While the risk to most Hawaii residents remains low, local transmission of monkeypox is occurring,” said Deputy State Epidemiologist Dr. Nathan Tan, in a statement. “The growing number of cases in Hawaii underscores the importance of vaccination—if you are eligible, please take this step to protect yourself and our community.” The DOH is reminding residents that the JYNNEOS vaccine is available to those who are eligible:See omnystudio.com/listener for privacy information.

New authorization for intradermal administration of monkeypox vaccine; a new approval for endometriosis pain; Amazon is among the companies to receive an FDA Warning Letter for selling unapproved drugs; a diabetes drug is found to contain a possible carcinogen; and a new scale that measures peripheral impedance and pulse rate.
Imagine this: A wedding photographer booked a couple earlier this year. As the countdown grew closer to the big day, the monkeypox outbreak spread across the country, public health emergencies were declared, and the client spirals into a downright panic. The photographer opens up their email to find their client trying to get out of their contract due to the “national emergency”. Despite the photographers' best customer service skills, the couple keeps persisting and states that the situation is Force Majeure- they can't continue the event as planned and need to cancel ASAP. What should the photographer do? Do they have to cancel the event, refund the client, and take a huge loss? What legal leg do the clients actually have to stand on and ultimately…when IS a national public health emergency a valid cause for cancellation? Recently, The Legal Paige Facebook Community has been getting TONS of questions regarding cancellations, public health emergencies, and monkeypox (cue horrific flashbacks to 2020). As an attorney, I can assure you that not every situation is a Force Majeure event (even if your client tries to say it is). This podcast episode will shed light on how to navigate public health emergencies, what constitutes a real Force Majeure event, and the tools you need to have in your legal toolkit to navigate situations like monkeypox moving forward (Covid-19 PTSD not included). PUBLIC HEALTH EMERGENCIES + MONKEYPOX A Public Health Emergency is declared by the Secretary of Health and Human Services when they have determined a disease that may be of public concern. Most Public Health Emergencies have an expiration of 90 days but can be extended if the Secretary feels appropriate. When a Public Health Emergency is declared it opens up more funding for vaccinations, a quicker response time, and additional resources (i.e. it fast tracks the issue). On August 4th, U.S. Department of Health and Human Services Secretary Xavier Becerra declared Monkeypox a Public Health Emergency. Monkeypox is defined by the CDC as “a rare disease caused by infection with the monkeypox virus. Monkeypox virus is part of the same family of viruses as variola virus, the virus that causes smallpox. Monkeypox symptoms are similar to smallpox symptoms, but milder, and monkeypox is rarely fatal.” If Monkeypox is a rare disease and is rarely fatal you are probably wondering why the White House decided to declare it a Public Health Emergency. In the last few weeks the US has seen a significant uptick in cases that has caused some concern. At the time of recording, there were 7,510 cases in the United States with numbers on the rise. There is a vaccination for Monkeypox called JYNNEOS, but there is a limited supply which is causing a shortage in hospitals and pharmacies. By declaring Monkeypox a Public Health Emergency the Biden Administration put the disease in the #1 line for funding and it prioritized the production of the JYNNEOS vaccine. WHAT ACTUALLY QUALIFIES AS A FORCE MAJEURE EVENT? During the beginning stages of the Covid-10 pandemic, many clients were able to cancel their contracts for their events by enacting the Force Majeure Clause. They were able to do this because it was unforeseeable, unpredictable, and the contractual obligations were impossible to perform (those three things have to be true to be able to enact Force Majeure). With clients beginning to invoke Force Majeure again for the latest public health crisis, it's important to note that Monkeypox does not qualify as a Force Majeure event. In fact, MOST Public Health Emergencies don't. Force Majeure was created for out of the blue, “acts of god” like tornadoes, hurricanes, wildfires, terrorism, war– events that no one could have imagined or anticipated. The beginning of Covid-19 qualified as Force Maejure because the world was unpredictably shut down. Nobody could leave their house. There was an inability for contracts to be executed. Even if your clients wanted to continue with their wedding in say April of 2020, they literally could not due to the shutdowns across the United States. That ‘impossibility of performance' we saw with Covid-19 is what you need to keep in mind when new public health emergencies pop up. .Is it impossible for you to perform your contractual obligations because of the emergency? If so, you could probably enforce Force Majeure. If you CAN perform your obligations then you are just in general cancellation territory. Vidcast: https://youtu.be/utqGh5aJvwg Monkeypox, the orthopox viral infection typically only occurring in Africa, is rapidly spreading around the world. As of August 4th, the total number of worldwide cases was 26,864 with all but 345 occurring in countries where this disease has not been previously reported. Though not usually fatal, at least 77 Africans have died and an additional 5 deaths have been reported outside of Africa. The total number of confirmed US monkeypox cases as of August 4th is 7102 with cases occurring in every state except Montana and Wyoming. Most current US cases associated with male-male sexual intimacy. The states hit the hardest are New York, Florida, Illinois, Texas, and California. The last major US outbreak occurred in 2003 with 47 confirmed or probable cases in 6 states, Illinois, Indiana, Kansas, Missouri, Ohio, and Wisconsin, traced to infected pet prairie dogs that had been housed near imported small mammals from Africa. Then, in 2021, two isolated cases of monkeypox were diagnosed in Maryland and in Texas when US residents contracted the disease on trips to Africa. Current efforts to stop the monkeypox epidemic include vaccination and use of anti-viral agents. A vaccination drive using the Danish monkeypox vaccine Jynneos or the older vaccinia viral vaccine ACAM2000 is underway but hampered by a vaccine shortage. The available supply is being optimized by only administering the first of 2 recommended doses, and virologists estimate that even this one dose offers 2 years of protection. The best defense is avoidance of person-to-person contact with a monkeypox-infected person or their personal articles as the virus can live for weeks on objects. Those who have a social or occupational exposure to monkeypox including medical teams and diagnostic lab personnel should receive monkeypox pre-exposure prophylaxis or PrEP and that is vaccination. How do you know you have monkeypox? Non-specific signs of illness such as fever and chills, headache, muscle aches, fatigue are accompanied by or followed by the appearance of a rash that can be all over your body or more likely limited to points of contact where you contracted the disease. Stunning JWST Images Show New Details Of The Universe After many delays, a Christmas launch, and a months-long period of travel and testing, the first science images from the James Webb Space Telescope (JWST) were unveiled this week. The JWST has a huge multi-segmented mirror that allows it to gather faint light—and it sees in the infrared, allowing it to see through dust and gas and reveal details about the universe that were previously unseeable. On Monday, a short ceremony at the White House unveiled the first image, a “deep field” image taken by staring for hours at a piece of sky the size of a grain of sand held at arm's length. The image shows thousands of galaxies, including ones so distant that their images have been warped by the gravitational lensing effect of massive objects in between. On Tuesday, four more images were unveiled, including a spectrograph describing the atmosphere of a distant exoplanet, a cluster of galaxies known as Stephan's Quintet, the dying stars of the Southern Ring Nebula, and the star formation region known as the Carina Nebula. Amber Straughn, an astrophysicist and deputy project scientist for James Webb Space Telescope Science Communications joins Ira to talk about the images, and what lies ahead now that the JWST has entered its operational phase. A Busy Time For Space Launches While much of the astronomical world was gazing at the first science images from the James Webb Space Telescope, there's been a lot of other space news to discuss—from launches and testing associated with the Artemis I mission to the moon to new data from the Martian rovers. U.S. Attempts To Catch Up With Rising Monkeypox Cases The outbreak of the orthopox virus currently known as monkeypox continues to spread in hotspots around the United States, with symptoms ranging from fever to intensely painful, contagious lesions. From five cases in late May, the known number has grown to at least 1,053 as of Wednesday afternoon, with epicenters including New York City, the Bay Area, Chicago, Washington D.C., and other major cities. But the current numbers most certainly are an undercount, as people seeking diagnosis report difficulty accessing tests. Meanwhile, the rollout of the existing monkeypox vaccine, Jynneos, remains slow and inadequate for demand, with more than a million doses still stuck in a stockpile in Denmark. So far, the virus, which is known to spread through respiratory droplets and skin-to-skin contact, has been detected predominantly in men who have sex with men. New York public health researcher Keletso Makofane and San Francisco AIDS Foundation CEO Tyler TerMeer speak to the frustration of LGBTQ men and nonbinary people in the most at-risk networks, as resources and response lag. And Ira talks to UCLA monkeypox researcher Anne Rimoin, who twelve years ago published a warning that cases were rising in African countries as immunity to the related smallpox virus waned. The CDC has confirmed over 300 cases in at least 28 states with the population-dense states of California, New York, Florida, and Illinois heading the pack. So far, most of the US cases have involved sexual transmission, but, as the amount of monkeypox virus circulating rises, the virus could spread by more casual contact or through the air on respiratory droplets. The CDC and the Biden administration are ramping up supplies of the preferred vaccine against monkeypox, Jynneos, but the demand far outpaces the available suppl. That being the case, your best bet is avoiding a monkeypox infection. If you received a smallpox vaccination years ago, you may have some residual protection. Don't count on it though. Monkeypox is spread by contact with the characteristic skin lesions, by bodily secretions from an infected person, and by aerosol droplets from such a person. Avoid prolonged contact with the skin of a person who may be infected or who has a rash with characteristic monkeypox macules, papules, or pustules. Don't be having intimate contact with people you don't know well. To avoid an infectious cloud of monkeypox in a crowd, use your dependable N95 mask. If you think you have monkeypox, immediately consult your personal medical team or a hospital team. Testing is available as is treatment with the antiviral agents TPOXX, cidofovir, and Vaccinia Immune Globulin IV. These agents are not proven to be effective for monkeypox, so avoiding it is the best strategy. What about vaccination? Healthcare personnel and first responders with a monkeypox exposure risk should be vaccinated. If you have been exposed to a person with known or suspected monkeypox, speak with your medical team or local public health department about vaccination. One more thing: monkeypox, like CoVid, has been shown to be mutating. The best way we can all protect ourselves from yet another devastating pandemic is to avoid being infected. So what should you know about monkeypox? Monkeypox is an orthopoxvirus, a cousin of the smallpox virus, that was first identified in 1958 in a colony of African monkeys. The first human case was noted in 1970, again in Africa, and the majority of infections occur in the Democratic Republic of the Congo. The virus is mostly carried by rodents such as mice, rats, and squirrels. Monkeypox does not occur naturally in the US, and all cases here stem from International travelers bringing in the virus. As of June 1st, monkeypox cases have been identified in 10 states and in more than 23 countries. Monkeypox begins with typical symptoms of viral infections including fever, headache, muscle aches, and fatigue after a 5 to 21 day incubation period. One to three days after symptom onset, a rash appears followed by discrete lesions that become bumpy, liquid-filled, and pus-filled, that break giving way to scabs after a total of 2 to 4 weeks. The disease is spread by infected secretions during close and intimate contact including sex but may also be spread through respiratory secretions. Spread is prevented by avoiding contact with sick animals and people, careful hand washing and use of alcohol hand sanitizers, and masking as well as use of eye protection around those who are infected. The very young, elders, pregnant women, and those with immune deficiencies are at particular risk. Those previously immunized against smallpox should have some protection, but it likely has waned over time. Most at risk currently are younger persons who never received the smallpox vaccine. At this time, vaccination is not being recommended but that may change if the disease is spreading. If it is, problems will arise as available vaccines to monkeypox, JYNNEOS and the older ACAM200, are in short supply. There are several anti-viral medications, tecovirimat and brincidofovir, that counteract monkeypox, but their effectiveness in treating human monkeypox has not been proven. Vaccinia immune globulin is available and may provide effective treatment for those infected.
