Brownble has given me more unexpected gifts than I can count, and by far, the greatest of gifts has been the people I've met, all through my work on the internet. Not only our amazing students, readers and listeners, but colleagues and friends, all on a similar journey. It's my version of online dating for someone who has been with her significant other for 24 years, but with none of the horror stories and solely ALL of the friendship. One of these gifts, has been my friend and registered dietitian nutritionist Amanda Wagner. Who is one of my favorite professionals when it comes to nutrition, but also one of my favorite people on Earth. Maybe one of the kindest, most compassionate, and deep hearted people I know that is also an expert in all things nutrition, dietetics, veganism and athletics. Amanda Wagner, M.Ed, RDN, LDN is passionate about evidence-based nutrition and fitness. She is a former high school science teacher with a M.Ed in science education who changed careers to become a Registered Dietitian Nutritionist (RDN) in December 2022. Amanda is also an avid runner and former Division 1 Cross Country/Track athlete, who is enthusiastic about and fueled by a vegan diet. Amanda is the owner and lead dietitian of her own private practice Amanda Wagner Nutrition. Amanda became a UESCA Certified Running Coach in December 2023 and also works as a running coach for distances up to the full marathon through Big River Running and Filla Endurance. As a RDN, Amanda finds it rewarding to share her love of nutrition, fitness, and plant forward eating with others and help them reach their own wellness goals. We talk about how plants can fuel athletic performance and even run of the mill exercise for us non athlete folks (although she has a slightly different perspective on what constitutes an athlete that I think you'll love). Outside of athletics we talked about the role of a registered dietitian nutritionist, why it's a professional that might just change the moment of eating for you and arm you with knowledge and support tailored specifically to you. Amanda was also gracious enough to give us answers to so many and I mean SO many of your nutrition questions including talking about carbohydrates, calcium, magnesium, supplementation, how to fuel before and after exercise, the soy myth, whether it's safe or not to eat and how much we should be eating, the biggest mistakes people make when it comes to nutrition when they go vegan, where to get enough choline, protein powders and their place, whether we should exercise on an empty stomach, taking multivitamins and so much more. She is so knowledgeable, and because of her background in education, she gives it to us in digestible and easy to apply tips and suggestions and she is there for you for whenever you decide you'd like individualized care and support. Amanda breaks down what Avoidant/Restrictive Food Intake Disorder (ARFID) is, debunks common misconceptions like it's just “picky eating” and only kids have it, and shares resources for those who may be affected or someone with ARFID. As someone with personal experience with ARFID, we were very excited to chat with Amanda about ARFID advocacy and its impact on everyday life. Amanda is passionate about evidence-based nutrition and fitness. She is a former high school science teacher with a M.Ed in science education who changed careers to become a Registered Dietitian Nutritionist (RDN) in December 2022. Amanda is also an avid runner and former division 1 cross country/ track athlete, who is enthusiastic about and fueled by a vegan diet. Amanda became a UESCA Certified Running Coach in December 2023. Blood sugar levels are higher than normal, but not yet high enough to be diagnosed as type 2 diabetes. With prediabetes, action is the best medicine.American Diabetes AssociationWhat is Diabetes?About 38 million Americans have diabetes (about 1 in 10), and approximately 90-95% have type 2 diabetes. Type 2 diabetes most often develops in people over age 45, but more and more children, teens, and young adults are also developing it.Preventing Type 2 DiabetesIf you have prediabetes, losing a small amount of weight if you're overweight and getting regular physical activity can lower your risk for developing type 2 diabetes.A small amount of weight loss means around 5% to 7% of your body weight, just 10 to 14 pounds for a 200-pound person.Regular physical activity means getting at least 150 minutes a week of brisk walking or a similar activity. That's just 30 minutes a day, five days a week.Tests for Diabetes and PrediabetesYour doctor will have you take one or more of the following blood tests to confirm the diagnosis:A1C TestThe A1C test measures your average blood sugar level over the past 3 months. An A1C below 5.7% is normal, between 5.7 and 6.4% indicates you have prediabetes, and greater than 6.5% indicates you have diabetes.Fasting Blood Sugar TestThis measures your blood sugar after an overnight fast (not eating). A fasting blood sugar level of 99 mg/dL or lower is normal, 100 to 125 mg/dL indicates you have prediabetes, and greater than 126 mg/dL indicates you have diabetes.GLP-1 Antagonists - Ozempic, Wegovy, Byetta, RybelsusThis class of drugs is commonly called glucagon-like peptide 1 (GLP-1) agonists.These drugs mimic the action of a hormone called glucagon-like peptide 1. When blood sugar levels start to rise after someone eats, these drugs stimulate the body to produce more insulin. The extra insulin helps lower blood sugar levels.Lower blood sugar levels are helpful for controlling type 2 diabetes. But it's not clear how the GLP-1 drugs lead to weight loss. Doctors do know that GLP-1s appear to help curb hunger. These drugs also slow the movement of food from the stomach into the small intestine.As a result, you may feel full faster and longer, so you eat less.Along with helping to control blood sugar and boost weight loss, GLP-1s and SGLT-2 inhibitors seem to have other major benefits. Research has found that some drugs in these groups may lower the risk of heart disease, such as heart failure, stroke and kidney disease. This was a very cool conversation with a kindred spirit, Dr David Wiss, where we do a deep dive into nutrition, especially as it pertains to mental health.  David enrolled at California State University, Northridge where he earned a Master's of Science in Nutrition, Dietetics, and Food Science. This led to a dietetic internship at the University of California Los Angeles (UCLA) Medical Center where he received specialized training on their eating disorders unit. This experience sparked a deep interest in the role of nutrition and the nutritionist profession as part of mental health recovery. David became a Registered Dietitian Nutritionist (RDN) in 2013 and founded Nutrition in Recovery, a group practice of RDNs specializing in the treatment of eating and substance use disorders. During this time, David developed a specialized nutrition curriculum that has been incorporated at more than fifty addiction treatment centers in Southern California and worldwide.In his nutritionist profession practice, David consults and develops nutrition protocols for clients in treatment. He also regularly conducts staff training for professionals and speaks at conferences on topics including disordered eating, gut health, and trauma-informed nutrition. In 2017, David received the “Excellence in Practice” award at the national Food and Nutrition Conference and Expo. Based on extensive clinical experience with clients in mental health recovery, he began collaborating with other experts in the field to publish book chapters and manuscripts in peer-reviewed journals. In 2017, David matriculated at UCLA's Fielding School of Public Health in the Community Health Sciences department, with a minor in Health Psychology. He earned his Doctor of Philosophy degree (Ph.D.) in 2022 by investigating the links between adverse childhood experiences and various mental health outcomes among socially disadvantaged men. During his doctoral training, Dr. Wiss published multiple peer-reviewed manuscripts in the fields of food addiction, eating disorders, substance use disorders, depression, trauma, and childhood sexual abuse. These papers have been published in high-impact journals such as Appetite, the Journal of the Academy of Nutrition and Dietetics, Frontiers in Psychiatry, Public Health Nutrition, Nutrients, Eating and Weight Disorders, and others. Today I'm talking to Dr. David Wiss who became a Registered Dietitian Nutritionist (RDN) in 2013 and founded Nutrition in Recovery, a group practice of RDNs specializing in treating eating disorders as well as substance use. He earned his Ph.D. in Public Health with a minor in Health Psychology from UCLA by investigating links between adverse childhood experiences and mental health outcomes among socially disadvantaged men. Dr. Wiss's newest venture is his app called Wise Mind Nutrition. Wise Mind Nutrition delivers educational content using nutrition and lifestyle medicine to improve mental health outcomes. In the episode today, Dr. Wiss explores how adverse life experiences can impact your eating. He dives into his own approaches in treating clients with eating disorders and disordered eating, very much supporting each client as an individual and appreciating a nuanced approach. Dr. Wiss talks ‘all foods fit' versus the abstinence model, noting how each model is imperfect but brings valuable thinking and tools. He reflects on how it is important to consider an individual's needs, taking into consideration their personality type, genetics, addiction history and other mental health when addressing nutrition. Dr. Wiss then talks about his app – Wise Mind Nutrition. This is an app that can be used by people in eating disorder recovery but also the wider population. He emphasises relationship with food and getting away from tracking metrics that can be unhelpful, whilst empowering individuals to explore their own food relationship, rather than take on any fixed advice. Here's the scenario: You have a, let's say, complicated relationship with food. You're trying to work toward healing and get rid of these freaking food obsessions and weird binges. You crack open the Intuitive Eating book and love it. Eat when hungry? Stop when full? Honor cravings? This sounds amazing! You try it. And it totally backfires. Then you start wondering if it even works at all. Today I am talking with Hilmar Wagner about what eating should actually look like at the beginning of recovery. Spoiler: not like the eating described above.  Hilmar Wagner is a Registered Dietitian/Nutritionist (RDN), licensed in Minnesota and Washington State. Hilmar has experience working in a variety of clinical, outpatient, community, and management settings. Hilmar joined the Emily Program in 2006 and has served in a number of clinical and management roles, both at the site, regional and organization-wide levels. He currently serves as the Dietetic Internship Coordinator and Clinical Outreach Specialist. Today we are talking about ultra processed food with Dr. David Wiss. David became a Registered Dietitian Nutritionist (RDN) in 2013 and founded Nutrition in Recovery, a group practice of RDNs specializing in treating eating and substance use disorders. He earned his Ph.D. in Public Health with a minor in Health Psychology from UCLA by investigating links between adverse childhood experiences and mental health outcomes among socially disadvantaged men. Dr. Wiss can be your nutrition and health consultant, functional medicine practitioner, recovery coach, or provide psychoeducation related to his expertise.  Ultra processed food is defined as food that is completely comprised of processed culinary ingredients. Meaning that the food does not contain any non-processed foods or minimally processed foods. The eating disorder community needs to recognize this exists because it is so alienating and invalidating to some people to hear that there is no difference between foods whatsoever. She is an incredibly talented health professional who has joined us on some of our first few podcasts. But this factor does not contribute to the problem on its own. We also have an issue of decreased insulin sensitivity. A problem in which cells do not respond well to insulin signalling. Hyperinsulinemia and less sensitive cells both interact and contribute to insulin resistance.  The old tale of the boy who cried wolf explains a lot about hyperinsulinemia because hyperinsulinemia is just like crying out loud more than you need to, to such an extent that people just start ignoring the message! Likewise, the constant overstimulation of insulin makes the cells less sensitive to insulin. These two changes are the key drivers behind this insulin resistance problem and can result in extreme forms of metabolic dysfunction such as diabetes. The human body is extremely complex which means that it is difficult to establish cause-and-effect relationships where one factor is directly responsible for bringing about a given set of changes. Likewise, with problems related to metabolic health, there are often multiple factors working collectively to bring about a certain change.  Diseases stemming from metabolic issues are also an output of a build-up of decade-long problems for eg a patient only receives a diagnosis for type 2 diabetes, it is estimated that 60 to 70% of our beta cells which produce insulin are damaged beyond repair! Poorly regulated insulin levels could be a consequence of a poorly balanced diet featuring high carbohydrate intake, more than required sugar, and in general low nutrient density and high energy consumption. Eating around the clock and constantly keeping insulin-stimulated can be problematic as well. Similarly, chronic stress and poor sleep quality can disrupt the sensitivity of the cells to insulin as well. [9:57] Advice for people who have received some early signals of insulin-related issues Unhealthy habits and life practices can result in a metabolic breakdown, therefore it is important to come up with a healthier lifestyle. Reduce your Carbohydrates to limit insulin load. You can have about 45 to 60 grams of carbohydrates for each meal. Focus on having nutrient-rich diets!. Exercise is great, especially weight lifting! [14:25] Insulin response: Protein VS Carbohydrate Protein can trigger an insulin response in some individuals but it is not the same as the one triggered by carbohydrates. With carbohydrates it can become extremely challenging to normalise glucose levels  [27:19} Is glucose never exceeding 90 a blessing or a problem in disguise? Keeping in mind the principles of metabolic flexibility, staying strictly below 90 can get problematic. It's not the end goal, having a balanced energy-efficient system is! [37:20] What is CGM? CGM stands for Continuous Glucose monitor.
