Episode #167 In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community: If the stomach rumbles about an hour after having a black coffee, does this mean that the coffee raised insulin? [01:43] Is insulin released only when the food is in the mouth? It feels like when a big, heavy meal is in the stomach for hours I could still add a supplement or snack to it and it wouldn't make a difference, but would there be an additional insulin release? [02:54] I have read that it is recommended to ‘anchor' your weight after a 10% reduction in BF%. Can you say a bit more about this? I presume you would ‘anchor' by scaling back on fasting, and adjusting frequency and intensity in response to any changes in BF% so as to keep it stable. But what happens if you go all the way in one go? Do you need to ‘anchor' for longer at the lower BF% in order for it to become your new set weight or does the research say it's not possible to reset it that low if you didn't pause along the way? [03:35] Is it true that a longer fast (or intense exercise) can stress your body, causing insulin to go up, and, therefore, might even be counterproductive? [14:08] One of the biggest effects of blood glucose and insulin spikes is the damage to small blood vessels. These spikes damage the organs that predominantly rely on small vasculature like kidneys, eyes, nerves in fingers and toes, leading to complications like diabetic kidney failure, macular degeneration, and neuropathy. Hyperinsulinemia is really a microvascular disease. Could you please share how you explain this process to non-medical folks? I fail to explain to my family members in layperson terms exactly what it is about the spikes of BG and insulin that damages the small blood vessels. [16:24] In The Obesity Code, you wrote about a study where they overfed people to get them to gain weight. The study showed that they had a hard time putting weight on because their bodies knew to burn it off. So my question is, if our bodies experience this effect as per the study, what signs would there be (such as raised temperature and/or heart rate) to get our body to burn the extra calories consumed? [00:40] Why Dr. Sowa Got Involved In The Health Space Even before Dr. Sowa went to college, she had discovered the field of public health. Plus, the idea of prevention always was very attractive to her. Sowa always wondered why doctors want to treat the disease if they could prevent it. At college, she really started to understand a holistic view of what it means to be healthy. Eventually, Dr. Sowa realized a lot of problems in health boils down to metabolic dysfunction. [04:40] Why Are 88% of Americans Metabolically Inflexible? So, Dr. Sowa says the solution is actually pretty straightforward. You can reverse type 2 diabetes; again, it is reversible! Many people think a low-carb and high-fat diet will give you a heart attack or a stroke; that's simply not true. The three macronutrients are carbohydrates, protein, and fat. Out of those three macronutrients, carbohydrates will spike glucose and insulin the most. It makes total sense that we should eat more protein and fat to reverse high insulin. [16:45] What Is An Optimal Range For Fasting Insulin? Optimal is usually under five. Even up to eight is okay for fasting insulin. Anything higher than eight means your body is making too much insulin in response to carbohydrates. When Dr. Sowa does bloodwork, she's looking for metabolic syndrome. [20:50] How Often Should Someone Retest Their Markers? You should test once a quarter. If you are embarking on a new lifestyle change, Dr. Sowa says to test right away. That way, you have the data. Once you start eating keto, then you can see what improves. Your markers are a better victory than what you see on the scale. [24:20] How Dr. Sowa Wants To Help People With Polycystic Ovary Syndrome PCOS is a disease of insulin resistance in the majority of cases. For years, the medical community has gone about treating this in the wrong way. Up to 50% of women with PCOS will go on to develop type two diabetes by the time they're 40. Women with PCOS can make themselves feel better by following a low carbohydrate diet. Sowa developed a kit to help women get a diagnosis. It usually takes women five years and three doctors to understand their diagnosis. [28:10] Why Dr. Sowa Is A Big Fan of Intermittent Fasting Fasting will reset your hormones. Plus, fasting is a great way to understand your relationship with food and hunger. Remember, you don't have to eat every two hours. In the beginning, it's not necessarily about eating less; it's just about eating less often. [31:10] Why Dr. Sowa Loves Walking After A Meal Walk after a meal, so your muscles have the ability to take up any glucose. We should be moving right after we eat to take up any free glucose and use it for energy instead of fat storage. Five minutes will make a difference! The sweet spot would be fifteen to twenty minutes. [34:10] How Important Is Quality Sleep Each Night? If you don't get good sleep, sleep, you will be hungry and crave carbohydrates. Good quality and consistent sleep will keep your cortisol levels down. When cortisol spikes from poor sleep, the hunger hormone will also spike. Plus, you will get a spike in insulin. When insulin is high, it will drive hunger. One of the silent epidemics in our country is sleep apnea, where people cannot get good sleep no matter how hard they try. Today, I am blessed to have here with me Shana Hussin. She grew up in Wisconsin, and earned her dietetics degree from the University of Wisconsin-Stevens Point in 1999. While very active throughout her entire lifetime, she struggled with weight gain in college, despite following conventional nutrition recommendations and exercising extensively. [00:45] The Story Behind Shana's Son and His Ulcerative Colitis Diagnosis One day, Shana's son got diarrhea; they didn't think much of it. After weeks, he still had diarrhea. So, they went and got testing, but nothing was coming back. Three months after he started having symptoms, he had a colonoscopy done, and he was diagnosed with ulcerative colitis. He had lost a bunch of weight and couldn't keep up with sports at the age of eleven. Later, the doctor said Shana needed to get ready to take his colon out. However, Shana found experts outside of the conventional medicine that helped heal her son. [13:30] Why Shana Started To Question Her Dietician Training Shana was a dietician for ten years before she started questioning things. She told people to eat low fat, mostly plants and whole grains. Ten years in, Shana's clients weren't getting any better. After having three children, Shana started to develop her own health problems. Shana had four or five canker sores in her mouth, bloating, and a lack of energy. She knew something in her diet had to be the problem. It took a long time to figure out what was wrong. Plus, Gary Taubes inspired him. Sam was really surprised to learn that that cancer was associated with diabetes, insulin resistance, obesity, and cardiovascular disease. He wanted to dig further on the connection between cancer and metabolic disease. [02:10] The Nazis Protected Warburg For His Cancer Research In 1923, Warburg makes this famous discovery called the Warburg Effect. Cancer cells take up a lot more glucose than other cells and burn it without oxygen; they ferment it, just like microorganisms do. Warburg also has made other discoveries about how cells breathe and use oxygen. He wins the Nobel Prize in 1931. In 1933, the Nazis come along. Despite having a Jewish father and being openly gay, the Nazis protected Warburg because they were interested in his cancer research. [06:50] Warburg's Studies Started To Disappear After The War After the war, Warburg's cancer research really starts to disappear. Australia in the 1950s famously discovered the structure of DNA. Eventually, we had a new sort of genetic understanding of cancer where we can now identify specific mutations. So, how cells take up glucose and shift their metabolism was considered irrelevant because of the study of genetics. Warburg's studies take peoeple back to diet, and there's a huge bias against taking diet seriously. [12:20] What Sugar Does To Promote Cancer Growth If cancer is about these metabolic shifts inside of a cell, what is it about the way we eat that affects the way a cell eats? In America, one out of three women are diagnosed with cancer within their lifetime. For men, it's about one in two. Cancer is closely linked to obesity. 13 of the deadliest cancers have been solidly linked to obesity. Sam wanted to know if obesity was causing cancer or if something was driving both. Elevated levels of the hormone insulin are responsible for obesity and give microscopic cancers a chance to grow and thrive. What is it about our diet that causes insulin to rise? Sugar! The increase in sugar consumption maps pretty closely to the rise of diabetes, heart disease, and cancer. [19:30] How A Keto Diet That Will Help You Prevent Cancer Sam is a huge believer that keto is a logical way to prevent cancer. The single most important thing you can do to improve your odds for cancer prevention is to keep your insulin levels low. Hyperinsulinemia promotes the growth of cancer. So, it's important to follow a diet that keeps your insulin levels down. [22:20] Intermittent Fasting Is Another Great Way To Lower Insulin Intermittent fasting is one very successful strategy for keeping insulin down. If you can fast consistently, it seems to work quite effectively on lowering insulin. Fasting can lead to weight loss because it lowers insulin levels in the body. Sam wants to point out that thin people can still have insulin resistance. In some ways, being naturally thin can actually be a disadvantage because you can't store subcutaneous fat. [26:30] How To Optimize Insulin and Glucose Levels To Be Proactive Instead of Reactive You can't outrun a bad diet. However, exercise can help you feel better and make you more inspired to follow a low-carb diet. After workouts, you can be inspired to eat healthily and continue to feel good. Once you understand that sugar is linked to cancer, it should help you ditch it during mealtime. [38:30] Finding Therapies To Target Specific Cancer Mutations When cancer can eat lots of glucose, it's no longer thinking about dying; it's thinking about doing a lot more things. Based on which particular mutation you have, the oncologist will devise a treatment. Unfortunately, targeted therapies to specific mutations haven't been as successful as we would have liked. However, there are some cases where therapies will successfully target mutations. [42:25] The Best Preventative Measures To Preventing Cancer In The Future Eliminate sugar from your diet. Definitely eliminate liquid sugar from your diet. If you are insulin resistant, you need to reduce those insulin levels. Stop smoking. Cut out refined carbohydrates. AND MUCH MORE! KEY TAKEAWAYS Hyper means too much, insulin is the hormone that regulates the amount of glucose in the blood, and ‘emia' means ‘in the blood.' Basically, it means your body is secreting too much insulin. When the body is trying to reduce the amount of glucose from the food you eat, it releases insulin to help bring the level of glucose down to a safe level. If this happens too often, your body develops an immunity to the insulin and has to produce more for the same effect. Carbohydrates become glucose when it is digested. The slower the carbohydrate releases the sugar the better it is. This is called the glycemic index. An insulin surge can cause a sudden drop in sugar levels resulting in lethargy. Lethargy leads you to intake more sugar causing another surge and so on…with enough occurrences you will develop diabetes and a fatty liver. [2:50] The problem with insulin resistance The physiology that can be used to explain insulin resistance is hyperinsulinemia which means having higher than normal insulin levels. But this factor does not contribute to the problem on its own. We also have an issue of decreased insulin sensitivity. A problem in which cells do not respond well to insulin signalling. Hyperinsulinemia and less sensitive cells both interact and contribute to insulin resistance. The old tale of the boy who cried wolf explains a lot about hyperinsulinemia because hyperinsulinemia is just like crying out loud more than you need to, to such an extent that people just start ignoring the message! Likewise, the constant overstimulation of insulin makes the cells less sensitive to insulin. These two changes are the key drivers behind this insulin resistance problem and can result in extreme forms of metabolic dysfunction such as diabetes. The human body is extremely complex which means that it is difficult to establish cause-and-effect relationships where one factor is directly responsible for bringing about a given set of changes. Likewise, with problems related to metabolic health, there are often multiple factors working collectively to bring about a certain change. Diseases stemming from metabolic issues are also an output of a build-up of decade-long problems for eg a patient only receives a diagnosis for type 2 diabetes, it is estimated that 60 to 70% of our beta cells which produce insulin are damaged beyond repair! Poorly regulated insulin levels could be a consequence of a poorly balanced diet featuring high carbohydrate intake, more than required sugar, and in general low nutrient density and high energy consumption. Eating around the clock and constantly keeping insulin-stimulated can be problematic as well. Similarly, chronic stress and poor sleep quality can disrupt the sensitivity of the cells to insulin as well. [9:57] Advice for people who have received some early signals of insulin-related issues Unhealthy habits and life practices can result in a metabolic breakdown, therefore it is important to come up with a healthier lifestyle. Reduce your Carbohydrates to limit insulin load. You can have about 45 to 60 grams of carbohydrates for each meal. Focus on having nutrient-rich diets!. Exercise is great, especially weight lifting! [14:25] Insulin response: Protein VS Carbohydrate Protein can trigger an insulin response in some individuals but it is not the same as the one triggered by carbohydrates. With carbohydrates it can become extremely challenging to normalise glucose levels [27:19} Is glucose never exceeding 90 a blessing or a problem in disguise? Keeping in mind the principles of metabolic flexibility, staying strictly below 90 can get problematic. It's not the end goal, having a balanced energy-efficient system is! [37:20] What is CGM? CGM stands for Continuous Glucose monitor. It is a small device that you put on your arm and leave it there for two full weeks to get a picture of your Glucose levels. Andrew Scarborough is a proactive cancer patient and public health advocate, scrutinising the latest research in order to devise his own optimal metabolic approach in an attempt to manage his disease. He is also an inquisitive researcher, using critical analysis and independent thought to support his university studies. Prior to his diagnosis of a highly vascular Anaplastic Astrocytoma brain tumour, he studied for a masters in Nutritional Therapy and worked as a personal trainer. He is currently studying Human Biology and Medical Science at the University of Westminster, where he is heavily involved with brain tumour research. He mixes practical experience of metabolic therapies with a detailed understanding of its potential for cancer management and seizure control at the cellular level. Isabella D Cooper majored with honours in biochemistry with medical physiology, molecular genetics, advanced cell biology, advanced cancer biology, physiological networks (neurology, endocrinology and immunology), proteins and enzyme biochemistry, bioinformatics in influenza, and metabolic biochemistry. She has won multiple awards in multiple consecutive years, those include the Faculty of Science and Technology Awards, Individual Teaching Award and Group Teaching Award for Medical Physiology, Dean's List Awards and Biochemical Society Award. ORN Zoë Harcombe show notes 9/6/22 Epert in nutritional research In a study in the 70s, she found no evidence dietary fat is harmful. Found more health incidents and mortality when PUFAs were substituted for saturated fats. Found that dietary interventions had no positive influence and quite likely evidence of harm. They are not evidence-based. With protein norm at 15%. a maximum of 30% dietary fat means carbohydrates will be 55%. Harder now to uncover author and sponsorship conflict of interests. Relentless campaigns now against meat and dairy. But no evidence there. Do they want us sick? Eat Real Food. Potato is a real food but not a high nutrient food. Will the starch mess you up? Not if you're metabolically healthy. Could cause oxalate issues. Real food doesn't put fat and carbohydrate together in one package, except for nuts. But people find the combo irresistible, and food manufacturers take advantage. If you're feeling well, don't go near a doctor and don't test. Don't have overwhelming evidence that processed foods are bad. Difference between sugar in fruit and packaged sugar? None. Body doesn't know where the glucose/fructose/sucrose molecule came from. Five A Day Fruits and Vegetables is a complete fairy tale. What does sugar do in the body? We have 4 grams of glucose in our bloodstream at all times. After consuming sugar, insulin gets released to get glucose out of the blood, fructose goes to the liver. Too much fructose causes NAFLD, too much glucose causes diabetes. Hyperinsulinemia – high insulin state as a response to habitually too much sugar. Too much sugar smashes body's delicate mechanism for glucose control. Need a new paradigm on cholesterol. Statins are not the answer. Dairy – Zoë personally eats a lot of it. We don't need fiber. Carbohydrates are not an essential need. Fiber is a subset of carbohydrates and therefore not needed. Can it be a benefit? No trial evidence that says it's any good. Population studies show people who eat fiber tend to be healthier, but they don't prove it's the fiber that is causing the health benefit. Cereal companies want you to think fiber is needed. Eggs are really good. The yolk has the nutrients. Main sugar in rice is glucose. Average serving has 100 grams carbohydrate. Overeating high carb sources can cause blood glucose to drop below 4 gm because of insulin response, causing hypoglycemia. Want blood glucose to be even. With a habitual keto or carnivore diet, the body runs on fats. Fat is the fuel we don't run out of. If eating high meat, do you need extra fat? No, eat the food as it comes. Eat the fat on the meat, don't add more. Don't add butter to the steak if trying to lose weight. Burn the body's fat, not dietary fat. Mixing fat and carbs is the best way to add weight. Studies showing association between red meat and heart disease are population studies showing association between heart disease and affluence. Why don't dairy and meat industries defend themselves? Today, I am blessed to have here with me Sam Apple. He is on the faculty of the MA in Science Writing and MA in Writing programs at Johns Hopkins. Prior to his arrival at Johns Hopkins, Apple taught creative writing and journalism at the University of Pennsylvania for ten years. Plus, Gary Taubes inspired him. Sam was really surprised to learn that that cancer was associated with diabetes, insulin resistance, obesity, and cardiovascular disease. He wanted to dig further on the connection between cancer and metabolic disease. [02:10] The Nazis Protected Warburg For His Cancer Research In 1923, Warburg makes this famous discovery called the Warburg Effect. Cancer cells take up a lot more glucose than other cells and burn it without oxygen; they ferment it, just like microorganisms do. Warburg also has made other discoveries about how cells breathe and use oxygen. He wins the Nobel Prize in 1931. In 1933, the Nazis come along. Despite having a Jewish father and being openly gay, the Nazis protected Warburg because they were interested in his cancer research. [06:50] Warburg's Studies Started To Disappear After The War After the war, Warburg's cancer research really starts to disappear. Australia in the 1950s famously discovered the structure of DNA. Eventually, we had a new sort of genetic understanding of cancer where we can now identify specific mutations. So, how cells take up glucose and shift their metabolism was considered irrelevant because of the study of genetics. Warburg's studies take peoeple back to diet, and there's a huge bias against taking diet seriously. [12:20] What Sugar Does To Promote Cancer Growth If cancer is about these metabolic shifts inside of a cell, what is it about the way we eat that affects the way a cell eats? In America, one out of three women are diagnosed with cancer within their lifetime. For men, it's about one in two. Cancer is closely linked to obesity. 13 of the deadliest cancers have been solidly linked to obesity. Sam wanted to know if obesity was causing cancer or if something was driving both. Elevated levels of the hormone insulin are responsible for obesity and give microscopic cancers a chance to grow and thrive. What is it about our diet that causes insulin to rise? Sugar! The increase in sugar consumption maps pretty closely to the rise of diabetes, heart disease, and cancer. [19:30] How A Keto Diet That Will Help You Prevent Cancer Sam is a huge believer that keto is a logical way to prevent cancer. The single most important thing you can do to improve your odds for cancer prevention is to keep your insulin levels low. Hyperinsulinemia promotes the growth of cancer. So, it's important to follow a diet that keeps your insulin levels down. [22:20] Intermittent Fasting Is Another Great Way To Lower Insulin Intermittent fasting is one very successful strategy for keeping insulin down. If you can fast consistently, it seems to work quite effectively on lowering insulin. Fasting can lead to weight loss because it lowers insulin levels in the body. Sam wants to point out that thin people can still have insulin resistance. In some ways, being naturally thin can actually be a disadvantage because you can't store subcutaneous fat. [26:30] How To Optimize Insulin and Glucose Levels To Be Proactive Instead of Reactive You can't outrun a bad diet. However, exercise can help you feel better and make you more inspired to follow a low-carb diet. After workouts, you can be inspired to eat healthily and continue to feel good. Once you understand that sugar is linked to cancer, it should help you ditch it during mealtime. [38:30] Finding Therapies To Target Specific Cancer Mutations When cancer can eat lots of glucose, it's no longer thinking about dying; it's thinking about doing a lot more things. Based on which particular mutation you have, the oncologist will devise a treatment. Unfortunately, targeted therapies to specific mutations haven't been as successful as we would have liked. However, there are some cases where therapies will successfully target mutations. [42:25] The Best Preventative Measures To Preventing Cancer In The Future Eliminate sugar from your diet. Definitely eliminate liquid sugar from your diet. If you are insulin resistant, you need to reduce those insulin levels. Stop smoking. Cut out refined carbohydrates. AND MUCH MORE! Researchers study preventing cancer and diabetes with the maqui berry NOVA Southeastern University of Florida, May 27, 2021 Aristotelia chilensis, also known as maqui berry, is a fruit-bearing shrub native to South America. According to a study published in the journal Phytochemical Analysis, maqui berries are rich in anthocyanins, which give the fruits their dark purple color. Anthocyanins are plant pigments that possess many remarkable biological properties, such as antioxidant, anti-inflammatory, anti-viral and anti-cancer activities. In a recent study, researchers at NOVA Southeastern University in Florida discussed the potential of Chilean maqui berry for use as a nutritional supplement that can help treat hyperinsulinemia and related diseases. Hyperinsulinemia, or higher-than-normal insulin levels, is often caused by insulin resistance, which is said to be the precursor to diabetes. Chronic hyperinsulinemia also promotes cancer growth by allowing insulin to exert its oncogenic effects, which include enhancing growth factor-dependent cell proliferation, among others. The researchers discussed how Chilean maqui berry can help with insulin resistance and reduce cancer risk in an article published in the journal Food Science and Human Wellness. The medicinal benefits of Chilean maqui berry Researchers have long considered nutritional supplementation to be a possible alternative or adjunct treatment to conventional therapies for common ailments and diseases. According to recent studies, maqui berries can reduce postprandial insulin levels by as much as 50 percent and are just as effective as metformin at increasing insulin sensitivity and stabilizing blood glucose levels. Maqui berries’ mechanism of action involves inhibiting sodium-dependent glucosetransporters in the small intestine and slowing the rate of entry of glucose in the bloodstream. Thanks to these actions, maqui berries can effectively reduce the likelihood of blood sugar spikes and prevent the corresponding rise in insulin levels that follows. At the same time, maqui berries contribute to cancer prevention since chronically high blood glucose levels — besides chronic hyperinsulinemia — are also linked to the development of cancer. In fact, numerous studies have shown that diabetics and prediabetics have an elevated risk of developing cancerous growths. Based on the findings of previous studies, the researchers believe that consistent supplementation with Chilean maqui berries could indirectly reduce the risk of cancer and other diseases that are promoted by hyperglycemia (high blood sugar) and hyperinsulinemia. Studies reveal that social isolation and quarantine throughout the COVID-19 pandemic may have a detrimental impact on physical and mental health of people living with pre-existing conditions University of Naples (Italy) and Teva Pharmaceuticals, May 30, 2021 Abstract 803: Impact of social isolation and quarantine on the course of diabetes mellitus and its complications during Covid 19 pandemic in Adjara Region Country of Georgia Abstract 1337: Psychological distress in patients with hypocortisolism during mass quarantine for Covid-19 epidemic in Italy Studies reveal that social isolation and quarantine throughout the COVID-19 pandemic may have a detrimental impact on people living with pre-existing conditions. Social isolation and quarantine can have a detrimental impact on physical and mental health of people living with pre-existing conditions, according to two studies being presented at the 23rd European Congress of Endocrinology (e-ECE 2021) The studies bring together research on the impact of social isolation and quarantine for people living with diabetes in the Adjara Region of Georgia, and on patients with hypocortisolism in Italy. Both studies reported that social isolation during the pandemic caused significant psychological and/or physical distress on the observed individuals. Data from the first study revealed that the impact of quarantine on people living with diabetes in the Adjara Region caused blood pressure (BP) levels to increase in 88.2% of patients with 50% of these cases resulting in high BP hospitalisation. In addition to these physical factors, increased feelings of anxiety and fear were observed on 82% of patients. In the second study, patients with hypocortisolism experienced increased anxiety and depression, associated with a dissatisfaction feeling of self and a reduced resiliency, when compared with Italian healthy controls. As these are all contributing factors to overall health deterioration, these findings suggest further research is required to allow patients with pre-existing conditions to remain fit and healthy during the current pandemic. In the Adjara Region study, Dr Liana Jashi and the research team disseminated an online questionnaire and collected answers from 16 endocrinologists and 22 family and general practice doctors. The study confirmed the negative, indirect effects social isolation and quarantine had on people living with diabetes. It reported a list of negative effects such as the reduced access to medical care, weight gain and increased cigarette and alcohol consumption. Physical activity decreased by 29.8%, a vital preventative to further physical and psychological problems. "This study highlights that people living with diabetes require greater support during pandemics to maintain exercise and protect their physical and mental health. National health services should use these data and future studies to implement better social care around supporting people with pre-existing conditions," commented Dr Jashi. In the second study, Dr Chiara Simeoli at the University of Naples reported data collected during the last three weeks of the mass quarantine lasted 2 months in Italy, in a web-survey-based, multicenter, case- control research involving 12 different Italian centres. The study confirmed that a large cohort of 478 patients with hypocortisolism, and particularly, 363 with adrenal insufficiency and 115 with congenital adrenal hyperplasia, adequately treated with glucocorticoids, showed higher anxiety and depression, associated with a dissatisfaction feeling of self and a reduced resiliency, when compared with Italian healthy controls, suggesting the detrimental impact of social isolation on mental health of these patients, particularly frail and vulnerable to infections and stress. Moreover, patients with adrenal insufficiency reported a worse quality of life than patients with congenital adrenal hyperplasia. "These findings confirmed that beyond the huge impact on physical health, COVID-19 epidemic, social isolation and mass quarantine represent significant psychological stressors, causing severe effects on mental health, even more on people with pre-existing conditions. An empowerment of psychological counselling for these vulnerable patients during COVID-19 should be considered by national health-care services," adds Dr Simeoli. Both studies indicate that additional larger studies over a longer period of time are needed for further investigation. Researchers discover link between local oxygen depletion in the brain and Alzheimer's disease University of Seville (Spain), May 24, 2021 The study, published in the journal Nature Aging and led by the laboratories of Dr. Alberto Pascual (CSIC), from the Neuronal Maintenance Mechanisms Group, and Prof. Javier Vitorica (University of Seville/CIBERNED) of the Physiopathology of Alzheimer's Disease Group at IBiS, demonstrates for the first time that low oxygen levels in the so-called senile plaques in the brain reduces the immune system's defensive capacity against the disease. The study also suggests that this lack of oxygen in the brain enhances the action of disorders associated with Alzheimer's disease that are characterized by low systemic oxygen levels, such as atherosclerosis and other cardiovascular diseases. What happens in the brain? A characteristic feature of Alzheimer's patients is the accumulation of highly toxic substances in their brains, known as senile plaques. The brain has an immune system whose main component are the microglial cells, which were first described and named 100 years ago by Pío del Río Hortega, a disciple of Ramón y Cajal. In the absence of damage, these cells facilitate the neurons' function. In response to Alzheimer's disease, microglia defend neurons by surrounding senile plaques, preventing their spread in the brain and decreasing damage. Alzheimer's disease is aggravated by other pathologies, such as cardiovascular diseases, which cause a decrease in oxygen levels in the body. This study has revealed reduced oxygen levels around senile plaques, compromising microglial activity (Image, center). When this is compounded by reduced oxygen supply to the brain due to other systemic pathologies, the microglia are unable to provide protection and there is an increase in the pathology associated with the disease. Relevance Alzheimer's disease is the leading cause of dementia in Spain and around the world. In Spain, its incidence is increasing dramatically as the population ages. Unfortunately, the origin of the disease remains unknown. The mechanism proposed in this study is mediated by the expression of the HIF1 molecule, whose discoverers received the Nobel Prize in Physiology or Medicine in 2019. Increased HIF1 levels compromise the mitochondrial activity of microglial cells and limit their protective capacity against disease. This study opens new lines of research to improve the metabolic capacity of microglia, which would enable a sustained response over time against the disease. Indirectly, the study supports previous work highlighting the importance of maintaining good cardiovascular health for healthy aging. Effect of different doses of melatonin on learning and memory deficit in Alzheimer model Guilan University of Medical Sciences (Iran), May 21, 2021 According to news reporting out of Rasht, Iran, research stated, “Alzheimer Disease (AD) is an age-related neurodegenerative disorder with a progressive impairment of cognitive function. The pineal gland hormone melatonin (MEL) has been known as a protection agent against AD.” Our news reporters obtained a quote from the research from Guilan University of Medical Sciences: “However, the effect of melatonin in various doses is inconsistent. In this study, we aimed to investigate two doses of MEL on learning and memory in the amyloid-beta (Ab)-induced AD in the rats. Forty-eight male Wistar rats were used in the experiment and randomly divided control, sham, vehicle, AD, AD+MEL10 mg/kg, and AD+MEL 20 mg/kg groups. Intracerebroventricular injection of Ab1-42 was used to develop the animal model of AD. Also, MEL-treated groups received an intraperitoneal injection of MEL for 4 next weeks. The Morris Water Maze (MWM) and Passive Avoidance Learning (PAL) tests were used to examine animals’ learning and memory. The brain of animals was removed for immunohistochemistry for anti- Amyloid Precursor Protein (APP). Intra-peritoneal injection of MEL significantly improve learning and memory in MWM (P=0.000) and PAL test (P=0.000), but there were no significant changes in the two groups that received the melatonin (P>0.05). Histopathological analysis revealed that the clearance of APP deposition in the AD+MEL20 group was considerable compared with the AD+MEL10 group (P=0.000).” According to the news editors, the research concluded: “Our findings indicate that 10 and 20 mg/kg doses of melatonin have similar results on learning and memory in the AD model. But 20 mg/kg of melatonin has significantly more effect on the clearance of APP deposition.” Effects of flaxseed on blood pressure, body mass index, and total cholesterol in hypertensive patients: A randomized clinical trial Lorestan University of Medical Sciences (Iran), May 25, 2021 Objectives Given the antioxidant properties of flaxseed and its biologically active ingredients, this study was conducted to determine the effects of flaxseed supplementation on body mass index (BMI), blood pressure, and total cholesterol levels in patients with hypertension. Methods In this triple-blind clinical trial, 112 patients, with an age range of 35 to 70 years, were randomized to 2 groups receiving 10 g (n=45) and 30 g (n=45) of flaxseed supplementation and 1 group receiving placebo (n=45) for 12 weeks by stratified block randomization. They were evaluated in terms of systolic (SBP) and diastolic blood pressure (DBP), BMI, and total serum cholesterol. Physical activity was measured using the International Physical Activity Questionnaire–Short Form (IPAQ–SF) and food intake was assessed using the Food Frequency Questionnaire (FFQ). The data were analyzed with SPSS, version 22, using the chi-square, Kruskal–Wallis, repeated measures analysis, ANOVA, and ANCOVA tests. Results The interaction effects among the study groups and time on the mean SBP (p = 0.001), DBP (p = 0.001), total cholesterol level (p = 0.032), and BMI (p < 0.001) were significant. During the study, the 30-g group achieved the best results, so that a 13.38-unit decrease in SBP was observed compared to a 1.72 unit increase in the placebo group and a 5.6-unit decrease in DBP was measured compared to a 2.39 unit increase in the placebo group. BMI decreased by 0.86 units compared to 0.06 units in the placebo group. Total cholesterol also decreased by 20.4 units compared to 11.86 units in the placebo group. Conclusion The results of this study showed that flaxseed can be effective in reducing blood pressure, total cholesterol, and body mass index in hypertensive patients in a twelve-week period. Study: Don't count on caffeine to fight sleep deprivation Michigan State University, May 27, 2021 Rough night of sleep? Relying on caffeine to get you through the day isn't always the answer, says a new study from Michigan State University. Researchers from MSU's Sleep and Learning Lab, led by psychology associate professor Kimberly Fenn, assessed how effective caffeine was in counteracting the negative effects of sleep deprivation on cognition. As it turns out, caffeine can only get you so far. The study -- published in the most recent edition of Journal of Experimental Psychology: Learning, Memory, & Cognition -- assessed the impact of caffeine after a night of sleep deprivation. More than 275 participants were asked to complete a simple attention task as well as a more challenging "placekeeping" task that required completion of tasks in a specific order without skipping or repeating steps. Fenn's study is the first to investigate the effect of caffeine on placekeeping after a period of sleep deprivation. "We found that sleep deprivation impaired performance on both types of tasks and that having caffeine helped people successfully achieve the easier task. However, it had little effect on performance on the placekeeping task for most participants," Fenn said. She added: "Caffeine may improve the ability to stay awake and attend to a task, but it doesn't do much to prevent the sort of procedural errors that can cause things like medical mistakes and car accidents." Insufficient sleep is pervasive in the United States, a problem that has intensified during the pandemic, Fenn said. Consistently lacking adequate sleep not only affects cognition and alters mood, but can eventually take a toll on immunity. "Caffeine increases energy, reduces sleepiness and can even improve mood, but it absolutely does not replace a full night of sleep, Fenn said. "Although people may feel as if they can combat sleep deprivation with caffeine, their performance on higher-level tasks will likely still be impaired. This is one of the reasons why sleep deprivation can be so dangerous." Fenn said that the study has the potential to inform both theory and practice. "If we had found that caffeine significantly reduced procedural errors under conditions of sleep deprivation, this would have broad implications for individuals who must perform high stakes procedures with insufficient sleep, like surgeons, pilots and police officers," Fenn said. "Instead, our findings underscore the importance of prioritizing sleep." Parkinson's disease more likely in people with depression, study suggests Umea University (Sweden), May 21 2021 People with depression may be more likely to develop the movement disorder Parkinson's disease, according to new research published in Neurology. According to the authors of the study, depression is more common in people with Parkinson's disease than those without the movement disorder. "We saw this link between depression and Parkinson's disease over a timespan of more than 2 decades, so depression may be a very early symptom of Parkinson's disease or a risk factor for the disease," says study co-author Prof. Peter Nordström, at Umeå University in Sweden. Parkinson's disease is a progressive disorder of the nervous system that affects how a person moves, including how they speak and write. As well as problems with movement, Parkinson's disease can also cause cognitive problems, neurobehavioral problems and sensory difficulties. The authors of the study state that depression is more common in patients with Parkinson's disease than in members of the general population. The mood disorder has a major influence on health-related quality of life and could also be involved in more rapid deterioration of cognitive and motor functions. However, few studies have investigated this association for periods of longer than 10 years, with any long-term findings so far inconclusive. For the study, the researchers used a cohort consisting of all Swedish citizens aged 50 years and above as of December 31st, 2005. From this group, they then took the 140,688 people diagnosed with depression . These individuals were each matched with three control participants (a total of 421,718 controls) of the same age and sex who had not been diagnosed with depression. The participants were then followed for up to 26 years. A total of 1,485 people with depression (1.1%) developed Parkinson's disease during this time, compared with 1,775 of those who did not have depression (0.4%). On average, Parkinson's disease was diagnosed 4.5 years after the beginning of the study, with the likelihood of the disorder developing decreasing over time. No sibling link found for depression and Parkinson's disease The researchers calculated that participants with depression were 3.2 times more likely than those without depression to develop Parkinson's disease within a year of the study beginning. After 15-25 years, the researchers found participants with depression were almost 50% more likely to develop the condition. If a participant's depression was severe, their likelihood of developing Parkinson's disease was also higher. For example, those who had been hospitalized for depression five or more times were 40% more likely to develop Parkinson's disease than participants who had been hospitalized for depression just once. In addition to these observations, the researchers examined siblings. No link was found between one sibling having Parkinson's disease and the other having depression. "This finding gives us more evidence that these two diseases are linked," says Prof. Nordström. "If the diseases were independent of each other but caused by the same genetic or early environmental factors, then we would expect to see the two diseases group together in siblings, but that didn't happen." The authors suggest there are a number of mechanisms that could explain their findings. Depression or antidepressive treatment could increase the risk of Parkinson's disease, depression could be an early symptom of Parkinson's disease, or that the two conditions could share environmental causative factors. In the paper, the authors acknowledge that they are unable to evaluate the potential role of substances used in antidepressive treatment as risk factors for Parkinson's disease. The study is an observational one and cannot determine causation. "Our findings suggest a direct association between depression and subsequent [Parkinson's disease], supported by a time-dependent hazard ratio, a dose-response pattern for recurrent depression, and a lack of evidence for coaggregation among siblings," the authors conclude. "Given that the association was significant over more than two decades of follow-up, depression may be a very early prodromal symptom of or a causal risk factor for [Parkinson's disease]." Elsewhere, a study published in December previously suggested that users of methamphetamine are at three times more risk of getting Parkinson's disease than people who do not use illegal drugs.

Abstract: Understanding kinetic control of biological processes is as important as identifying components that constitute pathways. Insulin-signaling (IS) is central for almost all metazoans and its perturbations are associated with various diseases and aging. While temporal phosphorylation changes and kinetic constants have provided some insights, constant or variable parameters that establish and maintain signal topology are poorly understood. Our iterative experimental and mathematical simulation-based approaches reveal novel kinetic parameters that encode concentration and nutrient dependent information. Further, we find that pulsatile fasting insulin rewires IS akin to memory and in anticipation of a fed response. Importantly, selective kinetic gating of signals and maximum connectivity, between metabolic and growth-factor arms under normo-insulinemic states, maintains network topology. In addition to unraveling kinetic constraints that determine cascade architecture, our findings will help in identifying novel therapeutic strategies that conserve coupling between metabolic and growth-factor arms, which is lost in diseases and conditions of hyperinsulinemia.
