In this week's episode, we'll discuss new research revealing that the mechanosensory ion channel Piezo1 is the elusive carrier molecule of the Er blood group antigens, thus establishing a new blood group system. Next, we review results of a randomized phase 3 trial of enasidenib versus conventional treatment in late-stage mutant-IDH2 relapsed or refractory AML. Although the primary endpoint of overall survival was not met, investigators say the risk benefit ratio remains positive. Finally, we'll review a study showing a clinically significant risk of breakthrough COVID-19 infections in patients with B-cell malignancies despite vaccination and pre-exposure prophylaxis with tixagevimab-cilgavimab during the Omicron era. However, hospitalization rates in the study were low and no deaths were reported.

By Chris Nichols Updated at 10:54 a.m. on September 24, 2021 Reports about vaccinated Americans catching COVID-19 are energizing vaccine skeptics and leading to more misinformation about efficacy and breakthrough cases. CapRadio's PolitiFact California Reporter Chris Nichols examined the facts about breakthrough infections in this week's Can You Handle The Truth. Nichols sat down and spoke with CapRadio's afternoon anchor Randol White and broke everything down. This interview has been edited for clarity and length. Interview Highlights On how rare breakthrough COVID-19 cases are feeding into misinformation One example is a popular and misleading post on Instagram. It wrongly claimed that the vaccines are “the first in history to not prevent people from catching or spreading a virus” and that they only last a few months. PolitiFact found that is completely false. No vaccine is 100% effective. That's the case for the flu vaccine and also the case for the COVID-19 vaccines. The U.S. Centers for Disease Control and Prevention says on its website some fully vaccinated people will get sick, some will even be hospitalized or die from COVID-19, and that vaccinated people with breakthrough infections can also spread the disease. On current COVID-19 cases in unvaccinated people Studies show vaccinated people are eight times less likely to be infected — and 25 times less likely to be hospitalized or die from COVID-19 than unvaccinated people, according to the CDC. And those who are fully vaccinated and get one of these breakthrough cases are less likely to suffer a serious illness. On the false claim that vaccine efficacy ‘only lasts for months' It's still not clear how long the protection provided by the vaccines will last. Health officials are still debating whether everyone will need a booster shot. But the last part of the social media post is misleading because plenty of other vaccines require boosters. People are asked to take the flu vaccine every year, and vaccines against Hepatitis B, whooping cough, chickenpox and measles all require multiple shots. On the odds of a fully vaccinated person developing a breakthrough case The New York Times found that the average vaccinated American's odds of getting a breakthrough infection are roughly 1 in 5,000 a day, or 1 in 10,000 in highly vaccinated states. They used data on daily average COVID-19 cases in different parts of the country to arrive at these odds. A recent CDC report includes data showing the odds of a breakthrough might be even lower, at about 1 in 5,000 per week. PolitiFact found all of these statistics have some limitations. For example, they don't take into account the undercounting of breakthrough infections. But, they do show the vaccines are reducing infections and the overall risk of a breakthrough is low. Correction: A previous version of this story misstated the risk of breakthrough infections. They are rare, but not exceedingly rare.

Epidemiology Counts from the Society for Epidemiologic Research
The arrival of COVID-19 vaccines that are highly effective against infection and severe disease in late 2020 appeared to be the silver bullet that would end the pandemic and bring life back to the way it was in pre-pandemic times. But the emergence of the highly infectious Delta variant of the virus, coupled with large portions of the eligible public remaining unvaccinated, has dampened much of this initial hope and led to what is being called the Fourth Wave of the pandemic. The surge in infections and hospitalizations in this latest wave is primarily in the unvaccinated; however, many vaccinated persons are experiencing “breakthrough infections” in which the virus evades the protection afforded by the vaccine. How can we interpret what these breakthrough infections mean regarding the effectiveness of the vaccine and the need for continuation of masking and other behaviors? In this episode, we aim to provide tools to prepare you to interpret the many reports on breakthrough infections encountered on the news and other media. Host Bryan James talks to Justin Lessler, now a professor of epidemiology at the University of North Carolina Gillings School of Global Public Health, and Lucy D'Agostino McGowan, assistant professor of statistics at Wake Forest University, about breakthrough COVID-19 infections and the Delta variant. They also talk about unvaccinated kids returning to school amongst all of this and the potential for booster shots.

"I believe that we should be radically transparent and honest," says Dr. Poland. "The more time this virus passes through one person after another, the more likely it continues to mutate. As a result of those mutations, two things are happening. Some of the mutations are making vaccines and plasma monoclonal antibodies less effective. The other thing is that the virus will likely become something that we have to live with for the rest of our lives." Dr. Poland reminds people that vaccine protection is not 100%. Breakthrough infections can occur. "Remember that in the clinical trials, 95% means that compared to unvaccinated people, your risk is reduced by 95% — not 100%," he says. "You might have a mild case of COVID-19,but you can still spread it to others, including those who are immune compromised, such as cancer patients. That's why we continue to wear masks until we get very widespread immunization." You can breathe a sigh of relief. You’ve gotten your COVID vaccine and now it’s been two weeks.You’ve got full protection against the virus now--and your friends (who haven’t gotten the shot yet) are feeling “vaccine envy.”That’s how it supposed to work, in an ideal world. But by the end of March, Minnesota had already documented around 90 cases of breakthrough COVID in people who were fully vaccinated.That’s what you and your non-vaccinated friend have in common. You can both still become infected with the COVID virus. But the differences are more significant. Though fully-vaccinated and non-vaccinated people can contract COVID, the fully-vaccinated person is less likely to experience severe symptoms. Of the nearly 90 cases of breakthrough COVID, not one has died.Another difference: fully-vaccinated individuals are far, far less likely to contract COVID. That shows the vaccine is doing its job. Health experts estimate that less than a tenth of one percent of fully-vaccinated Minnesotans will become infected.Breakthrough COVID is not a surprise. The Pfizer and Moderna vaccines were about ninety-five percent effective in clinical tests. The other five percent of the time, the virus was able to slip past the vaccine’s defenses.Yet even with that five percent, the vaccine is still the best bet for staying out of the hospital and staying healthy.
