Ever seen a patient need HUGE amounts of their intestine removed because of an awful foreign body? Or an intusussception? If so, you may well have come across short bowel syndrome before (and if you haven't, bank this info, because you'll likely need it in the future). Short bowel syndrome is a severe disorder that occurs due to massive intestinal resection. These patients can't absorb water, electrolytes and nutrients from their food, causing significant changes in their hydration, electrolyte and nutritional status - amongst many other things. They're incredibly challenging to manage, and they need intensive nursing care - which gives us a LOT of ways to help them, and many skills we can use in the process. In episode 59 of the Medical Nursing Podcast, we're diving into exactly what SBS is, how it impacts our patients, and (most importantly), how we can give them great nursing care. ---

"The first line treatment for adolescents with anorexia now is family-based therapy typically, which involves helping the parents facilitate the refeeding of the adolescent. So, I was working with the patient in that way and found it to be helpful and useful, but was consistently struck by the neglect of the patient's inner life, and found, at least based on my experience with many patients, that while you could get some symptomatic relief, if you didn't, in some way, address the deeper dynamics, the aspects of the patient's personality organization that drove the disorder, that were implicated at the disorder, there was a way that the patient would snap back to their old behaviors over time, that deeper change and a deeper understanding of what was going on was really necessary; and so that's been kind of evolution from my work over the past ten years from my first book, which was about anorexia in males, and tried to present a kind of Integrative understanding of that phenomena, increasingly over time I've become more and more interested in the deeper kind of analytic thinking that we can bring to bear on this kind of suffering.” Episode Description: We begin with a description of the common contertransferential pull to intervene behaviorally in the face of repetitive self-destructive eating disorder symptoms. This intention can inform but not compel the clinical decision as to the indicated treatment of choice for someone at any particular moment. Behavioral and pharmacologic treatments can be important in softening the pressure of eating disorder symptoms. They do not, however, give an individual access to their interoceptive life, from which these disturbing self-preoccupations emerge. We discuss the challenges of working with those who have limited capacities for mentalisation and as a result, live out their inner lives somatically and motorically. Immersive treatment leads the clinician to experience these proto-affects in one's own body and in one's own ruminations. Tom discusses alexithymia, typical family structures, and the presence of the 'abject' experience in the lives of these patients. He presents a disguised case of a patient who was able to work through both the early struggles and later neurotic aspects of these conflicts analytically. We close with his sharing with us his vision for the future which includes more integration between the dynamic and adynamic approaches to these challenging patients. Our Guest: Tom Wooldridge, PsyD, is Chair in the Department of Psychology at Golden Gate University as well as a psychoanalyst and board-certified, licensed psychologist. His first book, Understanding Anorexia Nervosa in Males, was published in 2016. His second book, Psychoanalytic Treatment of Eating Disorders: When Words Fail and Bodies Speak, an edited volume in the Relational Perspectives Book Series, was published in 2018. His third book, Eating Disorders (New Introductions to Contemporary Psychoanalysis), was released in 2022. His fourth book, co-edited with Burke, Michaels, and Muhr, is entitled Advancing Psychotherapy for the Next Generation: Rehumanizing Mental Health Policy and Practice. He has also written a novel about the process of psychotherapy, Ghosts of the Unremembered Past, additionally released as an audiobook. He is a Personal and Supervising Analyst at the Psychoanalytic Institute for Northern California and a Training Analyst at the San Francisco Center for Psychoanalysis. OB/GYN Dr. Julio Novoa, who not only provides excellence in women's health care but is renowned for his minimally invasive techniques, joins the HMP to discuss all things foreign body reactions, metal hypersensitivity, ASIA syndrome and beyond. Not only a OB/GYN, Dr. Novoa also specializes in cosmetic gynecology but might just shine as the brightest light in his concern for women's safety and advocacy, who agrees more with a patient-centric model for healthcare as opposed to approaching medicine strictly from a business perspective. Seeing a trend towards unnecessary medical procedures and not being a fan, he became alarmed with routine practices in surgical standards that harmed the patient. Noticing a large subset of patients with severe symptoms from foreign body implantation, such as the Essure permanent sterilization device, surgical clips and staples, leaky breast implants and more, he knew he had to speak out and continues to do so today. We discuss a newer-term and evolving diagnoses: ASIA Syndrome or Autoimmune Inflammatory Syndrome Induced by Adjuvants. Dr. Novoa explains what an adjuvant is and why it matters. We discuss various types of foreign body reactions and why surgeons should be able to readily explant anything they are keen to implant. We also discuss localized and systemic hypersensitivity reactions and whether or not everyone who has them can be classified under the definition of the ASIA effect. Dr. Novoa goes on to detail clinical observations and personally observed results from the large amount of explant surgeries he has performed and witnessed. More and more complete resolution of the patient's vast symptoms upon explantation became the rule, rather than the exception and he shares some of those experiences. He states his opinion on whether asymptomatic patients with implants should consider having them removed after healing has occurred. A lengthy discussion ensues surrounding how to affect change within the medical communities, physicians groups and governing bodies and Dr. Novoa offers his opinion as to why physicians aren't putting two and two together and completely missing the boat when patients present with systemic and localized foreign body reactions. We also go into the super-serious topics surrounding informed consent and how we need to up our game with our own surgical consents and medical records. This episode will encourage patients to look more closely prior to surgery and to find their voice when experiencing practices that could increase the risk of foreign body reactions. Pine Tree Foundation Endowed Lecture In conversation with Maiken Scott ''A historian of prodigious and varied gifts'' (San Francisco Chronicle), Simon Schama is the author of 20 books, including The Embarrassment of Riches; Scribble, Scribble, Scribble; and the National Book Critics Circle Award winner Rough Crossings, an account of the enslaved people who escaped to fight for the British during the American Revolutionary War. A professor of art history and history at Columbia University, he has written and presented more than 40 documentaries for the BBC, PBS, and The History Channel, including the seminal 15-part series A History of Britain, the Emmy-winning Power of Art, and The Story of the Jews, based on his two-volume millennia-spanning work. Schama is a fellow of the Royal Society of Literature and in 2018, he was knighted for his contributions to historical scholarship. In Foreign Bodies, he offers a vigorous cultural history of the complex relationship between pandemics and the crusaders who battle them. Maiken Scott is the host and executive producer of WHYY's The Pulse - a weekly, national health and science radio show and podcast that explores the people and places at the heart of health and science. Most clinicians have encountered the post-holiday tidal wave of patients that swallowed a pork rib or ate a chicken bone from the trash can. These pups just can't help themselves! So, what do you do? Bone foreign bodies are common, but when do you need to intervene versus letting the dog try to pass them on their own? To help answer these questions, Barash et al performed a study describing the outcome and the management of dogs with both esophageal and gastric bone foreign bodies.
