On this episode Razib talks to Jesse Singal, a journalist who has covered the social science beat for the last decade. Singal has an undergraduate degree in philosophy from University of Michigan and a master's in public affairs from Princeton. Currently a freelance journalist who writes his own Substack, Singal-Minded, and contributes to Blocked and Reported with Katie Herzog, Singal is formerly an editor at New York Magazine. His first book The Quick Fix: Why Fad Psychology Can't Cure Our Social Ills, covered the replication crisis. Razib and Singal first talk about what he learned, and unlearned, during his time as a reporter at New York Magazine, especially social psychology results that were long on glamor but short on robustness. They discuss how long we've known that social psychology had a problem, and whether it still hasn't reformed itself. Singal also reflects on his role in publicizing sexy findings, and how journalism has taken steps to be more careful lately. They also address some of the specific findings that came out of early 2010's social science, from implicit bias to power posing. Next, Razib asks Singal about youth gender medicine, and the major controversies over the last few years. Singal discusses the differences between female to male transitions as opposed to male to female, and relates the whole domain back to the replication crisis and the lack of good research. They also discuss political and social aspects, and where Singal sees youth gender medicine going in the next few years.

Since then, she has become known as the most prominent whistleblower in the effort to put the brakes on medicalized gender transition for kids. In this conversation, Jamie talks about what the last year and a half has been like for her, what the public still needs to understand about this issue, and why doctors and other medical providers are continuing to misrepresent their treatment protocols. She discusses how institutions serving the most vulnerable kids, including foster care systems (where large numbers of kids now identify as trans), have adopted affirmative care models and explains what it's like to Testify before state legislatures about restricting access to non-evidence-based gender-affirming care. As a self-described “queer woman who's married to a transgender person and is politically to the left of Bernie Sanders,” it's the last thing she ever thought she'd be doing. Now it's her life's work. GUEST BIO Jamie Reed is one of the first public whistleblowers from a pediatric gender clinic in the United States and is now the Executive Director and Co-Founder of the LGBT Courage Coalition, an American-based non-profit of LGBT adults seeking to reform youth gender medicine. She has spoken at numerous conferences including Genspect: The Bigger Picture in Colorado, at the International Perspectives on Evidence- Based Treatment for Gender-Dysphoric Youth in New York, and Psychotherapeutic Process with Young People Experiencing Gender Dysphoria in Tampere, Finland. Jamie Reed worked in a "gender clinic" and was shocked at the dearth of evidence and rush to transition minors that she saw—thus she stood up and spoke out. Aaron Kimberly has a similar story. Together they've founded "LGBT Courage Coalition" which is a group of LGBT adults that are working to rework standard practices in medicine, as well as public perception, of the so-called "gender affirming" model of youth health care. In this episode of the Year Zero podcast, which was recorded in July, we discuss the fast evolving state of the legislative bans on child sex trait modification procedures and the wider project to bring an ongoing medical scandal to heel. Excerpts:Parallels between the opioid epidemic and pediatric gender medicine“There are really interesting parallels between the opioid epidemic and the gender medicine issue. In both cases, for example, it started with medical authority, citing very, very low quality studies, claiming that the use of opioids in cancer patients could be scaled at large, and that the same type of risk benefit ratio would would apply to other types of patients — patients who are not, for example, terminally ill, or even patients with a history of addictions and patterns of addictive behavior. And that, of course, turned out not to be the case. The medical associations got involved and also fueled the opioid epidemic, sometimes for good intentions, sometimes, as in the case of the American Pain Society, because they were influenced by Big pharma money. So there, I think in the case of the opioid epidemic, there was just a clear villain, and the villain was Purdue Pharma. And it was a villain that the American Left loves to hate. Big corporate interests that make money off of health care. Here, I wouldn't want to argue that there aren't kind of pharma interests involved in general medicine, of course, there are, it's a big business, and it's growing too. And we have some evidence that clinics and hospitals that do the procedures as opposed to the psychotherapy get a net benefit from them. And they also, of course, boost their scores on the Corporate Equality Index, and, you know, ESG, and all that stuff. So there's definitely a lot of institutional self interest in doing so called gender affirming care. But I wouldn't argue that the financial motives are primary here. I think that they they definitely play a role. But I would say that the ideology is by far and away the most important factor. But in any case, there are strong parallels to the opioid epidemic. And, you know, this is another example of how we don't learn our lessons.On the capture of American policymaking by the NGO Borg “The ACLU never has to face voters. It's accountable only to its own funders. And those funders are increasingly a small number of deep pocketed foundations, individuals or corporations that give money. And often these people either don't know what's going on or they are themselves highly ideological and out of tune with what what most Americans agree on. And so the first problem is that with the ACLU, there's no mechanism of accountability between these interest groups and the general public in a way that there is with political parties in the general public. The second problem is that these interest groups almost by definition, are going to get more and more extreme over time, right? Because they usually they're founded for a certain purpose, they have a strong sense of mission. And that sense of mission tends to attract people who are ideologically aligned with that mission. So if it's the ACLU — first of all civil liberties, then it was civil rights, now it's what you might call — what you have called “Successor Ideology.” If it's environmentalism, who's gonna go work at the Environmental Defense Fund, who is going to go work at at the UN — all these organizations: environmentalists, people who are very, very gung ho about environmentalism, and importantly, people who are willing to impose very costly policies on the public, because the kind of trade offs that they're willing to accept are much more extreme than the kind of trade offs that the average citizen would accept. So these groups, the ACLU, for example, will attract young, very ideological, very woke lawyers — people like Chase Strangio to its ranks and promote them. And so gradually, over time, these groups tend to become more and more extreme in their positions. And because of the way in which the American political system gives them so much power and so much leverage over the policy process, we end up with the politics of radical policymaking. So it's not that other countries don't have similar problems. But here in the United States, because of how our system of government works, these problems tend to be much more extreme and much more intractable.”Thank you for reading Year Zero. Even in a time of culture wars and controversy about everything, there are few subjects as difficult to discuss as transgender rights. In less than a decade the number of young people identifying as trans has exploded, and with it the number of arguments about the subject. In British schools nearly 80% of teachers report trans-identified or non-binary students in their classroom. Supporters say being trans is innate, like being gay, and that this is like the gay-rights movement of 30 years ago, with lots of trans kids coming out of the closet to be themselves. Critics say being trans is not innate and that many kids are identifying as the opposite sex because it is cool, and because they are being indoctrinated by a new ideology, fed to them on social media and by activist teachers. The debates have become most heated when it comes to actual medical transitioning. In all of the disagreement, what frequently gets lost is the need for compassion for anyone struggling with dysphoria. But how far should compassion go? Should it allow any teenager who says they are trans to medically transition? What about the increasing number of studies showing that many such kids have other serious mental-health issues? Laura Perry Smalts who was born female, and lived as a young man for almost 9 years. She took cross-sex hormones for years and had two major surgeries. She now regrets ever having done it, has detransitioned, and wants to warn others of the dangers. Earlier this month, the Human Rights Campaign declared a national state of emergency for LGBTQ people, following "the worst year on record" for the number of anti-LGBTQ legislative proposals and wins. The ACLU mapped and tracked 491 bills this session, including two in Connecticut. Many of those proposals are aimed at gender-affirming health care. There are several protections for trans and nonbinary people in Connecticut, and in some cases, beyond the border. Connecticut's Safe Harbor Law, passed in response to the Supreme Court's overturning of Roe v. Wade in 2022, includes legal protections for people seeking gender-affirming health care from out-of-state. But advocates say the state is not immune from ideological or political attack. Diana Lombardi, the former executive director of the Connecticut TransAdvocacy Coalition, and a current member of Governor Lamont's Hate Crimes Advisory Council, notes the favorable political climate and support from "the governor on down." But "my concern here in the state of Connecticut is that pressure would be mounting to do away with our health care," she says. In the wake of such comprehensive exploration and analysis around the foundational medical research cited time and time again regarding child and adolescent gender transition, the Dutch Protocol, we extended an invitation to Dr. Stephen Levine and Dr. Julia Mason to join us in a discussion about their most recent paper, co-authored together with Zhenya Abbruzzese in a thorough critique of the Dutch Protocol.Dr. Stephen B. Levine is Clinical Professor of Psychiatry at Case Western Reserve University School of Medicine. He is the solo author of five books, all of which are concerned with love and sexuality. He has been teaching, providing clinical care, and writing since 1973 and has generated over 180 publications, 35 of which have been related to gender dysphoria. He and two colleagues received a lifetime achievement Masters and Johnson's Award from the Society for Sex Therapy and Research in March 2005. His recent publications on gender dysphoria have been read by thousands of people.Dr. Julia Mason is a pediatrician who started medical school in the Medical Scientist Training Program but emerged with a Master's degree in Nutritional Science rather than the planned PhD. This extended her medical education and kept her grounded in the scientific basis of medical care, which may have made her more likely to be skeptical of pediatric “affirmative gender care” when she saw it taking root in her hometown of Portland, Oregon. Julia is a founder and board member of SEGM, the Society for Evidence-based Gender Medicine. She is a full-time practicing pediatrician in Gresham Oregon, her gender work is extracurricular and entirely voluntary.Our conversation with Drs. Levine and Mason evolved into a pretty intense exploration of sexuality and the consequences that blocking an adolescent's puberty has on their developing sexuality. We explore why so many adolescents are consciously or subconsciously seeking to block their sexuality and seek, sometimes desperately, to avoid opportunities for experiencing natural sexual development. And what about the impact and consequences of easy access to extreme varieties of pornography? But most importantly, why are we robbing young people of the opportunity to develop their sexuality? The infusion of gender ideology into medicine has led politicized pockets of the medical industry to prey upon vulnerable children who experience distress with their sexed bodies. Rather than working to help these youth become comfortable with their bodies, many medical organizations and practitioners push “gender affirming care,” which starts with changed pronouns and “social transition," and moves on to puberty blockers, cross sex hormones, and even sterilizing surgery.When Chloe Cole was only twelve years old, she began taking puberty blockers. Soon after, she socially transitioned to begin presenting as a boy and began taking testosterone. At age fifteen, on the advice of her doctors, she underwent a double mastectomy. For years I covered stories about the LGBTQIA+ community with a focus on the transgender medicine, the community and their stories. With over 1.4 million Americans identifying as transgender or non-binary, there is still a lot of work to be done when it comes to showing up for others and ourselves. Ojeda se unió al Departamento de Pediatría del Centro Médico de la Universidad de Mississippi en marzo del 2009 como profesora asistente. Actualmente es Profesora y Vice-Decana de Investigación en el Departamento de Pediatría de esa institución y es miembro de varios comités de investigación a nivel Institucional. Es una investigadora altamente productiva y ha publicado su trabajo en Hypertension, American Journal of Physiology-Reanl Physiology, Gender Medicine y Advances in Chronic Kidney Diseases. The results from the first statewide LGBTQ+ needs assessment survey are in. More than 3,000 residents weighed in, informing the first-of-its-kind report for Connecticut's LGBTQ+ Health and Human Services Network. This hour, we focus on health care for transgender and nonbinary residents. "Transgender respondents were 11 times more likely than cisgender respondents to have been refused health care services because of their LGBTQ+ identity," according to the report. Dr. Gillian Einstein is a Professor in the Department of Psychology at the University of Toronto where she leads the Einstein Lab on Cognitive Neuroscience, Gender & Health. She is also the Founder of the Collaborative Specialization in Women's Health at the University of Toronto, The Wilfred and Joyce Posluns Chair of Women's Brain Health and Aging, and Guest Professor of Neuroscience and Gender Medicine, Linköping University, Linköping, Sweden.
