This week's expert, Hepatologist and Key Opinion Leader Scott Friedman, joins Roger to discuss advances in acceptance of gene therapy and knowledge in other areas of basic liver science. When discussing science, he pays particular attention to findings on the diversity of stellate cells and his interest in CAR-T as a therapy for liver disease.This conversation starts with Scott discussing gene therapy. Specifically, he applauds the idea that gene therapy is  becoming accepted in many diseases after a faulty start years ago, due to an unfortunate patient death in a badly controlled trial.  He comments that this acceptance has unique benefits in liver disease because the liver can regenerate so much faster and more efficiently than other organs. He mentions some of the rare liver diseases in which patients are benefiting from gene therapy, and notes that we now have gene therapies and early-stage trials to target PNPLA3 and  other genes associated with MASH and MASH cirrhosis. Next, Scott discusses stellate cells, which he has discussed in earlier episodes of SurfingMASH. Science is increasingly demonstrating how many different types of heterogeneous stellate cells exist. As Scott puts it, these cells "come in many flavors," each of which plays a different role in cell generation or cell death. In fact, the specific therapeutic challenges that present themselves may vary as a patient moves along the pathway from F1 to F2 to F3 to F4. Further, we are learning that there may be several different forms of MASH to present differently at a cellular level. This makes tremendous sense, given that no one drug has proven successful in even a significant majority of patients yet. As the conversation winds down, Scott shares what he describes as a "sobering note" about the state of research funding in America in 2025. As he notes, there are certain kinds of applied and developmental research that private companies do well, but other kinds of basic research that only occur when funded in public and not-for-profit sectors. As a specific example, he cites CRISPR, initially funded publicly and now in the hands of biotech companies, which is used to treat a variety of diseases more effectively than they could have been treated before, if at all. He also comments that a poor early commercial decision slowed the development of statins.

This week's expert, Hepatologist and Key Opinion Leader Mazen Noureddin, joins Roger to discuss major advances in drug development over the past year. He covers a range of different drug classes, focusing on stages of development and the range of options within each class. First, Mazen discusses a tremendously exciting group of FGF-21 agents, specifically mentioning Akero Therapeutics's efruxifermin, 89bio's pegozafermin, and Boston Pharmaceuticals's efimosfermin. He points to efruxifermin's 96-week results to suggest that FGF-21s might be appropriate for a wide range of patients, the idea that the drug's duration of effect may make the idea of “induction therapy” less appropriate, and the exciting early data on cirrhosis patients. He also mentions pegozafermin's publication of data in the New England Journal of Medicine and efimosfermin's promising data based on monthly dosing. Next, Mazen provides some detail on the various incretin agonist options, why hepatologists are particularly excited about combinations that include a glucagon agent, and what kinds of results we might expect in upcoming trials. Finally, Mazen discusses other promising compounds in later-stage development, including the pan-PPAR lanifibranor and the FASN inhibitor denifenstat. He notes ongoing work on new classes and combination therapies.

Dr. Steven Herrine is the chair of medical education and professor of medicine here at SKMC at Jefferson and a practicing hepatologist and gastroenterologist. He grew up in NYC and studied math and history at Oberlin College, after which he spent a few years as a chef, and then went to Jefferson medical school, Penn, for IM residency and Jefferson for GI/hepatology, where he has stayed since. He has won multiple awards for teaching, including IM residency teacher of the year, which makes sense because he now runs medical education as the chair here at SKMC. This is a scary interview for me because if I say anything bad, Dr. Herrine can of course, kick me right out of medical school with only a couple months left. Welcome to episode 31! In this week's coffee with consultants feature, we break new ground by featuring our first interview with a consultant from the Western Cape! Thanks to the nomination of their intern curator by a number of the Groote Schuur interns, I had the immense privilege and honour of interviewing hepatologist Professor Mark Sonderup at the Liver Clinic at Groote Schuur Hospital.  Professor Mark Sonderup is an internal medicine physician and specialist Hepatologist at the University of Cape Town (UCT) and Groote Schuur Hospital (GSH). He has research interests in the treatment of chronic and acute liver diseases, including HIV, viral hepatitis and drug-induce liver injuries. We discussed his journey into and through medicine, including his pharmacy degree which he completed prior to entering medicine at UCT, his internship experiences, and what led to him choosing to specialise in internal medicine, with the experience of becoming a registrar at the height of the HIV/AIDS crisis in South Africa, as well as some of the interesting factors around that time such as the HIV-denialism that had wide-reaching implications for patients and the public health system. In the latter half of this episode, we also discussed his role mentoring, monitoring and guiding interns and medical students, and some of the observations he has made from this and what advice he has for junior doctors. If you're someone who has been grappling with indecision, or experienced self-doubt in your journey to becoming and being a doctor, you'll definitely want to stick around for that because it is seriously rich and wise insight and advice to digest and take to heart. Professor Jacob George is a renowned hepatologist and liver research scientist who studies the causes of and mechanisms for the development of liver disease and liver cancer. Professor George's work has made significant contributions to clinical practice. His team first identified the role of interferon lambda 3 gene polymorphisms for predicting treatment response in chronic hepatitis C, and a second gene polymorphism that interacts with interferon lambda 3. These discoveries are considered major advances in the field and the finest examples of ‘personalized medicine'. Professor George was an author of the groundbreaking study of combination telaprevir, pegylated interferon, and ribavirin therapy for hepatitis C, which changed the landscape of hepatitis C treatments, heralding the era of direct-acting antivirals. His work identifying insulin resistance as the universal underlying pathophysiological abnormality in fatty liver disease underpins the majority of research on this condition and is the cornerstone of current therapy (lifestyle intervention; exercise; insulin-sensitizing agents). He also first described the association between hepatitis C and insulin resistance. Professor George is an advisor to Hepatitis Australia, the Transfusion Related AIDS and Infectious Diseases Service, and at state and national levels, on viral hepatitis. He oversees research for the Asian Pacific Association for the Study of Liver. He is a Fellow of the American Association for the Study of Liver Disease. The Impact Research Institute (IRI) is Waco, Texas' resource for extending care options for NAFLD (non-alcohol fatty liver disease.) Nadege Gunn is IRI's Medical Director and President. She joined Kevin Stevenson on I Don't Care to discuss her and her team's mission to treat NAFLD and other related diseases and conditions.Dr. Gunn is a Gastroenterologist and Hepatologist whose main passion is the liver. “The liver is an amazing organ,” Dr. Gunn said. “It's the only organ in the body that you could cut in half, and it can grow back in full size in about three months. The work we do in transplantation and replacing the liver is quite amazing, so that's why I chose the field in general.”Dr. Gunn helped open IRI in July of 2021. Necessity created the decision to develop the center. While practicing clinical trials in Austin, Texas, Dr. Gunn saw many patients traveling from the Waco area to receive treatment options for NAFLD. With no scan or clinical trial options available to people in Waco and the surrounding area, Dr. Gunn recognized an opportunity to open a clinic to serve the growing needs.Community outreach is critical to the clinic's role in Waco, TX. When they identify candidates who may have NAFLD, they do a non-invasive scan at the center to determine if the candidate does have the disease. “If they do, then we invite them to maybe participate in a clinical trial because there are no treatments for fatty liver,” Dr. Gunn said.For people wondering if they might have NAFLD, Dr. Gunn said the disease is often asymptomatic in the early stages. “It is when the disease is progressing, when it is starting to injure and harm the liver, that people might start firstly feeling fatigued,” Dr. Gunn said. “They might feel pain in the right upper quadrant of their abdomen and their belly.” Recognizing these warning signs and seeking a diagnosis is critical before full-on cirrhosis of the liver occurs.

In this episode, hepatologist Dr. Parvez Mantry joins our host Dr. Christopher Beck to discuss portal hypertension and ascites, two complications that arise from liver diseases and have a large effect on patients’ quality of life. Dr. Mantry starts the episode by sharing statistics on Chronic Liver Disease (CLD) and specifically highlights the burden of CLD on the U.S. population. He discusses various causes of CLD, including Hepatitis C, Alcohol Liver Disease, and Non-Alcoholic Steatohepatitis. Next, Dr. Mantry takes us through his diagnostic workup, including his physical examination, cross-sectional imaging, percutaneous and transjugular liver biopsies, and serological workup. He checks for complications such as portal hypertension, ascites, pedal edema, and hepatic encephalopathy. We also discuss the diagnosis of Hepatocellular Carcinoma (HCC). Then, we transition to strategies for managing ascites and portal hypertension through diuretics, paracentesis, Transjugular Intrahepatic Portosystemic Shunts (TIPS), liver transplantation, and a few experimental treatments that he is currently researching. Overall, Dr. Mantry advocates for close monitoring of symptoms and making treatment modifications as needed. Finally, when focusing on the patient experience, Dr. Mantry offers troubleshooting tips, especially for patients who struggle with leakage from the paracentesis site. To close, he gives insights on how physicians can best support chronically-ill patients who are awaiting transplantation. As the only liver specialist in the Baton Rouge region she has multiple jobs. She started The Liver Center Ochsner Baton Rouge to provide high level liver care to the region. Dr. Tyson is head of The Liver Center, which has grown significantly over the past 6 years and receives referrals from around the state. She is also a Clinical Assistant Professor of Medicine at Louisiana State University where she staffs the liver disease clinic with LSU Baton Rouge residents and provides care to the underserved. At LSU she gives didactic lectures to the residents and medical students. She also started a special program at LSU where primary care providers can give specialized care by treating hepatitis C virus (HCV) positive patients. Dr. Tyson utilizes her Master of Public Health degree and clinical expertise as the Clinical Network Specialist for the Louisiana Department of Health's initiative to eliminate hepatitis C (HepCured LA). Louisiana is the first start to have this type of public initiative. In this role she has helps create the training documents and provides the training to primary providers around the state of Louisiana to treat HCV. In less than one year, the HepCured LA initiative has created over 300 new hepatitis C treaters and treated more the 4,200 persons with HCV. More patients on Medicaid have been treated than the previous 5 years combined. She has published multiple peer reviewed articles on hepatitis C, hepatitis B and hepatocellular carcinoma. Dr. Tyson is also an entrepreneur. She is the founder and CEO of Med School Admit (MSA). MSA has programs focused on helping premedical students become competitive medical school applicants. She is proud of all the work she is doing to create healthier Louisianans and is particularly motivated to make an impact on students around the country who want to see their dreams of becoming a doctor realized. Dr. Tyson is a native of New Orleans, Louisiana who attend college at Howard University, Harvard Medical School, Johns Hopkins for residency, Baylor for GI fellowship and Tulane for transplant hepatology. She is a wife and mother of three children who in her spare time enjoys movies and endurance sports. While at Howard University Dr. Tyson was very involved. She was a student athlete, playing tennis for 3 years and serving as team captain. She served as junior class president in the College of Arts and Sciences. She was a member of Phi Sigma Pi honor fraternity and Alpha Kappa Alpha Sorority, Inc. During her senior year she was the president of Alpha Kappa Alpha. She took full advantage of her Howard years, researching at the National Institutes of Health and travelling to Africa during her summers. Given her well rounded activities while maintaining a 4.0 gpa Dr. Tyson applied for the Rhode's Scholarship and was a finalist. Ponni Perumalswami treats patients who have advanced liver diseases. She also is working to reach communities at risk for viral hepatitis B and C to connect them to testing, education, and healthcare. She explains The differences, such as transmission means, between hepatitis B and C; The reason why some foreign-born communities are at risk and how her group is trying to make their way into the center of these groups; and Why these diseases of the liver, while usually asymptomatic for years, can cause damage leading to treatments like liver transplantation.  Ponni Perumalswami is an associate professor of medicine in the Division of Liver Diseases at Mt. Sinai School of Medicine. She tells listeners that there's an at-risk population for hepatitis B and C that she and her colleagues are targeting. In the U.S., while the diseases are less common in the general population, at-risk groups who immigrate from areas with higher rates, specifically Asian and African-born communities, are hard to reach. Because they may not have insurance and are not English speakers, they aren't in touch with primary care doctors who might normally screen for these diseases and they can be asymptomatic for years. Her outreach efforts include testing within the communities and awareness-raising efforts. Dr. Perumalswami explains how these diseases work. She explains that hepatitis B is a DNA virus that largely infects foreign-born populations because the U.S. has had the means to vaccinate and test for this disease. It spreads by a vertical transmission from mother to child. Hepatitis C is transmitted through blood exposures, so through intravenous drug use, blood transfusions, and organ donations done before 1992 when testing became available. Even though each can exist in the body silently for years, they can still do tremendous damage in the liver, and increase one's risk for cirrhosis, cancer, and other disease of the liver that may need liver transplantation. Dr. Hina was born in Karachi, Pakistan but grew up in the United States from the age of 11. Her family settled in Darien, Connecticut where she attended middle and high school. After high school, she attended a combined 8 year BS/MD program at Union College / Albany Medical College. She received a Masters in Health Management at Union College as well. After completing medical school, Dr. Hina started Internal Medicine residency at Albert Einstein College of Medicine/ Montefiore Medical Center. She stayed on to do a one year fellowship to specialize in liver disease. She then did a three year fellowship in Gastroenterology at Northwell. After completing her education and training, she started her current position as Gastroenterologist and Hepatologist at ProHealth in Lake Success, NY. She lives in Long Island with her husband and children. In her free time, she enjoy reading and traveling with her family.

A hepatologist and academic, a wife and a mother of two, Dr Ellie Barnes delighted researchers at the Wellcome Trust Centre for Human Genetics’ Women in Science talk, held on Wednesday the 20th of November 2013 Her presentation entitled: Women in Science – the joys and the juggles, highlighted the ups and downs of balancing a career in science, particularly as a working mother. Currently balancing three major projects: leading a £5 million collaboration called Stop HCV, trialling a powerful new Hepatitis C vaccine, and studying B cell immunology and IgG4 systemic disease, Ellie is now at a senior level in her career. But she admited that getting to this point hasn’t exactly been smooth sailing. Having her first child while studying her PhD and second just before receiving her intermediate MRC award, Ellie said it’s often hard to manage the demands of work and home, but being able to do what you love day in and day out makes it worth the struggle.
