Few people have had a closer look at the inner workings of our government than Andy Slavitt. In 2013, Slavitt oversaw the turnaround of healthcare.gov after its disastrous rollout. He then worked as Acting Administrator for the Centers for Medicare & Medicaid Services (CMS) under President Barack Obama, which meant he was overseeing the Children's Health Insurance Program and the Health Insurance Marketplace, as well. In January of 2021, he was brought into the Biden administration as a Senior Pandemic Advisor to the Covid-19 pandemic response team — a role he stepped down from in June of 2021. A graduate of the University of Pennsylvania and Harvard Business School, Slavitt is now the founder and Board Chair Emeritus of United States Care, a national nonprofit health advocacy organization, and the founding partner of Town Hall Ventures, a health care firm focused on underrepresented communities. Perhaps most notably, Slavitt was one of the central figures of the Affordable Care Act, also known as Obamacare, which will be 15 years old next year. I wanted to talk to Slavitt about that anniversary, the state of health care in the U.S., his experience during the pandemic, and whether we are prepared for the next one. Now that the decline sustained itself past the holiday and its aftermath, we can be confident that existing strains continue to be on their way down. The question now shifts to the new variants, and when, whether and to what extent they will cause trouble. See the variant section for further discussion of this. Once again, the pandemic is ‘resetting' in this fashion. Deaths Indiana didn't report deaths or cases last week and this week likely filled in the backlog, which is probably most of why the Midwest went up instead of down. Cases The south continues to decline, half of which is Mississippi and North Carolina not reporting cases. I chose not to adjust. Variants Paper says BA.2.75.2 ‘exhibits extensive escape from neutralizing antibodies,' which is not news. Which new variant will take over next? In the equilibrium model, any reproduction advantage is scary, since it will continue to compound until behaviors adjust or enough people get infected. What we have learned over the last few years is that there is a big reproduction advantage to being the new variant and starting from a small base. It takes a huge edge during growth to be the kind of game changer that Alpha, Delta and Omicron were. Here is a graph for America. It is clear that BA.5 has peaked and is in decline, with BQ.1.1 looking most likely to be next and looking like it has a higher reproductive advantage than Moritz is suggesting. That is enough relative growth to be somewhat worrying. It looks like more than the ‘10% advantage' I see quoted in places like this Andy Slavitt thread. If I was convinced the advantage was only ~10%, I would be at least as confident as the 80% Slavitt reports from scientists that the winter will be highly manageable. As an alternative perspective, looking at variant groups as levels rather than looking at individual variants. As we saw last week, this is very concentrated in Germany and Austria, and to a lesser extent France. Graphs like this are effectively somewhat misleading in isolation. He gives a variety of other charts, including many variants of the one above. The variations for other nations all look broadly similar. An underappreciated fact is that case counts effectively fully reset from scratch every time a new variant takes over. If Western Europe (or at least Germany) is experiencing a surge of Level 5 cases now, and the big winter wave is going to be Level 6, then the current case counts don't matter for the future local path of the pandemic. Senior officials in the U.S. government, including White House lawyer Dana Remus, deputy assistant to the president Rob Flaherty, and onetime White House senior COVID-19 adviser Andy Slavitt, have been in touch with one or more major social media companies to try to get the companies to tighten rules on allegedly false and misleading information on COVID-19, and take action against users who violate the rules, the documents show. In July 2021, for instance, after Biden said that Facebook was “killing people” by not combating misinformation effectively, an executive at Meta reached out to Surgeon General Vivek Murthy, a Biden appointee, to say that government and Meta teams met after the remarks “to better understand the scope of what the White House expects from us on misinformation going forward.” The same executive later wrote to Murthy saying, “I wanted to make sure you saw the steps we took just this past week to adjust policies on what we are removing with respect to misinformation, as well as steps taken to further address the ‘disinfo dozen,'” including removing pages linked to the group. The White House publicly pressured social media companies to take action against a group officials dubbed the “disinformation dozen,” which a nonprofit claimed were producing the bulk of “anti-vaccine misinformation” on the platforms. Also in July 2021, Murthy said Facebook had not done enough to combat misinformation. Rob Flaherty, director of digital strategy for the White House, told Slavitt and others in April 2021 that White House staff would be briefed by Twitter “on vaccine misinfo,” with the meeting including “ways the White House (and our COVID experts) can partner in product work,” according to one of the messages. Greg Abbott Begins Busing Illegal Immigrants to Chicago Texas Gov. Greg Abbott on Wednesday said that illegal aliens apprehended along the U.S.-Mexico border are now being bused to Chicago in addition to New York City and Washington. Abbott said his strategy to bus illegal immigrants from Texas to the Democrat-led sanctuary cities makes a strategic point about the impact of President Joe Biden’s “open border policies” on American communities. “President Biden’s inaction at our southern border continues putting the lives of Texans—and Americans—at risk and is overwhelming our communities,” the Republican governor said in a statement. “To continue providing much-needed relief to our small, overrun border towns, Chicago will join fellow sanctuary cities Washington, D.C. and New York City as an additional drop-off location.” Acknowledging Chicago’s sanctuary city policies, Abbott said Mayor Lori Lightfoot “loves to tout” that her city welcomes illegal immigrants as a matter of “responsibility.” “I look forward to seeing this responsibility in action as these migrants receive resources from a sanctuary city with the capacity to serve them,” Abbott said. Chicago Calls Busing Program ‘Racist’ The Chicago mayor’s office confirmed that around 60 illegal aliens arrived at Union Square from Texas on Wednesday night and were being provided with essential services while they “navigate the next steps of their journey.” “As a city we are doing everything we can to ensure these immigrants and their families receive shelter, food, and most importantly protection,” the spokesperson said in a statement. “This is not new; Chicago welcomes hundreds of migrants every year to our city and provides much-needed assistance.” The spokesperson also accused Abbott of having no “shame or humanity” and described the busing program as “racist practices.” The mayors of Washington and New York City, Muriel Bowser and Eric Adams, have also criticized the busing program, calling it a political stunt. But Abbott has said the illegal immigrants voluntarily chose to go to their cities where there is more capacity to serve them. The Texas governor first directed the Texas Division of Emergency Management in April to charter buses to transport illegal immigrants who claimed asylum when apprehended along the U.S.-Mexico border to Washington. New York City was added as a second drop-off location in August. Since then, more than 8,900 illegal aliens have been bused out of Texas to those cities, providing “much-needed relief to Texas’ overwhelmed border communities,” according to Abbott’s office. Overwhelmed with the influx of illegal aliens arriving from Texas, Bowser has twice asked the Biden administration to deploy National Guard personnel to help her city cope. Both requests were denied. Lightfoot signed Chicago’s “Welcoming City Ordinance” measure in February 2021 to strengthen the city’s sanctuary policies, which prevented local police from cooperating with federal immigration authorities. “Chicago has been a welcoming city and a city of immigrants since its very founding, and proudly so,” Lightfoot said at the time. Abbott’s office noted that under this policy Chicago doesn’t deny city services to individuals based on their immigration status. “With its ‘Welcoming City Ordinance’ making it a sanctuary city, Chicago will not deny city services to individuals based on their immigration status. The city also does not require local police to cooperate with federal immigration authorities,” Abbott’s office said. Abbott promised to keep busing illegal aliens to so-called sanctuary cities until the Biden administration “does its job” to secure the border. In May, the Oregon Department of Transportation committed to spending $100 million in federal and state funds over the next five years to expand Oregon’s EV charging infrastructure. The transition to ZEVs seems to overlook increasing concern over their environmental impact and national security implications. Making ZEV batteries requires the use of fossil fuels to process ore and extract minerals, including lithium, cobalt, nickel, copper, and graphite. They also require steel, aluminum, plastic, and other materials. The extraction and processing of these minerals are dominated by China and the Democratic Republic of Congo. According to Bloomberg, China controls 80 percent of the market for EV batteries. The push to convert to ZEVs will also create challenges to the electrical grid. In a 2018 analysis, energy economists at the University of Texas found that if California drivers were to go fully electric overnight, the state would need about 47 percent more electricity than it currently consumes. All the states that follow California’s emission standards will face similar challenges. But California’s electrical grid is already strained and Oregon’s faces uncertainty. With a heatwave looming, California’s grid operator issued an alert on Aug. 31, urging residents to avoid using large appliances and charging electric vehicles between 4 p.m. and 9 p.m. over the Labor Day weekend. “Lowering electricity use during that time will ease strain on the system, and prevent more drastic measures, including rotating power outages,” the alert reads. Senior officials in the U.S. government, including White House lawyer Dana Remus, deputy assistant to the president Rob Flaherty, and onetime White House senior COVID-19 adviser Andy Slavitt, have been in touch with one or more major social media companies to try to get the companies to tighten rules on allegedly false and misleading information on COVID-19, and take action against users who violate the rules, the documents show. In July 2021, for instance, after Biden said that Facebook was “killing people” by not combating misinformation effectively, an executive at Meta reached out to Surgeon General Vivek Murthy, a Biden appointee, to say that government and Meta teams met after the remarks “to better understand the scope of what the White House expects from us on misinformation going forward.” The same executive later wrote to Murthy saying, “I wanted to make sure you saw the steps we took just this past week to adjust policies on what we are removing with respect to misinformation, as well as steps taken to further address the ‘disinfo dozen,'” including removing pages linked to the group. The White House publicly pressured social media companies to take action against a group officials dubbed the “disinformation dozen,” which a nonprofit claimed were producing the bulk of “anti-vaccine misinformation” on the platforms. Also in July 2021, Murthy said Facebook had not done enough to combat misinformation. Rob Flaherty, director of digital strategy for the White House, told Slavitt and others in April 2021 that White House staff would be briefed by Twitter “on vaccine misinfo,” with the meeting including “ways the White House (and our COVID experts) can partner in product work,” according to one of the messages. In another exchange that year, a Department of Treasury official working on “mis, dis, and mal-information” told Meta workers that the deputy treasury secretary wanted to talk about “potential influence operations.” In a text in February 2021, meanwhile, U.S. Cybersecurity and Infrastructure Security Agency (CISA) Director Jen Easterly, wrote to another agency official that she was “trying to get us in a place where Fed can work with platforms to better understand the mis/dis trends so relevant agencies can try to prebunk/debunk as useful.” The documents were part of a preliminary production in a lawsuitlevied against the government by the attorneys general of Missouri and Louisiana, later joined by experts maligned by federal officials. “If there was ever any doubt the federal government was behind censorship of Americans who dared to dissent from official Covid messaging, that doubt has been erased,” Jenin Younes, a lawyer with the New Civil Liberties Alliance who is representing some of the plaintiffs in the case, said in a statement. “The shocking extent of the government’s involvement in silencing Americans, through coercing social-media companies, has now been revealed.” Club Membership Plug: Let’s stop and take a moment to talk about Fight Laugh Feast Club membership. Greg Abbott Begins Busing Illegal Immigrants to Chicago Texas Gov. Greg Abbott on Wednesday said that illegal aliens apprehended along the U.S.-Mexico border are now being bused to Chicago in addition to New York City and Washington. Abbott said his strategy to bus illegal immigrants from Texas to the Democrat-led sanctuary cities makes a strategic point about the impact of President Joe Biden’s “open border policies” on American communities. “President Biden’s inaction at our southern border continues putting the lives of Texans—and Americans—at risk and is overwhelming our communities,” the Republican governor said in a statement. “To continue providing much-needed relief to our small, overrun border towns, Chicago will join fellow sanctuary cities Washington, D.C. and New York City as an additional drop-off location.” Acknowledging Chicago’s sanctuary city policies, Abbott said Mayor Lori Lightfoot “loves to tout” that her city welcomes illegal immigrants as a matter of “responsibility.” “I look forward to seeing this responsibility in action as these migrants receive resources from a sanctuary city with the capacity to serve them,” Abbott said. Chicago Calls Busing Program ‘Racist’ The Chicago mayor’s office confirmed that around 60 illegal aliens arrived at Union Square from Texas on Wednesday night and were being provided with essential services while they “navigate the next steps of their journey.” “As a city we are doing everything we can to ensure these immigrants and their families receive shelter, food, and most importantly protection,” the spokesperson said in a statement. “This is not new; Chicago welcomes hundreds of migrants every year to our city and provides much-needed assistance.” The spokesperson also accused Abbott of having no “shame or humanity” and described the busing program as “racist practices.” The mayors of Washington and New York City, Muriel Bowser and Eric Adams, have also criticized the busing program, calling it a political stunt. But Abbott has said the illegal immigrants voluntarily chose to go to their cities where there is more capacity to serve them. The Texas governor first directed the Texas Division of Emergency Management in April to charter buses to transport illegal immigrants who claimed asylum when apprehended along the U.S.-Mexico border to Washington. New York City was added as a second drop-off location in August. Since then, more than 8,900 illegal aliens have been bused out of Texas to those cities, providing “much-needed relief to Texas’ overwhelmed border communities,” according to Abbott’s office. Overwhelmed with the influx of illegal aliens arriving from Texas, Bowser has twice asked the Biden administration to deploy National Guard personnel to help her city cope. Both requests were denied. Lightfoot signed Chicago’s “Welcoming City Ordinance” measure in February 2021 to strengthen the city’s sanctuary policies, which prevented local police from cooperating with federal immigration authorities. “Chicago has been a welcoming city and a city of immigrants since its very founding, and proudly so,” Lightfoot said at the time. Abbott’s office noted that under this policy Chicago doesn’t deny city services to individuals based on their immigration status. “With its ‘Welcoming City Ordinance’ making it a sanctuary city, Chicago will not deny city services to individuals based on their immigration status. The city also does not require local police to cooperate with federal immigration authorities,” Abbott’s office said. Abbott promised to keep busing illegal aliens to so-called sanctuary cities until the Biden administration “does its job” to secure the border. The move comes as California urges residents to not charge their electric vehicles during a heatwave to avoid straining the electrical grid. An Oregon Department of Environmental Quality advisory committee is now in the process of creating what it calls “Advanced Clean Cars II Rules,” which will guide the state toward that new goal. California announced on Aug. 24 that it would begin to phase out gas-powered vehicles beginning in 2026, when 35 percent of all passenger vehicles and light duty trucks sold in the state must be zero-emission vehicles (ZEVs). With an anticipated increase of nearly 8 percent annually, the state has vowed that 100 percent of all vehicles sold in California will be ZEVs by 2035. Oregon aims to follow suit. “The transportation sector accounts for more than 40 percent of Oregon’s greenhouse gas emissions,” the Oregon Department of Energy said in a statement. “Moving to cleaner, alternative fuel transportation options, like zero-emission vehicles, can reduce those harmful Green House Gases (GHGs) and help Oregon reach its climate goals.” Oregon had already been working toward a goal set by its legislature in 2019 to have 250,000 registered ZEVs in the state by 2025. As of April, there were approximately 50,000 ZEVs on Oregon roads, falling well short of the pace to meet that goal in the first three years. “We’re committed to getting more electric vehicles [EVs] on Oregon’s roads and 50,000 registered EVs is a major milestone,” said Amanda Pietz, administrator of the Oregon Department of Transportation’s Climate Office. “EVs make up about 8 percent of new cars, trucks, and SUVs sold in Oregon, and we’ll support that growing market through partnerships to build out EV charging stations along major roads and in Oregon’s communities.” The state will also “support” that market by banning gas-powered vehicles and using taxpayer-funded subsidies to prime the pump. Oregon offers as much as $7,500 in taxpayer-funded rebates on top of the federal tax credit of $7,500 toward the purchase of a ZEV. Utilities in the state are offering additional rebates of up to $1,500 for a vehicle purchase and up to $1,000 to install a residential home charger or workplace-commercial charging station. The Go Electric Oregon website encourages buyers to “stack” government incentives to obtain more than $16,500 to use for the purchase of a ZEV. According to Oregon Public Broadcasting, taxpayers have already subsidized the purchase of electric vehicles (at an average cost of $66,000) to the tune of $55 million. One 2018 estimate puts the average income of an electric vehicle driver at $150,000 compared to the U.S. average of $63,000. In May, the Oregon Department of Transportation committed to spending $100 million in federal and state funds over the next five years to expand Oregon’s EV charging infrastructure. The transition to ZEVs seems to overlook increasing concern over their environmental impact and national security implications. Making ZEV batteries requires the use of fossil fuels to process ore and extract minerals, including lithium, cobalt, nickel, copper, and graphite. They also require steel, aluminum, plastic, and other materials. The extraction and processing of these minerals are dominated by China and the Democratic Republic of Congo. According to Bloomberg, China controls 80 percent of the market for EV batteries. The push to convert to ZEVs will also create challenges to the electrical grid. In a 2018 analysis, energy economists at the University of Texas found that if California drivers were to go fully electric overnight, the state would need about 47 percent more electricity than it currently consumes. All the states that follow California’s emission standards will face similar challenges. But California’s electrical grid is already strained and Oregon’s faces uncertainty. With a heatwave looming, California’s grid operator issued an alert on Aug. 31, urging residents to avoid using large appliances and charging electric vehicles between 4 p.m. and 9 p.m. over the Labor Day weekend. “Lowering electricity use during that time will ease strain on the system, and prevent more drastic measures, including rotating power outages,” the alert reads. Oregon Gov. Kate Brown has supported removing or breaching the Lower Snake River Dams to protect fish populations. National Review found that almost every team in the league “promote or fund groups that encourage or provide sex-change procedures and gender-transition hormone treatment for minors as young as 12. Other organizations promote ‘social transitions’ — i.e., nonmedical changes in ‘gender expression,’ including the adoption of new names, pronouns, and clothing — for children as young as three.” Much of the support these teams are lending is organized under the rubric of the widespread LGBTQ “Pride Nights” that the league pushes. Along with the yearly events in the stadiums, the teams also donate money to a number of advocacy groups — usually local groups, but often national groups, as well. The Detroit Tigers, for instance, support the Ruth Ellis Center and Corktown Health in Detroit, the magazine noted. At the checkout page for tickets to the Pride Night game, the Tigers website offered ‘a chance to support a local Pride organization of your choice,’ with a drop-down menu that included LGBT-focused clinics such as the Ruth Ellis Center and Corktown Health, both of which perform medical gender transitions on minors. On top of masculinizing and feminizing drugs, at least one — the Ruth Ellis Center — included irreversible ‘gender affirming surgery’ in its suite of ‘transition care for transgender youth’ options. Another charity promoted by the Tigers, the Trans Sistas of Color Project, was actively funding groups such as ‘Trans Minors Rights,’ which ‘advocates for empowering transgender youth to make their own decisions regarding puberty blockers’ — i.e., ‘without requiring the consent of a parent.’ (The group’s tagline: ‘Unblock blockers.’) The magazine noted that the Tigers are only one example and that the MLB “Pride Resource Guide” promotes a long list of “social justice” groups that also push transgenderism on kids. The report found that five teams promoted groups that push dangerous surgeries on trans kids, including the Detroit Tigers, the Boston Red Sox, the Milwaukee Brewers, Tampa Bay Rays, and the Cleveland Guardians. Four teams partner with organizations that provide counseling and referrals to youths for such medical procedures. That list includes the L.A. Dodgers, Pittsburgh Pirates, San Diego Padres, and the Oakland As. And another five teams promote groups that offer guides and promotion of the transgender lifestyle, including the Cincinnati Reds, Houston Astros, Arizona Diamondbacks, Philadelphia Phillies, and the Colorado Rockies. Finally, six teams support groups that have lobbyists for the trans cause and otherwise advocate for the transgender lifestyle. Those teams include the Chicago Cubs and White Sox, the L.A. Angels, Minnesota Twins, St. Louis Cardinals, and the Washington Nationals. “It’s a classic case of institutional capture by the Left,” the magazine concluded. America's response to the COVID-19 pandemic was driven as much by politics as it was by science as policymakers tried to balance public health with economic health. Andy Slavitt, who headed up the Centers for Medicare and Medicaid Services during the Obama administration says 15 months on, many of the choices made by our leaders don't look so great in hindsight.“It was a huge public health challenge, and that meant tough decisions and trade-offs. But a lot of what happened came back to politics, which caused us to take a very different approach. And it was to our detriment,” Slavitt said. Slavitt is the author of the new book Preventable: The Inside Story of How Leadership Failures, Politics, and Selfishness Doomed the U.S. Coronavirus Response. He's also the host of the “In the Bubble” podcast which examines the response to the COVID-19 pandemic. Earlier this year, Utah lawmakers passed the so-called “pandemic endgame” bill, which ended Utah's mask mandate on April 10 of this year. They also moved to rein in the power of public health officials.Despite those maneuvers, the number of cases in Utah is rising again as the delta variant of the virus takes hold. “I wish nothing more than a virus would respond to legislation. That would be really nice. We can fool ourselves for some amount of time, but that's not real political leadership,” Slavitt said. Andy Slavitt was shocked when he joined the Biden administration. “I was under the impression coming in—as many, many Americans were—that there were big stockpiles of vaccines waiting to go out the door that were produced over the course of the last year. It's been much, much less the case than I think we'd been led to believe,” the White House senior adviser for COVID response tells Molly Jong-Fast and Jesse Cannon on the latest episode of The New Abnormal. It was one of a host of shortcomings the Biden team found, Slavitt says: “Not enough vaccines, not enough vaccinators, not enough places for people to get vaccines.”“So when we got here January 20th, one of the things we learned as an only 46% of the vaccines delivered to states had actually made its way into people's arms. Now you'd never expect that to be 100%. But 46%? It was low,” Slavitt adds.There’s now a plan in place to purchase as many as 600 million doses, hopefully enough for every American. States are now using 75% of their doses, a big step up from a few weeks ago. And there may be another vaccine around the corner, from Johnson and Johnson. “I would love to tell you, we weren't going to be starting with a huge stockpile of Johnson & Johnson vaccine. The truth is we're starting with a small number and we've got to build it as quickly as possible.”Meanwhile, the virus is evolving. More contagious, deadlier variants are spreading, fast. But even here, Slavitt sees some signs for hope. “All of the vaccines work very well against the English B117 variant. So that's good. The South African variant—and there's another one that looks like the South African, that's the Brazilian variant—that one it's interesting. There is a degradation of performance of the vaccines against the South African variant. However, that degradation is thankfully still above the scientific threshold for effectiveness,” Slavitt says. “It still generates antibodies. It doesn't generate as many. But it’s good.”The vaccines may even be a bit better than advertised, Slavitt says. “You know, we may have done a little bit of a disservice to ourselves when with the Moderna and Pfizer vaccines, we talked about the end point as including [serve] and mild [COVID-19 cases], because we really don't care as much about the mild. And so we get very fixated on the fact that they're 95% percent effective against both, right. And we probably should have stepped back and said, ‘you know what, let’s just measure it against more severe symptoms.’ In which case, the Johnson and Johnson does very close to comparable.” And all of the vaccines seem to stop severe cases—from all the variants. There are even some indications that “viral load”—the amount of virus a person carries—“is decreased for people who are vaccinated,” indicating “that these vaccines not only reduce disease and save lives, but these vaccines also will reduce the ability of people to affect one another, which if it holds up, will be terrific news.”But only if the vaccine gets into the arms of the people who need it the most, Slavitt cautions. “It's not just how many vaccines are being administered, but also how equitable equitably they're being distributed. Because look, we all know there are half the population or more would crawl over broken glass to get the vaccine, right. But the problem is they’re crawling over other people,” he adds. “You know, they're getting online and going into communities, maybe neighborhoods they've never visited or visited in a long time, but [where] we put vaccines that are really hard hit by the virus. People are coming in, refreshing their browser, and going in and getting these appointments. It's really important that we not just focus on how many people we vaccinated, but that we do it as equitably as possible. Sign up today!Faced with mounting criticism over Virginia’s rate of vaccination, Governor Ralph Northam took to the podium this afternoon to address what his administration plans to do to speed up delivery of doses. “First of all I want all of you to know that I understand your frustration,” Northam said. “I know you’re out of patience and I am as well.”The first doses of the Pfizer vaccine became available in the United States in mid-December, followed soon after by the Moderna vaccine. “We told you then that the first shots would go to the people who are most vulnerable,” Northam said. “Health care workers and people living in long-term care. We estimated that would be about 500,000 people. Today about a month later, more than 520,000 shots have been given in our Commonwealth.Northam said that means more people have been vaccinated than diagnosed with COVID in the past year. Today that number hit 488,553 according to data from the Virginia Department of Health. Source: Virginia Department of HealthThe federal government is the entity that contracted with pharmaceutical companies to manufacture the vaccines. Northam said the national government has been distributing doses to states. The presidential transition has been a factor leading to public confusion. “Fifteen days ago, the outgoing federal Secretary of Health told states to open up eligibility to everyone 65 and over,” Northam said. “[They said] ‘we’ll send more doses. If you don’t expand eligibility, we’ll reduce your supply.’ Then two days later, states learned that there were no more doses. That made a confusing situation even more confusing across the entire country.” Northam said he has been in touch with the Biden administration including a call yesterday with all of the nation’s governors. “I also heard something that I have not heard since the pandemic began and that is a commitment at the national level to work together, find solutions, and get this moving so we can all get back to our lives,” Northam said. Earlier in the day, the White House COVID Response team held a briefing on their efforts. Andy Slavitt is Acting Administrator of the Centers for Medicare and Medicaid Services. “We’ve been hitting our target of an average of 1 million vaccinations per day, necessary to meet the president’s early commitment to administer 100 million shots in 100 days,” Slavitt said. But back to Virginia. Watch the whole video on Virginia Public Media. Northam said all states will immediately receive a 16 percent increase in supply. He also reported on a briefing from General Gustave Perna, the person in charge of Operation Warp Speed. “We asked how can we be confident that we will receive these additional doses in the weeks ahead,” Northam said. “He told us that after more than a month of working, the drug manufacturers have hit a stable, regular sustainable cadence of production and that is good news.”The national goal is now to have vaccinations available to everyone who wants one by the end of the summer. Today, the VDH reported the seven-day average for vaccinations is 26,010. That’s halfway to the goal of 50,000 a day. Northam invited representatives of CVS and Walgreens, the two companies that are working with the federal government to vaccinate people who live and work in long term care facilities. James Satterfield is a regional healthcare director at Walgreens. “We’ve completed 92 skilled nursing facilities and are in the process of pulling forward and completing our assisted living facilities as well,” Satterfield said. “Currently we’ve given over 18,500 shots to the most vulnerable in those long term care facilities and look forward to completing those doses. Dick Dakessian is a vice president at CVS Health. “As far as where we’re at today for CVS, our first round of COVID vaccination doses, we’ve gone to 195 skilled nursing facilities that we were dedicated to vaccinating and have completed those,” Dakessian said. “Right now we’re about halfway through those as well and plan to complete those, the second doses, by February 8. Today we have had 43,000 doses administered within skilled nursing facilities.”Northam said he is ordering hospitals to not hold on to any additional vaccine and to get doses in arms as soon as possible. “We’re also working with hospitals and local health districts to make sure they are not holding on to too much supply of second doses, especially if they won’t need it for several weeks,” Northam said. “By shifting inventory around, we’re going to be able to increase the number of shots this week by about 20 percent. That’s about 40,000 more shots by this Sunday on top of the 175,000 that were already planned.”Dr. Mike McDermott is the CEO of Mary Washington Health Care in the Fredericksburg area, as well as the past president of the Virginia Hospital and Health Care Association. “Overall, Virginia hospitals have administered more than 318,000 vaccine doses since mid-December,” McDermott said. “To give you an example of how fast vaccination capacity is scaling within our hospitals, Virginia hospitals have increased vaccinations by 35 percent in the last week.”McDermott said Operation Warp Speed was able to get vaccines manufactured, but he said there was not enough logistical work to get what he said was the “last twelve inches” of getting shots in arms. He also said the previous federal administration was not transparent in their operations. Vaccine supplies are still expected to be limited for some time. Northam said the Virginia Department of Health would issue more guidance about who is eligible. “Here is how to allocate your supply,” Northam said. “Half of available doses should go for people age 65 and older,” Northam said. “The other half should be used for frontline essential workers and people who are at increased risk for severe illness as the CDC identifies.”Northam also unveiled an updated VDH dashboard for vaccines that has more details on where doses are going. This dashboard also has more demographic information to ensure that doses are being distributed equitably. Many initial shots were recorded without identifying the race of the recipient. “On Monday, Health Commissioner Dr. Norm Oliver wrote to every clinician in Virginian and told them to collect this information, it matters for transparency,” Northam said. He added he will endorse legislation in the General Assembly to require this information. Right now, signing up for vaccines is handled by individual health districts. Northam said that system has been fragmented, so he has directed the Virginia Department of Health to create a statewide system. “That is not ready today but I expect it to be ready soon,” Northam said. “I know this has been a source of great frustration for a lot of Virginians. I hear you and we’re getting this fixed.” Northam has also amended Executive Order 72 continuing COVID-19 restrictions until at least February 28. Before we move on to the rest of the show, let’s hear more from Andy Slavitt. “Now as you heard from the president this week, he is pushing us to view 1 million a day as the floor, not the ceiling,” Slavitt said. “To that end, we are releasing more supply, activating [the Federal Emergency Management Agency] and deploying many more personnel. In fact, just this week, FEMA announced they had obligated $1 billion to support state vaccination sites.” Slavett said there are two limiting factors to distributing hundreds of millions of vaccines. First is getting the supply, and the second is to distribute them quickly and efficiently. “We are taking action to increase supply and increase capacity but even so it will be months before everyone who wants a vaccine will be able to get one,” Slavett said. Liz Russell is a member of the committee. “The challenge and sort of task presented by the Board of Supervisors and administered under OEI under the Community Remembrance project is to have five markers installed and we’d like to have them by the end of this year, five markers to better reflect the stories of women and people of color in the county,” Russell said. Two markers are close to being installed. One is for the Albemarle Training School and one is for Virginia L. Murray, the first Black woman to be supervise education in the county. The Albemarle Training School was the precursor to Jackson P. Burley High School. The committee needs to select three more topics for markers. Later that evening, the Pantops Community Advisory Committee learned more about a proposal to build a four-story hotel next to the Rivanna Ridge Shopping Center. Andy Reitelbach is the lead reviewer for the 125-room Pantops Overlook Hotel, which requires an amendment to the existing zoning. “Sometimes the zoning that designates what can be developed by-right and the future land uses identified in the master plan do not always line up,” Reitelbach said. “That is way a rezoning may have to come in.” In this case, the zoning states this land should be for office use and that a tree conservation area should be on the property. The Pantops Master Plan calls for most of the land to be community mixed-use and a small portion part of it be parks and recreation. Access would be right-in, right-out onto U.S. 250. One CAC member asked what the business viability would be for another hotel. “In this immediate area we have going up very soon if not already finished and operating, a Holiday Inn Express, a Hilton Garden Inn, a Comfort Inn and on the plans a Hampton Inn,” said Sara Robinson. “What justification did the owners that we needed another hotel?”Doug Ellis is with WS4 LLC, the company that owns the property. “We see Albemarle County as a great market for a well-developed and designed hotel that meets a different need than those what you listed off but we’re very aware that they are in the general area as well and hope to develop a product that is very different from those,” Ellis said. The new hotel would be all suites and would offer extended stays. The same company owns an adjacent parcel under the company Happy Boxes Pantops LLC. Ellis said there are no plans to develop that land. There is no date yet for public hearings at the Planning Commission or the Board of Supervisors. This is a public episode. Andy Slavitt was brought in by the Obama administration to fix Healthcare.gov after a disastrous rollout and then asked to stay on to head Medicare and Medicaid. Since leaving government, he's been one of the fiercest critics of the current administration's efforts to dismantle the Affordable Care Act and combat COVID-19. Yet his approach to changing health care and fighting the pandemic has been deliberately bipartisan, and he informally advises leading Republicans as well as Democrats. Slavitt teamed with former FDA chief Scott Gottlieb to propose a $46.5 billion plan for COVID-19 contact tracing and isolation and was lead author of an open letter entitled “Stay Home, Save Lives” signed by 16 top Republican and Democratic figures. President Trump has so far resisted imposing national rules to fight the coronavirus pandemic, but health policy expert Andy Slavitt says it's past time for the federal government to act. Slavitt, who was acting chief of the Centers for Medicare and Medicaid Services under President Obama, says the Trump Administration should take bold steps, such as issue a national stay-at-home order, bar creditors from foreclosing on businesses and convert stadiums to intensive care units. As Andy Slavitt, MBA, acting administrator of CMS, comes to the end of his tenure, he spoke with Mandi Bishop, MA, CEO of Aloha Health, about the task of making health policy translatable and the legacy of payment reform he leaves behind. During his tenure, Slavitt, who has held the position since March 2015, oversaw the rollout of a number of new payment models to move healthcare to a value-based care system, including the implementation of the Medicare Access and CHIP Reauthorization Act. In addition, he has made a point to work on providing Americans with tools to help them find the best hospitals. In the interview with Bishop, he offered insights for the incoming administration in the continued move toward value-based care.
