Dr. Sant'Anna describes the importance of multidisciplinary training, technical details such as proper prong sizing, fixation, humidification, and suctioning, as well as the challenges of sustaining practice change over a decade.The results of this initiative are striking: lower rates of delivery room intubation, reduced severe BPD, and improvements in outcomes particularly for infants born before 28 weeks. Globalization is dying, maybe even dead. Borders are back, baby. That's the message in Jonn Elledge's sparkling Brief History of the World in 47 Borders. In this romp around world history , Elledge introduces us to 47 of the world's oddest borders including particularly weird ones in Detroit, Kaliningrad and Bolivia. So should be celebrating or mourning the rebirth of the border? Elledge is in mourning. A self-described progressive who grew up on Star Trek dreams of planetary unity, he sees nationalism's resurgence since 2016 as "quite a bad thing." He blames economic stagnation—when the pie stops growing, generous approaches to migration and distribution become much harder to sustain. I'm more sanguine. Whatever globalist bureaucrats at the UN or EU promised us, borders were never going away. As a species, we humans are agoraphobic. The Trekkies are wrong. The claustrophobia of the border is what gives us our sense of space. 1. Borders are having a political moment - The "liberal hegemony" that promised borderless globalization has been collapsing since 2016 (Brexit, Trump), making nationalism and territorial division the dominant political force again.2. Economic stagnation drives border obsession - When economies aren't growing and people aren't getting richer, generous policies on migration and wealth distribution become much harder to sustain politically.3. Maps shape leaders' minds - Trump's fixation on his Oval Office Ukraine map shows how visual representations of territory directly influence foreign policy decisions and geopolitical thinking.4. Most "historic" borders are recent inventions - What we assume are natural, ancient boundaries (like the Berlin Wall, Bangladesh, or even Germany's division) are often just decades old, showing how arbitrary our sense of "normal" geography really is.5. Dr. McEvoy shares findings from two randomized controlled trials that explored whether an additional two weeks of CPAP could promote lung growth and improve longer-term outcomes.We review the physiologic rationale behind extended CPAP, including the role of mechanical stretch in stimulating alveolar and vascular development. Dr. McEvoy explains the stability criteria used to determine eligibility for extended CPAP and how her team measured pulmonary function in neonates. Results from her studies showed significant improvements in lung volume, diffusion capacity, and expiratory flows, with early signals of reduced wheezing at one year of age.The conversation also touches on feeding tolerance, the practicalities of implementing extended CPAP in the NICU, and the need for larger multicenter studies to confirm these findings. Emily McKinney and her boyfriend, Joe Johnson, are accused of lying to Kentucky State Police troopers about a dog attacking Emily's 10-month-old child. Experienced Child Abuser: Why the System Freed Jake Haro Before Baby Emmanuel's Death The disappearance and presumed murder of 7-month-old Emmanuel Haro has shocked California—and the entire country. What began as a frantic report of a kidnapping in a Big 5 parking lot quickly unraveled into something much darker: a staged story, a dead infant, and two parents now sitting in jail charged with first-degree murder. On today's episode, we break down the chilling timeline of what happened to baby Emmanuel, why investigators never believed the parents' story, and how prosecutors say the infant suffered months of abuse before his death. But this case isn't just about two parents facing murder charges. It's about the system that allowed it to happen. Emmanuel's father, Jake Haro, wasn't a man with a clean slate. He was a convicted child abuser, already guilty of nearly killing another baby in 2018—a crime that left his daughter permanently disabled. Despite that, a judge let him walk with probation. No prison. No long-term oversight. And when Jake later violated probation with a firearm offense, the case still dragged on, leaving him free to live in a home with children. Officials are now openly calling this tragedy preventable. Prosecutors and sheriffs alike are criticizing the judge's decision, saying Emmanuel's death should never have been possible. District Attorney Mike Hestrin didn't mince words: “If that judge had done his job, Emmanuel would be alive today.” This is more than a true crime story. It's a devastating reminder of how broken our protective systems can be.
