About this episode: In 2003, George W. Bush's President's Emergency Plan for AIDS Relief (PEPFAR) was the largest commitment to a single disease in U.S. history. Renewed under every subsequent presidency since, PEPFAR has saved more than 25 million lives and prevented some 5 million perinatal infections globally. But now, policy changes that effectively end PEPFAR have the potential to completely reverse course and the world could see HIV/AIDS infections on par with a time even before effective antivirals existed. In this episode: Dr. Chris Beyrer talks about his recent Lancet essay, “On Going Backwards,” why any retreat now will make it harder to regain lost ground, and how this policy change has the potential to impact millions of lives. With the International AIDS Society's 25th global conference taking place next week in Munich, Mark Lagon, Chief Policy Officer at Friends of the Global Fight Against AIDS, Tuberculosis, and Malaria and Bennett Freeman, Associate Fellow with Chatham House, joined Katherine to discuss a new Friends report regarding the role of the private sector and civil society organizations in advocating for global health programs, including HIV services. Lagon and Freeman argue that in a period during which restrictions on civic space seem to be increasing in many countries around the world, there is a business case to be made for the private sector in defending civil society organizations' efforts to promote respect for human rights, monitor for equitable access to services, and encourage transparency and accountability within global health programs and beyond.

Simultaneously.  In 1989 Dr Lewin (as she then was), instead of seeking a medical appointment in some prestigious city hospital, chose to work in a small hospital in Kenya. It was here she became passionate about infectious diseases, specifically HIV. And it would be a path she continues to tread to this day. She's determined to find the cure.  In 2014, she was appointed Director of the Peter Doherty Institute for Infection and Immunity and her research focuses on why HIV persists, and on developing clinical trials aimed at finding that cure. She's delivered more than 100 talks internationally and has published 280-plus papers. Whilst doing all that her other role in this field is as the President of the International Aids Society. A huge organisation which ‘convenes, educates and advocates for a world in which HIV no longer presents a threat to public health and individual well-being.' I believe there are about 14,000 members of the IAS.Oh, she's also been a leader and spokesperson for the Covid response since about January 2020 when her team were the 1st in the world to isolate, grow and then share the virus with other nations. Her ongoing education of others in this area has been a much wanted voice of reason with a steadying hand on the tiller. It was a privilege having a conversation with someone so focussed on helping humanity. You're essentially giving the cells the things that they need to   Dr. John E. Lewis So, it's kind of like if you have a car, I use this very poor analogy, because I can't think of a better one. But if your car has water in the gas tank, and it's kind of sputtering along, it still runs, but it's sputtering and doesn't really drive that well. You get that water cleared out, and then you give it high test 93 grade gasoline. And now boom, suddenly, your car is running like its brand new again. That's exactly what giving these polysaccharides does to ourselves.   Dr. John E. Between 5 and 6 million Americans (approximately 40 million worldwide) suffer from Alzheimer's disease today, and the Alzheimer's Association predicts that number to more than double by 2030. It is the 6th leading cause of death of Americans and has no known conventional medical treatment, prevention, or cure. The history of the HIV response is woven into the International AIDS Conferences. Ground-breaking science and political activism at the conferences have uniquely shaped the trajectory of the response.  Conferences in Canada have played key roles. In Montreal in 1989, protestors stormed the stage, establishing activism in the HIV response. In Vancouver in 1996, delegates celebrated a treatment revolution that saw HIV become a manageable condition. Today, AIDS 2022 returns to Montreal and also takes place virtually. But other crises like monkeypox, surging COVID-19 cases and climate change, remind us why now more than ever, we need to re-engage and follow the science.  In this episode of HIV unmuted, the award-winning IAS podcast, we take a deep dive into the breaking science being released at AIDS 2022, address important access concerns at the International AIDS Conferences and understand how to translate cutting-edge science into meaningful action. Meet our guests:  Erika Castellanos is Interim Executive Director of Global Action for Trans Equality (GATE). A epidemia de HIV nos anos 1980 representa um marco para a Ciência. Os tratamentos eficazes que surgiram ao longo das últimas quatro décadas são fruto de uma cooperação científica inédita e do trabalho de pesquisadores que ainda hoje tentam entender o vírus e suas mutações. A estrutura utilizada nos 40 anos de estudos dedicados à descoberta de uma vacina contra o HIV, lembra Roger Tatoud, diretor-adjunto dos programas HIV na IAS (International Aids Society), que organiza a Conferência Mundial sobre a Aids na Suíça, sustentou as pesquisas e o sucesso que resultaram na descoberta dos imunizantes contra o SARS-Cov-2. “Muitos dos centros clínicos onde foram realizados testes para a Covid-19 eram utilizados para a pesquisa do HIV. Os cientistas envolvidos nas pesquisas com a Covid-19 são os mesmos que trabalhavam em estudos sobre o HIV”, explica Além do investimento massivo, houve também um engajamento coletivo do mundo científico no combate à Covid-19, com aumento da cooperação e compartilhamento de dados, o que permitiu avanços em tempo recorde. Os organismos que financiam as pesquisas sobre o HIV também são os mesmos que investem nos estudos sobre o SARS-Cov-2, lembra o representante da International Aids Society.   “Houve um enorme suporte, que deve ser reconhecido, de 40 anos de investimento no HIV, que permitiu uma resposta rápida à Covid-19." Na entrevista à RFI Brasil, Roger Tatoud disse esperar que essa mesma “energia”, seja utilizada, no futuro, na busca por uma vacina contra o HIV. Há projetos em andamento, dificultados pela complexidade do vírus, mas uma das esperanças é a utilização do RNA mensageiro, adotada com sucesso nas vacinas contra a Covid-19. Essa tecnologia poderia “driblar” a alta capacidade de mutação do HIV, fazendo com que o organismo produza anticorpos independentemente da cepa. O pesquisador explica que o HIV é um vírus muito mais sofisticado do que o SARS-Cov-2, com cepas de diferentes famílias, o que dificulta o desenvolvimento de uma vacina. “Como vírus da Aids varia muito, bem mais do que o vírus da Covid-19, e tem muito mais variantes, os anticorpos, isolados, não reconhecem todas as cepas”, explica Tatoud sobre a dificuldade de criar uma vacina utilizando métodos clássicos, onde vírus inativados ou enfraquecidos provocam uma reação do sistema imunológico que protegem o corpo. Os cientistas detectaram até agora cinco novas variantes para o SARS-Cov-2: a britânica, a sul-africana, a brasileira, a nigeriana e a originária de Nova York. Essas mutações são esperadas e normais, explica o pesquisador. “A natureza é assim. Os vírus mudam, alguns mais do que outros”, diz. É o caso do HIV, mas também do vírus da gripe, por exemplo. “Por isso é necessário criar uma vacina diferente todos os anos, porque a cepa muda de um ano para outro. O HIV é assim, muda muito, dependendo da população e da área geográfica.” A triterapia, ou o famoso coquetel anti-HIV, ainda é tratamento mais eficaz no combate às cepas existentes, segundo ele. Mas há casos de resistência, que obrigam os cientistas a adaptar as moléculas para tratar os pacientes. Novas pistas de tratamento A IAS apresentou em janeiro resultados preliminares de um estudo que mostra que injeções regulares de um tipo específico de anticorpos monoclonais, chamado de VRC01, podem prevenir a contaminação. A pesquisa incluiu 4.600 homens e transgêneros no Brasil, Peru e Estados Unidos e mulheres na África subsaariana. Mas, mesmo tendo um amplo espectro, os anticorpos não reconhecem todos os vírus existentes. “Talvez seja necessário fazer mais estudos ou combinar diferentes anticorpos, para ver se funciona melhor”, explica o pesquisador. Roger Tatoud acredita também que a mudança da percepção coletiva em relação à gravidade do HIV influenciou os investimentos e as pesquisas. A imagem dos doentes em estado terminal nos hospitais, que aterrorizou o mundo nos anos 1980, ficou para trás. O coquetel faz com que os pacientes possam levar uma vida praticamente normal e novos medicamentos evitam a contaminação em caso de exposição. O HIV passou a ser visto como uma doença crônica. Além disso, a transmissão hoje pode ser totalmente evitada, diferentemente da Covid-19. “Todo mundo pode ser atingido, todo mundo pode ser contaminado e todo mundo tem medo. Já em relação ao HIV, existe um certo distanciamento”, ressalta. Roger Tatoud lembra que o vírus que gerou uma das piores epidemias do século 20 continua sendo uma doença que mata 800.000 pessoas por ano.

How have societies responded to pandemics, throughout the world, and throughout time? What are the new narratives, meanings and cultures that emerge and shape emerging realities? As this conversation will remind us, there is no simple answer to the problem of disease – but disease is also far more than a medical or scientific problem. Robin Gorna will draw on her experiences with social movements and cultural responses to AIDS since the 1980s, which brought hope and massive social change in the midst of rage and death. She will discuss the many connections between the two pandemics - of cultural change, politics and people and emerging narratives, with reflections on her current experience of living with Covid-19 in her own body. Erica Charters will discuss a just-published special issue of Centaurus on ‘The history of epidemics in the time of COVID-19’, reflecting on how the discipline of the history of science and medicine has responded to the current pandemic. Sharing historical approaches to understanding disease, she will explore how historians have framed pandemics and what a long-term context might offer for our understanding of COVID-19. Biographies: Dr Erica Charters (History Faculty and Wolfson College) examines the history of war, disease, and bodies, particularly in the British and French empires. Her current research focuses on manpower during the eighteenth century, examining the history of bodies as well as the history of methods used to measure and enhance bodies, labour, and population as a whole, including the history of statistics. Since disease was the biggest threat to manpower in the early modern world, Erica looks at how disease environments – throughout the world – shaped military, commercial, and agricultural power, as well as how overseas experiences shaped European theories of medicine, biology, and race alongside political methodologies such as statistics and censuses. Erica's monograph Disease, War, and the Imperial State: The Welfare of British Armed Forces during the Seven Years War (Chicago, 2014) traces how responses to disease shaped military strategy, medical theory, and the nature of British imperial authority (awarded the AAHM 2016 George Rosen Prize and the SAHR 2014 Best First Book). She set up the global AIDS Team for DFID (Department for International Development) in 2003, and then moved to South Africa to lead the UK’s regional and national HIV and health programmes. She co-founded, and now chairs, the St John’s College Women’s Network. She studied Theology but spent far too much time involved in student drama until the end of her 2nd year when she saw an early performance of The Normal Heart (by Larry Kramer) and signed up as a volunteer with the UK’s new AIDS Charity, the Terrence Higgins Trust. She remains fascinated by the ways in which culture and the arts inspire social movements, including the global AIDS response. Two things need to happen to achieve greater success in the global fight against HIV: dramatically reduce new infections and achieve viral suppression in those already living with the virus. In order to meet these goals, new infections must come down from an annual rate of 1.7-2 million that has been stagnant for a decade, viral suppression must be sustained and ensured for the more than 23 million people living with HIV (PLHIV) on anti-retroviral treatment (ART), and treatment must be provided to the more than 14 million PLHIV not on ART.     New technology offers the potential to facilitate these efforts if they can be utilized effectively.  The July 2019 International AIDS Society science conference in Mexico City highlighted several important technological innovations either already on the market or that will be available in the coming years including long-acting injectable treatment, pre-exposure prophylaxis (PrEP), implants, and broadly neutralizing antibodies as a vaccine.  However, the ongoing experience of starting oral PrEP programs has demonstrated that taking new products to scale for impact is complex and that there are significant obstacles to ensuring access to those in need.   On Tuesday, October 22, 2:30-5:00 pm, the CSIS Global Health Policy Center will host a public event to explore recent developments in HIV prevention and treatment technology, examine successes and challenges in integrating those products into existing service delivery, and discuss how global programs, such as PEPFAR and the Global Fund, need to adapt to ensure access and uptake.  Fireside ChatDeborah Waterhouse CEO ViiV Healthcare  Moderated byJ. HIV, democracy and adolescents in South Africa: An introduction to the Mzantsi Wakho study, by Rebecca Hodes. In this first episode we speak to Rebecca Hodes, who is the principal investigator of Mzantsi Wakho’s qualitative research. She is the Director of the AIDS and Society Research Unit, University of Cape Town, and an Honourary Affiliate of the Department of Social Policy and Investigation, Oxford University. Hodes studied history at Rhodes University, and received an M.Sc in the History of Medicine, Science and Technology and a D.Phil from Oxford University. She has published a monograph, Broadcasting the Pandemic: A History of HIV on South African Television (HSRC: Cape Town, 2014). Her academic work has been published widely in peer-reviewed journals, including African Affairs, the South African Medical Journal, the Journal of the International AIDS Society, and the Journal of Southern African Studies, as well as numerous edited volumes and collected editions.

An international AIDS conference has been held in Australia with tribute to several delegates who were killed en route to the gathering when their plane was shot down over Ukraine. Officials at the opening ceremony for the 20th International AIDS Conference in Melbourne held a moment of silence for the six AIDS researchers and activists killed aboard Malaysia Airlines Flight 17. All 298 people on board the plane died when it was shot down by a surface-to-air missile on July 17th as it flew over rebel-held eastern Ukraine. Among the passengers was prominent Dutch researcher Joep Lange, former president of the International AIDS Society, and spokesman Glenn Thomas of the World Health Organization based in Geneva. Around 12,000 scientists and activists from 200 countries attended the conference to discuss the latest developments in HIV and AIDS research. This is NEWS Plus Special English. A new government study shows the rate of HIV infections diagnosed in the United States each year fell by one-third over the past decade. Experts hailed it as hopeful news that the AIDS epidemic may be slowing in the U.S. Researchers say that the reasons for the drop are not clear. It might mean fewer new infections are occurring, or that most infected people already have been diagnosed so more testing won't necessarily find many more cases. The study result was released ahead of the International AIDS Conference which was held between July 20th and 25th in Melbourne, Australia. HIV is the virus that causes AIDS, which destroys the immune system. The World Health Organization estimates that 35 million people globally have the virus. In the United States, 1 million people are thought to be infected, though many don't know it. You are listening to NEWS Plus Special English. I'm Mark Griffiths in Beijing. Now is the time to get moonstruck. Forty-five years ago on Sunday, Apollo 11's Neil Armstrong and Buzz Aldrin became the first humans to set foot on the moon. Armstrong's "one small step for man and one giant leap for mankind" on the dusty lunar surface on July 20th, 1969, still stirs hearts. At the launch site at Florida's Kennedy Space Center, NASA honored Armstrong on Monday with a renaming ceremony of the historic Operations and Checkout Building. Both Aldrin and Michael Collins, the Apollo 11 command module pilot who orbited the moon, were there. People can join the celebration, at the very least, by walking out and looking at the moon over the weekend. The Armstrong family had said following his death in 2012 that those who wish to honor Neil can simply honor his example of service, accomplishment and modesty; and the next time we walk outside on a clear night and see the moon smiling down at us, we can think of Neil Armstrong and give him a wink. This is NEWS Plus Special English. China's first lunar rover, Yutu, or Jade Rabbit, is still functioning despite the malfunction of some of its equipment. According to Wu Wei-ren, chief designer of China's lunar probe project, Yutu has been woken up after the past dormant period, but the problems still exist. The rover will embrace another "working day", which is about 14 Earth days, in an abnormal state caused by mechanical failures. During the lunar night, there is no sunlight to provide power to Yutu's solar panel, and the extremely low temperatures can damage its delicate electronics, so the rover must "hibernate". The 140-kilogram, six-wheeled Yutu, is part of the Chang'e-3 lunar probe. It has outlived its designed lifespan of three months since it reached the moon in mid-December last year. The rover has completed its designated scientific and engineering tasks, analyzed major elements on the lunar surface and studied mineral resources. However, after almost six weeks of operation, the moon buggy reported a mechanical control problem in January, before entering its second period of dormancy. Since then, it has been unable to move any farther, remaining around 20 meters southwest of where it landed. Its antenna and solar panels cannot be folded, either. Chinese engineers blamed the problem on the "complicated lunar surface environment". You are listening to NEWS Plus Special English. I'm Mark Griffiths in Beijing. A 38-year-old man has died of plague in northwest China's Gansu Province. The case was reported in Jiuquan City, and the man died on July 16th. He had been in contact with a dead marmot, which is a member of the squirrel family. A total of 150 people who had close contact with him, have been put in quarantine and are under medical observation. None of them has reported any symptoms of the disease so far. China's National Health and Family Planning Commission has sent disease prevention and control specialists to the city to prevent the plague from spreading. Plague is categorized as a Class A infectious disease, the most serious category under China's Law on the Prevention and Treatment of Infectious Diseases.
