This is the story of how I knew I needed to start an anti-anxiety and anti-depressant medication. I was recently diagnosed with ADHD, Anxiety and Depression. I also had to switch Primary Care Physicians because my old PCP was not listening to me and seemed very resistant to prescribing any medication for mental health. Greif, postpartum and the chaos of raising two toddlers sent me spiraling with both anxiety and depression. I got to a point where managing my mental health felt like a full time job, I did not have the time or the energy to manage my depression. My anxiety was making my ADHD worse, I couldn't get anything done and I knew in my gut that it was time to talk with my doctor about getting on something for my depression. I was breastfeeding at the time, and I had read that anti-anxiety medication can help with the symptoms of ADHD. I also knew that often-times one medication can reduce symptoms of anxiety and depression. After talking with my doctor, she recommended Zoloft or Sertraline. I had very few side effects, and after about a week or 10 days, I noticed a difference. After two or three weeks I felt like my old self again. I was laughing and could enjoy my children and my life as it was, instead of drowning everything in toxic positivity and hoping for the best. This week on the Codcast, John McDonough of the Harvard T.H. Chan School of Public Health and Paul Hattis of the Lown Institute talk to Michael Barnett, who is both a primary care physician at Brigham and Women's Hospital and a professor of health policy at the T.H. Chan School, about the ongoing effort to unionize PCPs across the Mass General Brigham system.
“Primary care physicians are your first line of defense against cancer,” said Matthew Farrell, M.D., an Ohio State Wexner Medical Center primary care physician and a clinical associate professor of family and community medicine. Prevention, screenings and early detection are crucial and begins with a patient's family history with cancer and other medical issues. “We are thorough and ask about their first-degree relatives, their parents, siblings and children, and their secondary relatives, such as grandparents and aunts and uncles,” Farrell said. “If I see a pattern, a certain number of relatives with the same cancer, especially at an early age, there could be a genetic factor.” A patient with an inherited genetic mutation will then be screened at an earlier age and more often. Dr. Farrell also talked about the importance of children receiving the Human Papillomavirus (HPV) vaccine that prevents certain types of cancer. He examines patients to detect skin cancer and melanoma in the early stages. “Skin cancer is the most common form of cancer and it's personal for me; my mother was diagnosed with melanoma three times … and she's doing fine.” Breast cancer and prostate cancer are two of the most common forms of cancer. Dr. Farrell discussed when women should begin getting yearly mammograms, and why and when men should begin to undergo regular Prostate-Specific Antigen (PSA) tests. Men and women without a family history of colorectal cancer should get their first colonoscopy at 45. “My brother-in-law was 52 when he had his first colonoscopy,” Dr. Farrell said. “They found a large tumor in his colon; it was removed, and he's been cancer free for 20 years now.” Lung cancer screenings are relatively new and recommended for long-time smokers. They are vital because “if you wait until there are symptoms it's very hard to treat and cure,” Dr. Farrell said, adding “I'm fortunate to be part of the Ohio State system and the James where we have experts and specialists for every type of cancer and medical issue my patients have.”

Anytime I have the opportunity to learn from a biological dentist, I'm all in. The healthcare ecosystem in Minnesota is a dynamic and complex network that influences the well-being of its residents and impacts the state's economic landscape. As one of the most health-conscious states in the nation, Minnesota has long been a leader in healthcare innovation, access, and quality. On October 9, 2024, the Economic Club of Minnesota hosted a discussion centered on the Economic Ecosystem of Minnesota Healthcare. The discussion featured Andrea Walsh, President and CEO of HealthPartners and Dr. Rahul Koranne, President and CEO of Minnesota Hospital Association and was moderated by retired EVP UnitedHealth Group; Director & Secretary, Economic Club of Minnesota, Jeannine Rivet. The conversation was informative and robust. Andrea Walsh emphasized the current state of healthcare versus actual wellness. Dr. Koranne talked about how community infrastructure is critical to being able to release patients from hospitals to either home or a care facility. Boarding patients in hospitals is not a sustainable model. Another discussion point was staffing. The nursing shortage that was exacerbated during and following the COVID pandemic has been eased slightly. Dr. Koranne explained that the shortage of Primary Care Physicians and Pediatricians is a challenge facing the state of Minnesota.

Madison Cotton went to her Primary Care Physician because of troubling cysts in her breast in 2017 and the fact that her mother had already been through breast cancer. Madison was basically dismissed by her doctor and eventually in 2022 was diagnosed with Breast cancer. More recently, Vivek also led the development of Med-PaLM M, the first demonstration of a generalist biomedical AI system and AMIE, a research AI system, which surpassed Primary Care Physicians on multiple axes pertaining to diagnostic dialogue in an randomized study conducted in the style of a virtual Objective Structured Clinical Examination (OSCE). Over the years, Vivek's research has been published in well-regarded journals and conferences like Nature, Nature Medicine, Nature Biomedical Engineering, JMLR, CVPR, and NeurIPS. It also forms the basis for several regulated medical device products under clinical trials at Google, including the NHS AI award winning breast cancer detection system Mammo Reader and the skin condition classification system DermAssist.

Dr. Ian Lake is a Primary Care Physician in the UK. After 20 years of living with Type 1 diabetes, he accidentally discovered the ketogenic lifestyle which had a profound transformation in his diabetes control from the first day; he halved his insulin doses, reduced his hypos, and had much-improved blood glucose. Previously, he was in the same situation as two-thirds of people with Type 1 diabetes who are losing 100 days of life every year because of their HbA1c being above 7.5%. (EASD 2020) He feels so healthy and alert that he has had the confidence to resume sports in the great outdoors solo, which he had to stop because of safety concerns around diabetes. In 2020, he organised the Zerofive100 project, a 100-mile run over five days with zero calories, water only. One other person with Type 1 was in the team of eight. It was designed to address clinical concerns about the keto diet in type 1 diabetes and was a success. There were no issues at all. He has written professional courses on keto diets, which are intended for clinical teams and will also appeal to people living with Type 1. He also runs residential courses on insulin optimisation for people living with type 1 diabetes, and has a website dedicated to providing information for fellow T1s. ‘There is a lot of misinformation out there and people are getting confused.
