Dr. Atul Gawande is the Assistant Administrator for Global Health at the U.S. Agency for International Development, where he oversees a bureau that manages more than $4 billion with a footprint of more than 900 staff committed to advancing equitable delivery of public health approaches around the world. The Bureau for Global Health focuses on work that improves lives everywhere--from preventing child and maternal deaths to controlling the HIV/AIDS epidemic, combating infectious diseases, and preparing for future outbreaks. Prior to joining the Biden-Harris Administration, he was a practicing surgeon at Brigham and Women's Hospital in Boston and a professor at the Harvard Medical School and the Harvard T.H. We are strange creatures. It is hard for us to speak about, or let in, the reality of frailty and death — the elemental fact of mortality itself. In this century, western medicine has gradually moved away from its understanding of death as a failure — where care stops with a terminal diagnosis. Hospice has moved, from something rare to something expected. And yet advances in technology have made it ever harder for physicians and patients to make a call to stop fighting death — often at the expense of the quality of this last time of life. Meanwhile, there is a new longevity industry which resists the very notion of decline, much less finitude. Fascinatingly, the simple question which transformed the surgeon Atul Gawande's life and practice of medicine is this: What does a good day look like? As he has come to see, standing reverently before our mortality is an exercise in more intricately inhabiting why we want to be alive. This conversation evokes both grief and hope, sadness at so many deaths — including our species-level losses to Covid — that have not allowed for this measure of care. Twila Reynolds and I talk about the inevitable moment coming to all of us: the day of our death.   Twila Reynold is retired.  She spent most of her professional years in the commercial insurance industry.  But, later, she changed fields and worked in the administrative medical field, which has always interested her. When she began working at the VA Medical Center, she started as an Invasive Procedure Coordinator in the CT area. She later moved to Human  Resources, where she handled all the worker compensation claims for that facility.   Twila directed me to the book Being Mortal by Atul Gawande. A PBS documentary by the same name is available on YouTube at  https://www.youtube.com/watch?v=lQhI3Jb7vMg Atul Gawande is a surgeon who explores the inevitable process of aging and ultimately death, and takes a critical look at how the medical profession does not yet have a firm grip on how to handle these processes well. He explores the birth of nursing homes, independent and assisted living facilities, and the various roles modern hospice programs can play to support those nearing the end of their lives. Gawande shares several stories of those who were adeptly aided through the process of growing frail as well as those were not so lucky. Finally, Gawande shares what he has learned about how each of us should approach our mortality as well as that of those we love.

About ten years ago, Dr. Atul Gawande published Being Mortal and it became a New York Times bestseller. A trained surgeon, Dr. Gawande was trained to target a specific problem and fix it. In this book, however, he uses a mix of facts and stories to reveal a truth we must all accept. The body and the mind break down. Gawande skillfully captures the way people age and the ways we have supported older adults over time. More important, he issues a challenge. Over time, medicine, safety, and health standards have been given a supreme role in the care and support of older adults. Being Mortal warns against the over-medicalization of care for older people at the expense of finding what truly makes each adult's life worth living. Being Mortal is not just a reflection on the ideal form of caring for older adults. It's a call to accept that we will all become frail, to plan for that inevitable stage of life, and to consider what a good life will look like for us at that time.

On Episode 10 of Season 3 , I spoke with my grandmother, Sydell Aaron. Ultimately, we all become consumers of Healthcare, like it or not. On Healthcare Reimagined, I typically showcase healthcare innovation - the truth is that innovations are only interesting in so far as they are making life  better for patients. Last week I spoke with my grandmother about her experience as a consumer of U.S. healthcare over the past 9 decades. Sydell, or Meema as I call her, was born in 1932. In 1929, 3 years before she was born, the first polio patient was saved.  In the 1940's when Meema was a teenager, scientists succeeded in isolating penicillin and antibiotics became widely available for the first time. Before that, you could die from a simple infection. The first kidney transplant was done in 1952, when Meema was 20.  In 1964, for the first time human blood was successfully stored. Meema was 32 years old, with 3 children.Meema has already lived 50% longer than the average life expectancy for a woman the year she was born (it was 62 back then). We spoke about her family doctor making house calls, the awe and wonder of medicine before technology that made medical information available to all, and about the trade off between safety and independence as one gets older.We discussed a few quotes from Atul Gawande's book Being Mortal, and the loss of independence as one ages.  One of the quotes from Gawande's book really captured the essence of the challenge Meema faces in her interactions with her adult children. They want the best for her, as she knows, but at times, they infringe upon her freedom in an effort to protect her: "We want autonomy for ourselves and safety for those we love.” We went on to discuss the framework in which death is addressed in U.S. Healthcare, and a system that selects for those who can and want to fix things (Doctors), when sometimes the best option is not to fix but to provide comfort in one's final days. We closed with a discussion about Meema's own hopes, desires, and observations after over 9 decades on this pale blue dot we all call home. Eleanor Rutter is Assistant Director of Public Health at Sheffield City Council, and Leader of Sheffield's Compassionate Sheffield programme.A talented mathematician as a child, Eleanor went to medical school out of a need to seek the approval of other people. Following a complicated pregnancy, she was away from work as a hospital doctor for 18 months, after which she went into public health. She had a further two children and time off through mental ill-health, and the training programme, nominally five years, took her 12 years to complete.She had a false start in an authority with what she feels was an ‘over-medicalised' model of public health, but has now found her feet in what she describes as her “dream job.”In her current role, Eleanor leads the Compassionate Sheffield programme. It is in fulfilment of the city's 2018 public health objective to ensure that everyone has a dignified death in a place of their choice. She soon found out that there were a lot of compassionate communities doing good work in this area.Eleanor's approach is informed by the academic work of Professor Allan Kellehear at the University of Bradford. It recognises dying as a social and spiritual process first and foremost, rather than a medical one. She says that communities and neighbourhoods are best placed to allow people to live the complete lives they choose to value.Eleanor's team comprises two community development workers, one of which is an end-of-life doula, a communications officer, a clinical lead, and a programme manager. They are funded by Public Health Sheffield City Council, the ICB (Integrated Care Board), and St Luke's Hospice. She says the team is an enabler, building capacity, confidence and connections within and between communities. The main strands of the team's work to date have been advance care planning, developing training to help people navigate the end of life, building ‘death literacy' through death cafes, and leading Sheffield's covid memorial project.Atul Gawande's book ‘Being Mortal' has also had a strong influence on Eleanor's thinking. She says that by not listening to people and over-medicalising their problems we are at risk of stripping away their humanity.The next stage for Compassionate Sheffield is to build on the work that people did in the pandemic as compassionate neighbours. In the longer term, Eleanor feels that compassion runs through everything we do and its potential is far greater than transforming the end of life. For example, in Sheffield's economic anchor organisations many people are in a conversation with Michael West concerning compassionate leadership. She says “I don't think it's just a silly pipe dream, this idea of Sheffield becoming a compassionate city in its entirety.”Sheffield has not intentionally diverged from the Frome Model, which is the basis of Compassionate Communities UK. Rather, Sheffield's Health and Wellbeing Board, aware of the compassion that was already manifest in Sheffield's communities, wanted to grow Compassionate Sheffield using an asset-based approach.As white and middle class, Eleanor is very conscious of her privilege. Therefore, she has a problem with the term ‘achievements' and feels that often she has just needed to “scoop up the opportunities that were given to me.” Only two or three times in her career has she been faced with making a genuinely tough choice, which on one occasion involved insisting on doing the right thing even though her position was unpopular with some very senior colleagues.Through therapy Eleanor has learnt to see life as a learning process. One of the things she has learnt is the power of saying sorry and actually meaning it. Eleanor credits therapy as being the experience that has changed her the most. She put herself “heart and soul” into it. It was gruelling, but she is “massively transformed” and no longer driven by self-loathing.Otherwise,...
