In this episode, Drs. Patrick Georgoff, Teddy Puzio, and Jason Brill are joined by special guest Dr. Pat Murphy, who helps us delve into the evolving field of acute care surgery (ACS), exploring its history, challenges, and the nuances of defining full-time employment in this demanding specialty. The discussion highlights the origins of ACS as a response to unmet emergency surgical needs and its three foundational pillars: trauma surgery, emergency general surgery, and surgical critical care, with additional roles like surgical rescue evolving over time. Dr. Murphy share insights into the workload, including night shifts, call schedules, and the toll on surgeons' health, emphasizing the importance of fair compensation, equitable shift distribution, and transparency in job expectations. The episode underscores the value ACS surgeons bring to hospitals, likening them to essential infrastructure like firefighters, with their impact often unrecognized in traditional productivity metrics like RVUs.    Dr. Murphy would like to thank the many collaborators who made this volume of work possible including the many acute care surgeons who have taken the time to participate in the research and their dedication to patient care and surgeon wellbeing Learning Objectives: 1)        Define and understand the evolution of acute care surgery as a surgical subspecialty, including its historical development, key components (trauma, surgical critical care, emergency general surgery, surgical rescue), and its unique role within the surgical landscape. 2)        Analyze the concept of "full-time equivalent" (FTE) for acute care surgeons, considering factors such as call schedules, shift length, service demands, and the impact of varying case volumes and intensities on workload. 3)        Discuss the challenges of defining and measuring the value of acute care surgeons, considering factors beyond traditional productivity metrics (e.g., RVUs) such as the impact of surgical rescue, patient safety, and the value of 24/7 availability in preventing adverse outcomes. 4)        Explore the importance of recognizing the unique demands and contributions of acute care surgeons, including the impact of high-stress environments, irregular schedules, and the importance of work-life balance and clinician well-being on long-term sustainability within the specialty. In Part 2 of this podcast Hugh Montgomery, Liz Crowe, and Shelly Dev along with Peter Brindley continue their discussion on wellness, resilience, burn out and being a healthcare worker in the world now. IN THIS EPISODE THE TEAM DISCUSSES THE BROADER ORGANISATION STRUCTURE AND HOW THIS CONTRIBUTES TO (OR DETRACTS FROM) TEAMWORK AND HEALTHCARE WORKER WELLNESS AND SATISFACTION. Shelly delves into the topic of the organisations and whether they are supporting the clinicians on the ground in the best possible way. Senior leadership, in her opinion, has done a major disservice to healthcare workers in their support and leadership roles. The support needed on the ground transcends yoga classes and healthy cooking recipes. Organisational support needs to acknowledge the needs and desires of healthcare staff, namely, to deliver excellent care and have good days at work in the context of a healthy life. Liz suggests that although the organisational leadership is important for the overall wellbeing of the workforce, they are one aspect of a broader picture. She believes that leaders should be mentored in leadership. Teaching people basic communication and feedback skills would make a huge difference. Similarly, fostering a culture of togetherness and unity amongst separate entities of a larger organisation would lead to greater worker satisfaction and lead to better outcomes for patients. HUGH RAISES THE POINT OF CLINICAL OUTCOMES BEING INFLUENCED BY ENGAGEMENT OF HEALTHCARE PROVIDERS BY SENIOR MANAGEMENT. Management teams engaging with clinical staff seem to increase the patient care being delivered. Hugh provides his thoughts as to why this may be the case. Without senior management support, clinicians are increasingly overworked in a system that is constantly pushing back. This can, and does, lead to staff finally breaking and resigning on the spot. The core the issue of healthcare worker burnout and dissatisfaction is simple Shelly states. In her view, everyone in healthcare at their core are good and decent people. They desire support and structures that allow them to enact this value in their everyday work. Although the solutions to the broad range of problems facing health systems across the globe are not as straight forward, remembering this fact is a good starting point. From here, the team provide some of their insights into the way forward.     Tune in to this authentic perspective on healthcare worker wellbeing with Peter Brindley, Hugh Montgomery, Liz Crowe & Shelly Dev. This episode discusses the effect of the pandemic on healthcare professionals at an individual level, and how this has broad reaching ramifications at a team and industry level across different country contexts.   IN THIS PODCAST PETER BRINDLEY IS JOINED BY HUGH MONTGOMERY, LIZ CROWE, AND SHELLY DEV TO DISCUSS WELLNESS, RESILIENCE, BURN OUT AND BEING A HEALTHCARE WORKER IN THE WORLD NOW. THIS EPISODE EXPLORES JOB SECURITY, PUBLIC RECOGNITION, AND THE EFFECT OF THE PANDEMIC AT A PERSONAL LEVEL FOR DOCTORS AND NURSES.   In the context of the pandemic, the good comes with the bad – as Liz explains. Throughout the pandemic we have seen health care professionals experience trying work conditions the world over. However, it is one of the few industries that did not experience staff layoffs and work reduction. On the other hand, all healthcare systems in the world are imperfect. Throwing a pandemic into the mix produced even more challenges. It was therefore unlikely that the mental health and satisfaction from work was going to improve over the past two years. Hugh discusses the disposition of healthcare workers in London during the pandemic – one of the hardest hit regions in the world. Whilst the pandemic initially provided an opportunity for intensive care doctors and nurses to do what they are trained to do; the ongoing nature has proven to be challenging. The doctors in his system are weary – both mentally and physically. Shelly highlights the touching nature of working within a close team during this difficult period. In her experience there is a comradery that has been emphasised through the pandemic. However, Shelly states that even in non-pandemic times healthcare workers have struggled to cultivate a healthy relationship with the rest of their lives outside of work. Therefore, her first thoughts at the start of the pandemic were not of the intellectually interesting challenge, but rather what was going to happen with her family. On a broader scale, Shelly posits these hardships may lead to more and more healthcare professionals leaving the industry.   Tune in to this authentic perspective on healthcare worker wellbeing with Peter Brindley, Hugh Montgomery, Liz Crowe & Shelly Dev. Join us on Thursday at the Medtech Conference in Minneapolis to hear from Congressman Erik Paulsen. A longtime proponent of the Medtech industry, Rep. Paulsen most recently co-authored a letter asking the Trump administration to remove medical technology products from a list of Chinese imports subject to protective tariffs. The five-term Congressman also has fought against the Medical Device Tax. The Medtech Conference co-chairs and advisory board also are pleased to honor Dave Amerson, CEO of NeoTract, as the recipient of this year’s Dealmaker of the Year Award. The award recognizes Amerson's leadership of the urology start-up as it established its UroLift System as a critical treatment of lower urinary tract symptoms due to benign prostatic hyperplasia. Sales of UroLift were surging when Teleflex agreed to acquire the company, paying $725 million up front with the promise of another $375 million in milestone payments over the next two years. In an interview, Justin Klein, general partner of NEA, and Amerson will discuss: The steps Neotract took to adopt a winning culture? What strategies helped the company secure support from payers, providers? How the deal with Teleflex came to be? What's next for Teleflex in Urology? Join us on May 31st to explore this singular successful story and to better understand our entire Medtech sector.

Rick Mangum, CPA, joined us for this episode of Life In Accounting: The Where Accountants Go podcast. I invited Rick on the show for several reasons, but primarily for the discussion of a venture he was involved in that was backed by private equity where he served as the CFO on the executive team that built the company for eventual sale. The company we are referring to was Vidacare, a medical device company that sold to Teleflex just a few years back. Golf wasn't going to work out… As with all our interviews, we started with the question of how Rick decided to pursue accounting as a career in the first place. Interestingly enough, Rick started out as a golf major but when he realized that wasn't going to work out for him as a long-term career option, he decided to look into accounting. Taking risks… Although his career started in a more ‘traditional' manner with his first job being at EY (Ernst & Whinney at the time), he later got into roles that were higher risk / higher reward with his moves to Clear Channel and later Vidacare. Both organizations were going through substantial changes internally, and both included ownership transitions. This change of direction with his career correlates to the time that he decided to pursue an MBA, much later in his career than many do. It was this exposure as well as the knowledge that came through the program that allowed him to be more comfortable with riskier moves and also equipped him with the knowledge necessary to take on more dynamic roles. Selling for $300 million… You will need to listen to the audio for this episode to get all the details, but in a nutshell the Vidacare organization was doing around $20 million in revenue when he joined them, moved up to $70 million as Rick and their executive management team worked on refinements within the organization, and then eventually sold for $300 million to Teleflex a few years back. Vidacare is a great story of inventing a needed product, taking it to market, and then reaping the rewards. I hope you enjoy all the career insights that Rick provides throughout this episode. He has some great stories and has learned much through his experiences. Stay tuned for another exciting guest next week!
